Acute disseminated encephalomyelitis (ADEM) after allogeneic bone marrow transplantation for acute myeloid leukemia.

Acute disseminated encephalomyelitis (ADEM) is a rare inflammatory demyelinating disease of the central nervous system. We describe here a patient who developed ADEM after allogeneic bone marrow transplantation (BMT). A 48-year-old woman with acute myeloid leukemia (M2) underwent allogeneic BMT from her HLA-identical sister. Cyclosporin for prophylaxis of acute graft-versus-host disease (GVHD) was discontinued from day 15 because of its toxicity. She was relatively well after the resolution of cytomegalovirus reactivation and chronic GVHD. Nine months after BMT, she suddenly developed diplopia

An outbreak in Italy of botulism associated with a dessert made with mascarpone cream cheese.

In the late 1996, an outbreak of botulism affected eight young people (age of patients ranged from 6 to 23 years) in Italy. The onset of the illness was the same for all of these patients: gastrointestinal symptoms (nausea and vomiting) followed by neurologic symptoms. The most common neurologic symptoms were dysphagia, respiratory failure (100%), diplopia (87%), dysarthria, ptosis (75%) and mydriasis (50%). All patients required mechanical ventilation. Botulinum toxin was detected from two of respectively five sera and six stool samples analysed, while spores of Clostridium botulinum type A w

[Paroxysmic anarthria in multiple sclerosis].

INTRODUCTION: The paroxystic clinical features of multiple sclerosis (MS) include trigeminal neuralgia, itch, transient diplopia, Lhermitte's sign, akinesia, dystonia, Uhthoff's phenomenon and others which are very characteristic, such as paroxystic ataxia and dysarthria. CLINICAL CASE: We present the case of a 30 year old man who consulted for multiple episodes lasting only a few seconds, of complete inability to speak. This symptom recurred several times a day and in many different situations. It was often triggered off by external stimuli such as having to speak in front of several people.

[Orbital myopathy in metastatic malignant paraganglioma: a paraneoplastic syndrome?].

BACKGROUND: Several disease entities of the orbit are known which display an autoimmunological pathogenesis. Among these disease entities the thyroid-related immune orbitopathy and the idiopathic orbital inflammatory syndrome (orbital pseudotumor). The pathogenesis of these diseases is not well understood; however, an autoimmune inflammatory reaction histologically characterized by a chronic inflammatory cell infiltrate has been suggested. One care of orbital pseudotumor presumably of paraneoplastic origin occurring in a patient with a non-Hodgkin's lymphoma has been reported. We report a pati

[A midbrain arteriovenous malformation at quadrigeminal plate completely obliterated by embolization].

An eighteen year-old boy presented sudden loss of consciousness and tetraparesis. Radiological examinations revealed a ruptured midbrain arteriovenous malformation (AVM) at the right quadrigeminal plate. The patient recovered from symptoms by conservative treatment over 6 months with residual right hemisensory disturbance and mild diplopia during bilateral horizontal gaze. One year later, he was admitted to our institute for radical treatment of the AVM. On the angiography, the AVM was fed by two branches of the right long circumferential artery and a left paramedian penetrating artery of the

Nonepithelial tumors of the nasal cavity, paranasal sinuses, and nasopharynx. A clinicopathologic study. XIV: Chordomas.

Twenty cases of a rare tumor, chordoma involving the nasal cavity, paranasal sinuses, or nasopharynx, are reported. Patients most often had localized headache, nasal obstruction, decreased hearing, or symptoms related to cranial nerve involvement, especially diplopia. A mass bulging into the nasopharynx, posterior nasal cavity, or pharynx was found on physical examination in 13 of these 20 patients, and in another 6 patients on radiologic examination. Paralysis of one or more cranial nerves was identified in the majority of patients who had tumors involving the upper nasopharynx. On radiologic

Computer-assisted planning, stereolithographic modeling, and intraoperative navigation for complex orbital reconstruction: a descriptive study in a preliminary cohort.

