OBJECTIVE: In see-and-treat protocols, women referred to colposcopy are treated with a loop electrosurgical excision procedure at the first visit when findings suggest high-grade cervical intraepithelial neoplasia (CIN), making accurate colposcopic diagnosis essential. This systematic review evaluates colposcopy guidelines and supporting evidence to identify macroscopic indicators that should guide patient selection for see-and-treat management to minimize over- and undertreatment.
METHODS: TRIP and Google Scholar were searched in January 2021 to identify guidelines describing colposcopic ind

BACKGROUND: Recent years have seen significant changes in the management approach for hypospadias in Germany, influenced by legislative modifications (German Civil Code §1631e), as proximal hypospadias are listed as DSD 46 X, Y, resulting in a family court decision prior to surgery. According to the revised clinical guidelines (S2k guideline, first published 2016, updated 2021), surgical treatment is now mainly indicated for proximal and middle hypospadias.
OBJECTIVE: To analyze trends in hypospadias diagnoses and surgical treatments in Germany from 2006 to 2024, focusing on potential impacts

INTRODUCTION: Hypertension is a major modifiable risk factor for stroke. Studies between 1996 and 2013 reported high rates of non-achievement of guideline recommended blood pressure (BP) targets following stroke/TIA. Few recent large studies of long-term post-stroke BP management exist, and no studies have reported findings since the introduction of new guideline recommendations (systolic blood pressure [SBP] < 130 mmHg) in 2022.
PATIENTS AND METHODS: We analysed BP control in the CONVINCE trial, which randomised 3154 recent ischaemic stroke/high-risk TIA patients to colchicine 0.5 mg daily o

Angina in non-obstructive coronary artery disease (ANOCA) is a common clinical condition affecting 20-50% of patients undergoing coronary angiography for suspected angina. Despite recent 2024 European guidelines on chronic coronary syndrome emphasizing its significance and clinical implications, ANOCA remains often underdiagnosed and undertreated. The management of ANOCA differs from obstructive coronary artery disease and requires careful interpretation of diagnostic tests and tailored treatment based on accurate clinical assessment. ANOCA diagnosis may involve microvascular dysfunction or va

A recent review of 11 international clinical guidelines for borderline personality disorder (BPD) and its features in adolescents, found broad agreement on core recommendations: structured assessment, psychotherapy as first-line treatment and family involvement. This is a meaningful step forward. But it raises a question the guidelines themselves do not answer: If clear recommendations exist, why are so many adolescents still not receiving a timely diagnosis? This commentary focuses on that gap. Clinicians frequently hesitate to diagnose BPD in young people, often out of concern about stigma o

Congenital hyperinsulinism (CHI) is a rare, genetically heterogeneous disorder characterized by inappropriate hypersecre tion of insulin by pancreatic beta cells and is the most common cause of persistent hypoglycemia in young children. The in cidence of CHI in the general population ranges from 1:30,000 to 1:50,000 live births. Timely diagnosis and treatment of CHI are critical, as persistent hypoglycemia can lead to irreversible damage to the central nervous system. This review summa rizes current understanding of the etiology and pathogenesis of CHI and presents an up-to-date classification

INTRODUCTION: Minimally invasive surgical therapies (MISTs) for benign prostatic hyperplasia (BPH) are increasingly favored for their efficacy in symptom relief and reduced morbidity. Prostatic urethral lift has gained traction in the last decade due to its non-ablative properties and favorable clinical outcomes.
AREAS COVERED: In this article, we provide a narrative review including articles prior to July 2026 focusing on prostatic urethral lift technology. We provide a brief technical overview of the implant and implantation process, outlining key differences in the first- and second-genera

Chronic kidney disease (CKD) is closely intertwined with obesity, diabetes, hypertension, dyslipidemia, and cardiovascular disease. In 2023, the American Heart Association formally recognized these interconnections as a unified entity, the cardiovascular-kidney-metabolic (CKM) syndrome. The CKM syndrome brings renewed attention to the importance of CKD in cardiovascular disease and reinforces the need for effective, evidence-based, interdisciplinary approaches to diagnose and prevent its intersecting components. The Kidney Disease: Improving Global Outcomes (KDIGO) organization has long led ef

Although Sjögren's disease (SjD) is the most frequent connective tissue disease, there are still no approved medications for systemic treatment. Many disease-modifying antirheumatic drugs (DMARD) and biologicals have not shown sufficient efficacy in studies. The reasons for this are the complex immunological mechanisms and a high degree of clinical and immunological heterogeneity of the patients; however, significant progress has been made in recent years: for the first time 2 phase 3 studies (ianalumab and telitacicept) reported positive results. The current treatment recommendations are base

Thyroid eye disease (TED), the most common extrathyroidal manifestation of Graves' disease, requires individualized management that integrates medical, radiotherapeutic, and surgical interventions. In this article, we present a society-endorsed, survey-informed Korean clinical practice framework, including consensus elements, for TED management. This framework integrates the 2022 American Thyroid Association/European Thyroid Association consensus, the 2021 European Group on Graves' Orbitopathy (EUGOGO) guideline, and contemporary Korean specialist practice patterns. A 2025 nationwide survey of

PURPOSE OF REVIEW: The 2025 ESC/EACTS Guidelines for valvular heart disease introduce substantial changes in timing and modality of intervention, making a focused appraisal of their surgical implications highly relevant for contemporary cardiac practice.
RECENT FINDINGS: The guidelines formalize imaging-based staging of cardiac damage, lower thresholds for intervention in aortic stenosis and primary mitral regurgitation, and refine age- and risk-stratified allocation of surgery vs. transcatheter therapies across aortic, mitral, and tricuspid disease. They expand indications for transcatheter

Maternal sepsis remains a major and largely preventable contributor to maternal morbidity and mortality, with intra-amniotic infection and postpartum endometritis remaining common obstetric sources of sepsis. Intra-amniotic infection complicates approximately 2%-5% of term deliveries, and postpartum endometritis occurs in up to 7% of births. Both conditions are typically polymicrobial, arising from ascending infection involving gram-positive, gram-negative, and anaerobic organisms. Despite rising antimicrobial resistance, the recommended antibiotic regimens for obstetric infections have remain

Cardiac sarcoidosis is a rare but potentially life-threatening condition characterized by the formation of non-caseating granulomas in the myocardium. Clinical manifestations range from asymptomatic forms to atrioventricular blocks, ventricular arrhythmias, heart failure, and sudden cardiac death. Diagnostic work-up requires a multimodality approach combining advanced imaging, clinical criteria, and, when possible, histological confirmation. Immunosuppressive therapy remains the cornerstone of treatment, aimed at suppressing myocardial inflammation and preventing irreversible damage. Risk stra
