Dosimetric Comparisons of Volumetric Modulated Arc Therapy and Tomotherapy for Early T-Stage Nasopharyngeal Carcinoma.

PURPOSE: To compare the dosimetric differences between volumetric modulated arc therapy (VMAT) and helical tomotherapy (HT) in treating early T-stage nasopharyngeal carcinoma (NPC). METHOD: Ten patients with early T-stage NPC who received tomotherapy using simultaneously integrated boost (SIB) strategies were replanned with VMAT (RapidArc of Varian, dual-arc). Dosimetric comparisons between the RapidArc plan and the HT plan included the following: (1) D98, homogeneity, and conformity of PTVs; (2) sparing of organs at risk (OARs); (3) delivery time and monitor units (MUs). RESULTS: (1) Compar

Do recurrent seizures cause neuronal damage? A series of studies with MRI volumetry in adults with partial epilepsy.

Despite optimal treatment, 30% of epilepsy patients develop intractable epilepsy and continue to have recurrent seizures or other symptoms of epileptic syndrome restricting their ability to lead a full life. Hippocampal sclerosis is found in 60-70% of patients with intractable temporal lobe epilepsy (TLE). However, it is not known whether the damage in the hippocampus is the cause or the consequence of TLE. The purpose of the present series of studies was to investigate with magnetic resonance imaging (MRI) the appearance of medial temporal lobe damage during the course of partial epilepsy, an

Brief Encounters with Real Objects Modulate the Medial Parietal But Not Occipitotemporal Cortex.

Humans are skilled at recognizing everyday objects from pictures, even if we have never encountered the depicted object in real life. However, if we have encountered an object, how does that real-world experience affect the representation of its photographic image in the human brain? We developed a paradigm that involved brief real-world manual exploration of everyday objects prior to the measurement of brain activity with fMRI while viewing pictures of those objects (40 participants, 28 females). We found that while object-responsive regions in the lateral occipital and ventral temporal corte

The semantic network supports approximate computation.

OBJECTIVE: The present study used complex approximate and exact arithmetic computation problems to dissociate the brain networks for strategy-based approximate computation and procedure-based exact computation. METHOD: Twenty-eight college students were scanned with MRI while they were solving complex approximate and exact computation problems, including addition, subtraction, multiplication, and division of multidigit integers and fractions. The neuroimaging data were analyzed using whole brain, region of interest, and functional connectivity approaches. RESULTS: Results showed that approxi

Uncoupled relationship in the brain between regional homogeneity and attention function in first-episode, drug-naïve schizophrenia.

The relationship between the cognitive impairment and the structural and functional abnormalities in the brains of patients with schizophrenia (SZ) is not yet clear. This study aims to investigate the relationship, thereby exploring the neuromechanism underlying SZ. We collected multimodal MRI data from 68 first-episode, drug-naïve patients with SZ, and 64 well-matched healthy controls, and used regional homogeneity (ReHo) and gray matter volume (GMV) to assess the functional and structural integrity of the brains, respectively. We then evaluated in the entire brain the correlations between Re

Inferior-frontal cortex phase synchronizes with the temporal-parietal junction prior to successful change detection.

The inferior frontal gyrus (IFG) and the temporo-parietal junction (TPJ) are believed to be core structures of human brain networks that activate when sensory top-down expectancies guide goal directed behavior and attentive perception. But it is unclear how activity in IFG and TPJ coordinates during attention demanding tasks and whether functional interactions between both structures are related to successful attentional performance. Here, we tested these questions in electrocorticographic (ECoG) recordings in human subjects using a visual detection task that required sustained attentional exp

White matter hyperintensities are positively associated with cortical thickness in Alzheimer's disease.

White matter hyperintensities are associated with an increased risk of Alzheimer's disease (AD). White matter hyperintensities are believed to disconnect brain areas. We examined the topographical association between white matter hyperintensities and cortical thickness in controls, mild cognitive impairment (MCI), and AD patients. We examined associations between white matter hyperintensities and cortical thickness among 18 older cognitively healthy participants, 18 amnestic MCI, and 17 mild AD patients. These associations were cluster-size corrected for multiple comparisons. In controls, a po

Differences and the relationship in default mode network intrinsic activity and functional connectivity in mild Alzheimer's disease and amnestic mild cognitive impairment.

There is evidence that the default mode network (DMN) functional connectivity is impaired in Alzheimer's disease (AD) and few studies also reported a decrease in DMN intrinsic activity, measured by the amplitude of low-frequency fluctuations (ALFFs). In this study, we analyzed the relationship between DMN intrinsic activity and functional connectivity, as well as their possible implications on cognition in patients with mild AD and amnestic mild cognitive impairment (aMCI) and healthy controls. In addition, we evaluated the differences both in connectivity and ALFF values between these groups.

Neural substrates for judgment of self-agency in ambiguous situations.

The sense of agency is the attribution of oneself as the cause of one's own actions and their effects. Accurate agency judgments are essential for adaptive behaviors in dynamic environments, especially in conditions of uncertainty. However, it is unclear how agency judgments are made in ambiguous situations where self-agency and non-self-agency are both possible. Agency attribution is thus thought to require higher-order neurocognitive processes that integrate several possibilities. Furthermore, neural activity specific to self-attribution, as compared with non-self-attribution, may reflect hi

Intact but less accessible phonetic representations in adults with dyslexia.

Dyslexia is a severe and persistent reading and spelling disorder caused by impairment in the ability to manipulate speech sounds. We combined functional magnetic resonance brain imaging with multivoxel pattern analysis and functional and structural connectivity analysis in an effort to disentangle whether dyslexics' phonological deficits are caused by poor quality of the phonetic representations or by difficulties in accessing intact phonetic representations. We found that phonetic representations are hosted bilaterally in primary and secondary auditory cortices and that their neural quality

Empirical validation of the triple-code model of numerical processing for complex math operations using functional MRI and group Independent Component Analysis of the mental addition and subtraction of fractions.

