Aneurysmal bone cyst of the larynx.

Aneurysmal bone cyst is a rare lesion of the head and neck, and the mandible and maxilla are the most common sites of occurrence. To the authors' knowledge, two cases of aneurysmal bone cyst of the larynx described here are the first to be reported. Although the exact pathogenesis of this lesion is unknown, several theories of its origin are discussed. Complete surgical excision is the treatment of choice. Radiation therapy for this benign lesion is not indicated because of a possible association with the development of sarcoma. Histologic misdiagnosis of carcinoma or sarcoma of the larynx lea

Malignant histiocytosis and encephalomyeloradiculopathy complicating coeliac disease.

A 62 year old Irish woman with an eight year history of probable coeliac disease developed brain stem signs, unilateral facial numbness and weakness, wasting and anaesthesia in both lower limbs. Over the next two years, a progressive deterioration in neurological function and in intestinal absorption, and the development of anaemia led to a suspicion of malignancy. Bone marrow biopsy revealed malignant histiocytosis. Treatment with cytotoxic drugs led to a transient, marked improvement in intestinal structure and function, and in power of the lower limbs. Relapse was associated with bone marro

[Nieman-Pick disease revealed by a pulmonary miliary tuberculosis (author's transl)].

A diagnosis of Niemann-Pick disease was made in a 26 years old man with chronic pulmonary miliary tuberculosis and splenomegaly. It was confirmed by the drop of sphingomyelinase level in leucocytes and fibroblasts of the skin. The authors showed the presence of foamy cells with sphingomyelin in the spleen and bone marrow. They underlined the value of dosing sphingomyelinase in leucocytes for diagnosis purposes. They also observed blue histocytes in the spleen and bone marrow, next to the foamy cells which are not specific of Niemann-Pick disease and can be found in numerous other affections. T

Anorexia Nervosa and Osteoporosis: A Possible Complication to Remember.

Anorexia nervosa (AN) belongs to the spectrum of food disorders and affects approximately 2.9 million people worldwide. It is responsible for numerous and serious medical complications. Osteoporosis is a common complication, and the decrease in bone mineral density (BMD) is one of the few potentially irreversible consequences of AN. When associated with AN, it can manifest at a very young age, possibly leading to irreparable damage. We describe the case of a 30-year-old woman with a one-year evolution diagnosis of AN, complaining of back pain. Physical examination revealed a slight elevation o

Role of Infiltrating Monocytes in the Development of Radiation-Induced Pulmonary Fibrosis.

Lung exposure to radiation induces an injury response that includes the release of cytokines and chemotactic mediators; these signals recruit immune cells to execute inflammatory and wound-healing processes. However, radiation alters the pulmonary microenvironment, dysregulating the immune responses and preventing a return to homeostasis. Importantly, dysregulation is observed as a chronic inflammation, which can progress into pneumonitis and promote pulmonary fibrosis; inflammatory monocytes, which are bone marrow derived and express CCR2, have been shown to migrate into the lung after radiat

Malignant fibrous histiocytoma arising in the area of total hip replacement.

A patient developed a rapidly progressive and extensive periprosthetic osteolysis after a cemented total hip arthroplasty for postradiation necrosis of pelvic bone and femoral head. Malignant tumor is one of the causes of periprosthetic bone loss. The biopsy confirmed the malignant fibrous histiocytoma (MFH). However, majority of periprosthetic bone loss is due to wear debris induced osteolysis. Usually, wear debris induced periprosthetic osteolysis is developed later and the progression is much slower than there of malignant tumor. Also wear debris induced osteolysis is confirmed by chronic i

Biomechanical Analysis and Design Method for Patient-Specific Reconstructive Implants for Large Bone Defects of the Distal Lateral Femur.

This study aims to develop a generalizable method for designing a patient-specific reconstructive scaffold implant for a large distal lateral femur defect using finite element (FE) analysis and topology optimization. A 3D solid-core implant for the distal femur defect was designed to withhold the femur load. Data from FE analysis of the solid implant were use for topology optimization to obtain a 'bone scaffold implant' with light-weight internal cavity and surface lattice features to allow for filling with bone material. The bone scaffold implant weighed 69.6% less than the original solid-cor

Pathogenesis of Osteoporosis.

Osteoporosis is a condition where bone resorption exceeds bone formation leading to degeneration. With an aging population, the prevalence of osteoporosis is on the rise. Although advances in the field have made progress in targeting the mechanisms of the disease, the efficacy of current treatments remains limited and is complicated by unexpected side effects. Therefore, to overcome this treatment gap, new approaches are needed to identify and elucidate the cellular mechanisms mediating the pathogenesis of osteoporosis, which requires a strong understanding of bone biology. This chapter will f

Geriatric Type II Fractures of the Dens Axis: Biomechanical Superiority of a New Osteosynthesis Plate Compared to Common Anterior Screw Fixation.

Study DesignBiomechanical Basic Research.ObjectivesType II Fractures of the dens axis (DFTII) are common fractures of the cervical spine in geriatric patients. A surgical approach is the anterior screw fixation (ASF). However, a disadvantage is the high post-operative rate of non-union. This study aimed to develop and biomechanically test an osteosynthesis plate (OSP) for the anterior treatment of DFTII in comparison to ASF.MethodsA novel OSP was developed and manufactured. The OSP was biomechanically tested in comparison to ASF in synthetic bone models and geriatric human cadaveric bone. An a

Ewing tumor family consists of Ewing tumor of bone, extraosseous Ewing tumor, primitive neurectodermal tumor and Askin tumor. All of them share genetic abnormality, reciprocal translocation (11; 22) (q24; q12), and originate from the same primordial stem cell. Ewing tumor is the most common form, found in 60% of cases. It is the second primary malignant bone tumor. Localized lesion is found in nearly 80% and metastatic disease in 20% of cases. Patients present mostly due to pain and palpable tumefaction, and pathological fracture as the initial problem develops in long bones. Ewing tumor can d

[Healing of autologous cancellous bone transplants and hydroxylapatite ceramics in tibial segment defects. Value of ultrasonic follow up].

