The effects of parity, oral contraceptive use and hormone replacement therapy on the incidence of varicose veins.

Three population-based age cohorts (40-, 50- and 60-years old) of women (n = 3,590) were followed up to find out if pregnancies, use of oral contraceptives (OCs) or hormone replacement therapy (HRT) affect the appearance of varicose veins. Results were presented as odds ratios (OR) of prevalences at entry (POR) and as incidence odds ratios (IOR) during the 5-year follow-up. Parity with three or more births was an independent risk factor for varicose veins IOR 2.0 (95% confidence interval (CI), 1.0 - 3.9). OC use showed a small and not significant protective effect for varicose veins, both POR

Over-the-counter access, changing WHO guidelines, and contraindicated oral contraceptive use in Mexico.

This study examines the prevalence of contraindications to the use of oral contraceptives in Mexico by sociodemographic characteristics and by whether this family planning method was obtained with or without a doctor's prescription. Using data on smoking behavior and blood-pressure measurements from the 2000 Mexican National Health Survey, the authors found that, under the 1996 World Health Organization (WHO) medical eligibility guidelines, the prevalence of contraindications is low and that no significant differences in contraindications exist at any level between those who obtain oral contra

Maternal use of oral contraceptives and risk of hypospadias - a population-based case-control study.

The aim of this population-based case-control study was to examine the risk of isolated hypospadias in boys born to mothers who have used oral contraceptives in early pregnancy. The study was based on data from the Hungarian Case-Control Surveillance of Congenital Abnormalities from 1980 to 1996, and included 3,038 boys with hypospadias (cases), 24,799 boys without congenital abnormalities (CA-free controls), and 11,881 boys with abnormalities other than hypospadias. We used unconditional logistic regression to adjust for birth order, maternal age, maternal employment status, maternal diabetes

Ethinylestradiol + etonogestrel contraceptive vaginal ring: new drug. Possibly useful in some situations.

(1) The standard hormonal contraception consists of a combined oral contraceptive containing about 30 micrograms of ethinylestradiol and a well-evaluated and widely used progestin such as levonorgestrel or norethisterone. (2) A vaginal ring delivering about 120 micrograms of etonogestrel and 15 micrograms of ethinylestradiol daily for 3 consecutive weeks per cycle, which is then removed for one week, is now marketed in France as a contraceptive. Etonogestrel, a desogestrel metabolite, is classified as a 'third-generation' progestin. (3) On the basis of 2 non-comparative trials involving a tota

Unusual case of mesenteric venous thrombosis associated with oral contraceptive use in an adolescent girl.

A 16-year-old girl presented with hematochezia and abdominal pain. The patient developed a small bowel obstruction that required a laparotomy, which showed a segment of grossly infarcted small bowel with 3 perforations, localized abscesses, and creeping fat. Resection and an end-to-end anastomosis were performed. Pathologic examination showed underlying mesenteric venous thrombosis with no evidence of Crohn's disease. Hematology workup showed no evidence of a congenitally inherent hypercoagulable state. The use of oral contraceptives was the only predisposing factor identified.

Are affluent, well-educated, career-orientated women knowledgeable users of the oral contraceptive pill?

OBJECTIVE: Studies have shown poor knowledge of oral contraceptives among women attending government health clinics and women in rural areas. Little is known about the level of contraceptive knowledge in educated, affluent, career-orientated women, although it could be expected that access to information would be greater. The study objective was to describe the profile, knowledge and understanding of oral contraceptive users in a private general practice in Johannesburg, South Africa. METHODS: Over a period of 3 months, all women attending a private general practice who were using an oral con

Contraceptive use--United States and territories, Behavioral Risk Factor Surveillance System, 2002.

PROBLEM: Contraceptive use is an important determinant of unintended pregnancy. In the United States, approximately half of all pregnancies are unintended. Population-based information about contraceptive use patterns is limited at the state level. Information about contraceptive use for states can be used to guide the development of state programs and policies to decrease unintended pregnancy and the spread of sexually transmitted infections. Information about contraceptive use for specific subpopulations can be used to further refine state efforts to improve contraceptive use and subsequentl

Use of combined oral contraceptives among women with migraine and nonmigrainous headaches: a systematic review.

This systematic review examines evidence evaluating whether women with headaches who use combined oral contraceptives (COCs) have a greater risk of stroke than women with headaches who do not use COCs. We searched MEDLINE for articles published from 1966 through March 2005 relevant to headaches and COC use as risk factors for stroke. Of the 79 articles identified, nine met our selection criteria (eight reports of six observational studies plus one meta-analysis). All studies reported specifically on migraine headaches. Evidence from six case-control studies suggested that COC users with a hist

Comparison of two regimens of a monthly injectable contraceptive containing dihydroxyprogesterone acetophenide and estradiol enanthate.

