Glucose-dependent insulinotropic polypeptide deficiency reduced fat accumulation and insulin resistance, but deteriorated bone loss in ovariectomized mice.

Given the established roles of glucose-dependent insulinotropic polypeptide (GIP) in promoting fat storage and bone formation, we assessed the contribution of GIP to obesity and osteopenia in ovariectomized mice with a gene encoding green fluorescent protein (GFP) inserted into the GIP locus, in which GIP was either reduced (GIPgfp/+ ) or absent (GIPgfp/gfp ). In GIPgfp/gfp mice, weight gain, subcutaneous and visceral fat mass were reduced, and glucose intolerance was improved compared with wild-type mice with the same magnitude of insulin responses. Cancellous bone mineral density and bone co

Use of DXA-based technology for detection and assessment of risk of vertebral fracture in rheumatology practice.

Measurement of bone mineral density (BMD) by dual-energy X-ray absorptiometry (DXA) is the recommended method for diagnosis of osteoporosis and assessment of future fracture risk. However, most patients who will suffer fractures do not have osteoporosis by DXA (T-score of -2.5 or less). Bone strength, which is most closely associated with resistance to fracture, is a composite of both bone density and bone quality, and the latter is not measured by DXA. Thus, other technology is needed for non-invasive and inexpensive assessment of bone strength and fracture risk. Vertebral fractures, the most

[Postmenopausal osteoporosis. Digital Rx radiogrammetry in the diagnosis and follow-up of treatment with alfacalcidol].

Postmenopausal osteoporosis is characterised by an increased resorption of trabecular bone, a consequence of estrogen deficit. Changes in vitamin D metabolism are also an important contributors to the development of osteoporosis in postmenopausal women. Vitamin D and its active metabolites (Alfacalcidol, Calcitriol) perform important functions in regulation of the calcium balance and the bone metabolism. Aim of our study was to determine the efficacy of Alfacalcidol (Alpha D3) in reducing the loss of bone mass. 391 postmenopausal women with osteopenia and 165 postmenopausal women with osteopor

Rapid long-term bone loss following stroke in a man with osteoporosis and atherosclerosis.

Bone loss in humans has been reported where there is reduced mechanical loading such as in space flight, spinal cord injury, and stroke. Whether osteoporotic patients are susceptible to further bone loss in states of underloading such as hemiparesis is unknown. Here we report the case of a 64-year-old man with established idiopathic osteoporosis and atherosclerosis who presented with a right middle cerebral artery territory stroke. Annual bone mineral density measurements were made at the left hip and spine before and after left hemiparesis. The left total hip T-score was -3.2 before the strok

Transplantation osteoporosis and corticosteroid-induced osteoporosis in autoimmune diseases: experience with alfacalcidol.

The effect of alfacalcidol therapy on bone mineral density at the spine and proximal femur was evaluated in 112 transplant recipients (59 heart, 26 liver and 27 lung); 45 transplant cases served as controls (included in a randomised way in a placebo group) and in 42 rheumatoid arthritis cases. Liver and lung transplantation cases had before transplantation a lower bone density at the spine and femur compared to heart transplant cases. Heart transplant cases lost considerably more bone immediately after transplantation than liver and lung transplant recipients. A positive effect of 2 years alfa

Single photon bone densitometry in hemodialysis patients.

Renal osteodystrophy is a common finding in patients with renal insufficiency. The maximum of its intensity is found in hemodialysis patients. Bone densitometry is so far the best method for non-invasive assessment of the extent of the illness. Some densitometric studies in hemodialysis patients have already been published but their results differ in prevalence and intensity of renal osteodystrophy. They also demonstrated a slight relationship between intensity of renal osteodystrophy and duration of the dialysis treatment. Opinions vary on the relationship between bone mineral density and mar

Reduced axial bone mineral content in patients taking an oral anticoagulant.

Recent literature indicates that the vitamin K cycle plays a role in the calcification process, presumably via its intervention on gamma-carboxylation of the noncollagenous proteins of bone osteocalcin and matrix gamma-carboxyglutamic acid protein. The major clinical evidence of this interference is fetal bone defect caused by oral anticoagulants given to the mother during the first trimester of pregnancy. No bone abnormalities have been reported so far in adults receiving oral anticoagulants. We studied 56 women who had had cardiac valve replacement and who were given acenocoumarol as anticoa

Suppressed TSH levels secondary to thyroxine replacement therapy are not associated with osteoporosis.

OBJECTIVE: Recent studies have suggested that patients receiving thyroxine are at increased risk of osteoporosis. We set out to measure bone mineral densities in two groups of post-menopausal women receiving thyroxine replacement therapy (those with serum TSH levels persistently suppressed or non-suppressed) and to compare the results in both groups with those of the local control population. DESIGN: Cross-sectional study. PATIENTS: Seventy-eight post-menopausal women who had been treated with thyroxine for primary autoimmune or idiopathic hypothyroidism for a minimum of 5 years, 44 with TSH

Serum-derived exosomes of young rats protect bone of ovariectomized rats after fatigue loading in vivo.

