OBJECTIVE: To investigate the treatment and clinical efficacy of postoperative plate fracture and in situ fracture of the femoral stem.
METHODS: We have retrospectively analyzed the clinical data, revised surgery information, and clinical efficacy of patients with postoperative plate fracture of the femoral stem in our hospital. A total of 33 cases were included whose original fractures were located in the upper and cadaveric femur and treated with paralleling intramedullary pins for revision surgery, as well as patients whose original fractures were located in the lower femur which were fixe

OBJECTIVE: To evaluate the functional outcome and the complication rate after open reduction and internal fixation of proximal humeral fractures with a "time-angle measurement" reduction evaluation technique, to assess the acceptance of the fracture reduction and to estimate the result of the sur-gery.
METHODS: Forty-six patients [mean age: (66.2±14.9) years] with an acute proximal humeral fracture were managed with open reduction and internal fixation with this reduction evaluation technique from January 2012 to December 2013. According to the Neer classification, there were 6 two- part frac

BACKGROUND: Hip fracture is associated with high mortality. Cardiovascular disease and other comorbidities requiring long-term anticoagulant medication are common in these mostly elderly patients. The objective of our observational cohort study of patients undergoing surgery for hip fracture was to study the association between preoperative use of low-dose acetylsalicylic acid (LdAA) and intraoperative blood loss, blood transfusion and first-year all-cause mortality.
METHODS: An observational cohort study was conducted on patients with hip fracture (cervical requiring hemiarthroplasty or pert

BACKGROUND: Sustentaculum tali fractures are rare foot fractures accounting for 1% of all calcaneal fractures, which accounts for about 1-2% of all bone fractures. Since the importance of sustentaculum tali in maintaining the medial foot arch, its fractures should be dealt with anatomical fixation to avoid post-traumatic arthrosis.
OBJECTIVE: Is to delineate the importance of sustentacular tali fractures fixation mentioning operative technique and the post-operative outcome.
METHODOLOGY: Ten cases with 10 consequent closed sustentaculum tali fractures were treated by ORIF. Nine of them were

INTRODUCTION: Pertrochanteric femur fracture fixation with use of cephalomedullary nails (CMN) has become increasingly popular in recent past. Known complications after fracture consolidation include peri‑implant fractures following the use of both short and long nails, with fracture lines around the tip of the nail or through the interlocking screw holes, resulting in secondary midshaft or supracondylar femur fractures, respectively. Limited research exists to help the surgeon decide on the use of short versus long nails, while both have their benefits. The aim of this biomechanical study is

Complex pelvic fractures are infamously challenging to fix surgically because of their fine anatomy and proximity to vital neurovascular structures. Traditional open reduction and internal fixation (ORIF) improves stability but is complicated by excessive blood loss, longer operative time, and morbidity. Robotic-assisted surgical methods, i.e. Robot-Assisted Fracture Reduction (RAFR) and the TiRobot platform, provide a paradigm shift toward precise, minimally invasive fracture reduction and fixation. The RAFR system blends preoperative high-definition 3D CT imaging with intraoperative cone-bea

Tourniquet-related nerve injury is an uncommon but clinically important complication of extremity surgery. Postoperative radial nerve palsy after humeral shaft fixation is usually attributed to fracture-related injury or surgical manipulation, particularly when a posterior approach is used. We present a case of transient radial-dominant neuropathy following the use of a sterile exsanguination tourniquet during the posterior plating of a distal humeral shaft fracture. A 27-year-old male construction worker with an AO/OTA (Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Associatio

Each year, millions of Americans suffer bone fractures, often requiring internal fixation. Current devices, like plates and screws, are made with permanent metals or resorbable polymers. Permanent metals provide strength and biocompatibility, but cause long-term complications and may require removal. Resorbable polymers reduce long-term complications, but are unsuitable for many load-bearing applications. To mitigate complications, degradable magnesium (Mg) alloys are being developed for craniofacial and orthopedic applications. Their combination of strength and degradation make them ideal for

BACKGROUND: Heterotopic ossification (HO) is a serious complication of traumatic elbow injuries, particularly fracture-dislocations. Limited data exist in the literature regarding the risk factors associated with HO formation in these injuries. The purpose of this study was to review the incidence of HO after fracture-dislocation of the elbow and to identify potential risk factors associated with its formation.
METHODS: Twenty-seven patients (28 elbows) were surgically treated for elbow fracture-dislocations during 8 years, with an average follow-up of 14 months. Records were reviewed with at

INTRODUCTION: Distal radius fractures constitute one of the most prevalent upper extremity orthopedic injuries, predominantly resulting from falls onto an outstretched hand with an extended wrist. Despite volar locking plate fixation emerging as the gold standard treatment for distal radius fractures, it is associated with well-documented complications, such as flexor pollicis longus (FPL) tendon irritation and even rupture. The surgical management of FPL tendon rupture following volar plate fixation poses significant challenges due to hardware-induced tendon attrition and compromised surgical

