The pivotal role and intervention strategies of BMAL1 mediated circadian clock dysregulation in intervertebral disc degeneration.

BACKGROUND: Intervertebral disc degeneration (IVDD), a primary cause of chronic low back pain, involves extracellular matrix (ECM) degradation and nucleus pulposus cell apoptosis. While traditionally linked to mechanical stress, inflammation, oxidative stress, and metabolic dysfunction, emerging evidence positions circadian clock disruption as the central hub integrating these factors. Under physiological conditions, the core clock genes BMAL1/CLOCK regulate ECM homeostasis. RESULTS: In IVDD, however, this rhythm is disrupted: abnormal mechanical stress inhibits BMAL1 via the RhoA/ROCK pathwa

Phytochemicals in arthritis management: insight into pharmacodynamic potency with pharmacokinetic profile.

Arthritis, especially osteoarthritis (OA) and rheumatoid arthritis (RA), is a chronic, multifactorial disorder characterized by ongoing inflammation, oxidative stress, immune dysregulation, cartilage degradation and abnormal bone remodeling. These factors significantly contribute to global disability. Traditional treatments, including nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroids, disease-modifying antirheumatic drugs (DMARDs), and biologics can provide symptomatic relief and immunosuppression. However, their long-term use is often limited due to adverse effects and insufficie

Therapeutic Strategies for Endothelial Dysfunction in Cardiovascular Disease, Rheumatoid Arthritis, and Diabetes: Current and Emerging Approaches.

The endothelium forms the inner vital lining of blood vessels and controls vascular tone, hemostasis, and inflammation, maintaining cardiovascular homeostasis. Endothelial dysfunction, characterized by abnormal vasodilation, increased oxidative stress, and decreased nitric oxide bioavailability, is a key pathophysiological mechanism underlying many cardiovascular diseases. The review examines current approaches for addressing endothelial dysfunction in various disease states, with a special emphasis on cardiovascular disease, rheumatoid arthritis, and diabetes mellitus. We performed a thorough

PRDM16 protects retinal ganglion cells from ischemia-reperfusion Injury by regulating the RPL5/p53 axis.

Ischemia-reperfusion (I/R) injury represents a prevalent etiology of permanent vision loss across multiple ocular pathologies. Apoptotic death and inflammation of retinal ganglion cells (RGCs) induced by I/R injury constitutes a major contributor to visual dysfunction. Nevertheless, the molecular mechanisms that initiate the apoptotic and inflammation cascade in RGCs remain largely uncharacterized. Recent investigations have demonstrated that PRD1-BF1-RIZ1 homeodomain protein 16 (PRDM16) attenuates renal cell death in acute kidney injury. However, its specific function and regulatory mechanism

Systemic lupus erythematosus-driven accelerated atherosclerosis: the immune-metabolic-vascular axis and therapeutic implications.

Patients with systemic lupus erythematosus (SLE) are at markedly increased risk of premature atherosclerosis (AS) and atherosclerotic cardiovascular disease (ASCVD), and this excess risk is not fully explained by traditional Framingham factors. Increasing evidence suggests that SLE does not merely coexist with AS; rather, persistent immune activation and immunometabolic dysregulation reshape the vascular microenvironment toward endothelial dysfunction, lipoprotein impairment, maladaptive myeloid activation, and immunothrombosis. This review synthesizes current epidemiologic, mechanistic, and t

Treatment of pediatric chronic nonbacterial osteomyelitis - a systematic review.

Chronic nonbacterial osteomyelitis (CNO) is an autoinflammatory bone disease most commonly affecting children and young people. CNO can cause bone pain, hyperostosis and fractures, thereby significantly impacting on patients' wellbeing. The molecular pathophysiology of CNO is characterized by NLRP3 inflammasome activation and a pronounced imbalance between pro- and anti-inflammatory cytokines. In the absence of clinical trials, treatment of CNO remains empiric and is based on personal experience and published case series. This project systematically reviewed the available literature in pediatr

From Bench to Bedside: Personalized Genomics in the Diagnosis and Treatment of Osteomyelitis.

