[A case report: an 89-year-old man taking prednisolone for chronic inflammatory demyelinating polyradiculoneuropathy diagnosed with human herpesvirus 6 variant A encephalitis].

Human herpesvirus 6 (HHV-6) is a neurotropic virus comprising two variants, HHV-6A and HHV-6B. Although HHV-6B is known to cause exanthem subitum in children, the clinical significance of HHV-6A remains poorly understood, particularly in adult and elderly populations. Here, we report a rare case of HHV-6A encephalitis in an immunosuppressed elderly patient. An 89-year-old male patient with chronic inflammatory demyelinating polyradiculoneuropathy was administered immunosuppressive therapy with prednisolone. His consciousness progressively worsened over 4 days. Brain magnetic resonance imaging

High KIR diversity in Uganda and Botswana children living with HIV.

Killer-cell immunoglobulin-like receptors (KIRs) are critical regulators of the innate immune system and are found on the surfaces of natural killer (NK) cells. The KIR encoding genes, located on chromosome 19q13.4, are genetically diverse and associated with HIV progression. However, there is limited knowledge on the diversity of KIR from Uganda and Botswana HIV-infected paediatric cohorts. We applied next-generation sequencing technologies on 312 participants (Uganda: n = 246, Botswana: n = 66), Pushing Immunogenetics to the next-generation (PING) bioinformatics pipeline, logistic regression

Twisting the End Game: How Telomere Chromatin Modifications Shape Telomere Maintenance.

Cell division inevitably shortens telomeric DNA owing to the end-replication problem. Eukaryotic chromosomes possess specialized telomere structures to maintain genomic stability. In most proliferative cells, telomerase adds telomeric repeats during S-phase. In differentiated cells where telomerase is silenced, telomeres shorten progressively, thereby compromising genomic integrity. Consequently, cancer cells universally activate alternative telomere maintenance mechanisms during malignant transformation: ~80% reactivate telomerase, while a portion of the rest rely on BIR (break-induced replic

Dasatinib is superior to imatinib in adult Ph + ALL: a propensity score-matched analysis of pooled JALSG trial data.

This study compared dasatinib and imatinib in adult Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph + ALL). Using pooled data from three JALSG prospective trials (Ph + ALL202, Ph + ALL208, Ph + ALL213), we analyzed outcomes for 206 patients aged 15-64 years treated with dasatinib (n = 74) or imatinib (n = 132) in combination with chemotherapy. We applied propensity score matching (1:1) and inverse probability of treatment weighting to minimize selection bias and balance baseline characteristics. Dasatinib plus chemotherapy was associated with significantly higher complete mol

Upfront Immunotherapy Approaches in the Management of Adults with Acute Lymphoblastic Leukemia.

The therapeutic landscape of adults with B-cell precursor acute lymphoblastic leukemia (BCP-ALL) is undergoing a paradigm shift, driven by the development of immunotherapy-based "chemo-free" and "chemo-light' regimens. These strategies aim to achieve high efficacy with reduced toxicity, particularly in older adults who may not tolerate intensive chemotherapy. In Philadelphia chromosome-positive (Ph+) BCP-ALL, the incorporation of ABL tyrosine kinase inhibitors (TKIs) with blinatumomab (CD3/CD19 bispecific T-cell engager) has shown remarkable efficacy, with some studies reporting molecular resp

Impact of placental and peripheral blood DNA methylation on celiac disease susceptibility.

OBJECTIVES: Several studies suggest that the first immunogenic insult in celiac disease (CeD) could occur during fetal development. The placenta is a key organ that could link the environment with the genome and future outcomes, including CeD. Our objective is to determine the involvement of placental DNA methylation (DNAm) as potential mediator of the genetic susceptibility to CeD. METHODS: We used Summary-data-based Mendelian Randomization to infer what part of the susceptibility to CeD acts through DNAm in placenta or peripheral blood. We interrogated whether DNAm of the CpGs identified co

Overview of 1q abnormalities in multiple myeloma: scientific opinions from Italian experts.

Multiple myeloma (MM) is a haematological malignancy characterised by high genomic heterogeneity. One of the most common cytogenic abnormalities in MM is the gain of genetic material at the long arm (q) of chromosome 1 (+ 1q). While many mechanisms of resistance have been associated with + 1q alterations (e.g. CD38 downregulation, impairment of complement-dependent cytotoxicity, or induction of immunosuppression), the precise genetic or pathogenetic factors responsible for these alterations are still being investigated. Although interphase fluorescence in situ hybridisation (iFISH) is the gold

Characterization of RNA editing gene APOBEC3C as a candidate tumor suppressor in prostate cancer.

