Late-onset esophagogastric anastomotic fistula managed without continuous fasting: a case report.

Esophagogastric anastomotic fistula is a common complication following esophageal cancer surgery, typically occurring within the first postoperative week. Conventional management requires prolonged fasting until complete fistula closure, which significantly impacts patient quality of life. We present a case of a male who developed an esophagogastric anastomotic fistula 2 months postoperatively, complicated by a concurrent gastrobronchial fistula. Endoscopic evaluation revealed persistent gastric wall defects at the fistula site. As the disease progressed, thickened visceral pleura formed dense

[Contribution Of Ultrasound In The Management Of Hypertrophic Pylore Stenoses At Bouake CHU: About 4 Cases].

The overall incidence of hypertrophic pyloric stenosis is between 2 and 3 per 1000 live births. Classically, newborns only develop this pathology between 3 weeks and 3 months of life, with a predominance of males at a ratio of 5:1. The diagnosis is clinically evoked by a characteristic pattern of vomiting and progressive malnutrition. Medical imaging, particularly abdominal ultrasound, is used to make the diagnosis, and treatment is surgical.

[Long‒Term Survival in a Case of Unresectable Sigmoid Colon Cancer Managed with Multidisciplinary Treatment].

We report a 58‒year‒old woman with Stage Ⅳb sigmoid colon cancer, presenting with hematochezia and lower abdominal pain, and diagnosed with over 15 liver metastases and para‒aortic lymph node (PALN) metastases. After laparoscopic sigmoid colectomy, FOLFOXIRI plus bevacizumab achieved marked liver tumor reduction and complete PALN response, enabling hepatic resection. Multiple recurrences were managed with repeated liver resections, PALN dissection, and chemoradiotherapy. Following fifth‒line of chemotherapy, oxaliplatin was reintroduced, and fruquintinib initiated, leading to 6 years and 3 mon

Clinical experience in the management of penetrating stab wounds to the neck: A retrospective analysis of 5 cases.

PURPOSE: Penetrating neck trauma can frequently be life-threatening, including those that appear minor. Consequently, thorough examination and quick operative treatment have been the standard measure. In addition, modern precision medicine has shifted the procedure towards more selective non-operative management, whereas advanced imaging like computed tomography angiography plays a key role. The diagnostic and therapeutic protocol remains to be adapted and optimized. The purpose of this study was to present our experience, outcomes, and management strategy, and contribute to establishing an ev

Management and Long-Term Monitoring of a Young Patient with Pheochromocytoma and RET Mutation: A Case Report.

BACKGROUND: Pheochromocytoma is a rare catecholamine-secreting tumor that can present with severe hypertensive episodes and other symptoms due to excessive catecholamine release. Approximately 30% of pheochromocytomas are associated with hereditary syndromes, including multiple endocrine neoplasia type 2A (MEN2A), an autosomal dominant disorder caused by mutations in the RET proto-oncogene. MEN2A is characterized by the presence of medullary thyroid carcinoma, pheochromocytoma, and primary hyperparathyroidism. CASE PRESENTATION: We report the case of a 19-year-old female who presented with ph

Case report of preservative management of a giant broad ligament fibroid: A challenge in diagnosis and surgery.

INTRODUCTION: Broad ligament fibroids are unusual types of leiomyomas. They usually present as pelvic masses with pressure-related symptoms. The purpose of this case report is to shed light on the importance of keeping these fibroids in mind when approaching pelvic and abdominal masses, despite their low prevalence. CASE PRESENTATION: Herein, we present a case of broad ligament fibroid in a 24-year-old female, who presented with gradual abdominal distension. On physical examination, an abdominal mass was palpated, extending from the hypogastric to the epigastric region of the abdomen. Ultraso

Intraoperative management of Takotsubo cardiomyopathy secondary to parathyroidectomy in the setting of myocardial bridge: a case report.

