Patterns and correlates of cervical cancer screening initiation in a large integrated health care system.

BACKGROUND: The latest 2012 US Preventive Services Task Force cervical cancer screening guidelines recommended screening initiation at age 21 years. Little is known about the cervical cancer screening initiation practices in the community and whether there are critical gaps with respect to adherence to current clinical guidelines. Despite an overall decline in cervical cancer incidence across women of all ages, the incidence rate has not declined among 24-25 year olds between 2000 (2.79 per 100,000) and 2013 (2.93 per 100,000). Thus, it is important to understand cervical cancer screening init

Prevalence and Reactivation of Hepatitis B and C in Patients Undergoing Chemotherapy for Solid Tumors.

PURPOSE: Hepatitis B virus (HBV) and hepatitis C virus (HCV) infections are clinically significant in patients with cancer, as chemotherapy can trigger viral reactivation, leading to liver failure, treatment interruption, or death. Despite this risk, routine viral screening in patients with solid tumors remains inconsistent across institutions and guidelines. The purpose is to determine the frequency of viral screening and the prevalence of HBV, HCV, and HIV infections-and to assess the rate of viral reactivation-in a large cohort of newly diagnosed adult patients with cancer receiving chemoth

Perianal lesions in men who have sex with men living with HIV: Risk of precancer and cancer.

OBJECTIVES: Men who have sex with men living with HIV (MSMLWH) are at highest risk for human papillomavirus (HPV)-associated anal cancer, which may originate from the anal canal, verge or perianal skin. Perianal lesions are frequently overlooked during examination, and their (pre)malignant burden in this population remains poorly characterized. METHODS: A total of 308 MSMLWH who underwent high-resolution anoscopy (HRA)-guided perianal biopsy between 2018 and 2024 were analysed. Demographics, clinical HIV parameters, smoking history, HPV vaccination status, anal cytology, high-risk HPV test re

Liver cancer and hepatic decompensation events in patients hospitalized with viral hepatitis in Spain.

BACKGROUND: Chronic viral hepatitis B, C, and D are the main causes of decompensated cirrhosis and liver cancer worldwide. Newborn HBV vaccination was implemented more than 2 decades ago in most EU countries. Furthermore, potent oral antivirals have been available to treat HBV for 15 years and to cure HCV since 2014. The real-life clinical benefits of these interventions at country level have not been assessed, especially regarding major hepatic outcomes such as cirrhotic decompensation events and hepatocellular carcinoma (HCC). METHODS: Retrospective study of all hospitalizations in Spain ha

Serum Immune Profiling for Early Detection of Cervical Disease.

BACKGROUND: The most recent (2012) worldwide estimates from International Agency for Research on Cancer indicate that approximately 528,000 new cases and 270,000 deaths per year are attributed to cervical cancer worldwide. The disease is preventable with HPV vaccination and with early detection and treatment of pre-invasive cervical intraepithelial neoplasia, CIN. Antibodies (Abs) to HPV proteins are under investigation as potential biomarkers for early detection. METHODS: To detect circulating HPV-specific IgG Abs, we developed programmable protein arrays (NAPPA) that display the proteomes o

Knowledge and occupational hazards of barbers in the transmission of hepatitis B and C was low in Kumasi, Ghana.

INTRODUCTION: Blood borne viral hepatitis transmission still ranges between 4-20% in many Ghanaian communities. Hepatocellular carcinoma (HCC) also called liver cancer is reported as the leading cause of cancer mortality among males in Ghana. We studied the knowledge and risk factors associated with barbers' occupation in the transmission of hepatitis B virus (HBV) and hepatitis C virus (HCV). METHODS: A randomized cross-sectional survey of 200 barbershops was conducted in Kumasi between January and August 2013. Barbershops, which operated continuously for more than 8 months, were selected fo

Time trade-off procedure for measuring health utilities loss with human papillomavirus-induced diseases: a multicenter, retrospective, observational pilot study in Italy.

