[The clinical application of compound induced hypotension during functional endoscopic sinus surgery].

OBJECTIVE: To assess the clinic efficacy of compound induced hypotension during functional endoscopic sinus surgery. METHOD: Sixty patients with chronic sinusitis and nasal polyps were divided into three groups: Sodium nitroprusside (SNP) group (A), diltiazem+SNP group (B), labetalol+SNP group (C). We started induced hypotension with beginning of functional endoscopic sinus surgery. Mean arterial pressure (MAP), heart rate (HR), cardiac output (CO), cardiac index (CI) and ejection fraction (EF) were recorded during induced hypotension. Sugar and lactic acid in arterial and venous blood were a

A rare complication of endoscopic sinus surgery: necrotizing fasciitis of the eyelid.

Necrotizing fasciitis is a soft tissue infection characterized by necrosis of fascia and subcutaneous tissue. It frequently involves the groin, abdomen and lower extremities, but rarely involves the head and neck region. An unusual occurrence of periorbital necrotizing fasciitis after a routine endoscopic sinus surgery in a 57-year-old woman with a history of type II diabetes mellitus is presented. Although the disease is very rare, all the physicians should be aware of the manifestations of this disorder and its treatment. Early recognition and prompt intervention plays an important role in m

[Macrolide antibiotic-induced vasculitis (Churg-Strauss syndrome)].

BACKGROUND: Macrolides are known to have relatively few side effects and are prescribed in cases of allergic reaction to penicillin. The new macrolides, for example Azithromycin and Roxithromycin, are increasingly preferred over erythromycin at the ear, nose, and throat out-patient department due to improved oral reabsorption (acid resistance), better penetration into tissue, prolonged half-life, extended antibacterial activity, modest side effects, and better pharmacokinetics. There are only few case reports concerning side effects of macrolides. We report on the appearance of a Churg Strauss

Microscopic endonasal transsphenoidal approach for repair of sellar defect from penetrating trauma during wartime: illustrative case.

BACKGROUND: Penetrating head trauma caused by blast injuries can result in intracranial foreign bodies (FBs), which pose significant risks, including CSF leaks, pneumocephalus, and infections, particularly when paranasal sinuses are involved. OBSERVATIONS: A 54-year-old male sustained a blast injury in Ukraine, resulting in shrapnel embedded in the sella turcica. The trajectory of the shrapnel traversed the right medial orbital wall into the ethmoid and sphenoid sinuses, ultimately reaching the sella and causing a skull base defect with CSF rhinorrhea. CT imaging revealed subarachnoid hemorrh

Beyond the Maxillary Sinus: A Proposed Classification of Chronic Sinus Atelectasis.

BACKGROUND: Chronic maxillary atelectasis (CMA) describes the loss of maxillary sinus volume and has been well-described in the literature under many different names, creating inconsistency in reporting. A recent proposal has standardized the nomenclature but does not include disease of other paranasal sinuses. OBJECTIVE: We describe a unique case of chronic ethmoid atelectasis, briefly review literature associated with CMA, and propose an expanded classification of chronic sinus atelectasis. RESULTS: A 70-year-old man presented to the emergency department with transient vision loss and was

Nasal septal abscess complicating acute pansinusitis in a child: A case report.

INTRODUCTION AND IMPORTANCE: Nasal septal abscess (NSA) is a rare complication of acute sinusitis in children. It is very difficult to find in the literature. NSA can have devastating complications. These include potentially life-threatening intracranial infection and cosmetic deformity of the nose. These complications, which affect not only functional but also vital prognosis, can be prevented by early diagnosis and treatment. CASE PRESENTATION: We report the case of a 13-year-old boy who presented to the emergency department with a 2-day history of bilateral eyelid oedema extending to the n

Technique notes on the management of superior sagittal or transverse sinus during the falcotentorial meningioma surgery: a case report.