INTRODUCTION AND IMPORTANCE: Intraoperative Takotsubo cardiomyopathy (TTC) is a rare, stress-induced condition that mimics acute coronary syndrome and can arise intraoperatively, even during low-risk surgeries. Prompt recognition is essential, as TTC may lead to significant hemodynamic instability in an otherwise stable patient. CASE PRESENTATION: We report a 54-year-old female with a history of hyperparathyroidism and prior non-ischemic cardiomyopathy who presented for parathyroidectomy. Despite a negative preoperative cardiac work-up, including stress echocardiogram and transthoracic echoca

Managing Chemotherapy-Associated Cardiotoxicity: A Case of 5-Fluorouracil and Mitomycin-Induced Chest Pain.

Fluoropyrimidine-based chemotherapy drugs such as 5-fluorouracil (5-FU) have documented cardiotoxic effects from mild chest discomfort to severe ischemia. Early detection of complications is important to ensure patient safety and outcomes. We report the case of a 68-year-old woman with invasive non-keratinizing squamous cell carcinoma who developed recurrent chest pain during chemotherapy with 5-FU and mitomycin. While she had no history of coronary artery disease, she experienced mid-sternal chest pressure radiating to the neck and left arm during both cycles of chemotherapy. Initial presenta

Pediatric Rapunzel Syndrome Presenting With Jejunojejunal Intussusception Managed Surgically: A Rare Case Report.

Bezoars are masses of indigestible material within the gastrointestinal tract. Trichobezoars, composed of hair, are rare and typically occur in young females with psychiatric conditions. Rapunzel syndrome is a severe variant where the trichobezoar extends into the small intestine. We report a case of a 7-year-old girl presenting with acute abdominal pain, reduced appetite, and a palpable epigastric mass. She had a history of trichophagia and weight loss. Investigations revealed microcytic hypochromic anemia and a large intragastric mass extending into the duodenum, with associated jejunojejuna

Surgical management of tic-induced myelopathy and the importance of perioperative tic control: illustrative cases.

BACKGROUND: Degenerative cervical myelopathy (DCM) is the most common cause of adult spinal cord dysfunction. Tourette syndrome (TS) represents a rare cause of DCM that is incompletely characterized in the literature. Patients with TS might develop repetitive involuntary cervical movements that can accelerate spondylotic changes. Therefore, patients with myelopathy secondary to TS or other tic disorders might exhibit DCM symptoms at younger ages. The present study examines the surgical management of DCM secondary to TS in the literature and describes the outcomes of 3 patients who were treated

Challenges in the Diagnosis and Management of the Miller Fisher Variant of Guillain-Barré Syndrome: A Case Report.

Miller Fisher syndrome (MFS) is a rare variant of Guillain-Barré syndrome (GBS), characterized by the triad of ophthalmoplegia, ataxia, and areflexia, often following a bacterial or viral illness. This case report presents a 58-year-old Hispanic female patient with a preceding streptococcal pharyngitis who developed symptoms of bilateral facial nerve palsy and lower limb ataxia. Cerebrospinal fluid (CSF) analysis revealed albuminocytologic dissociation and, combined with her presentation of ophthalmoplegia, ataxia, and areflexia, supported the diagnosis of MFS. Magnetic resonance imaging (MRI)

Management of a First Branchial Cleft Fistula in an Adult Patient: A Case Report and Literature Review.

First branchial cleft anomalies (FBCAs) are uncommon congenital malformations that develop due to incomplete closure of the first branchial cleft during embryonic development. They represent a small proportion of all branchial cleft anomalies. These lesions, which may present as cysts, sinuses, or fistulas, pose significant diagnostic and therapeutic challenges due to their nonspecific clinical manifestations and complex anatomical relationships, particularly with the facial nerve and parotid gland. This case report describes a 47-year-old male patient presenting with recurrent infections and

Challenges in Diagnosing and Managing Ovarian Hydatidosis: A Case Report and Literature Review from Iran.

Hydatidosis is an infection caused by the parasite Echinococcus granulosus (E. granulosus). The liver and lungs are the most commonly affected organs. However, ovarian hydatid cyst (OHC) is a relatively rare manifestation. Herein, we report a case of primary OHC in a 65-year-old woman. Additionally, upon review of reported OHC cases in Iran, a total of 13 cases have been documented in various studies, with our case being the second reported from Tehran. Most cases originated from different regions of Iran, except for the southwest and southeast, with an average patient age of 38 years. The pre
