Outcome following acute supratentorial subdural hematoma in pediatric age.

The authors report their experience in surgical treatment of 39 cases of acute subdural hematoma, followed by intensive therapy and physiokinesitherapy. The causes of head injuries, types of fractures, localization of the hematomas, concomitant lesions, diagnostic and therapeutic clinical features are discussed. The most common neurological evaluation scales for rating the state of comas are used for comparing the cases of head injury in adults (Glasgow Coma Scale) and in children (Children's Coma Scale). The Authors also report clinical follow-up, morbidity, and general mortality rate. Moreov

Duane's syndrome associated with crocodile tear and ear malformation. A case of stapes anomaly and its reconstruction.

Duane's syndrome is a congenital eye movement disorder characterized by marked limitation or absence of abduction, variable limitation of adduction, and narrowing of the palpebral fissure with retraction of the globe on attempted adduction. We treated a patient with bilateral Duane's syndrome associated with crocodile tear and external, middle, and inner ear anomalies. Exploratory tympanotomy showed an absence of the oval window and a rudimentary superior structure of the stapes that was connected to the abbreviated incudal lenticular process with connective tissue. A small hole was drilled in

Nervous and ocular disorders in children with cholestasis and vitamin A and E deficiencies.

Thirteen children, aged 10 months to 20 years, presenting with chronic cholestasis from the first month of life and with low serum levels of vitamins A and/or E, have been investigated for neurological and ophthalmological symptoms. Clinical findings consisted of 4 types: peripheral neuropathy; cerebellar dysfunction; abnormalities of eye movement, and retinal degenerative changes. The results of electrophysiological and morphological studies of muscle and nerves were consistent with neurono-axonal degeneration. Electrical abnormalities of the retina, especially a decrease of the b wave of ele

The narcoleptic approach paradigm (NAP) for the direct study of dreams and dream sleep functions.

A new sleep research paradigm, the Narcoleptic Approach Paradigm (NAP), was developed to enable direct psychological and physiological investigations of isolated dream sleep (also called rapid eye movement or REM sleep) and nondream sleep (non-REM or NREM). Essential to NAP is the cooperation of individuals with narcolepsy, who frequently have REM sleep onset. Narcoleptic animals may also be used for more convenient and extensive physiological NAP investigations. Several cogent psychological and physiological design variations of the general NAP model are outlined and one investigation success

Characterological depressions. Clinical and sleep EEG findings separating 'subaffective dysthymias' from 'character spectrum disorders'.

Early-onset characterological depressions are distinguished from late-onset chronic depressions that complicate the long-term course of unipolar and nonaffective illnesses. In turn, characterological depressions are divisible into at least two subtypes: (1) "Subaffective dysthymias" have even sex distribution, are often complicated by superimposed depressive episodes, rapid eye movement latency is shortened, and they tend to respond to tricyclics of lithium carbonate. In brief, they share many features of primary affective illness. (2) "Character spectrum disorders," by contrast, represent a h

Sleep apneic episodes as indications for adenotonsillectomy.

Fourteen children with adenoid and tonsillar hyperplasia were studied for one night in the sleep laboratory. Six of the children had at least 40 apneic episodes per night, mostly central or obstructive. The largest number of episodes was found in rapid eye movement sleep. Mixed episodes were the longest and central episodes the shortest. Adenotonsillectomy was performed on children with at least 40 episodes and on children with predominantly obstructive episodes. In two of these children, preoperative and postoperative recordings were taken, revealing a reduction in the total number of apneic

Sleep breathing abnormalities in kyphoscoliosis.

Five patients with severe kyphoscoliosis were studied during sleep. A spectrum of breathing abnormalities was found ranging from no abnormalities to severe episodes of prolonged central apnea. Rapid-eye-movement (REM) sleep was the period of greatest physiologic disturbance in all the patients, being the time of greatest oxygen desaturation. Within this small group, those with the most clinical evidence of chronic hypoxemia, polycythemia, and cor pulmonale, had the most severe derangements during sleep. Derangements in breathing pattern and arterial oxygen saturation (SaO2) had no apparent rel

Cranio-orbito-facial injuries: technical suggestions.

Eight per cent of 387 acutely head-injured patients had concomitant orbitofacial injuries. Irrespective of the magnitude of brain injury, the main surgical goals in such cases treated were closure of CSF fistulas, maximizing recovery of eye movement, visual and masticatory function, and esthetic reconstruction. The strictly neurosurgical lesions should be dealt with at once because of the risk of intracranial infection. Antibiotic acrylic and antibiotic wax are valuable in such intervention. In most cases it is advantageous to treat lesions of the upper two thirds of the facial complex in the

Apneic spells and sleep states in preterm infants.