Osteomyelitis (OM), an inflammatory condition of the bone tissue, is a complex orthopedic condition marked by chronic inflammation, diagnostic uncertainty, and recurrent infections. Despite standard treatments-including surgical debridement, antimicrobial therapy, and bone reconstruction-many patients continue to experience recurrence and treatment failure. Growing molecular evidence indicates that host genetic factors play a crucial role in shaping immune responses and influencing disease progression in OM. This narrative review synthesizes current knowledge from candidate gene single-nucleot

SLC31A1 knockdown mitigates post-MI heart failure via regulation of copper metabolism.

INTRODUCTION: Cuproptosis due to copper overload is a contributor to the progression of cardiovascular diseases, especially heart failure (HF) after acute myocardial infarction (AMI). Solute carrier family 31 member 1 (SLC31A1) is a major Cu2+ transporter responsible for intracellular Cu2+ uptake. In this study, we investigated the role and detailed mechanism of SLC31A1 in post-AMI HF. METHODS: Mouse left anterior descending coronary artery was ligated to produce an in vivo post-AMI HF model. These mice were subjected to treatments with short hairpin RNA targeting SLC31A1, the copper chelator

The diagnostic value and pathological application of TREM2 in patients with osteoarthritis.

BACKGROUND: Osteoarthritis (OA) is a prevalent degenerative joint disease with limited diagnostic biomarkers. Triggering receptor expressed on myeloid cells 2 (TREM2) is a key regulator of inflammation and bone metabolism, but its role in OA remains unclear. METHODS: Plasma and synovial fluid samples were collected from patients with OA and patients with joint trauma serving as controls. Levels of TREM2 and IL-6 were measured using RT-qPCR and ELISA. Correlation analyses were conducted to assess relationships among TREM2, IL-6, and clinical parameters. The diagnostic value of TREM2 was evalua

Signaling pathways and ion channels in osteoarthritis: a review of recent advances.

The understanding of osteoarthritis (OA) has gradually evolved from the traditional perspective of it being a mere "wear-and-tear" joint disease to a more comprehensive view that characterizes it as a degenerative joint disease marked by chronic low-grade inflammation and immune imbalance. The progression of OA is closely linked to abnormal mechanical loading and the activation of the innate immune system. This narrative review examines the role of mechanosensory-immune signaling networks in OA, with a specific focus on ion channels as receptors for upstream mechanical and chemical stimuli. Th

MAFB in immunity and disease: An evidence-graded perspective on context-dependent immune regulation.

MAFB is a large Maf family bZIP transcription factor involved in myeloid differentiation, macrophage-state regulation, inflammatory control, and tissue adaptation. Accumulating evidence indicates that MAFB biology is highly context dependent rather than uniformly anti-inflammatory, pro-inflammatory, protective, or pathogenic. In macrophages and related myeloid cells, MAFB has been linked to inflammasome regulation, lipid-mediator switching, efferocytosis, tissue repair, and cell-state transitions. Across diseases, its interpretation depends on cellular source, disease stage, tissue niche, pert

Biomarkers in Chronic Nonbacterial Osteomyelitis: Bridging Autoinflammation and Classical Inflammatory Diseases.

Chronic nonbacterial osteomyelitis (CNO) is a rare autoinflammatory bone disorder characterized by sterile bone lesions, heterogeneous clinical presentation, and the absence of validated diagnostic or prognostic biomarkers. Traditionally considered a distinct disease entity, emerging genetic, mechanistic, and translational evidence challenges this view. Emerging genetic, mechanistic, and translational evidence suggests that CNO may represent a convergent clinical phenotype of sterile bone inflammation arising from diverse disturbances in innate immunity. Central to its pathogenesis is the infl

Mitochondrial-ER stress crosstalk in osteoarthritis: From ageing-associated chondrocyte dysfunction to emerging therapeutics.

Osteoarthritis (OA) is a common age-associated joint disorder driven not only by mechanical wear but also by progressive intracellular stress, metabolic imbalance, and chronic inflammation that culminate in cartilage degeneration and functional disability. Increasing evidence identifies mitochondrial dysfunction and endoplasmic reticulum stress (ERS) as central pathological hubs regulating chondrocyte survival, extracellular matrix (ECM) integrity, and inflammatory signaling. Mitochondrial impairment promotes excessive reactive oxygen species (ROS) generation, defective ATP production, disturb

Elevated sympathetic tone exacerbates myocardial ischemia reperfusion injury through enhancing endothelial cell inflammation and apoptosis in chronic pain mice.

