Feminizing Sertoli cell tumors associated with Peutz-Jeghers syndrome: an increasingly recognized cause of prepubertal gynecomastia.

Testicular sex cord tumors with annular tubules are an increasingly recognized cause of prepubertal gynecomastia typically accompanied by accelerated linear growth and advanced bone maturation. Serum estrogen levels may be elevated. Testicular ultrasound and biopsy are diagnostic, and mastectomy is indicated. Although these tumors can occur independently, causing gynecomastia in 10 percent of cases, they usually occur in patients with Peutz-Jeghers syndrome. In any Peutz-Jeghers syndrome patient developing gynecomastia, a testicular tumor should be sought. Conversely, because a significant pro

Successful treatment of a rectal anastomotic stenosis by transanal endoscopic microsurgery (TEM) using the contact Nd:YAG laser.

We report the advantage of employing transanal endoscopic microsurgery (TEM) using the contact Nd:YAG laser for the treatment of a rectal anastomotic stenosis. A 72-year-old woman was admitted to our hospital with a postoperative rectal anastomotic stenosis. Twenty months prior to admission, the patient underwent a low anterior resection for the treatment of the rectal cancer using an EEA stapling device. A barium enema and colonoscopy revealed a rectal stenosis, 0.8-cm diameter. This stenosis was at the anastomotic site, approximately 4.0 from the dental line. An endoscopic treatment was perf

Colonic adenomas in asymptomatic women with a history of breast cancer.

One hundred ninety-three asymptomatic women with a personal history of breast cancer underwent screening colonoscopy. One hundred sixty-eight women had breast cancer as their only potential risk factor for colonic neoplasia, and 25 had a family history of colorectal neoplasia in addition to their personal history of breast cancer. Among women with breast cancer, increasing age and body weight were each predictive of an increasing prevalence of colonic adenomas. The prevalence of colonic adenomas in women aged 50-75 yr whose only potential risk factor was breast cancer was 18%, and was identica

Upper and lower gastrointestinal endoscopy in children and adolescents with Crohn's disease: a prospective study.

Fifty-six children and adolescents with Crohn's disease were prospectively investigated with gastroscopy and colonoscopy irrespective of localizing symptoms or signs. Routine biopsies were taken from endoscopically normal and abnormal areas. A high incidence (71%) of upper gastrointestinal (GI) involvement was found. In 41%, these findings were instrumental in making the diagnosis. The ileum was viewed in 49 of the 56 cases. Overall, the upper GI tract was involved in 71%, the terminal ileum in 53%, and the colon in 86% (oesophagus 16%, body of stomach 46%, antrum 36%, duodenum 21%, terminal i

[Sonography in the diagnosis and follow-up of colonic diverticulitis].

17 patients with colonic diverticulitis of various degree were examined by conventional abdominal ultrasound in comparison to either colonoscopy or operation. The sonography of the inflammated region showed an echo-poor enlargement of the colonic wall (normal less than 3 mm) ranging from 5 to 15 mm (mean 7.8 mm) over a short distance. In cases with bowel wall enlargement of more than 11 mm a spontaneous evacuation of pus from the diverticles was endoscopically seen. A sonographic enlargement of the colonic wall of more than 15 mm is very suspicious to be a peridiverticulitis or a beginning abs

Contribution of modern imaging in rectosigmoid neoplastic disease (comparison with fiberoptic endoscopy).

Double-contrast enema (DCE) remains the primary diagnostic exploration. Its specific method and interpretation require skillful handling. Unlike colonoscopy, DCE results in false-positives, the frequency of which varies widely in function of the skill of the interpretation. False negatives reach a significant level from a threshold of size which can be fixed at 5 mm. Despite these drawbacks, double-contrast enema, which is an exploration involving a two level of invasiveness and which provides a good panoramic image of the colon should remain the primary exploration in routine check-ups. [Systematic monitoring of the postoperative rectum].

This study concerns the detection and treatment of recurrence after operation for colo-rectal cancer. The recurrences were classified as being in the suture line, peri-colonic, pelvic or distant metastasis. The factors which determine the appearance of recurrence depend on the tumour (development of the tumour and degree of anaplasia), the surgical technique (removal of the metastatic pathway and techniques intended to prevent the implantation of migrating tumour cells) and the patient (age, immunological factors, etc.). Our survey for the detection of recurrence consists of history and clinic

Mathematical Modeling of Vedolizumab Treatment's Effect on Microbiota and Intestinal Permeability in Inflammatory Bowel Disease Patients.

Growing evidence suggests that impaired gut permeability and gut microbiota alterations are involved in the pathogenesis of Inflammatory Bowel Diseases (IBDs), which include Ulcerative Colitis (UC) and Crohn's Disease (CD). Vedolizumab is an anti-α4β7 antibody approved for IBD treatment, used as the first treatment or second-line therapy when the first line results in inadequate effectiveness. The aim of this study is to develop a mathematical model capable of describing the pathophysiological mechanisms of Vedolizumab treatment in IBD patients. In particular, the relationship between drug con

Unveiling the Enigma of a Colonic Neuroendocrine Tumor Causing Ileocolic Intussusception: A Case Report.

Intussusception in adults is rare and is often associated with a pathologic lead point. While colonic adenocarcinoma is a common cause, well-differentiated colonic neuroendocrine tumors are exceedingly rare. We present a unique case of an ileocolic intussusception due to a distal ascending colonic neuroendocrine tumor, emphasizing the diagnostic challenges and importance of prompt intervention. A 60-year-old male with a previous screening colonoscopy in June of 2022 presented to the Emergency Department with two days of cramping, right upper abdominal pain with associated nausea and two episod

Adenocarcinoma of Sigmoid Colon Diagnosed in Pregnancy: A Case Report.