The human genome encodes 19 adenosine and cytidine deaminase genes, classified as A-to-I versus C-to-U editors. A-to-I editors have been widely identified as a promising therapeutic target in various cancers. Conversely, the investigation into C-to-U editors is relatively limited. This study evaluated RNA-editing genes in prostate cancer (PCa). Notably, the APOBEC3 genes are clustered in terms of their chromosomal locations, and their transcriptional changes exhibit significant positive correlations in both primary PCa and castration-resistant prostate cancer (CRPC). One member of this family,

Deep characterization of females with heterozygous Duchenne muscular dystrophy mutations.

OBJECTIVE: Duchenne muscular dystrophy (DMD) is an X-linked muscular dystrophy due to null mutations in the DMD gene that predominantly affects males, while heterozygous females are usually asymptomatic carriers. In approximately 10-20% of cases, they may present with muscle weakness and/or cardiomyopathy. We aimed to describe clinical and molecular characteristics of DMD heterozygous females. METHODS: A monocentric, observational, and cross-sectional study was designed. Clinical and molecular data were collected along with, when available, muscle biopsies. The pattern of X inactivation was d

A case report of mixed-phenotype acute leukemia with atypical BCR::ABL1 e13a3 fusion gene.

RATIONALE: Mixed-phenotype acute leukemia (MPAL) is a type of acute leukemia which is characterized by immunophenotypic features of myeloid, T-lymphoid, and/or B-lymphoid differentiation. The Philadelphia chromosome-positive (Ph+) MPAL, a rare subtype of MPAL, represents <1% of adult acute leukemia cases and typically has a poor prognosis. Here we report a very unique case of MPAL with Ph + atypical e13a3 breakpoint cluster region (BCR)::ABL1 fusion protein and provide new perspectives on the diagnosis and management of Ph + MPAL. PATIENT CONCERNS: We present a 64-year-old male who experience

Prenatal exposure to bisphenols, metals, and risk of fetal chromosome numerical abnormalities in high-risk pregnancies: Independent, combined, and interactive effects.

Prenatal exposure to bisphenols and metals has raised significant concerns regarding their potential impact on fetal development, particularly the risk of fetal chromosome numerical abnormalities (CNA). In this case-control study, we analyzed bisphenol and metal concentrations in amniotic fluid of high-risk pregnant women undergoing amniocentesis. Concentrations of bisphenols and metals were measured using ultra-performance liquid chromatography-tandem mass spectrometry and inductively coupled plasma-mass spectrometry, respectively. Logistic regression and quantile-based g-computation were app

Expectations and the Patient-Doctor Relationship: Ethical Considerations in a Case of Triploidy.

Objectives: Monitoring pregnancies is essential for community well-being. However, not all pregnancies progress normally, and some require termination. The objective was to emphasize the importance of trust in the doctor-patient relationship during this challenging time for expectant parents. Case report: During fetal morphology examination, parents were warned of a poor fetal prognosis, prompting a request for pregnancy termination. They consulted another specialist, who reassured them that the fetus appeared normal, though slightly hypotrophic. The child was born at 35 weeks gestational age

Global Challenges in Paediatric Acute Lymphoblastic Leukaemia.

<p>Background: Acute lymphoblastic leukaemia (ALL) is the commonest paediatric cancer and represents a fifth of adult leukaemias. Global outcome disparities are linked to variations in socio-demographic indices (SDIs). Summary: In high-SDI regions, established collaborative groups report cure rates surpassing 90% in paediatric ALL. The focus is on reducing treatment toxicity using chemotherapy-free strategies, principally T-cell-directed immunotherapies and targeted small molecules, as exemplified in adult Philadelphia-chromosome-positive ALL. High cure rates limit testing of novel approaches

Complete genome sequence of Heyndrickxia oleronia EISK28 isolated from human facial skin.

OBJECTIVES: Heyndrickxia oleronia is a Gram-positive, rod-shaped bacterium originally isolated from termites but has gained clinical importance due to its association with Demodex mites and rosacea, a chronic inflammatory skin disease. Despite its clinical relevance, genomic resources from human-associated environments remain limited, with existing reference genomes derived from ecologically distant sources. This study aimed to provide the first complete genome sequence of H. oleronia EISK28 isolated from human facial skin to support future research on its role in skin health and disease. DAT

Recurrent Intracranial Atherosclerotic Disease (ICAD)-Large-Vessel Occlusion (LVO) in Acute Lymphoblastic Leukemia Treated by Rescue Angioplasty: A Case Report.

Malignancy-associated stroke results from diverse cancer-related coagulopathies, and therapeutic options remain limited beyond management of the underlying malignancy. In particular, a large-vessel occlusion (LVO) that occurs despite multiple antithrombotic agents is difficult to attribute to routine atherothrombotic mechanisms and warrants consideration of occult malignancy. An 80-year-old man presented with left hemiparesis and dysarthria. Magnetic resonance imaging revealed an acute infarction in the right middle cerebral artery territory, and magnetic resonance angiography demonstrated rig