BACKGROUND: The economic evaluation of any human papillomavirus (HPV) vaccination strategy requires the measurement of clinical benefits (quality-adjusted life-years [QALY]) gained to reflect both the increase in life expectancy and the economic benefits associated with an effective intervention. OBJECTIVE: The purpose of this pilot study was to investigate the feasibility of a standardized time trade-off (TTO) procedure to quantify utilities loss in health states affected by HPV-induced pathologies in Italy. METHODS: This multicenter, retrospective, observational, cross-sectional study was

HPV detection and genotyping as an earlier approach in cervical cancer screening of the female genital tract.

Human papillomavirus (HPV) infection is the leading risk factor for cervical intraepithelial neoplasia (CIN) and cervical cancer. More than 100 virus genotypes have been identified so far, some of them strongly associated with the development of neoplasia. The aim of this study was to evaluate the prevalence of the different HPV genotypes in women presenting no cytological alterations in cervical cells, in women presenting light alterations, and in women presenting severe alterations at routine gynecological examination. We retrospectively analyzed 97 HPV results of women submitted to cervical

Bioengineering strategies to enhance BCG immunotherapy against bladder cancer.

Bacillus Calmette-Guérin (BCG) remains the frontline immunotherapy for non-muscle-invasive bladder cancer (NMIBC), yet recurrence and non-response occur in over half of patients, highlighting the need for improved therapeutic strategies. Recent advances in bioengineering including genetic modification, synthetic biology, biomaterials, and delivery systems are expanding BCG from a traditional vaccine into a more versatile immunotherapeutic platform. In this review, we summarize the mechanisms of BCG-induced antitumor immunity and discuss current bioengineering strategies aimed at enhancing tumo

Nonoxidative pentose phosphate pathway regulates CD8+ T cell immunity by maintaining NADPH homeostasis.

NADPH is essential for cellular biosynthesis and redox balance in CD8+ T cells. Here, we demonstrate that the nonoxidative pentose phosphate pathway (non-oxPPP), mediated by transketolase (TKT) and transaldolase (TALDO1), is critical for CD8+ T cell activation, proliferation, and memory formation by maintaining NADPH homeostasis. Metabolomic profiling and isotopic tracing revealed upregulated non-oxPPP flux in effector (Teff) and memory (Tm) CD8+ T cells, enabling a pentose cycle that amplifies NADPH yield and sustains metabolic fitness for T cell immunity. Genetic knockdown or pharmacological

Gene Editing of a Carcinogenic Liver Fluke Tetraspanin Impairs Parasite Surface Biogenesis and Extracellular Vesicle Uptake by Human Host Cells.

Opisthorchiasis remains a significant public health concern throughout Southeast Asia. The liver fluke Opisthorchis viverrini resides within the biliary tract, and chronic infection leads to bile duct cancer, or cholangiocarcinoma. Here, we examined the functions of liver fluke tetraspanins, 4-transmembrane domain proteins expressed on the surface of the fluke tegument, and extracellular vesicles (EVs) derived from this syncytial surface. We undertook CRISPR-Cas9 gene knockout (KO) of the O viverrini tetraspanin 2 (Ov-tsp-2) gene and found that Ov-tsp-2-KO flukes had abnormal tegument biogenes

Acceptance and the willingness to pay for human papilloma virus (HPV) vaccine: A systematic review.

Human papillomavirus (HPV) is a major global health concern due to its link to cervical and other cancers. Although HPV vaccination is highly effective, acceptance and willingness to pay (WTP) differ widely across populations. This review summarizes global evidence from 2015-2025. A systematic search of PubMed, Scopus, CENTRAL, Web of Science, and Google Scholar was conducted in 2025 following PRISMA guidelines. Studies reporting data on knowledge, acceptance, attitudes, and WTP across any population were included. Quality assessment used ISPOR checklists, and data were synthesized in Excel 20

Cofactors in Human Papillomavirus Carcinogenesis.

