Acute and chronic effects of hypercalcaemia on cortical excitability as studied by 5 Hz repetitive transcranial magnetic stimulation.

We designed the present study to disclose changes in cortical excitability in humans with hypercalcaemia, by delivering repetitive transcranial magnetic stimulation (rTMS) over the primary motor area (M1). In 22 patients with chronic hypercalcaemia related to primary hyperparathyroidism and 22 age-matched healthy subjects 5 Hz-rTMS was delivered at rest and during a sustained voluntary contraction of the target muscle. Changes in the resting motor threshold (RMT), motor evoked potential (MEP) amplitudes and cortical silent period (CSP) duration were measured and compared in patients and health

Changes in corticospinal excitability evoked by common peroneal nerve stimulation depend on stimulation frequency.

The afferent volley generated during neuromuscular electrical stimulation (NMES) can increase the excitability of the human corticospinal (CS) pathway. This study was designed to determine the effect of different frequencies of NMES applied over the common peroneal nerve on changes in CS excitability for the tibialis anterior (TA) muscle. We hypothesized that higher frequencies of stimulation would produce larger increases in CS excitability than lower frequencies. NMES was applied at 10, 50, 100, or 200 Hz during separate sessions held at least 48 h apart. The stimulation was delivered in a 2

Mirror movements and ipsilateral motor evoked potentials in ALS.

It can be difficult to detect upper motor neuron (UMN) involvement in early amyotrophic lateral sclerosis (ALS). The present study tested the usefulness of mirror movements (MMs), i.e., contralateral coactivation of hand muscles, as a UMN sign in ALS. Thirty-seven patients with possible, probable or definite ALS and 19 patients with suspected ALS without clinical signs of UMN involvement, 21 disease controls and 15 healthy volunteers were included. MMs were studied clinically, electromyographically and by transcranial magnetic stimulation (TMS), looking for ipsilateral motor evoked potentials

Changes in the alpha and beta amplitudes of the central EEG during the onset, continuation, and offset of long-duration repetitive hand movements.

Electroencephalographic alpha and beta activities recorded from central electrodes are known to display movement-related suppression or enhancement. We investigated whether the suppression that is known to occur during the onset of a single movement would persist or otherwise habituate when the movement is continuously repeated for a long period of time. Fourteen subjects took part in the experiments. They performed repetitive simultaneous extension-flexions of the fingers II-V in one hand, continuously for a period of at least 30 s. They then stopped this self-paced movement and rested for at

Two cases of quadriparesis following anterior cervical discectomy, with normal perioperative somatosensory evoked potentials.

Two cases illustrate an uncommon failure of perioperative somatosensory evoked potential (SEP) monitoring to detect iatrogenic lesions causing temporary quadriparesis during straightforward cervical surgery. In both cases, anterior cervical discectomy at one or two levels was undertaken with bone graft and titanium implants, and cortical SEP were monitored to alternate stimulation of the left and right median or ulnar nerves. The SEP showed only minor changes during surgery, not considered pathologically significant, and were normal when recorded postoperatively. Both patients, however, experi

Oscillatory activity in sensorimotor cortex of awake monkeys: synchronization of local field potentials and relation to behavior.

1. Oscillations of 20-40 Hz were observed in local field potentials (LFPs) and unit activity in sensorimotor cortices of three awake monkeys while the monkeys performed trained wrist movements and untrained exploratory arm movements. The mean frequency of LFP oscillations was 25.9 +/- 1.4 (SD) Hz and the number of cycles of oscillations per episode was variable, with a mean of 4.2 +/- 0.5 (mean +/- SE). 2. Oscillatory episodes occurred most often when the monkeys retrieved raisins from a Klüver board (0.59 +/- 0.23 episodes per s, mean +/- SD) or from unseen locations with the use of somatosen

Effects of aging on signal transmission and transduction systems in the gerbil brain: morphological and autoradiographic study.