INTRODUCTION: This study compared two regimens of a monthly injectable contraceptive containing dihydroxyprogesterone acetophenide 150 mg and estradiol enanthate 10 mg (Perlutan) over 12 cycles of use. METHODS: Three hundred sixty-five adolescents were randomized into two groups. The patients in Group 1 received an initial injection of Perlutan on the 1st-5th day of their menstrual cycle and subsequent injections every 30 +/- 3 days, whereas those in Group 2 followed the traditional schedule of administration in which the first injection is administered between Days 7 and 10 of their menstrua

Oral contraceptive use and ovarian cancer risk among carriers of BRCA1 or BRCA2 mutations.

Women with mutations of the genes BRCA1 or BRCA2 are at increased risk of ovarian cancer. Oral contraceptives protect against ovarian cancer in general, but it is not known whether they protect against the disease in carriers of these mutations. We obtained self-reported lifetime histories of oral contraceptive use from 451 women who carried mutations of BRCA1 or BRCA2. We used conditional logistic regression to estimate the odds ratios associated with oral contraceptive use, comparing the histories of 147 women with ovarian cancer (cases) to those of 304 women without ovarian cancer (controls

Emergency contraceptive pills over-the-counter: practices and attitudes of pharmacy and nurse-midwife providers.

Deregulation of emergency contraceptive pills (ECP) has led to pharmacy staff becoming a new provider group of ECP, together with nurse-midwives, who are already experienced in prescribing contraceptives. This postal questionnaire survey aimed to assess practices and attitudes towards ECP and the over-the-counter (OTC)-availability among pharmacy staff (n=237) and nurse-midwives (n=163). The overall response rate was 89%. Both study groups were positive to ECP and the OTC-availability and the vast majority agreed that sexually active women should be aware of ECP and that routine information ab

Emergency contraceptive pills over-the-counter: a population-based survey of young Swedish women.

One year after emergency contraceptive pills (ECP) had become prescription-free, we sent a questionnaire to 800 randomly selected women in mid-Sweden. The aim was to investigate women's knowledge, attitudes and practices regarding the method. The response rate was 71% (n = 564). The majority of the women, 65%, would prefer to purchase ECP over-the-counter (OTC) in a pharmacy. Attitudes toward the method were predominantly positive, but one fourth (24%) had worries about side effects and one third (33%) considered ECP to be a kind of abortion. Logistic regression showed that correct knowledge o

Risk of stroke in women exposed to low-dose oral contraceptives: a critical evaluation of the evidence.

BACKGROUND: Use of the oral contraceptive pill (OCP) has been reported to be associated with stroke. With current OCPs containing less than 50 micro g of ethinyl estradiol, and many earlier studies reporting the association between OCPs and stroke, subjected to biases, we determined whether such an association exists and, if so, the magnitude of the risk. METHODS: Two independent searches were conducted to obtain relevant articles from MEDLINE, EMBASE, and Science Citation (1970 to June 2000). Eligible articles published in English describing OCP use and stroke outcomes were retrieved, and re

Are oral contraceptive use and pregnancy complications risk factors for atopic disorders among offspring?

In utero programming of atopic manifestations has been suggested. We investigated the association between oral contraceptive (OC) use before, and complications during pregnancy (CDP) and asthma, along with other atopic manifestations. The study is based on neonates from Kingston and St Andrew, a geographic subcohort from the Jamaican Perinatal Morbidity, Mortality Survey conducted in 1986-1987. Information on OC use and CDP was extracted from maternal interviews and medical records. In a follow up in 1997-1998, via interviews with mothers, trained nurses collected information on asthma/wheezin

Combination estrogen-progestin contraceptives and body weight: systematic review of randomized controlled trials.

OBJECTIVE: Many women and clinicians believe that combination estrogen-progestin contraceptive use can lead to weight gain. This concern can deter women from starting hormonal contraception or lead to premature quitting. This review evaluated the association between combination contraceptive use and change in body weight. DATA SOURCES: The computerized databases CENTRAL, MEDLINE, EMBASE, Popline, and LILACS (from their inception to 2002) were used to conduct this review. Known investigators and manufacturers were contacted for information about other trials not discovered in the database sear

[Effect of low-dose oral contraceptive regulon-Gedeon Richter on some hemostatic variables].

The authors have investigated the changes of some haemostatic variables at 33 women, who used low-dosed oral contraceptive REGULON--Gedeon Richter in the duration of six months. Statistically significant decrease is found in the activity of Protein S and Protein C, but plasminogen level is increased. The levels of the other variables remained in referential ranges. The use of REGULON doesn't influence the coagulation system.
