Anomalous origin of the left main coronary artery misdiagnosed by coronary angiography and correctly detected with multidetector computed tomography.

We describe a case of a very anomalous origin of the left main coronary artery, misdiagnosed as occluded at previous invasive coronary angiography. Multidetector computed tomography coronary angiography demonstrated the vessel to be patent, with an anomalous origin from the commissural region between the right sinus of Valsalva and posterior sinus of Valsalva. A large coronary fistula between proximal left anterior descending coronary artery and the right sinus of Valsalva was also demonstrated. This is an example of the additional clinical role of multidetector computed tomography coronary an

Successful management by provocative angiography and endovascular stent of Ureteroarterial fistula in a patient with a long-term indwelling ureteral stent.

We present the clinical course of a ureteroiliac arterial fistula in a patient who had been managed by ureteral stenting for 8 years for severe ureteral stricture after abdominoperineal resection with pelvic irradiation for advanced rectal cancer. A multidisciplinary team approach including provocative angiography and an endovascular stent saved the life of the patient. Ureteroarterial fistula is a rare complication of a long-term indwelling ureteral stent that is potentially fatal unless a prompt diagnosis and adequate therapy are provided. Heightened awareness and a high index of suspicion f

Prognostic value of coronary computed tomographic angiography for prediction of cardiac events in patients with suspected coronary artery disease.

OBJECTIVES: We assessed the rate of cardiac events after detection or exclusion of obstructive coronary artery disease (CAD) by coronary computed tomography angiography (CCTA). BACKGROUND: Several studies have demonstrated a high diagnostic accuracy of CCTA for detection of obstructive CAD compared with invasive angiography, but data regarding the clinical prognostic value of CCTA are limited. METHODS: In all, 1,256 consecutive patients with suspected CAD undergoing 64-slice CCTA in our institution between October 2004 and September 2006 were observed prospectively for the occurrence of seve

Impact of tricuspid regurgitation and prior coronary bypass surgery on the geometry of the coronary sinus: a rotational coronary angiography study.

INTRODUCTION: The coronary sinus (CS) is often distorted in patients with advanced cardiomyopathy, making CS cannulation difficult. The objective of this study was to examine the impact of the underlying cardiac pathology on the variability of the CS anatomy, using rotational coronary venous angiography (RCVA). METHODS AND RESULTS: Seventy-nine patients undergoing RCVA for cardiac resynchronization therapy (CRT) were evaluated: age 63 +/- 15 years, 43% with prior coronary artery bypass grafting (CABG). Aspects of the CS anatomy which could impact cannulation were examined: the CS ostial angle

Frequency of renal artery stenosis and variants of renal vascularization in hypertensive patients: analysis of 1550 angiographies in one centre.

Renal artery stenosis (RAS) is an important cause of arterial hypertension and chronic kidney disease. The aims of our study were to assess the prevalence of RAS and to examine the frequency of variants of renal vasculature, that is, multiple and/or accessory renal arteries in hypertensive patients referred to renal angiography. We evaluated retrospectively 1554 arteriographies of hypertensive patients. Angiograms were evaluated to find RAS, significant RAS (>60% stenosis of the lumen), radiological signs of atherosclerosis, aneurysms of the renal arteries or aorta and variants of kidney vascu

Prevalence and characteristics of major and minor coronary artery anomalies in an adult population assessed by computed tomography coronary angiography.

AIMS: There is conflicting data regarding the prevalence and characteristics of coronary artery anomalies (CAAs). We sought to explore the prevalence and characteristics of major and minor CAAs using computed tomography coronary angiography (CCTA). METHODS AND RESULTS: We prospectively studied 577 patients in sinus rhythm referred for control of stents or for screening due to the presence of multiple risk factors (53%), or due to chest pain, equivalent symptoms or inconclusive stress tests (47%). 40 and 64 slice CT scanners were used. The mean age was 61 +/- 11 years and 81% were male. We ide

Ultrasound and magnetic resonance angiography features of post-traumatic ulnar artery pseudoaneurysm: a case report and review of the literature.

Ulnar artery pseudoaneurysms are very uncommon. The least common etiological mechanism is a single direct trauma. It is important to identify these lesions, which may have important clinical complications such as distal thrombosis with digital ischemia or gangrene. This report describes the features of sonography and magnetic resonance angiography of a histologically confirmed ulnar artery pseudoaneurysm.

Hemispheric revascularization in the setting of carotid occlusion and subclavian steal: a diagnostic and management role for quantitative magnetic resonance angiography?

OBJECTIVE: Advances in phase contrast magnetic resonance imaging have facilitated the noninvasive assessment of blood flow rates and flow direction in patients with complex cerebrovascular disorders. METHODS: We describe a case of right hemispheric hypoperfusion in which, on noninvasive assessment with quantitative magnetic resonance angiography, the patient was found to harbor an occult subclavian steal with flow reversal in the left vertebral artery. RESULTS: The presence of posterior communicating arteries noted on quantitative magnetic resonance angiography suggested that normalization o

Rupture of an internal carotid artery aneurysm during angiography with leakage of contrast medium via an external ventricular drain.

With a ruptured intracranial aneurysm producing subarachnoid haemorrhage (SAH) cerebral angiography is currently used for identification of the affected vessel. Aneurysm rerupturing is one of the more serious complications of cerebral angiography and has been frequently described. We report a 61-year-old man who presented with SAH who had rerupture of a large aneurysm of the internal carotid artery during angiography. A substantial amount of contrast medium escaped via a ventricular drain. The three main risk factors for rerupture of an aneurysm are: angiography performed within 6 h of the pri

Duplex ultrasound imaging with an ultrasound contrast agent: the economic alternative to CT angiography for aortic stent graft surveillance.

The use of endovascular stent graft repair for aortic aneurysmal disease has become increasingly common, with the added requirement for close postoperative surveillance to detect the presence of endoleaks or graft migration. The most commonly used technique for surveillance is computed tomography (CT) angiography, with the need for intravenous contrast posing 1 limitation in those patients with renal dysfunction and the cost of this testing presenting an economic limitation. Early results of duplex imaging in the authors' Vascular Laboratory using an intravenous ultrasound contrast agent have

Cerebral angiography in the rat with mammographic equipment: a simple, cost-effective method for assessing vasospasm in experimental subarachnoid haemorrhage.

We report quantification of vasospasm following subarachnoid haemorrhage (SAH) and the effect of a new antivasospastic free radical scavenger (AVS) in rats, using an angiographic technique developed in our laboratory. We acquired single-exposure angiograms with mammographic equipment, using a 0.1 mm diameter focal spot and single-emulsion mammographic films. Contrast medium was injected through a PE50 catheter in the common carotid artery, after the external carotid artery had been ligated to divert flow towards the internal carotid artery territory. Measurements of the M1 and A1 segments and