Anteromedial coronoid facet fractures typically occur with varus, pronation, and axial forces applied to the elbow. Due in part to the high rate of concomitant lateral collateral ligament (LCL) injuries, untreated anteromedial facet fractures can result in varus and posteromedial rotatory elbow instability. Although small fractures that are not amenable to open reduction and internal fixation can be treated with isolated LCL repair, larger fragments are treated with buttress plating on the anteromedial surface of the coronoid with or without LCL repair. The "over-the-top" approach via a split

Extensor mechanism deficiency in the knee may occur due to neglected patellar and quadriceps tendons rupture or may be caused by chronic fractures of the patella. Older patients can tolerate nonunion with impaired function including extension limitation or persistent muscle weakness. In young patients, performing rigid internal fixation with reoperation should be considered when a nonunion occurs. However, delayed and neglected nonunion in patella fractures require performing different surgical procedures. We report two cases, operated for a patella fracture, in whom nonunion occurred and acco

Isolated fractures of the capitellum are rare injuries and account for 1% of all elbow injuries. The purpose of this study is to evaluate the clinical outcomes of 11 Type I capitellum fractures treated by open reduction and internal fixation using at least two standard Herbert screws between 1998 and 2003. The average age of the patients was 27.5 years. The mean follow-up time was 23.4 months. The final postoperative assessment was made at the 12th month. The results were evaluated according to the Mayo Elbow Performance Index. We obtained excellent result in eight patients and good result in

AIMS: To investigate whether there are extractable conclusions for limb fracture management in dependent alcoholics.
METHODS: We discuss four cases of dependent alcoholics who presented in our department over a 12-month period, and who developed significant complications owing to non-compliance with treatment.
RESULTS: Initial treatment, although appropriate, failed because of non-compliance. This led to further admissions, wound infections and surgery to enable cure.
CONCLUSIONS: Our case reports indicate that for upper limb fractures of the middle third of the humerus, non-operative treat

The authors evaluate retrospectively in a group of 121 children with a dislocated supracondylar fracture in 1984-1993 the results of different therapeutic methods. The highest percentage of excellent and satisfactory results was recorded in children treated by closed reposition with percutaneous pinning with Kirschner wires (88%). In open reposition with Kirschner wires 60% of the results were excellent of satisfactory. The lowest percentage of excellent and satisfactory results was recorded when fixation with a plaster bandage was used (48%). The highest percentage of poor results was observe

Forty-eight pathologic and impending pathologic fractures of the proximal femur were surgically treated in 38 patients averaging 70 years of age. The most common tumors were breast (45%), multiple myeloma (24%), and lung (11%). Tumor size, aggressiveness, and location influenced the type of fixation. Sixty-four percent of the lesions were treated by flexible intramedullary nails. This "stress sharing" device afforded sufficient stability to provide pain relief, restore function, and permit healing. Pain was relieved in 92% of the patients, and 84% became ambulatory. The rate of fracture union

In Orthopedics, damage control is indicated in patients with pelvic and/or long bone fractures associated with hemodynamic instability. It is inappropriate to perform a complex definitive reduction and fixation surgery for severely injured trauma patients with hemodynamic instability. In these cases, it is recommended to perform minimally invasive procedures that temporarily stabilize the fractures and bleeding control. Closed or open fractures of the long bones such as femur, tibia, humerus, and pelvis can lead to hemodynamic instability and shock. Thus, orthopedic damage control becomes a pr

Calcaneal fractures are very common injuries, and closed reduction-percutaneous pinning, open reduction-internal fixation, and primary arthrodesis are the procedures used in the surgical treatment of these injuries. The aim of surgical treatment is to restore the normal biomechanics of the Achilles tendon and rearfoot by anatomic reduction of the articular surfaces. Minimization of the soft tissue complications commonly associated with the open treatment of calcaneal fractures can be achieved using indirect closed reduction and Kirschner wire or screw-assisted minimally invasive percutaneous o

We report a 38-year-old man with a combination of fractures in his left shoulder involving the middle and distal clavicle, the scapular neck, and the coracoid process, associated with scapulothoracic dissociation and an upper brachial plexus injury. The middle clavicular fracture was fixed with a plate, whereas the distal clavicular fracture was fixed with tension band wiring. Internal fixation for fractures of the scapular neck and coracoid process was not performed, as they were in an acceptable position after fixation of the clavicular fractures. At the 14-month follow-up, the range of moti

Proximal humeral fractures account for 4% to 5% of all fractures. Most of these fractures are nondisplaced or minimally displaced and amenable to nonsurgical treatment or open reduction and internal fixation. Complex proximal humeral fractures with displaced three- and four-part fragments, fracture-dislocations, and humeral head splits are more difficult to treat. In older patients, hemiarthroplasty or reverse shoulder arthroplasty is often the indicated treatment. Arthroplasty in this patient cohort is very technique-dependent and relies on preserving deltoid function, proper component placem