Human papillomavirus (HPV) infection is the necessary cause of cervical cancer (CC) and other HPV-related malignancies, yet by itself is not sufficient for malignant progression. A myriad of co-factors influences the risk that an HPV infection persists and progress to precancerous lesions and invasive disease. Understanding these cofactors is crucial for risk stratification and informing comprehensive preventive strategies-complementing HPV vaccination and screening-to further reduce the incidence of cervical and other HPV-associated cancers. This chapter reviews the epidemiological and mechani

Enfortumab vedotin induces immunogenic cell death and shows enhanced preclinical antitumor activity when combined with a PD-1 inhibitor.

Enfortumab vedotin is a Nectin-4-directed antibody-drug conjugate designed to deliver the microtubule-disrupting agent monomethyl auristatin E (MMAE) to tumor cells. Using preclinical models of urothelial cancer (UC), we expand the understanding of the multifaceted mechanism of action for enfortumab vedotin that includes direct cytotoxicity on Nectin-4-positive tumor cells, indirect bystander effect on neighboring Nectin-4-negative tumor cells, and MMAE-mediated induction of immunogenic cell death (ICD) and associated increase in activated immune cells in the tumor microenvironment. Viral warriors: Unlocking the immune system's potential with oncolytic viruses in cancer immunotherapy.

Oncolytic viruses (OVs) represent an exciting avenue in tumor immunotherapy, harnessing the power of viruses to overcome resistance and enhance immune responses. OVs achieve this by selectively targeting and destroying tumor cells. This cell lysis releases tumor-specific antigens (TAAs), effectively creating an in situ vaccine that educates the immune system. Moreover, OVs directly trigger both innate and adaptive immunity and can be genetically engineered to secrete immune-boosting factors. Early studies have shown promising synergistic antitumor effects when OVs are combined with other cance

Endotoxin's Impact on Organism: From Immune Activation to Tolerance and Beyond.

Endotoxin, a key component of Gram-negative bacterial membranes, remains a central focus in understanding host-pathogen interactions and immune modulation. In this review, we examine the multifaceted roles of endotoxin, with particular emphasis on how its structural variants modulate host immune recognition and inflammatory signalling. We explore the complex mechanisms by which endotoxin activates innate immune pathways and how these responses evolve under repeated or chronic exposure conditions. Special attention is given to the phenomenon of endotoxin tolerance, an adaptive reprogramming of

Infectious Mononucleosis.

Infectious mononucleosis is a clinical entity characterized by a sore throat, cervical lymph node enlargement, fatigue, and fever most often seen in adolescents and young adults. Infectious mononucleosis is most often caused by a primary Epstein-Barr virus (EBV) infection. EBV is a γ-herpesvirus that infects at least 90% of the population worldwide. The virus is spread by intimate oral contact among teenagers and young adults. How preadolescents acquire the virus is not known. A typical clinical presentation with a positive heterophile antibody test is usually sufficient to make the diagnosis,

Heat-killed Mycobacterium tuberculosis induces trained immunity in vitro and in vivo administered systemically or intranasally.

Trained immunity (TI) represents a memory-like process of innate immune cells. TI can be initiated with various compounds such as fungal β-glucan or the tuberculosis vaccine, Bacillus Calmette-Guérin. Nevertheless, considering the clinical applications of harnessing TI against infections and cancer, there is a growing need for new, simple, and easy-to-use TI inducers. Here, we demonstrate that heat-killed Mycobacterium tuberculosis (HKMtb) induces TI both in vitro and in vivo. In human monocytes, this effect represents a truly trained process, as HKMtb confers boosted inflammatory responses ag

Learning-based sensitivity analysis and feedback design for drug delivery of mixed therapy of cancer in the presence of high model uncertainties.

In this paper, a methodology is proposed that enables to analyze the sensitivity of the outcome of a therapy to unavoidable high dispersion of the patient specific parameters on one hand and to the choice of the parameters that define the drug delivery feedback strategy on the other hand. More precisely, a method is given that enables to extract and rank the most influent parameters that determine the probability of success/failure of a given feedback therapy for a given set of initial conditions over a cloud of realizations of uncertainties.
