Structure and function of the superior temporal plane in adult males with cleft lip and palate: pathologic enlargement with no relationship to childhood hearing deficits.

BACKGROUND: In a previous study from our lab, adult males with non-syndromic cleft lip and/or palate (NSCLP) were shown to have significantly lower temporal lobe gray matter volume than matched controls. The current study was designed to begin a regional analysis of specific subregions of the temporal lobe. The superior temporal plane (STP) is a brain region involved in the governance of auditory processing and aspects of language. The cognitive deficit of subjects with NSCLP is characterized by specific deficits in language; therefore this region of the temporal lobe is particularly important

Interstitial deletion of a proximal 3p: a clinically recognisable syndrome.

Interstitial deletions of the proximal short arm of chromosome 3 occurring as constitutional aberrations are rare and a defined clinical phenotype is not established yet. We report on a 30-months-old girl with distinct facial features (square facies, plagiocephaly, broad forehead, broad nasal bridge, long philtrum and low set ears) and psychomotor/speech delay associated with an interstitial deletion of 3p12 chromosomal band, del(3)(p12p12). Clinical manifestations of our child were compared with those of other eight patients with the same deletion previously described to further delineate the

Large vestibular aqueduct syndrome: a case study.

A 23-month-old female was referred for hearing aid fitting after failing newborn hearing screening and being diagnosed with significant hearing loss through subsequent diagnostic testing. Auditory brainstem response (ABR) and behavioral testing revealed a moderate-to-severe bilateral mixed hearing loss. Prior to the hearing aid evaluation, tympanostomy tubes had been placed bilaterally with little or no apparent change in hearing sensitivity. Initial testing during the hearing aid fitting confirmed earlier findings, but abnormal middle ear results were observed, requiring referral for addition

Neural mechanisms of language-based learning impairments: insights from human populations and animal models.

The acquisition of speech perception and consequent expression of language represent fundamental aspects of human functioning. Yet roughly 7% to 8% of children who are otherwise healthy and of normal intelligence exhibit unexplained delays and impairments in acquiring these skills. Ongoing research has revealed several key features of language disability that may provide more direct insight into underlying anomalous neural functioning. For example, evidence supports a strong association between basic defects in processing rapidly changing acoustic information and emergent disruptions in speech

Language lateralization in patients with temporal lobe epilepsy: a comparison of functional transcranial Doppler sonography and the Wada test.

This study prospectively investigates whether noninvasive functional transcranial Doppler sonography (fTCD) is a useful tool to determine hemispheric language lateralization in the presurgical evaluation of patients with medically intractable temporal lobe epilepsy (TLE). fTCD results were compared with the Wada test as the gold standard. Wada test and fTCD were performed in 13 patients suffering from TLE. fTCD continuously measured blood flow velocities in both middle cerebral arteries, while the patient was performing a cued word generation task. During the Wada test, spontaneous speech, com

Studies of exclusion in individuals with severe mental retardation.

Exclusion performances in matching to sample are demonstrated when subjects select experimentally undefined comparison stimuli in the presence of undefined sample stimuli, apparently by rejecting defined comparison stimuli. Several studies have documented exclusion performances in a small number of individuals with severe mental retardation. These studies also demonstrated the potential of exclusion procedures for establishing prerequisites for emergent naming performances. The present study examined exclusion in a larger cohort of subjects. Initial experiments asked two questions. First, how

Parent and professional perspectives on behavioral inflexibility in autism spectrum disorders: A qualitative study.

Restricted and repetitive behaviors are a core feature of autism spectrum disorder; however, research on the functional impact of these behaviors on the quality of life for individuals with autism spectrum disorder and their families remains scarce. We conducted focus groups with parents of children with autism spectrum disorder and clinicians in order to better characterize the functional impact of behavioral inflexibility, which represents one potential dimensional construct that could account for the breadth of behaviors comprising the restricted and repetitive behavior domain. Is children's listening effort in background noise influenced by the speaker's voice quality?

The present study aims at exploring the influence of voice quality on listening effort in children performing a language comprehension test with sentences of increasing difficulty. Listening effort is explored in relation to gender ( = cisgender). The study has a between-groups design. Ninety-three mainstreamed children aged 8;2 to 9;3 with typical language development participated. The children were randomly assigned to two groups (n = 46/47) with equal allocation of boys and girls and for the analysis to four groups depending of gender and voice condition. Differences in speech and language abilities between children with 22q11.2 deletion syndrome and children with phenotypic features of 22q11.2 deletion syndrome but without microdeletion.

BACKGROUND: 22q11.2DS is the most common microdeletion syndrome in humans, usually associated with speech and language delay (SLD). Approximately 75% of children with 22q11.2 microdeletion have congenital heart malformations (CHM) which after infant open-heart surgery might lead to SLD. AIMS: The purpose of this study was to determine whether factors associated with microdeletion contribute to SLD in children with 22q11.2DS. METHODS AND PROCEDURES: We compared speech and language abilities of two groups of school-aged children: those with 22q11.2 microdeletion (E1) and those with the phenoty

An etiologic classification of autism spectrum disorders.