The Mongolian gerbil was used as a model of aging because of its relatively short lifespan, genetic homogeneity and the fact that data had been collected previously. Furthermore, gerbils have been widely used in biomedical investigations of stroke and epilepsy. Age-related differences in signal transmission and transduction systems were investigated in brains of three-, 11- and 21-month-old gerbils by morphological and in vitro receptor autoradiographic studies. Morphometric analysis revealed a decreased number of neurons in layer III of the occipital cortex and also a decrease in cerebellar P

Effects of oropharyngeal exercises on orofacial function and corticomotor excitability in healthy individuals.

Oropharyngeal exercises/myofunctional therapy (OE/MT) offer complementary benefits for obstructive sleep apnea. However, the complex nature of the intervention impedes the understanding of the underlying mechanisms related to sensorimotor function and corticomotor control of involved muscles. This study explored the sensorimotor rehabilitation effects of OE/MT tasks in twenty-two healthy adults (24.1 ± 3.1 years) using a cross-over design to better understand the normal physiology of the tongue before and after these different tasks. Sessions involved breathing training (BT), tongue training (

Beta transcranial alternating current stimulation over the dorsal premotor area impairs response initiation but not inhibition in humans.

BACKGROUND: Response initiation and inhibition are core components of movement control, supported by a distributed frontal network including the dorsal premotor cortex (PMd) and inferior frontal gyrus (IFG). Converging evidence suggests that neural oscillations, particularly in the β- and γ bands, contribute to both response initiation and inhibitory control. Transcranial alternating current stimulation (tACS) enables non-invasive, frequency-specific modulation of cortical oscillations, offering a tool to causally probe these mechanisms. This study investigated whether β- and γ-tACS applied ov

Prior use-dependent plasticity triggers different individual corticomotor responses during persistent musculoskeletal pain.

Movement repetition is crucial for pain interventions. It facilitates the rehabilitation of motor patterns, the acquisition of motor skills, and the genesis of adaptive use-dependent plasticity. However, the influence of prior motor experience and preexisting use-dependent plasticity on pain severity and progression remains poorly investigated. This study investigated the effects of preexisting use-dependent plasticity during the development of prolonged experimental musculoskeletal pain. Using transcranial magnetic stimulation, corticospinal excitability was assessed by measuring the Rest mot

Differential modulation of corticomotor excitability in older compared to young adults following a single bout of strength -exercise.

Evidence shows corticomotor plasticity diminishes with age. Nevertheless, whether strength-training, a proven intervention that induces corticomotor plasticity in younger adults, also takes effect in older adults, remains untested. This study examined the effect of a single-session of strength-exercise on corticomotor plasticity in older and younger adults. Thirteen older adults (72.3 ± 6.5 years) and eleven younger adults (29.9 ± 6.9 years), novice to strength-exercise, participated. Strength-exercise involved four sets of 6-8 repetitions of a dumbbell biceps curl at 70-75% of their one-repet

Duration but not intensity influences transcranial direct current stimulation (tDCS) after-effects on cortical excitability.

Recent studies have shown heterogeneous results regarding the influence of intensity and duration of motor transcranial direct current stimulation (tDCS) on cortical excitability. In this pilot crossover study including 14 healthy participants, we compared the effects of a single session of anodal-tDCS set with two commonly used durations (20 and 30minutes) and intensities (1 and 2mA) on short-interval intracortical inhibition (SICI) and intracortical facilitation (ICF). Regardless of the current intensity, 20minutes of tDCS increased SICI (3ms inter-stimulus interval ISI) and decreased ICF (7

Supplementary motor area syndrome after surgery for parasagittal meningiomas.

BACKGROUND: Resection within the supplementary motor area (SMA) may be accompanied by dramatic motor deficits and speech arrest when the dominant hemisphere is involved, termed the SMA syndrome. Typically, the muscle tone of the paralyzed extremities is preserved, and in most cases, a complete or near complete recovery is seen within a few months. The SMA syndrome has not been recognized for extra-axial tumor surgery in approximation of the SMA. METHODS: We observed the SMA syndrome in a patient operated for a parasagittal meningioma in the posterior frontal region, and this observation intri

The influence of central neuropathic pain in paraplegic patients on performance of a motor imagery based Brain Computer Interface.

