Impact of case-mix adjustment on observed variation in the healing of diabetic foot ulcers at 12 weeks using data from the National Diabetes Foot Care Audit of England and Wales: A cohort study.

AIM: This cohort study investigates the extent to which variation in ulcer healing between services can be explained by demographic and clinical characteristics. METHODS: The National Diabetes Foot Care Audit collated data on people with diabetic foot ulcers presenting to specialist services in England and Wales between July 2014 and March 2018. Logistic regression models were created to describe associations between risk factors and a person being alive and ulcer-free 12 weeks from presentation, and to investigate whether variation between 120 participating services persisted after risk fact…

Combined Core Stability and Short Foot Exercises Improve Medial Longitudinal Arch and Dynamic Balance More Than Short Foot Exercises Alone in Female Child Badminton Players with Pes Planus: A Randomized Controlled Trial.

BACKGROUND: Badminton involves rapid, multidirectional movements and unilateral landings that impose substantial mechanical stress on the foot-ankle complex. Athletes with pes planus are at an increased risk of instability and injury due to altered kinetic chain function. While short foot exercises (SFE) enhance medial longitudinal arch (MLA) integrity in individuals with flat feet, core stability training (CST) is increasingly incorporated into sports rehabilitation programs. However, the effects of CST on pes planus remain insufficiently explored. This study investigates the combined effects…

Mechanobiological and neuromuscular responses to foot-position variations during front and back squat exercises.

INTRODUCTION: This study investigated the mechanobiological and neuromuscular effects of three foot-position techniques-flat heel (FH), heel-elevated (HE), and forefoot-elevated (FE)-during front and back squat exercises. Variations in foot positioning were expected to influence muscle activation, joint kinematics, ground reaction forces, and postural stability. METHODS: Twelve resistance-trained male athletes (mean age 23.4 ± 3.8 years; training experience 8.1 ± 2.7 years) performed squats at 70% of their one-repetition maximum (1RM) under each foot-position condition. Surface electromyograp…

Aggressive lumbar vertebral hemangioma presenting with foot drop mimicking spinal tuberculosis: A case report.

BACKGROUND: Aggressive vertebral hemangiomas are uncommon vascular lesions that may present with epidural extension and neurological deficit. In tuberculosis-endemic regions, atypical vertebral hemangiomas may mimic spinal tuberculosis, resulting in diagnostic confusion and delay in definitive management. CASE DESCRIPTION: A 20-year-old male presented with a 1-year history of left lower-limb radiculopathy followed by progressive left foot drop for 1 month. The patient had previously received empirical antitubercular therapy without clinical improvement. Magnetic resonance imaging demonstrated…

Reporting of social demographics in diabetic foot ulcer randomized controlled trials: a scoping review.

BACKGROUND: Diabetic foot ulcerations (DFUs) remain a major public health issue, disproportionately affecting diverse populations. The extent to which race, ethnicity, and social demographics are reported in randomized controlled trials (RCTs) on graft treatments for DFU remains unclear. OBJECTIVE: To assess the reporting frequency of these patient characteristics and social determinants of health in order to provide insight into providing more representative, evidence-based care. METHODS: Following Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines, a sco…

Foot orthoses alter kinematics in runners with patellofemoral pain: a statistical parametric mapping analysis.

Clinical guidelines recommend foot orthoses for patients with patellofemoral pain (PFP) and excessive subtalar pronation. Although Statistical Parametric Mapping (SPM) is established for analysing continuous kinematic waveforms in biomechanics, foot orthoses research in PFP frequently relies on discrete variables (e.g. peak angles). Applying SPM allows detailed assessment of movement adaptations throughout the stance phase that isolated data points may overlook. This study evaluated the immediate effects of medially wedged orthoses on lower limb kinematics, pain, and self-perceived improvement…

Morphological Similarities of Chronic Ankle Instability and Cavus Foot Type Using Statistical Shape Modeling.

Chronic ankle instability (CAI) can develop in up to 40% of patients after a first-time ankle sprain. Advanced imaging and statistical shape modeling (SSM) provide the opportunity to evaluate if subtle differences in foot and ankle morphology, such as hindfoot alignment or cavus features, may contribute to structural predispositions for recurrent instability. In this study, a 14-bone SSM was created from weight-bearing CT data (n = 80) from patients with CAI (n = 23), cavus foot type (n = 29), and rectus (n = 28) foot type. Scans for the CAI group were from pre-operative imaging of patients un…

Clinical validity of radiographic parameters for evaluating pediatric foot deformities: correlation with pedobarographic analysis.

BACKGROUNDS: Pediatric foot deformities disrupt biomechanics and can lead to long-term musculoskeletal complications. Radiographic parameters, particularly the anteroposterior talo-1st metatarsal angle (AP talo-1MT), are widely used; however, their accuracy may be compromised in the presence of hallux valgus. The clinical utility of the anteroposterior talo-2nd metatarsal angle (AP talo-2MT) has not been well established. This study evaluated radiographic parameters by correlating them with pedobarographic indices, hypothesizing that AP talo-2MT would demonstrate comparable or superior functio…

Advances in Wound Healing Strategies for Diabetic Foot Ulcers: From Conventional to Regenerative Therapies.

Diabetic Foot Ulcer (DFU) is a serious and chronic complication of uncontrolled diabetes, significantly increasing the risk of amputation and mortality in diabetic patients. Conventional wound healing techniques often fail to manage chronic wounds in DFU due to several limitations, which eventually raises the need for advanced, innovative, and efficient treatment strategies for the better management of DFU. Many novel interventions, such as growth factor therapy, stem cell therapy, gene therapy, nanotechnology, bioengineered skin substitutes, 3D-bioprinting, and regenerative medicine, have sho…

[Offloading and immobilization therapy for diabetic foot: Germany experience from theory to clinical practice].

Diabetic foot is a common condition that imposes a substantial burden on patients and society. Although prolonged activity restriction in traditional management may facilitate ulcer healing, it is detrimental to overall health. Therefore, a balance must be achieved between promoting wound healing and preserving mobility. Offloading and immobilization constitute the core principles of diabetic foot management. Offloading reduces wound stress by redistributing plantar pressure, whereas immobilization promotes local healing by limiting joint motion. To effectively balance these two therapeutic pr…

Low-level laser therapy and conventional treatment for diabetic foot ulcers: a case series.

Diabetic foot ulcers (DFUs) are a serious complication of diabetes mellitus and are associated with prolonged healing, increased hospitalization, and risk of amputation. Low-level laser therapy (LLLT), a non-invasive and low-cost modality, has shown potential to accelerate wound healing by promoting tissue repair, cellular proliferation, and improved microcirculation. This study aimed to descriptively evaluate healing outcomes in patients with DFUs treated with LLT combined with conventional treatment care with conventional treatment alone. This prospective observational comparative study incl…

Mechanistic Evaluation of Luteolin in Wound Healing of Diabetic Foot Ulcer via Nrf2 Signaling.

Diabetic foot ulcers (DFUs) are a leading cause of lower-limb amputation, with oxidative stress and ferroptosis playing critical roles in impaired wound healing. This study aimed to evaluate the therapeutic potential of luteolin (LU), a naturally occurring plant-derived flavonoid, in DFU repair and to elucidate its underlying molecular mechanisms. A DFU rat model was established using a high-fat/high-glucose diet combined with streptozotocin injection, followed by full-thickness skin wounding. LU was topically administered at different concentrations. Wound closure was monitored macroscopicall…