BACKGROUND: Autism spectrum disorders (ASD) represent a common phenotype related to multiple etiologies, such as genetic, brain injury (e.g., prematurity), environmental (e.g., viral, toxic), multiple or unknown causes. OBJECTIVES: To devise a clinical classification of children diagnosed with ASD according to etiologic workup. METHODS: Children diagnosed with ASD (n = 436) from two databases were divided into groups of symptomatic cryptogenic or idiopathic, and variables within each database and diagnostic category were compared. RESULTS: By analyzing the two separate databases, 5.4% of th

[Gonosomal trisomy syndrome. Five case reports and review of literature].

Gonosomal trisomies (GT) or so called sex chromosome trisomies (SCTs) are the most common chromosomal abnormalities in humans. The addition of extra X and/or Y chromosomes leads to neurodevelopmental differences, with increased risk for developmental delays, cognitive impairments, executive dysfunction, and behavioural and psychological disorders. Attentional problems, hyperactivity, autistic spectrum disorders and impulsivity are commonly described. Rates of language and communication problems are high in all 3 trisomies. Especially in cases of language impairment ENT specialists may be the m

[Developmental amnesia in the premature infant].

All types of memory disorders have been observed in children, although these reports are rare. Developmental amnesia selectively involves episodic daily life memory while semantic learning is respected and general intelligence is not affected. Daily life is severely disturbed by this cognitive disorder usually occurring after hypoxic ischemic injury with bilateral hippocampal atrophy on MRI. Memory disorders are underdiagnosed in at-risk patients and rarely reported. We report on a former small-for-gestational-age preterm infant with no obvious hypoxic event during perinatal life. The follow-u

Estimating the neurocognitive effects of an early intervention program for children with prenatal alcohol exposure.

BACKGROUND: Animal studies suggest that early intervention in pups exposed heavily to ethanol in utero can mitigate their neurocognitive damage. No human studies on this promising mechanism exists. METHODS: Breaking the Cycle is an early intervention program for drug-and alcohol addicted mothers and their young children. We compared BSID-III scores between infants heavily exposed to ethanol and a group exposed only to drugs of abuse, mainly cocaine. Both groups benefited from all aspects of our early intervention program. RESULTS: The two groups did not differ in any aspect of the BSID-III.

Validity of the aberrant behavior checklist in a clinical sample of toddlers.

We investigated the congruent and criterion validity of the Aberrant Behavior Checklist (ABC) in a clinical sample of toddlers seen over 1 year in Turkey. All consecutive patients (N=93), 14-43 months old (mean, 30.6 mos.), in a child psychiatry outpatient clinic were included. The ABC, Autism Behavior Checklist (AuBC), and Child Behavior Checklist 2/3 (CBCL) were completed by the children's parents. Internal consistency for ABC subscales was moderate to high. The total ABC score, which is interdependent with subscales (e.g., Irritability, Social Withdrawal) of the ABC, was significantly corre

Cyclothymic temperament as a prospective predictor of bipolarity and suicidality in children and adolescents with major depressive disorder.

INTRODUCTION: Although several recent studies suggest that bipolar disorder most commonly begins during childhood or adolescence, the illness still remains under-recognized and under-diagnosed in this age group. As part of the French Bipolar network and in line with the hypothesis that juvenile depression is pre-bipolar , we evaluated the rate of onset of bipolar disorders in a naturalistic 2-year prospective study of consecutive, clinically depressed children and adolescents, and to test whether the cyclothymic temperament underlies such onset. METHODS: Complete information was obtained from

[Neuropsychological assessment of ADHD subtypes].

Starting from a discussion of the validity of neuropsychological methods for the diagnosis of ADHD the results of an own study are presented. What is the diagnostic value of the German version of the CPT (Continuous Performance Task) and the DAT (Dortmunder Aufmerksamkeitstest) in discriminating ADHD subtypes (according to DSM-IV) and in which areas do the children of each subtype differ from a control group of normal children? The computer versions of the CPT and DAT were administered to 14 boys with the "combined subtype" of ADHD, 14 boys with the "predominantly inattentive type" of ADHD and

Autism: in search of susceptibility genes.

Autism is a neurodevelopmental disorder of unknown etiology. There is convincing data for the involvement of genetic factors in the development of autism, and the absence of any consistent evidence for an environmental, neuroanatomical, or biochemical cause has led to an increasing number of genetic studies to determine the basis of this complex disorder. The results of recent genetic linkage and candidate gene studies are reviewed in relation to the challenge of clinical and genetic heterogeneity, and prospects for the future of genetic research in autism are considered.