The aim of this study was to develop and test a scoring system based on real-time ultrasonography (US), which is able to predict the healing of a bone defect filled with a bone graft substitute or cancellous bone graft. We implanted porous hydroxy-apatite (HA) ceramic blocks into a segmental defect in the tibia of 51 sheep; 14 sheep received autologous bone graft from the iliac crest. Follow-up times were 3, 6 and 12 months. With the exception of the 12-month animals, there was a minimum of 6 animals in each group. At the end of follow-up the tibiae were tested in torsion to failure. These res

An Integrated Approach Reveals the Pre-Osteoblast-Driven Metrnl Synergizes With Circadian Genes to Inhibit Osteogenesis.

BACKGROUND: Current osteoporosis (OP) therapies predominantly suppress osteoclastic bone resorption, highlighting a critical unmet need for anabolic strategies that directly stimulate bone formation. Meteorin-like (Metrnl) is a recently identified adipokine whose role in bone metabolism remains poorly defined and controversial. This study systematically investigated the expression profile, physiological function, and molecular mechanism of Metrnl in skeletal biology. METHODS: An integrated approach combining clinical bone marrow specimen analysis, proteomics, single-cell RNA sequencing, and c

Bone marrow adipose tissue expansion and bone loss in experimental chronic kidney disease is independent of altered bone marrow stromal cell lineage determination.

INTRODUCTION: Chronic kidney disease-mineral bone disorder is the irreversible loss of kidney function leading to altered mineral homeostasis and bone loss, commonly referred to as renal osteodystrophy. Bone marrow adipose tissue (BMAT) accumulates in clinical CKD and animal models of this disease, but the mechanism(s) responsible are unclear. This study sought to determine the relationship between BMAT distribution and bone structure and to establish whether disease progression directly affected: 1) the commitment of bone marrow mesenchymal stromal cells (BMSCs) to osteoblastic (OPC) and adip

The Concept of Scaffold-Guided Bone Regeneration for the Treatment of Long Bone Defects: Current Clinical Application and Future Perspective.

The treatment of bone defects remains a challenging clinical problem with high reintervention rates, morbidity, and resulting significant healthcare costs. Surgical techniques are constantly evolving, but outcomes can be influenced by several parameters, including the patient's age, comorbidities, systemic disorders, the anatomical location of the defect, and the surgeon's preference and experience. The most used therapeutic modalities for the regeneration of long bone defects include distraction osteogenesis (bone transport), free vascularized fibular grafts, the Masquelet technique, allograf

Convergence of scaffold-guided bone regeneration and RIA bone grafting for the treatment of a critical-sized bone defect of the femoral shaft.

BACKGROUND: Critical-sized bone defects, mainly from trauma, infection or tumor resection are a challenging condition, often resulting in prolonged, complicated course of treatment. Autografts are considered as the gold standard to replace lost bone. However, limited amount of bone graft volume and donor-site morbidity have established the need for the development of alternative methods such as scaffold-based tissue engineering (TE). The emerging market of additive manufacturing (3D-printing) has markedly influenced the manufacturing of scaffolds out of a variety of biodegradable materials. Pa

Suboptimal Level of Bone-Forming Cells in Advanced Cirrhosis are Associated with Hepatic Osteodystrophy.

Bone loss is common in advanced cirrhosis, although the precise mechanisms underlying bone loss in cirrhosis are unknown. We studied the profile and functionality of bone-forming cells and bone-building proteins in bone marrow (BM) of individuals with cirrhosis (n = 61) and individuals without cirrhosis as normal controls (n = 50). We also performed dual energy X-ray absorptiometry for clinical correlation. BM mesenchymal cells (MSCs) were analyzed for colony-forming units-fibroblasts and their osteogenic (fibronectin-1 [FN1], insulin-like growth factor binding protein 3 [IGFBP3], collagen typ

Prevention and treatment of bone fragility in cancer patient.

It is well known that fractures increase the risk of morbidity and mortality. The various mechanisms responsible for bone loss in cancer patients may have a different impact depending on the characteristics of the clinical case and correlates with the therapies used, or caused by the therapies used against cancer. Some hormonal treatments cause hypogonadism, event which contributes to the progressive loss of bone mass. This is detectable in patients with breast cancer receiving determines that estrogen-deprivation and in men with prostate cancer with therapies that determine androgen deprivati

Potential of (99m)Tc-MIBI for detecting bone marrow metastases.

UNLABELLED: In this study, we evaluated the potential of (99m)Tc-hexakis-2-methoxyisobutylisonitrile (MIBI) for detecting bone metastases in comparison with a conventional bone tracer. METHODS: (99m)Tc-MIBI and (99m)Tc-hydroxymethylene diphosphonate (HMDP) scans were obtained from 99 patients with proven malignant diseases and suspected bone metastases. We compared 373 lesions that showed abnormal uptake on (99m)Tc-MIBI scans or (99m)Tc-HMDP scans (or both). RESULTS: Bone metastases were confirmed in 334 of 373 lesions. Thirty-nine lesions on (99m)Tc-HMDP scans had false-positive findings, b