The incidence of apneic spells during different sleep states active sleep, quiet sleep, and undifferentiated sleep was determined in eight preterm infants of 30 to 35 weeks' conceptional age, by means of a polygraphic recording technique. They were free of perinatal and postnatal complications other than apnea. During their active or rapid eye movement (REM) sleep they showed significantly more apneic episodes which were also longer lasting and they were accompanied by bradycardia of a greater severity. The organization of the immature nervous system with a preponderance of inhibitory synaptic

Sensitivity to temporal synchrony and selective attention in audiovisual speech in infants at elevated likelihood for autism: A preliminary longitudinal study.

Autism Spectrum Disorder is a highly heritable condition characterized by sociocommunicative difficulties, frequently entailing language atypicalities that extend to infants with a familial history of autism. The developmental mechanisms underlying these difficulties remain unknown. Detecting temporal synchrony between the lip movements and the auditory speech of a talking face and selectively attending to the mouth support typical early language acquisition. This preliminary eye-tracking study investigated whether these two fundamental mechanisms atypically function in infant siblings. We lon

Evaluation of a Novel Virtual Reality Simulated Alternate Cover Test to Assess Strabismus: A Prospective, Masked Study.

PURPOSE: A sensorimotor examination is the gold standard for strabismus diagnosis and quantification but requires a highly skilled examiner and may be limited by a child's cooperation. Virtual reality (VR) employs eye-tracking technology to monitor eye position and may be able to measure strabismus. The aim of this study was to assess a prototype VR-simulated alternate cover test to detect and measure strabismus. DESIGN: Prospective, masked diagnostic test study. METHODS: Patients aged 5-18 years with visual acuity of 20/80 or better were prospectively enrolled to undergo strabismus measurem

Tears and Eyewear in Forensic Investigation-A Review.

Body fluid analysis has played a crucial role in ascertaining various characteristics and has greatly aided in reconstructing events during crime scene investigation. It is often presumed that crimes that involve violence and mental disturbances such as murder or sexual assault provide good sources of body fluids such as blood, saliva, semen, vaginal secretions, urine and tears. Tears are secreted in response to any emotional or stressful situations and may be found deposited on surfaces such as bedding, tissue paper or cloth. In the absence of the commonly noted body fluids such as blood or s

Contribution of malocclusion and female facial attractiveness to smile esthetics evaluated by eye tracking.

INTRODUCTION: There is disagreement in the literature concerning the importance of the mouth in overall facial attractiveness. Eye tracking provides an objective method to evaluate what people see. The objective of this study was to determine whether dental and facial attractiveness alters viewers' visual attention in terms of which area of the face (eyes, nose, mouth, chin, ears, or other) is viewed first, viewed the greatest number of times, and viewed for the greatest total time (duration) using eye tracking. METHODS: Seventy-six viewers underwent 1 eye tracking session. Of these, 53 were

Effectiveness of inferior transposition of the horizontal rectus muscles for acquired inferior rectus paresis.

Full inferior transposition of the horizontal recti (inverse Knapp procedure) was performed on four patients with unilateral inferior rectus muscle weakness. The patients were followed up over time to determine the medium- and long-term effectiveness of the procedure. All of the patients had single binocular vision without the use of prisms in the primary position of gaze and three in the reading position after a mean postoperative follow-up interval of 21/2 years. The mean primary position preoperative vertical deviation was 20.3 delta and the mean primary position postoperative vertical corr

Does visual attention to diagnostic facial features predict eyewitness identification accuracy? Implications for diagnostic feature detection theory.

A police lineup includes a suspect and known-innocent fillers. The purpose of fillers is to protect an innocent suspect as witnesses must rely on their memory to make a lineup decision (i.e., select or reject). Diagnostic Feature Detection theory proposes that witnesses compare facial features across lineup members, weighting shared and non-shared features differently during the decision process (Wixted & Mickes, 2014). According to this view, only non-shared features are diagnostic because they provide distinguishing information, compared to features that are shared amongst all lineup members

Central retinal artery occlusion following COVID-19 vaccine administration.

INTRODUCTION: Increased risk of thromboembolic events has been associated with SARS-CoV-2 infections and more recently, with COVID-19 vaccination. To date, however, there are no reports of an association between the COVID-19 vaccination and retinal artery occlusions. We report a case of a patient who developed central retinal artery occlusion (CRAO) 2 days following the administration of the AstraZeneca COVID-19 vaccine. CASE DESCRIPTION: A 76-year-old woman presented to our Department of Ophthalmology complaining of painless vision loss in her left eye 48 hours after she had received her fir