PURPOSE: Post-traumatic or postablative enophthalmos and diplopia and/or facial asymmetry resulting from inaccurate restoration of orbital anatomy remain relatively frequent sequellae of complex orbital reconstruction. Recently, preoperative computer-assisted planning with virtual correction and construction of stereolithographic models have been combined with intraoperative navigation in an attempt to more accurately reconstruct the bony orbit and optimize treatment outcomes. The purpose of the present study is to review the authors' early experience with computer planning, stereolithographic

Outcomes of endoscopic sinus surgery (ESS) on sino-orbital osteomas: ophthalmic manifestations and visual outcomes from a 12-year multicenter experience and literature review.

BACKGROUND: Osteomas are the most common benign craniofacial bone tumors that may extend into the orbital cavity. While the majority of them remain asymptomatic, orbital involvement can lead to clinically significant ophthalmologic manifestations. Although endoscopic sinus surgery (ESS) has been increasingly utilized, data focusing specifically on ophthalmologic outcomes following ESS in sino-orbital osteomas are limited. METHODS: This 12-year multicenter descriptive cohort evaluated 12 patients with symptomatic sino-orbital osteomas managed with ESS, including advanced approaches such as Dra

Isolated clival Langerhans cell histiocytosis in a pediatric patient: systematic review and illustrative case report.

BACKGROUND: Langerhans cell histiocytosis is a rare clonal myeloid disorder with heterogeneous clinical manifestations, predominantly affecting children. Osseous involvement is common; however, isolated localization to the clivus is exceptionally rare and poses significant diagnostic and therapeutic challenges due to its proximity to critical neurovascular structures. We present an illustrative pediatric case alongside a systematic review of the literature to better characterize this uncommon entity. METHOD: This study presents a rare case of a 3.5-year-old boy diagnosed with isolated clival

Multimodal surgical management of idiopathic intracranial hypertension presenting with chiari malformation, syringomyelia, and venous sinus stenosis: A case report.

BACKGROUND: Idiopathic intracranial hypertension (IIH) and Chiari I malformation (CMI) may present with overlapping symptoms and imaging findings, including cerebellar tonsillar descent, making accurate diagnosis challenging. This case illustrates the diagnostic complexity and highlights the role of venous sinus stenosis as an underlying etiology in IIH. CASE DESCRIPTION: We present the case of a 65-year-old obese woman with a history of CMI previously treated with suboccipital decompression. She re-presented with recurrent headaches, diplopia, and left upper extremity paresthesia. Imaging re

Is Isolated Inferior Rectus Weakness a Strong Indicator for Myasthenia Gravis?

BACKGROUND: Isolated inferior rectus weakness (IRW) has historically been reported as a possible presenting sign of myasthenia gravis (MG). Orthoptists often identify IRW, prompting further investigations. This service evaluation aims to determine whether patients with IRW or other symptoms were subsequently diagnosed with MG. METHODS: A retrospective review of patient case notes was conducted. The cohort included consecutive patients who presented to the adult strabismus service and received acetylcholine receptor antibody (ACR) testing between 2010 and 2023 at a tertiary referral centre in

Case Report: Anti-cavin-4-positive immune-mediated rippling muscle disease co-occurring with isolated ocular myasthenia gravis.

Immune-mediated rippling muscle disease (iRMD) is a rare autoimmune myopathy characterized by mechanically triggered, wave-like skeletal muscle contractions. Its coexistence with myasthenia gravis (MG) has been described, but detailed clinicopathological and longitudinal characterization of anti-cavin-4-positive iRMD coexisting with isolated ocular MG remains limited. Here, we report a 35-year-old man who developed painless, water-like rippling movements of the right thigh in 2023, followed by occasional palpitations and mild creatine kinase elevation in 2024. In July 2025, he developed fluctu