BACKGROUND: Falcotentorial meningiomas (FM) are surgical challenges for protecting sinus, and the technique notes on the management of superior sagittal or transverse sinus are required for good results. METHODS: We improved the technique notes on the management of superior sagittal or transverse sinus in three FM patients with signs of increased intracranial pressure or chronic headache. RESULTS: All patients underwent surgeries in the prone position, and occipital/sup-occipital/sub-occipital craniotomy was performed. In one patient, the skull was removed traditionally with exposure of the

Automated MRI pituitary structured reporting from free-text using a fine-tuned Llama model: a feasibility study.

BACKGROUND AND OBJECTIVE: Structured reports in radiology have demonstrated substantial advantages over unstructured ones. However, the transition from unstructured to structured reporting can face challenges, as experienced radiologists worry about the potential loss of valuable information. In this study, we fine-tuned the Llama 2 model capable of generating structured pituitary MRI reports from unstructured reports. METHODS: We used a training set comprising 104 pituitary MRI reports to fine-tune Llama 2 and 26 reports as a test set to evaluate the system. The dataset was annotated manuall

Sex Estimation From the Paranasal Sinus Volumes Using Semiautomatic Segmentation, Discriminant Analyses, and Machine Learning Algorithms.

The aims of this study were to determine whether paranasal sinus volumetric measurements differ according to sex, age group, and right-left side and to determine the rate of sexual dimorphism using discriminant function analysis and machine learning algorithms. The study included paranasal computed tomography images of 100 live individuals of known sex and age. The paranasal sinuses were marked using semiautomatic segmentation and their volumes and densities were measured. Sex determination using discriminant analyses and machine learning algorithms was performed. Males had higher mean volumes

Sinonasal Mucosal Melanoma: An Update and Review of the Literature.

Primary sinonasal mucosal melanoma (SNMM) is an aggressive tumor with high metastatic potential and poor outcomes. Presenting symptoms are nonspecific, and the nasal cavity is the most common site of origin followed by the maxillary and ethmoid sinuses. Histopathologically, SNMMs are pleomorphic and predominantly composed of epithelioid cell type. Identifying these tumors requires a high index of suspicion for melanoma and the use of a panel of immunohistochemical markers when typical histopathological features are missing. Not infrequently, these tumors are undifferentiated and/or amelanotic.

Lemierre's syndrome complicated by cerebral venous sinus thrombosis: A life threatening and rare disease successfully treated with empiric antimicrobial therapy and conservative approach.

Lemierre's syndrome (LS) is a "forgotten" condition characterized by septic thrombophlebitis of the jugular vein that follows an otolaryngological infection. Fusobacterium necrophorum is the aetiological agent responsible for the syndrome in adolescents and young adults whereas in older people even common bacteria are involved. Complications arise from spreading of septic emboli distally, i.e. to the brain, lungs, bones and internal organs everywhere in the body. We report a middle-aged woman who presented with headache and bilateral sixth cranial nerve palsy following a sphenoidal sinusitis a

Rapidly Progressive Orbital Apex Syndrome Due to Scedosporium apiospermum Following Endoscopic Sinus Surgery.

Scedosporium apiospermum is a ubiquitous, highly resistant opportunistic fungus found in sewage and polluted waters and may infect the paranasal sinuses. Orbital Apex Syndrome may occur following trauma, surgery, or infection. An 80-year-old male with diabetes mellitus and mild dementia underwent uncomplicated, bilateral functional endoscopic sinus surgery for chronic sinusitis with polyposis. Initial pathology was reported as non-invasive bacterial and fungal species. On postoperative day 4, he had sudden right vision loss and abducens nerve palsy. Imaging noted violation of the lamina papyra

A Rare Case of Sinonasal Lymphoepithelial Carcinoma Presented With Clinically Stage IV Disease.

Sinonasal lymphoepithelial carcinoma (SLEC) is an extremely rare malignancy. We present a case of SLEC in a 77-year-old man who presented with nasal congestion and persistent sinusitis. Imaging revealed a large right nasal mass involving right paranasal sinuses along with bulky bilateral cervical lymphadenopathy. In addition, there was a fluorodeoxyglucose avid L1 vertebral lesion. Biopsy of nasal mass and cervical lymph nodes showed syncytial growth of tumor cells in a lymphoplasmacytic background. Immunohistochemical stains showed positivity for pankeratin, CK5/6, epithelial membrane antigen

Study of factors influencing preoperative detection of alveolar antral artery by CBCT in sinus floor elevation.