Although extensive evidence links chronic pain (CP) to an increased cardiovascular risk, its direct impact on myocardial ischemia-reperfusion (MI/R) remains poorly understood. In mice, CP was induced using chronic constriction injury (CCI) of the brachial plexus. We found that CP significantly increased myocardial infarction area, endothelial cell apoptosis, and inflammation, accompanied by the increase in sympathetic nerve tone around the endothelial cells. Viral tracer results demonstrated the existence of anatomical connections between the superior cervical ganglion (SCG) and the heart. The

The Effects of CB2R Activation on Inflammatory Pathways in Dermatomyositis.

Background/Objectives: Dermatomyositis is an autoimmune disease with heterogeneous symptoms and many potential drivers. Nonpsychoactive cannabinoids have shown promise in treating some subtypes of DM; however, the reasons behind this were unclear. In this project, we tested the effects of CB2R activation on PBMCs from amyopathic and classic DM patients to determine its anti-inflammatory effects on pathways biologically relevant to DM. Methods: We determined the % CB2R positivity and intracellular cytokines in PBMCs from amyopathic DM and classic DM patients. CB2R positivity was determined by a

Antithrombin in Hyperuricemic Nephropathy: Unveiling Its Noncanonical Renoprotective Roles and Therapeutic Repurposing Potential.

The pathogenesis of hyperuricemic nephropathy (HN) is increasingly linked to a self-perpetuating cycle of thromboinflammation. This process begins with hyperuricemia-induced endothelial dysfunction, which activates both the coagulation cascade and inflammatory pathways, notably the pyrin domain-containing protein 3 inflammasome. This creates a vicious cycle of which inflammation promotes coagulation and coagulation products further amplify inflammation, ultimately leading to renal microvascular thrombosis, ischemia, and progressive fibrosis. This review critically examines the therapeutic pote

Evaluation of gossypetin's effects on gut microbiota profile and TLR4, Myd88, NFKB, and NLRP3 signaling pathways in rats.

Gut microbiota plays a crucial role in maintaining host homeostasis by regulating metabolic processes and immune responses. Disruptions in microbial composition are closely associated with inflammatory diseases and are often linked to the activation of key signaling pathways such as Toll-like receptor 4/myeloid differentiation primary response 88/nuclear factor kappa TLR4/MyD88/NF-κB and NLR family pyrin domain-containing 3 (NLRP3) inflammasome. Natural bioactive compounds, particularly flavonoids, have gained attention due to their potential to modulate both gut microbiota and inflammation-re

Medicinal Plant Polysaccharides as Microbiota-Directed Modulators of Immunosenescence: Structural Determinants, Metabolite Reprogramming, and Host Immune Regulation.

Immunosenescence is a major contributor to age-associated morbidity, yet microbiota-directed strategies capable of restoring immune homeostasis remain insufficiently validated. Medicinal plant polysaccharides (MPPs) are structurally diverse macromolecules that often resist host digestion and undergo microbial transformation in the colon, but their effects cannot be interpreted as those of a homogeneous intervention class. In this structured narrative review, we critically synthesize evidence across structural carbohydrate biology, microbial ecology, metabolite signaling, and immune aging and p

Epilepsy is a chronic neurological disorder characterized by recurrent seizures, neuroinflammation, and epigenetic instability, with around one-third of patients experiencing drug-resistant epilepsy. While ion channels and neurotransmitter imbalances have been thoroughly examined in relation to epilepsy, the significance of piRNAs and their interaction with PIWI proteins in gene regulation, transposon silencing, and inflammatory signaling inside the epileptic brain is gaining recognition. This summary encapsulates the existing understanding of piRNA synthesis, the epigenetic control through PI

Oxidative Stress and Alcohol-Related Hepatitis: A Role for Future Therapies.

Alcohol-associated Hepatitis (AH) is a rare acute injury caused by alcohol consumption, which can lead to one of the most severe manifestations of liver disease. It is part of the alcohol-related liver diseases (ArLD) spectrum, which represents a major global health burden, with oxidative stress and inflammation serving as central, interconnected pathogenic mechanisms. Chronic alcohol (ethanol) consumption induces hepatic reactive oxygen species (ROS) generation through multiple pathways, including cytochrome P450 2E1 (CYP2E1) induction, mitochondrial dysfunction, and NADPH oxidase activation.
