Determinants of fertility in Guatemala.

The 1978 Contraceptive Prevalence Survey for Guatemala provides an opportunity to examine fertility levels and a number of determinants of fertility for three broad segments of the country: the Department of Guatemala and, in the remainder of the country, the Ladino and Indian populations. While Ladinos had a much higher rate of contraceptive use than did Indians, the two groups had similar birth rates. The lack of difference in fertility appears to be due to the pattern of prolonged breastfeeding among Indians and perhaps to differences in the rate of conception due to nutrition, coital frequ

Inositol for subfertile women with polycystic ovary syndrome.

BACKGROUND: Subfertile women are highly motivated to try different adjunctive therapies to have a baby, and the widespread perception is that dietary supplements such as myo-inositol (MI) and D-chiro-insoitol (DCI) are associated with only benefit, and not with harm. Many fertility clinicians currently prescribe MI for subfertile women with polycystic ovary syndrome (PCOS) as pre-treatment to in vitro fertilisation (IVF) or for ovulation induction; however no high-quality evidence is available to support this practice. This review assessed the evidence for the effectiveness of inositol in subf

Neighborhood Income Mobility and Risk of Neonatal and Maternal Morbidity.

IMPORTANCE: Residing in a low-income neighborhood is generally associated with worse pregnancy outcomes. It is not known if moving from a low- to higher-income area between 2 pregnancies alters the risk of adverse birth outcomes in the subsequent birth compared with women who remain in low-income areas for both births. OBJECTIVE: To compare the risk of adverse maternal and newborn outcomes among women who achieved upward area-level income mobility vs those who did not. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study was completed in Ontario, Canada, from 2002 to 2019, w

Impact of rapidly cleaving embryos on blastocyst formation and single-blastocyst transfer outcomes.

BACKGROUND: Clarifying the impact of day-3 cell number on blastulation and subsequent pregnancy outcomes is essential for optimizing blastocyst selection criteria and refining embryo assessment protocols. While slow cleavage on day 3 is well-recognized as detrimental, the prognostic value of rapid cleavage (>8 cells) remains ambiguous. METHODS: This retrospective cohort study (January 2015-April 2024) included 64,853 embryos undergoing blastocyst culture (Cohort 1) and 2,669 single-blastocyst frozen embryo transfer (FET) cycles (Cohort 2) at a large tertiary assisted reproduction center. Risk factors of mortality in outborn neonates, an experience from central India: A prospective observational study.

BACKGROUND: Despite various measures, the neonatal mortality rate in outborn neonates is significantly high and makes impossible to achieve sustainable developmental goals. OBJECTIVES: To recognize the risk factors of mortality in outborn neonates. METHODS: A prospective observation cohort was studied at Government Medical College, Nagpur, for a duration of two and a half years on all outborn neonates. Neonates who died within an hour, whose parents left the hospital against medical advice, and not willing to participate were excluded. Demographic, maternal, and neonatal clinical parameters

Cross-sectional survey in Central African Republic finds mortality 4-times higher than UN statistics: how can we not know the Central African Republic is in such an acute humanitarian crisis?

BACKGROUND: CAR is one of the poorest countries in the world. While UN statistics suggest that there is no health emergency in the country, two recently published mortality surveys contradict this. Moreover, recent accusations of massive scale human rights abuses by mercenaries suggested the need for a nationwide mortality survey. METHODS: Two stage cluster surveys were conducted in two different strata: one in the roughly half of the country within the Government's control, and one in the areas mostly outside of the Government's control. We randomly selected 40 clusters of 10 households in e

Maternal complications in pregnancy and childbirth for women with epilepsy: Time trends in a nationwide cohort.

OBJECTIVE: Obstetric trends show changes in complication rates and maternal characteristics such as caesarean section, induced labour, and maternal age. To what degree such general time trends and changing patterns of antiepileptic drug use influence pregnancies of women with epilepsy (WWE) is unknown. Our aim was to describe changes in maternal characteristics and obstetric complications in WWE over time, and to assess changes in complication risks in WWE relative to women without epilepsy. METHODS: This was a nationwide cohort study of all first births in the Medical Birth Registry of Norwa

ConFIRM trial - conversion of in vitro fertilization cycles to intrauterine inseminations in patients with a poor ovarian response to stimulation: a protocol for a multicentric, prospective randomized trial.

BACKGROUND: To date, there is no consensus on the ideal management strategy of patients with poor ovarian response (POR) to controlled ovarian stimulation (COS) for in vitro fertilization (IVF). Currently, these patients are given the choice of: (1) canceling the cycle; (2) proceeding with COS regardless of the poor response, and performing the oocyte retrieval and transfer of embryos when available; or (3) conversion to an intrauterine insemination (IUI). When the decision to proceed with the COS cycle is taken, it is not clear whether IVF or conversion to IUI is the best choice. If live birt

Time trends in autism and in MMR immunization coverage in California.

CONTEXT: Considerable concern has been generated in the lay and medical communities by a theory that increased measles-mumps-rubella (MMR) immunization among young children may be the cause of an apparent marked increase in autism occurrence. OBJECTIVE: To determine if a correlation exists in secular trends of MMR immunization coverage among young children and autism occurrence. DESIGN, SETTING, AND PARTICIPANTS: Retrospective analyses of MMR immunization coverage rates among children born in 1980-1994 who were enrolled in California kindergartens (survey samples of 600-1900 children each ye

Maternal and infant complications in high and normal weight infants by method of delivery.

OBJECTIVE: To estimate the population risks of maternal and infant complications with the birth of macrosomic (at least 4000 g) compared with normal weight infants. METHODS: Term, singleton infants were identified from the state of Washington's birth event records database for 1990. Diagnosis codes from the Internal Classification of Diseases (9th revision) were used to identify delivery method and previously defined complications. We adjusted for maternal demographic and clinical factors using multivariable logistic regression to derive the risk of each maternal and infant complication. RES

Obesity, Twin Pregnancy, and the Role of Assisted Reproductive Technology.

IMPORTANCE: The prevalence of overweight and obesity (body mass index [BMI] ≥25) has increased globally, and high BMI has been linked to higher rates of twin birth. However, evidence from large population-based studies is lacking; the issue needs careful study, as women with obesity are also more likely to use assisted reproductive technology (ART), which frequently results in twin pregnancy. OBJECTIVE: To examine the association between BMI and twin birth and the role of ART as a potential mediator in this association. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study inclu

Low-dose oral misoprostol for induction of labour.

BACKGROUND: Misoprostol given orally is a commonly used labour induction method. Our Cochrane Review is restricted to studies with low-dose misoprostol (initially ≤ 50 µg), as higher doses pose unacceptably high risks of uterine hyperstimulation. OBJECTIVES: To assess the efficacy and safety of low-dose oral misoprostol for labour induction in women with a viable fetus in the third trimester of pregnancy. SEARCH METHODS: We searched Cochrane Pregnancy and Childbirth's Trials Register, ClinicalTrials.gov,  the WHO International Clinical Trials Registry Platform (14 February 2021) and referenc
