[X-ray diagnosis and prognosis of post-traumatic cervical myelopathies].

X-ray examination (pneumomyelography, vertebral angiography, phlebospondylography) of 192 patients with disorders of functions of the cervical spinal cord in a late period after the injury demonstrated residual local and diffuse compressions and atrophy of the spinal cord. Operations were conducted on 186 patients; atrophy of the spinal cord aggravated restoration of functions after decompression operations, which must be borne in mind in prognosticating the disease.

Accuracy and clinical outcomes of TiRobot-assisted pedicle screw fixation for thoracolumbar fractures: a meta-analysis.

Accurate pedicle screw placement is critical for achieving optimal stability and reducing procedure-related complications in thoracolumbar fracture surgery. Robotic navigation systems, such as TiRobot, have been increasingly adopted to improve surgical precision; however, their overall clinical value remains to be clarified. Following rigorous study selection, data extraction, and methodological quality assessment, quantitative synthesis was performed using RevMan 5.4. Primary outcomes covered screw placement accuracy and intraoperative parameters, while secondary outcomes included radiologica

Biomechanical optimization of iliolumbar fixation strategies for unilateral vertical sacral fractures: Prioritizing stability-mobility balance via finite element analysis.

OBJECTIVE: This study aims to optimize iliolumbar fixation strategies for unilateral vertical sacral fractures via finite element analysis, by comparing stability, implant stress, and lumbar mobility to identify the optimal clinical option. METHODS: A finite element model of the pelvis and L3-L5 lumbar spine was constructed to simulate four fixation models (L4L5IS, L5S1IS, L5IS, S1IS) under a 600 N vertical load. Sacral vertical displacement, implant stress, and fracture line separation were analyzed. RESULTS: Double-segment fixation (L4 + L5 + iliac screw and L5 + S1 + iliac screw) demonstr

Is There a Difference in Postoperative Outcomes Between Kyphoplasty and Vertebroplasty in the Management of Vertebral Compression Fractures?: A Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Cement augmentation using vertebroplasty (VP) or kyphoplasty (KP) can be employed to manage vertebral compression fractures (VCFs). Randomized controlled trials (RCTs) have disagreed about the superiority of one technique over the other. Therefore, a meta-analysis of RCTs is warranted. METHODS: PubMed, Cochrane, Embase, and Google Scholar were searched for articles from database inception to July 15, 2024. The inclusion criteria consisted of English and non-English-language RCTs comparing KP to VP in the management of VCFs. The studied outcomes were the risks of cement leakage and

Navigated Osteosynthesis for Unstable Atlas Fractures: Technical Note and Case Series.

OBJECTIVE: Fractures of the atlas are typically considered stable or unstable based on the integrity of the transverse ligament. Whereas stable Jefferson burst fractures can be treated nonoperatively, unstable fractures with disruption of the transverse ligament often require surgical intervention. Atlas osteosynthesis has been proposed as a motion-preserving alternative to atlantoaxial fusion. Intraoperative navigation may facilitate safe placement of C1 instrumentation. METHODS: Cases of patients with unstable atlas fractures treated with navigated osteosynthesis at a single level I trauma

Incidence of deep vein thrombosis and symptomatic pulmonary embolism in Taiwanese patients with pelvic and/or acetabular fractures: a retrospective study.

Venous thromboembolism (VTE) is common in patients with trauma, and thromboprophylaxis has been advocated. However, conflicting results regarding VTE rates in the Asian population following orthopaedic procedures have been presented. We aimed to investigate the VTE incidence in Taiwanese patients with pelvic and/or acetabular fractures and identify the associated risk factors. We included 402 patients who underwent surgery for pelvic and/or acetabular fractures. All patients received mechanical thromboprophylaxis with graduated compression stockings. Duplex scanning was performed postoperative

Can a Nomogram Predict Survival After Treatment for an Ankylosing Spondylitis Cervical Fracture in a Patient With Neurologic Impairment? A National, Multicenter Study.

BACKGROUND: Ankylosing spondylitis-related cervical spine fracture with neurologic impairment (ASCF-NI) is a rare but often lethal injury. Factors independently associated with survival after treatment remain poorly defined, and identifying patients who are likely to survive the injury remains challenging. QUESTIONS/PURPOSES: (1) What factors are independently associated with survival after treatment among patients with ASCF-NI? (2) Can a nomogram be developed that is sufficiently simple for clinicians to use that can identify patients who are the most likely to survive after injury? METHODS

Prediction of fragility fractures in men with prostate cancer under androgen deprivation therapy: the importance of a multidisciplinary approach using a mini-invasive diagnostic tool.

Bone fragility in men who are treated with androgen deprivation therapy (ADT) has a complex pathophysiology that differs from that of primary and post-menopausal osteoporosis. Fracture risk assessment based on bone mineral density (BMD) and Fracture Risk Assessment Tool (FRAX) score might not be effective in this patient setting, since high frequency of fragility fractures has been reported even in subjects with low FRAX risk and normal BMD. In this paper we want to emphasize the importance in the individual assessment of bone fragility and prediction of fractures by measuring parameters of bo

Effect of a new transpedicular vertebral device for the treatment or prevention of vertebral compression fractures: A finite element study.

BACKGROUND: A finite element study was performed to investigate the biomechanical performance of a novel transpedicular implant (V-STRUT©, Hyprevention, France) made of PEEK (polyetheretherketone) material in terms of strengthening the osteoporotic vertebra and the thoraco-lumbar spine. The objective was to assess numerically the efficacy of the implant to reduce the stress distribution within bone and absorb part of the stress by the implant thanks to its optimized material selection close to that of normal bone. METHODS: A numerical model was generated based on a scan of an osteoporotic pat

Finite element analysis of sacral-alar-iliac screw fixation for sacroiliac joint dislocation.

The percutaneous sacroiliac (SI) screw is a common fixation option for posterior ring disruption in pelvic fractures. However, SI screw placement is difficult and can injure adjacent neurovascular structures. The sacral-alar-iliac screw (SAI) is a safe, reliable free-hand sacral pelvic fixation technique. To investigate the biomechanical stability of SAI for SI joint dislocation, finite element analysis was performed in unstable Tile-Type B and C pelvic ring injuries. The displacement in S1 (fixation of a unilateral S1 segment with one SI screw), TS1 (fixation of the S1 segment with a transsac

Automated detection and classification of acute vertebral body fractures using a convolutional neural network on computed tomography.

BACKGROUND: Acute vertebral fracture is usually caused by low-energy injury with osteoporosis and high-energy trauma. The AOSpine thoracolumbar spine injury classification system (AO classification) plays an important role in the diagnosis and treatment of the disease. The diagnosis and description of vertebral fractures according to the classification scheme requires a great deal of time and energy for radiologists. PURPOSE: To design and validate a multistage deep learning system (multistage AO system) for the automatic detection, localization and classification of acute thoracolumbar verte

Sarcopenic obesity defined by visceral adiposity was associated with osteoporotic vertebral fracture.

BACKGROUND: Previous studies have reported that the fracture risk related to sarcopenic obesity (SO) may be influenced by the distribution of fat mass. Therefore, it is useful to explore a body component suitable for defining obesity when predicting fracture risk. This study was an attempt to explore the contribution of SO defined by visceral adiposity on the incidence of osteoporotic fracture. METHODS: We enrolled 736 Chinese patients aged > 60 years in this prospective study. Sarcopenia was defined as low skeletal muscle index (SMI) with muscle strength or low SMI with low physical performa
