Obstetrician call schedule and obstetric outcomes among women eligible for a trial of labor after cesarean.

BACKGROUND: Reducing cesarean deliveries is a major public health goal. The low rate of vaginal birth after cesarean has been attributed largely to a decrease in the likelihood of choosing a trial of labor after cesarean, despite evidence suggesting a majority of women with 1 prior low transverse cesarean are trial of labor after cesarean candidates. Although a number of reasons for this decrease have been explored, it remains unclear how systems issues such as physician call schedules influence delivery approach and mode in this context. OBJECTIVE: The objective of the study was to investiga

Choice of surgical approach for capitellar fractures based on pathoanatomy of fractures: outcomes of surgical management.

UNLABELLED: The understanding of the anatomic configuration of the fractures of the capitellum has enhanced in the last few years, and the classification of these fractures continues to evolve. It is essential to tailor the surgical approaches to these fractures based on the state-of-the-art classifications. Thirty-three patients with fractures of the capitellum were included in this retrospective study. The mean age of the patients was 37.9 years, and the mean follow-up period was 24.6 months. Seventeen patients had Bryan-Morrey type I fracture, three had type II fracture, two had type III fr

Current management of the mangled upper extremity.

Mangled describes an injury caused by cutting, tearing, or crushing, which leads to the limb becoming unrecognizable; in essence, there are two treatment options for mangled upper extremities, amputation and salvage reconstruction. With advances in our understanding of human physiology and basic science, and with the development of new fixation devices, modern microsurgical techniques and the possibility of different types of bony and soft tissue reconstruction, the clinical and functional outcomes are often good, and certainly preferable to those of contemporary prosthetics. Early or even imm

Altruistic discourse in the informed consent process for childhood cancer clinical trials.

Scholars have debated the role that altruistic considerations play--and should play--in recruitment and decision-making processes for clinical trials. Little empirical data are available to support their various perspectives. We analyzed 140 audiotaped pediatric informed consent sessions, of which 95 (68%) included at least one discussion of how participation in a cancer clinical trial might benefit: 1) the pursuit of scientific knowledge generally; 2) other children with cancer specifically; and 3) "the future" and other vaguely defined recipients. Clinicians initiated most (80%) of these dis

Perceived risk of deteriorating periodontal conditions.

BACKGROUND: Interpretation of risk for periodontitis is critical for treatment planning. How periodontists assess risk for periodontitis is unclear. PURPOSE: To study (1) what factors periodontists use when assessing the risks for worsening periodontal conditions anticipating that no treatment would be provided, and (2) if risk assessment is consistent and independent of specialty background training. MATERIAL AND METHODS: Medical history, clinical dental data, full-mouth intra-oral radiographs, and slide pictures were obtained from each of 51 subjects, and the information was provided to 23

Methodological questions in sentinel lymph node analysis in breast cancer patients.

BACKGROUND: The sentinel lymph node (SLN) procedure has been proposed to women with breast cancer with clinically negative axillary lymph nodes, in order to avoid conventional axillary lymph node dissection and its associated side-effects. Methodological aspects of the validation of the SLN procedure are questioned here. MATERIALS AND METHODS: The results of relevant published studies are reviewed, with emphasis on pathological techniques. The ability of the SLN procedure to diagnose lymph node metastases, the extent to which axillary lymph node dissection contributes to treatment, apart from

The role of the laboratory in the diagnosis and management of typhoid fever.

In typhoid-endemic areas the results obtained from the laboratory are important in confirming the clinical diagnosis of typhoid and may also contribute to decisions made on the management and treatment of typhoid cases. Isolation of Salmonella typhi remains the gold standard, with culture from bone marrow aspirate or a combination of specimens from other body sites resulting in the greatest sensitivity. Antibody detection techniques may still provide valuable information, but only if the results are interpreted in the context of the background antibody levels in the local population. None of t

Explainable and evidence-linked recommendations for spine surgery via a retrieval-augmented LLM agent.

BACKGROUND: Clinical decision-making in spinal and spinal cord diseases requires a comprehensive assessment of imaging findings, neurological status, bone integrity, and patient-centered goals. Recently, the emergence of Large Language Models (LLMs) has provided new tools for intelligent decision support; however, their reliability and clinical interpretability remain to be systematically evaluated. METHODS: We propose a novel retrieval augmented generation (RAG)-based framework specifically tailored for spinal disease decision support and systematically evaluated four advanced LLM agents (Ge

Use and satisfaction of the internet in obtaining information on brachial plexus birth palsies and its influence on decision-making.

The purpose of this study was to determine the influence of the Internet on parents regarding decisions for children with brachial plexus birth palsies (BPBP). A power analysis was performed to determine the appropriate sample size needed for estimating proportions in the general BPBP population. At least 100 surveys were needed for appropriate data analysis and 122 surveys were collected (100% response rate). Each participant filled out a 32-item survey on demographics, questions on Internet usage, and the influence of the Internet on care decisions. It is evident that parents commonly access

Treatment decisions in "white coat" hypertension: do we need the whole 24 hours?

Twenty-four hour ambulatory blood pressure monitoring (24BP) is an emerging technology used to further evaluate elevated blood pressure readings obtained during office visits. We wondered whether a less costly approach than 24BP would lead clinicians to similar treatment decisions. Eleven faculty general internists sequentially evaluated sample patients' office blood pressure readings, four intermittent blood pressures (IBP), and 24-hour ambulatory blood pressure reports and made treatment decisions at each step. This yielded 187 "cases," in 173 of which the physicians were able to make treatm

Artificial Intelligence and Multi-Omics Approaches in the Precision Management of Pulmonary Hypertension: From Early Diagnosis to Therapeutic Stratification.

