Three-Year Outcomes in Recipients of Mismatched Unrelated Bone Marrow Donor Transplants Using Post-Transplantation Cyclophosphamide: Follow-Up from a National Marrow Donor Program-Sponsored Prospective Clinical Trial.

The use of post-transplantation cyclophosphamide (PTCy) as graft-versus-host disease (GVHD) prophylaxis has resulted in reductions in GVHD and improved outcomes in allogeneic hematopoietic cell transplantation (HCT) using HLA-mismatched related donors. We report the 3-year outcomes of the first multicenter prospective clinical trial using PTCy in the setting of mismatched unrelated donor (MMUD) bone marrow HCT. The study enrolled 80 patients, treated with either myeloablative conditioning (MAC; n = 40) or reduced-intensity conditioning (RIC; n = 40), with the primary endpoint of 1-year overall

Serial Cross-Sectional Mapping of Bone Marrow Microenvironment Remodelling During Fracture Repair Using Multiparametric MRI and Histopathology.

Restoring bone marrow microenvironment (BMM) homeostasis is critical for fracture healing, yet non-invasively tracking physiological events like apoptosis, angiogenesis and matrix remodelling remains challenging. This study evaluated multiparametric 7.0T MRI (T2 mapping and proton density-weighted imaging [PDWI]) for in vivo tracking of BMM spatiotemporal remodelling during natural and pharmacologically accelerated bone repair. Utilizing a rat tibial drill-hole injury model (N = 36), we compared a natural healing control with a Huoxue Qiangji Decoction (HQD) intervention group. Serial high-res

Relapse in childhood B-cell precursor acute lymphoblastic leukemia: insights from transcriptomic signatures of diagnostic bone marrow.

Survival rates exceeding 85% are now achieved for most children diagnosed with B-cell precursor acute lymphoblastic leukemia (BCP-ALL), the most common subtype of childhood ALL. Advances in the understanding of disease biology, the identification of robust prognostic factors, and the implementation of risk-adapted treatment protocols have enabled increasingly personalized therapeutic approaches. The integration of RNA sequencing into routine diagnostics has further refined subtype classification and facilitated the detection of cryptic rearrangements and pathogenic variants. Despite these impr

Extracellular Vesicles from Bone Marrow Mesenchymal Stem Cells Modulate Proliferation, Migration, and Chemosensitivity in Ovarian Cancer Cells.

Ovarian cancer is the most lethal gynecologic malignancy, with chemoresistance and recurrence driven by cancer stem cells (CSCs). Mesenchymal stem cell-derived extracellular vesicles (MSC-EVs) mediate tumor-stroma communication, but their role in ovarian cancer progression and therapy remains unclear. Here, we investigated bone marrow (BM)-MSC-EVs, their effects on ovarian cancer cells, and the underlying molecular mechanisms. BM-MSCs were isolated, confirmed using flow cytometry and trilineage differentiation, and their EVs characterized using nanoparticle tracking analysis, transmission elec

Performance evaluation of a CD34 expression assay by digital PCR in bone marrow harvest from healthy donors.

BACKGROUND: The aim of this study was to investigate a CD34 expression assay as an additional tool for the total nucleated cell (TNC) count assessment in bone marrow harvest (BMH) obtained from healthy donors. STUDY DESIGN AND METHODS: Samples obtained from bone marrow donors (n = 71) were analyzed for CD34 expression using a digital PCR assay. The assay was extensively validated and the obtained data was correlated with standard BMH assessment methods such as TNC counts performed using a hematology analyzer and CD34 cell count measured with flow cytometry. RESULTS: The digital PCR CD34 assa

Healing of Perforating Inflammatory Root Resorption by Allogeneic Bone Marrow Mesenchymal Stromal Cells Transplantation: A Case Report.

INTRODUCTION: Perforating inflammatory root resorption (PIRR) in permanent teeth can cause extensive dentin loss, periodontal breakdown, and eventual tooth loss. Regenerative endodontic procedures (REPs) have been proposed to treat PIRR. Among them, cellular therapy based on transplantation of mesenchymal stromal cells (MSCs) has been suggested as a potential option for treating PIRR, because of their paracrine and anti-inflammatory effects during tissue repair. METHODS: A 13-year-old girl presented with a traumatized tooth #9 showing discoloration, a buccal sinus tract, 8-mm probing depth, t

Serous atrophy of bone marrow manifested as subcutaneous and visceral fat depletion on CT: two case reports.

Serous atrophy of the bone marrow (SABM) is primarily caused by the breakdown of bone marrow fat due to the depletion of body fat reserves. SABM is linked to poor nutritional status or cachexia in advanced cancer stages and exhibits distinct characteristics on magnetic resonance imaging (MRI). However, these adverse conditions create challenges for undergoing MRI, thereby delaying timely diagnosis. This case report details changes in body fat tissue observed through CT scans during the progression of two SABM cases. Two patients were elderly males, and both had undergone chemotherapy and immun

Ningxue shengban decoction regulates T-cell immune balance in immune thrombocytopenia via the bone marrow hematopoietic microenvironment.

