The Global Spine Care Initiative: applying evidence-based guidelines on the non-invasive management of back and neck pain to low- and middle-income communities.

PURPOSE: The purpose of this review was to develop recommendations for the management of spinal disorders in low-income communities, with a focus on non-invasive pharmacological and non-pharmacological therapies for non-specific low back and neck pain. METHODS: We synthesized two evidence-based clinical practice guidelines for the management of low back and neck pain. Our recommendations considered benefits, harms, quality of evidence, and costs, with attention to feasibility in medically underserved areas and low- and middle-income countries. RESULTS: Clinicians should provide education and

Treatment preferences amongst physical therapists and chiropractors for the management of neck pain: results of an international survey.

BACKGROUND: Clinical practice guidelines on the management of neck pain make recommendations to help practitioners optimize patient care. By examining the practice patterns of practitioners, adherence to CPGs or lack thereof, is demonstrated. Understanding utilization of various treatments by practitioners and comparing these patterns to that of recommended guidelines is important to identify gaps for knowledge translation and improve treatment regimens. AIM: To describe the utilization of interventions in patients with neck pain by clinicians. METHODS: A cross-sectional international survey

Effectiveness of three vestibular rehabilitation exercises for treating acute unilateral peripheral vestibular dysfunction: a multicenter randomized study.

BACKGROUND: Vestibular rehabilitation therapy (VRT) is an established intervention for patients with acute unilateral peripheral vestibular dysfunction (aUPVD). However, the relative effectiveness of different VRT strategies remains uncertain. This multicenter randomized pilot trial aimed to compare the effects of customized vestibular rehabilitation therapy (CVRT), generic Cawthorne-Cooksey exercises (GVRT), and vestibulo-ocular reflex (VOR) adaptation exercises on vestibular symptom recovery and functional improvement. METHODS: A total of 34 patients with aUPVD were enrolled across three ce

The response to individualized treatment after a standardized treatment protocol among neck pain sufferers: a secondary analysis of a randomized controlled trial.

BACKGROUND: Manual therapy and exercise are recommended for patients with neck pain. In a recent randomized controlled trial, home stretching exercises with or without manual therapy were offered to subjects with persistent or recurrent neck pain. No difference in pain or disability between the treatment groups were found after the two-week intervention period. We aimed to investigate whether these patients had a better outcome after individual tailoring of the treatment content two months after the initial structured intervention period. METHODS: This manuscript is a secondary analysis of a

Use of manipulative therapy for mechanical pain of spinal origin.

Manipulation of the spinal joints for the relief of pain is an ancient remedy that is in widespread use today. Clinical trials investigating the effectiveness of manipulative therapy are briefly reviewed, and some experimental evidence suggesting possible causes of spine pain is presented. The rationale for the use of spinal manipulative therapy within the context of R. A. McKenzie's diagnostic and therapeutic system is detailed and explicitly stated. Spinal mobilization and manipulation are presented as a logical treatment progression from the use of postural instruction and exercise therapy.

Effectiveness of remote graded motor imagery therapy for patients with central sensitisation low back pain: study protocol for a randomised controlled trial.

INTRODUCTION: Chronic low back pain (CLBP) is a prevalent global condition that significantly impairs quality of life, reduces work productivity and imposes substantial healthcare burdens. Approximately 70% of adults worldwide experience at least one episode of low back pain during their lifetime. Central sensitisation, a common complexity of CLBP, involves heightened responsiveness of the central nervous system, resulting in amplified neural signalling and increased pain sensitivity. This phenomenon suggests that pain in CLBP extends beyond purely biomedical origins, involving multiple factor

Three Adult Siblings with Mucopolysaccharidosis Type II (Hunter Syndrome): A Report on Clinical Heterogeneity and 12 Months of Therapy with Idursulfase.