Pulmonary hypertension (PH) is a heterogeneous clinical syndrome in which similar haemodynamic abnormalities may arise from distinct vascular, cardiac, pulmonary, thromboembolic, and molecular mechanisms. This complexity limits the ability of conventional classifications and risk scores to fully capture individual disease trajectories and treatment responses. Artificial intelligence (AI) offers a framework for integrating clinical data, electrocardiography, multimodal imaging, invasive haemodynamics, biomarkers, and multi-omics information across the PH care pathway. This review summarises cur

The Need to Improve the Medical Subject Headings (MeSH) and the Excerpta Medica Tree (EMTREE) Thesauri to Perform Systematic Review on Oral Potentially Malignant Disorders.

BACKGROUND: Despite recent advancements in the understanding and classification of oral potentially malignant disorders (OPMD), their terminology remains inconsistent and heterogeneous throughout the scientific literature, thus affecting evidence-based decision-making relevant for clinical management of these disorders. Updating this classification represents a necessity to improve the indexing and retrieval of OPMD publications, in particular for systematic reviews and meta-analysis. METHODS: Through a critical appraisal of the Medical Subject Headings (MeSH) and Excerpta Medica Tree (EMTREE

New grading scale based on early factors for predicting delayed cerebral ischemia in patients with aneurysmal subarachnoid hemorrhage: a multicenter retrospective study.

Delayed cerebral ischemia (DCI) could lead to poor clinical outcome(s). The aim of the present study was to establish and validate a predictive model for DCI after aneurysmal subarachnoid hemorrhage (aSAH) based on clinical data. Data from a series of 217 consecutive patients with aSAH were reviewed and analyzed. Related risk factors within 72 h after aSAH were analyzed depending on whether DCI recurred. Least absolute shrinkage and selection operator (LASSO) analysis was performed to reduce data dimensions and screen for optimal predictors. Multivariable logistic regression was used to establ

A conventional content analysis of clinicians' perspectives on patient-based factors that influence surgical prioritization of abdominal trauma.

Operative triage of abdominal trauma, whether blunt or penetrating, remains a major challenge in emergency care. Although quantitative decision-support tools, such as Pediatric Emergency Care Applied Research Network (PECARN) rule, Focused Assessment with Sonography in Trauma (FAST), Shock Index, Pediatric Age-Adjusted (SIPA), and Blunt Abdominal Trauma Scoring System (BATSS), are widely used, they do not fully capture the complex clinical reasoning and experiential judgment required for real-world decision-making. This limitation is particularly important because blunt and penetrating abdomin

Duhamel versus transanal endorectal pull through (TERPT) for the surgical treatment of Hirschsprung's disease.

For the surgical treatment of Hirschsprung's disease, several surgical techniques are used to resect the distal aganglionic colon. Two frequently used techniques are the Duhamel procedure and the transanal endorectal pull-through procedure. During the '8th Pediatric Colorectal Course' in Nijmegen, November 2015, a workshop was organized to share experiences of both techniques by several experts in the field and to discuss (long term) outcomes. Specifically, the objective of the meeting was to discuss the main controversies in relation to the technical execution of both procedures in order to m

Chronic Pain and Opioid Prescribing: Three Ways for Navigating Complexity at the Clinical‒Population Health Interface.

Clinically focused interventions for people living with pain, such as health professional education, clinical decision support systems, prescription drug monitoring programs, and multidisciplinary care to support opioid tapering, have all been promoted as important solutions to the North American opioid crisis. Yet none have so far delivered substantive beneficial opioid-related population health outcomes. In fact, while total opioid prescribing has leveled off or reduced in many jurisdictions, population-level harms from opioids have continued to increase dramatically. We attribute this failu

Enhancing Engagement to Support Shared Decision Making Within Haemophilia: Development of Materials for People With Haemophilia, Caregivers and Healthcare Professionals.

INTRODUCTION: Treatment decision making can be complex and challenging for people with haemophilia (PwH) and their caregivers. A shared decision-making (SDM) approach can ensure that decisions about treatment and care incorporate patients' beliefs, values, and preferences. AIMS: The aims were to: (1) identify potential drivers and barriers to SDM between PwH/caregivers and clinicians, (2) develop tools to facilitate SDM in clinic appointments, and (3) gain feedback on the usability and design of these tools to inform refinements. METHODS: Virtual focus groups were conducted with adolescent a

Enhancement of Patient-Centered Lung Cancer Screening: The MyLungHealth Randomized Clinical Trial.

IMPORTANCE: Lung cancer screening (LCS) with low-dose computed tomography (CT) remains underused in the US, partly because of incomplete smoking history documentation in electronic health records (EHRs) and limited time for shared decision-making in primary care. OBJECTIVE: To determine whether a patient-facing, EHR-integrated tool combined with clinician-facing clinical decision support improves the identification of LCS-eligible patients and the ordering of low-dose CT compared with clinician-facing tools alone. DESIGN, SETTING, AND PARTICIPANTS: This pragmatic, unstratified, randomized cl

Efficacy of Electronic Reminders in Increasing the Enhanced Recovery After Surgery Protocol Use During Major Breast Surgery: Prospective Cohort Study.

BACKGROUND: Enhanced recovery after surgery (ERAS) protocols are patient-centered, evidence-based guidelines for peri-, intra-, and postoperative management of surgical candidates that aim to decrease operative complications and facilitate recovery after surgery. Anesthesia providers can use these protocols to guide decision-making and standardize aspects of their anesthetic plan in the operating room. OBJECTIVE: Research across multiple disciplines has demonstrated that clinical decision support systems have the potential to improve protocol adherence by reminding providers about departmenta