BACKGROUND: Immune thrombocytopenia (ITP) is characterized by increased platelet clearance and decreased platelet production. Hematopoietic stem and progenitor cell (HSPCs) abnormalities are a key mechanism of ITP, contributing to megakaryocyte defects and aberrant lymphocyte differentiation. Ningxue Shengban Decoction (NXSBD) has proven to be an effective therapeutic option for ITP. AIM: This study aimed to explore the therapeutic mechanism of NXSBD in ITP. METHOD: First, we evaluated the therapeutic effect of NXSBD on thrombocytopenia. T-lymphocyte subsets were analyzed by flow cytometry,

Neurofibromatosis type I -related severe aplastic anemia: an unusual association with a complicated bone marrow transplantation course.

Severe aplastic anemia (SAA) is a life-threatening condition characterized by bone marrow failure, requiring immunosuppressive therapy or hematopoietic stem cell transplantation (HSCT). Neurofibromatosis type I (NF1) is an autosomal dominant disorder with diverse systemic manifestations. Herein, we report a pediatric patient with unusual coexisting SAA and NF1 who underwent HSCT, facing multiple infectious and hematologic complications. This case highlights the challenges of managing immunosuppression, infections, and engraftment complications in such a rare clinical scenario.

Preoperative Knee Bone Marrow Edema Is Associated With Poor Clinical Outcomes Following Posterior Medial Meniscal Root Repair.

PURPOSE: To determine if preoperative femoral or tibial bone marrow edema (BME) is indicative of higher rates of re-tear and conversion to total knee arthroplasty (TKA) after posterior medial meniscal root tear repair, and, secondarily, whether preoperative BME impacted patient-reported outcome measures and rates of infection and arthrofibrosis. METHODS: Patients that underwent posterior medial meniscal root tear repair between 2013 and 2021 with a 2-year minimum follow-up were retrospectively analyzed. Primary outcome measures included re-tears and TKA conversion. Secondary outcomes included

Bone marrow failure in telomere biology disorders: Current understanding and the emerging landscape of non-transplant therapies.

Telomere biology disorders (TBDs) are a heterogeneous group of inherited systemic diseases caused by pathogenic variants in genes encoding telomerase or proteins involved in telomere maintenance or protection. With very short and/or dysfunctional telomeres, patients with TBD have clinical manifestations clustered in tissues with high rates of cell proliferation. Bone marrow failure (BMF) is the most common hematologic complication of a TBD and can precede the development of more difficult-to-treat malignant transformation. However, not all patients with TBD develop BMF, and some genotype/pheno

How I utilize somatic alterations in the diagnosis, risk stratification, and therapy of hypocellular bone marrow failure.

Hypocellular bone marrow failure (BMF) may be acquired due to immune-mediated disease, the prototype being immune aplastic anemia (IAA), or inherited, due to germ line defects in genes important for hematopoietic stem cell function and maintenance (inherited BMF syndromes [IBMFS]). Proper diagnosis of the underlying etiology of hypocellular BMF, particularly distinguishing IAA, myelodysplastic syndrome (MDS; most relevant in this setting is hypoplastic MDS), and the IBMFS, is important given the differences in clinical management. Clonal hematopoiesis (CH), in this context comprising somatic m

Bone marrow mesenchymal stem cell-derived exosomes ameliorate intervertebral disc degeneration by regulating HMGB1 acetylation via SIRT6.

BACKGROUND: Intervertebral disc degeneration (IVDD) is a leading cause of low back and leg pain and is primarily driven by inflammation-induced apoptosis of nucleus pulposus (NP) cells. Bone marrow mesenchymal stem cell-derived exosomes (BMSC-exos) have shown strong therapeutic potential for degenerative diseases. Sirtuin 6 (SIRT6), a key regulator of aging and inflammation, has been closely associated with IVDD progression. Therefore, we hypothesized that BMSC-exos might alleviate IVDD by promoting SIRT6-mediated deacetylation of high mobility group box 1 (HMGB1), thereby suppressing inflamma

Bone marrow mesenchymal stem cells-derived exosomes repair degenerative intervertebral discs by modulating macrophage M1/M2 phenotype.

The effect of bone marrow mesenchymal stem cell-derived exosomes (BMSC-Exos) on intervertebral disc degeneration (IVDD) repair and healing and its possible mechanism were investigated. Tail IVDD puncture was performed on mice, and BMSC-Exos were injected into the joint cavity. The disc morphology was observed by HE staining, apoptosis rate of nucleus pulposus (NP) tissues was determined by TUNEL, and polarization of macrophages and NP cell damage were detected by Western Blot. An in vitro experimental model was established by co-culture of nucleus pulposus (NP) cells and M1 macrophages in a co