OBJECTIVE: The aim of this study was to test how the presence of central neuropathic pain (CNP) influences the performance of a motor imagery based Brain Computer Interface (BCI). METHODS: In this electroencephalography (EEG) based study, we tested BCI classification accuracy and analysed event related desynchronisation (ERD) in 3 groups of volunteers during imagined movements of their arms and legs. The groups comprised of nine able-bodied people, ten paraplegic patients with CNP (lower abdomen and legs) and nine paraplegic patients without CNP. We tested two types of classifiers: a 3 channe

Clarifying variability of corticomotoneuronal function in kennedy disease.

INTRODUCTION: Although Kennedy disease (KD) is regarded as a pure lower motor neuron disorder, recent transcranial magnetic stimulation (TMS) studies have reported subclinical corticomotoneuronal dysfunction in KD. To clarify these findings, this study applied novel triple stimulation (TST) and threshold tracking TMS techniques to gain further insights into corticomotoneuronal function in KD. METHODS: TMS studies were undertaken in 12 KD patients and 38 age-matched controls. Motor evoked potentials were recorded over the abductor pollicis brevis (threshold tracking TMS) and abductor digiti mi

Sex Differences in Progression of Neurodegeneration: The Age, Gene/Environment Susceptibility-Reykjavik Study.

INTRODUCTION: Advancing age is associated with global brain atrophy. Cross-sectional studies have found sex differences in neuroanatomy; however, longitudinal studies assessing sex differences in neurodegeneration are currently scarce. The effects of age and sex on brain atrophy may not be uniform across the whole brain and may partially explain the sex differences observed in dementia. The current study aimed to examine sex differences in longitudinal atrophy patterns in gray and white matter regions in older adults. METHODS: The study sample included 1,480 individuals from the Age, Gene/Env

Precise detection of awareness in disorders of consciousness using deep learning framework.

Diagnosis of disorders of consciousness (DOC) remains a formidable challenge. Deep learning methods have been widely applied in general neurological and psychiatry disorders, while limited in DOC domain. Considering the successful use of resting-state functional MRI (rs-fMRI) for evaluating patients with DOC, this study seeks to explore the conjunction of deep learning techniques and rs-fMRI in precisely detecting awareness in DOC. We initiated our research with a benchmark dataset comprising 140 participants, including 76 unresponsive wakefulness syndrome (UWS), 25 minimally conscious state (

Optimal Brain Targets for Enhancing Vocal Performance With Transcranial Direct Current Stimulation.

Transcranial direct current stimulation (tDCS) can be used to temporarily and reversibly modulate brain functions and is an increasingly utilized tool to investigate the relationships between brain and behavior. This study aimed to identify the brain area where tDCS has the greatest effect on vocal quality, vocal range, neuromotor functioning of the larynx, and self-reported vocal effort in vocally healthy individuals. Sixteen adults, non-singers, with a mean age of 22.5 (±1.9) years and no vocal symptoms or brain dysfunction, participated in this double-blinded randomized crossover clinical t

Cortical microstructural gradients capture memory network reorganization in temporal lobe epilepsy.

Temporal lobe epilepsy (TLE), one of the most common pharmaco-resistant epilepsies, is associated with pathology of paralimbic brain regions, particularly in the mesiotemporal lobe. Cognitive dysfunction in TLE is frequent, and particularly affects episodic memory. Crucially, these difficulties challenge the quality of life of patients, sometimes more than seizures, underscoring the need to assess neural processes of cognitive dysfunction in TLE to improve patient management. Our work harnessed a novel conceptual and analytical approach to assess spatial gradients of microstructural differenti

BACKGROUND: Benign epilepsy with centrotemporal spikes (BECTS) is one of the most common childhood epilepsy syndromes. The neural basis of BECTS is still poorly understood. This study aimed to further investigate the possible neural mechanisms of BECTS by comparing percent amplitude of fluctuation (PerAF) of resting-state functional magnetic resonance imaging (RS-fMRI) signal of each brain voxel and connectivity within and between related networks in children with BECTS and healthy controls (HCs). METHODS: Firstly, we used PerAF method to investigate brain functional alteration and defined th