This study aimed at assessing the prevalence of alveolar antral artery (AAA) detection by CBCT, its related variables, and at describing explanatory models useful in surgical planning, by retrospective evaluation of CBCT explorations. The modelling of the probability for detecting AAA was undertaken using logistic generalized additive models (GAM). The capacity for discriminating detection/no detection was assessed by receiver operating characteristic curves. A total of 466 sinuses were studied. Univariate models showed detection probability was linked to sinus width and thickness of the later

Nasal mites (Mesostigmata: Rhinonyssidae) in African penguins (Spheniscus demersus).

Rhinonyssids are obligate haematophagous mites that parasitize the nasal cavity of vertebrates, and occur in a wide range of birds worldwide. Two species of nasal mites are known to occur in penguins: Rhinonyssus sphenisci, which has been recorded from Humboldt and Magellanic penguins (Spheniscus humboldti and S. magellanicus, respectively), and Rhinonyssus schelli, which has been recorded in Adélie and Gentoo penguins (Pygoscelis adeliae and P. papua, respectively). We examined the nasal cavity of African penguins (Spheniscus demersus) that died while under care at a rehabilitation centre (N

Bacterial community collapse: a meta-analysis of the sinonasal microbiota in chronic rhinosinusitis.

Chronic rhinosinusitis (CRS) is a common, debilitating condition characterized by long-term inflammation of the nasal cavity and paranasal sinuses. The role of the sinonasal bacteria in CRS is unclear. We conducted a meta-analysis combining and reanalysing published bacterial 16S rRNA sequence data to explore differences in sinonasal bacterial community composition and predicted function between healthy and CRS affected subjects. The results identify the most abundant bacteria across all subjects as Staphylococcus, Propionibacterium, Corynebacterium, Streptococcus and an unclassified lineage o

Endovascular plug for internal carotid artery occlusion in the management of a cavernous pseudoaneurysm with bifrontal subdural empyema: technical note.

The authors demonstrate the use of an endovascular plug in securing a carotid artery pseudoaneurysm in an emergent setting requiring craniotomy for a concurrent subdural empyema. They describe the case of a 14-year-old boy with sinusitis and bifrontal subdural empyema who underwent transsphenoidal exploration at an outside hospital. An injury to the right cavernous segment of the ICA caused torrential epistaxis. Bleeding was successfully controlled by inflating a Foley balloon catheter within the sphenoid sinus, and the patient was transferred to the authors' institution. Emergent angiography

A review of nasal, paranasal, and skull base tumors invading the orbit.

Tumors that invade the orbit are uncommon. The majority are meningiomas arising from the sphenoid ridge (66%). Others are bone and cartilage tumors arising from the surrounding bones of the orbit, pituitary adenomas, and epithelial tumors arising from the paranasal sinuses and nasal cavity. Meningiomas occur more often in women, whereas epithelial tumors have a predilection for men. Meningiomas and epithelial tumors typically present in the sixth decade of life, whereas bone tumors tend to affect individuals in their third decade of life. Patients often present with a combination of ophthalmol

A literature review of radiological findings to guide the diagnosis of gallbladder adenomyomatosis.

BACKGROUND: Gallbladder adenomyomatosis (GA) is a benign gallbladder entity discovered as an asymptomatic gallbladder mass. Since gallbladder cancer is in the differential diagnosis for gallbladder masses, the ability to differentiate benign disease avoids a more extensive oncologic resection. This study sought to review imaging modalities used to diagnose GA. METHODS: PubMed and SciVerse Scopus were systematically searched using the terms: "gallbladder adenomyomatosis" and "gallbladder imaging" for articles published between January 2000 and January 2015. RESULTS: A total of 14 articles wer