Mucopolysaccharidosis type II (MPS II - Hunter syndrome) is a rare X-linked recessive disease of lysosomal glycosaminoglycan metabolism leading to a systemic storage disorder. We report three adult brothers (aged 46-52 years) with attenuated Hunter syndrome after 12 months of enzyme replacement therapy with idursulfase. Before enzyme replacement therapy, each had serious complications of their disease: in addition to all requiring urgent cervical spinal canal decompressions in middle age, one required emergency aortic and mitral valve surgery, another had stage IV airways disease, and the thir

The factors affecting glycemic control in Japanese adolescents with type 2 diabetes mellitus.

The factors affecting glycemic control were examined using HbA(1C) as an index in a total of 22 patients with type 2 diabetes mellitus whose ages at onset were less than 18 years old. As a result, the presence or absence of the following cases were considered possible factors for significant exacerbation of glycemic control: diabetic microangiopathy; school phobia or nonworking situation; drug therapy; use of more than two kinds of oral hypoglycemic agents (OHAs) or insulin for the drug therapy cases. No improvement in glycemic control could be achieved even by increasing the number of OHAs fo

Lower than conventional doses of captopril in the initiation of converting enzyme inhibition in patients with severe congestive heart failure.

Angiotensin converting enzyme (ACE) inhibitors are effective long-term therapy for congestive heart failure, improving symptoms, exercise tolerance, biochemical anomalies, and mortality. Captopril, in doses of up to 150 mg daily is relatively free of serious adverse effects. A test dose of 6.25 mg normally precedes regular therapy in an attempt to avoid first-dose hypotension, which may also be prevented by reduction in diuretic dosage and by the avoidance of over-diuresis leading to volume depletion. We report two patients who, despite the above precautions developed symptomatic first-dose hy

Evaluation of a Rehabilitation Program With Blood Pressure-Guided Exercise Intensity Restrictions for Patients With Thoracic Aortic Dissection or Aneurysm.

PURPOSE: Aortic disease presents a significant risk of mortality and morbidity, with hypertension considered the primary driver of disease progression. Blood pressure naturally increases during physical activity, and patients need guidance on the optimal level of exercise. The purpose of this study was to evaluate change in cardiopulmonary fitness after attending a rehabilitation program using blood pressure-guided exercise intensity. METHODS: The cohort included patients with thoracic aortic dissection (type A and B) and surgically operated thoracic aortic aneurysm. All participated in a reh

Gas exchange threshold to guide exercise training intensity of older individuals during cardiac rehabilitation.

The gas exchange threshold (GET), which is determined during incremental exercise (Inc-Ex) testing, is often considered a safe training intensity for cardiac rehabilitation. However, there are only a limited number of reports on the actual implementation of this method. We assessed the applicability of GET-guided exercise using a constant load exercise (CL-Ex) protocol. We recruited 20 healthy older individuals (healthy, age: 69.4 ± 6.8 years) and 10 patients with cardiovascular diseases or risk factors (patient, age: 73.0 ± 8.8 years). On day 1, we determined the GET during symptomatic maximal

Cardiorespiratory Fitness after Transient Ischemic Attack and Minor Ischemic Stroke: Baseline Data of the MoveIT Study.

BACKGROUND: Cardiorespiratory fitness (CRF) is reduced in patients with stroke. It is unclear whether it is also reduced in patients with a transient ischemic attack (TIA) or minor stroke. We investigated the CRF in patients with a recent TIA or minor stroke and explored which determinants are associated with a lower fitness. METHODS: In 113 patients with a recent TIA or minor ischemic stroke (64 (SD = 10) years of age; 49 (IQR 27-86) days post TIA or stroke), the peak oxygen consumption (VO2peak) was determined in a symptom-limited ramp exercise test. Exercise capacity and participation of children with a ventricular septal defect.

Existing data on exercise performance in children with a ventricular septal defect (VSD) are scarce and inconclusive. We aimed to elucidate whether and why exercise capacity and physical activity level are decreased in children with VSD. Children 9 to 17 years of age with a surgically (operated, n = 13) or conservatively (unoperated, n = 14) managed VSD were compared with healthy peers (controls, n = 15) regarding (1) anthropometric and spirometric measurements; (2) cycle ergometric performance in terms of peak oxygen uptake, peak heart rate, and peak workload; and (3) mean daily energy expend