In patients with postmenopausal osteoporosis, the accumulation of bone microdamage further increases fracture risk. Exosomes derived from the circulatory system of young individuals can reverse age-related defects during bone repair. Therefore, the present study aimed to elucidate the mechanisms underlying the protective effects of exosomes against structural degradation under fatigue-induced damage. To this end, a rat tibial fatigue injury model was established to investigate the protective effects of serum-derived exosomes (SDEs) isolated from young rats on bone after fatigue damage. SDEs we

Intake of neutral core human milk oligosaccharides (HMOs) during early life improves long-term bone quality.

Infancy is a key period for bone growth, determining bone strength later in life. Studies have provided early insights into the role that human milk oligosaccharides (HMOs) may play in growth, potentially through their effects on the microbiome. However, focus was on sialylated HMOs whereas neutral core HMOs represent the most abundant class in human breast milk. We explored impact on bone quantity, quality and strength in 1year old female minipigs exposed to neutral HMOs during preweaning. Milk formula led to significant lower bone mineral density and quality compared to sow-fed (positive con

Pathophysiology of bone loss in patients receiving anticonvulsant therapy.

Many studies have shown that patients taking antiepileptic drugs (AEDs) are at increased risk for metabolic bone disease and low bone mineral density. Although early reports of bone disease in patients with epilepsy often involved institutionalized patients, who may be at risk because of lack of physical activity, reduced sunlight exposure, and poor nutrition, low bone density has also been reported in well-nourished, ambulatory outpatients with epilepsy. Traditionally, attention to the problem of AED-induced bone loss has been focused on those drugs that induce the hepatic cytochrome P450 enz

New observations on bone quality in mild primary hyperparathyroidism as determined by quantitative backscattered electron imaging.

UNLABELLED: Bone mineralization density distribution, an important aspect of bone material quality, was determined in mild primary hyperparathyroidism using quantitative backscattered electron imaging. A strong correlation between bone turnover status and degree and heterogeneity of mineralization was found. Further studies are needed before we can draw conclusions about fracture risk in this disorder. INTRODUCTION: Mild primary hyperparathyroidism (PHPT) is best characterized by asymptomatic hypercalcemia, most commonly in the absence of classical signs and symptoms. Hence, there is need to

Negative impact of Crohn's disease on bone mineral mass.

Prolonged chronic inflammation and corticosteroid therapy increase the risk of osteoporosis in patients with Crohn's disease. It has been estimated that 30% of these patients, who take steroids for prolonged periods, will suffer a vertebral fracture. Patients with Crohn's disease are difficult to wean from corticosteroids and therefore are at risk of developing bone complications. The purpose of this cross-sectional study was to examine the relationship between cumulative steroid dose, duration of the disease and the development of osteopenia in patients with Crohn's disease. We studied 28 pat

[Update 2017 of the KDIGO guidelines on Chronic Kidney Disease-Mineral and Bone Disorder (CKD-MBD). What are the real changes?].

Guidelines for the assessment, diagnosis and therapy of the alterations that characterize the CKD-MBD are an important support in the clinical practice of the nephrologist. Compared to the KDIGO guidelines published in 2009, the 2017 update made changes on some topics on which there was previously no strong evidence both in terms of diagnosis and therapy. The recommendations include the diagnosis of bone anomalies in CKD-MBD and the treatment of mineral metabolism abnormalities with particular regard to hyperphosphataemia, calcium levels, secondary hyperparathyroidism and anti-resorptive thera

Effect of growth hormone therapy and puberty on bone and body composition in children with idiopathic short stature and growth hormone deficiency.

The state of bone health and the effect of growth hormone (GH) therapy on bone and body composition in children with idiopathic short stature (ISS) are largely unknown. A direct role of GH deficiency (GHD) on bone density is controversial. Using dual-energy X-ray absorptiometry, this study measured total body bone mineral content (TB BMC), body composition, and volumetric bone mineral density (vBMD) at the lumbar spine (LS) and femoral neck (FN) in 77 children (aged 3-17 years) with ISS (n = 57) and GHD (n = 20). Multiparametric assessment of bone health in metastatic hormone-sensitive prostate cancer patients receiving androgen deprivation + enzalutamide ± zoledronic acid (BonEnza study).

Androgen deprivation therapy has a negative effect on bone mineral density and trabecular bone score in prostate cancer patients. The addition of androgen receptor pathway inhibitors can result in worsened skeletal fragility. BonEnza is a prospective phase II trial in which metastatic hormone sensitive prostate cancer patients were randomized to receive androgen deprivation therapy plus enzalutamide with (EZ arm) or without (E arm) the addition of zoledronic acid. Bone quantity and quality parameters were evaluated by dual-energy x-ray absorptiometry (DXA) scan at baseline and after 18 months

Circulating Proneurotensin Levels Predict Impaired Bone Mineralisation in Postmenopausal Women With Type 2 Diabetes Mellitus.

CONTEXT: The mechanisms underlying bone fragility and increased fracture risk observed in individuals with type 2 diabetes (T2D) are not yet fully elucidated. Previous research has suggested a role for neuropeptides in regulating bone metabolism; however, the contribution of the neuropeptide Neurotensin (NT), which is thoroughly implicated in T2D and cardiovascular disease, has not been investigated in this context. OBJECTIVE: To study the relationship between circulating levels of the NT precursor proneurotensin (proNT) and bone mineralisation in T2D women. MATERIALS AND METHODS: This is a