With more women getting pregnant at later ages than in the past, the incidence of malignancies in pregnancy is on the rise. Common malignancies of pregnancy are melanoma, breast cancer, cervical cancer, lymphomas, and leukemias. Colorectal carcinoma is rare in pregnancy, with an estimated incidence of 1 in 13',000 cases. We describe such a case of colorectal carcinoma in pregnancy (CRC-p), in a 31-year-old patient. She presented in the 21st week of her second pregnancy with constipation of two weeks duration despite appropriate medical management. This prompted further evaluation with abdomina

Study protocol of the RaPS study: novel risk adapted prevention strategies for people with a family history of colorectal cancer.

BACKGROUND: People aged 40-60 years with a family history (FH) of colorectal cancer (CRC) in 1st degree relatives (FDRs) have a 2- to 4-fold increased risk of CRC compared to the average risk population. Therefore, experts recommend starting CRC screening earlier for this high-risk group. However, information on prevalence of relevant colonoscopic findings in this group is sparse, and no risk adapted screening offers are implemented in the German health care system. For example, screening colonoscopy is uniformly offered from age 55 on, regardless of family history. Thus, we initiated a multic

High complete resection rate for pre-lift and cold biopsy of diminutive colorectal polyps.

BACKGROUND AND STUDY AIMS: The majority of polyps removed at colonoscopy are diminutive (≤ 5 mm) to small (< 10 mm) and there are few guidelines for the best way for these polyps to be removed. We aimed to assess the feasibility and effectiveness of cold biopsy forceps polypectomy with pre-lift (CBPP) for polyps ≤ 7 mm. Our aims were to assess completeness of histological resection of this technique, to identify factors contributing to this and assess secondary considerations such as timing, retrieval and complication rates. PATIENTS AND METHODS: We conducted a prospective cohort study on con

Distribution and Characteristics of Colonic Diverticula in a United States Screening Population.

BACKGROUND & AIMS: Colonic diverticula are the most common finding from colonoscopy examinations. Little is known about the distribution of colonic diverticula, which are responsible for symptomatic and costly diverticular disease. We aimed to assess the number, location, and characteristics of colonic diverticula in a large US screening population. METHODS: We analyzed data from a prospective study of 624 patients (mean age, 54 years) undergoing screening colonoscopy at the University of North Carolina Hospital from 2013 through 2015. The examination included a detailed assessment of colonic

Microsimulation model to identify suboptimal recurrence detection in patients with colorectal cancer following the current standard of care.

OBJECTIVES: Colorectal cancer (CRC) is one of the most common cancers worldwide. Stage II/III patients undergo curative-intent surgery yet still face the recurrence risk. Detecting recurrences early provides the best opportunity for optimal treatment. We aimed to develop a microsimulation model to evaluate CRC management-associated outcomes based on current guidelines, including the performance of guideline-recommended surveillance in detecting recurrences. DESIGN: Two separate individual-level state transition (microsimulation) models for colon and rectal cancer were built with a lifetime ho

Artificial intelligence detection of endoscopic moderate-to-severe ulcerative colitis: a novel tool to enhance clinical trial recruitment.

BACKGROUND AND AIMS: Artificial intelligence (AI) may detect endoscopically active ulcerative colitis (UC) and streamline clinical trial enrollment. Here, we investigate the performance of a commercially available AI model, autoinflammatory bowel disease (AutoIBD)-UC (Virgo Surgical Video Solutions, Inc, San Francisco, Calif, USA), to detect moderate-to-severe UC in colonoscopy videos collected as part of routine clinical practice. METHODS: AutoIBD-UC was applied to consecutive endoscopy videos between August 30, 2024, and October 17, 2024. Each video received an AutoIBD score (0-1); videos w

Non-invasive breath testing to detect colorectal cancer: protocol for a multicentre, case-control development and validation study (COBRA2 study).

BACKGROUND: Colorectal cancer (CRC) is the fourth most common cancer in the United Kingdom. The five-year survival rate from CRC is only 10% when discovered at a late stage, but can exceed 90% if diagnosed early. Symptoms related to CRC can be non-specific, and therefore the decision to refer for a colonoscopy can be challenging. Breath analysis potentially offers a simple and quick method to detect CRC specific volatile organic compounds (VOCs) in breath. This protocol describes the COBRA2 study which aims to develop and validate the clinical prediction model (CPM) in the detection of CRC bas

Familial adenomatous polyposis: a case report.

BACKGROUND: Familial adenomatous polyposis is characterized by the presence of multiple colorectal adenomatous polyps and caused by germline mutations in the tumor suppressor gene and adenomatous polyposis coli, located on chromosome 5q21-q22. Familial adenomatous polyposis occurs in approximately 1/10,000 to 1/30,000 live births, and accounts for less than 1% of all colorectal cancers in the USA. It affects both sexes equally and has a worldwide distribution. The incidence of colon cancer in low- and middle-income countries is rising. In addition to the increasing incidence, lack of early det

Bleeding Risk Following Stereotactic Body Radiation Therapy for Localized Prostate Cancer in Men on Baseline Anticoagulant or Antiplatelet Therapy.

PURPOSE: Patients on anticoagulant/antiplatelet medications are at a high risk of bleeding following external beam radiation therapy for localized prostate cancer. SBRT may reduce the bleeding risk by decreasing the volume of bladder/rectum receiving high doses. This retrospective study sought to evaluate the rates of hematuria and hematochezia following SBRT in these patients. METHODS: Localized prostate cancer patients treated with SBRT from 2007 to 2017 on at least one anticoagulant/antiplatelet at baseline were included. The minimum follow-up was 3 years with a median follow-up of 72 mont