The suitability of a previously hypothesized triple-code model of numerical processing, involving analog magnitude, auditory verbal, and visual Arabic codes of representation, was investigated for the complex mathematical task of the mental addition and subtraction of fractions. Functional magnetic resonance imaging (fMRI) data from 15 normal adult subjects were processed using exploratory group Independent Component Analysis (ICA). Separate task-related components were found with activation in bilateral inferior parietal, left perisylvian, and ventral occipitotemporal areas. These results sup

Epilepsy in vacuolating megalencephalic leukoencephalopathy with subcortical cysts.

Vacuolating megalencephalic leukoencephalopathy with subcortical cysts (MLC) is a disorder characterised by acquired macrocephaly, developmental motor delay of varying degrees, slowly progressive cerebellar and pyramidal signs, and initially preserved intellectual function. More than 60% of the published cases had epileptic seizures. In this study, we analysed the seizures and EEG findings of nine patients with MLC. Six patients (66.6%) with moderate to severe neurological impairment had epilepsy, four with partial and two with generalised seizures. The EEG of five epileptic patients revealed

Multiple levels of visual object constancy revealed by event-related fMRI of repetition priming.

We conducted two event-related functional magnetic resonance imaging (fMRI) experiments to investigate the neural substrates of visual object recognition in humans. We used a repetition-priming method with visual stimuli recurring at unpredictable intervals, either with the same appearance or with changes in size, viewpoint or exemplar. Lateral occipital and posterior inferior temporal cortex showed lower activity for repetitions of both real and non-sense objects; fusiform and left inferior frontal regions showed decreases for repetitions of only real objects. Repetition of different exemplar

Motion-responsive regions of the human brain.

Functional magnetic resonance imaging was used to map motion responsive regions of the human brain by contrasting passive viewing of moving and stationary randomly textured patterns. Regions were retained as motion responsive if they reached significance either in the group analysis or in the majority of hemispheres in single-subject analysis. They include well-known regions, such as V1, hMT/V5+, and hV3A, but also several occipito-temporal, occipito-parietal, parietal, and frontal regions. The time course of the activation was similar in most of these regions. Motion responses were nearly ide

[Hemianopic visual field defects--methods of study and localization problems].

In a survey of methods for determining hemianopic visual field defects, a distinction is made between gross visual field screening and actual visual field examination with kinetic and automatic static perimetry. These methods may be arranged according to increasing the concentration required as well as to ability to cooperate, as follows: hand-movement stimulation of shifts of gaze, finger-counting with repetition by the patient, brightness and color comparison among the respective halves and quadrants of the visual field, then kinetic perimetry with the Goldmann perimeter, and, most exacting

Subarachnoid metastases from a prolactinoma.

A 37-year-old man developed a left frontal metastasis from a prolactin-secreting pituitary tumour, which had been operated on nine years before. The metastatic tumour was totally excised. One and a half years later he was found to have multiple left temporal and parietal subarachnoid metastases. Because treatment with bromocriptine for five months and subsequent radiation proved ineffective, the tumour nodules were removed surgically.

Cortical and subcortical projections of the middle temporal area (MT) and adjacent cortex in galagos.

Projections of the middle temporal visual area, MT, and of visual cortex adjoining MT were investigated with autoradiographic methods in the prosimian primate, Galago senegalensis. Ipsilateral cortical targets of MT included area 17, area 18, cortex caudal to MT, cortex ventral to MT, and parietal-occipital cortex dorsal to MT. This pattern of projections suggests that extrastriate cortex contains a number of visual subdivisions in addition to MT. Contralateral projections were to MT and parietal-occipital cortex. Projections from MT to areas 17 and 18 connected regions representing similar pa

Spatiotemporal characteristics of the neural representation of event concepts.

Events are a fundamentally important part of our understanding of the world. How lexical concepts denoting events are represented in the brain remains controversial. We conducted two experiments using event and object nouns matched on a range of psycholinguistic variables, including concreteness, to examine spatial and temporal characteristics of event concepts. Both experiments used magnitude and valence tasks on event and object nouns. The fMRI experiment revealed a distributed set of regions for events, including the angular gyrus, anterior temporal lobe, and posterior cingulate across task

Zero-crossing patterns reveal subtle epileptiform discharges in the scalp EEG.

Clinical diagnosis of epilepsy depends heavily on the detection of interictal epileptiform discharges (IEDs) from scalp electroencephalographic (EEG) signals, which by purely visual means is far from straightforward. Here, we introduce a simple signal analysis procedure based on scalp EEG zero-crossing patterns which can extract the spatiotemporal structure of scalp voltage fluctuations. We analyzed simultaneous scalp and intracranial EEG recordings from patients with pharmacoresistant temporal lobe epilepsy. Our data show that a large proportion of intracranial IEDs manifest only as subtle, l

Remote photonic sensing of cerebral hemodynamic changes via temporal spatial analysis of acoustic vibrations.

A novel photonic method for remote monitoring of task-related hemodynamic changes in human brain activation is presented. Physiological processes associated with neural activity, such as nano-vibrations due to blood flow and tissue oxygenation in the brain, are detected by remote sensing of nano-acoustic vibrations using temporal spatial analysis of defocused self-interference random patterns. Temporal nanometric changes of the speckle pattern due to visual task-induced hemodynamic responses were tracked by this method. Reversing visual checkerboard stimulation alternated with rest epochs, and
