A rare seizure: A case of seizure-induced central acetabular fracture-dislocation in a patient with chronic kidney disease and hepatitis-B.

UNLABELLED: Seizure induced fractures are uncommon and usually involve long bone fractures like proximal humerus, proximal femur, shoulder dislocations etc. These occur due to intense muscle contractions or may result from indirect trauma post-seizure episode in the patient. Acetabulum fracture with central head dislocation, as seen in this case, is very rare following a seizure episode. It was further complicated by the presence of Hepatitis-B and dialysis dependent CKD both of which impact the surgical outcomes as well as pose challenges in the management of patient in the perioperative peri

Superior Efficacy of Denosumab Over Zoledronic Acid in Increasing Femoral Neck Bone Mineral Density in Osteoporosis Patients With Type 2 Diabetes Mellitus.

OBJECTIVES: This study aimed to compare the antiosteoporotic efficacy of zoledronic acid (ZOL) with denosumab (DEN) in osteoporosis patients with type 2 diabetes mellitus (T2DM). METHODS: This was a prospective, open-label, nonrandomized clinical study. Osteoporotic women with T2DM aged 50 to 80 years were enrolled and assigned to either the ZOL group (n = 45) or the DEN group (n = 75) based on patient preference. The efficacy endpoint included the percent change from baseline in bone mineral density (BMD), bone turnover markers, and the fracture risk evaluated by the fracture risk assessment

Seizure-Induced Periprosthetic Femoral Fracture After Total Hip Arthroplasty in a Patient With Epilepsy: A Case Report.

Fractures secondary to seizure activity are a well-known complication in patients with epilepsy; however, periprosthetic fractures following total hip arthroplasty (THA) due to seizures are rarely documented. Nonadherence to anti-epileptic medications (AEMs) significantly increases the risk of seizure recurrence, particularly in individuals with a history of seizure-induced skeletal trauma. We present the case of a 27-year-old male with a documented history of epilepsy who sustained a right periprosthetic femoral fracture following a generalized tonic-clonic seizure. One year prior, the patien

Chronic Medial Dislocation of the Proximal Ulna: A Rare Delayed Complication of a Supracondylar Fracture of the Humerus in Children.

Osteonecrosis of the humeral trochlea is an uncommon but challenging complication of pediatric elbow fractures. We herein report a case of osteonecrosis of the trochlea, a rare and delayed complication of a supracondylar humerus fracture that led to a chronic dislocation of the ulna. A five-year-old boy arrived at the Emergency Room following a fall from a tree on an outstretched hand with an extended elbow. Radiographic studies of the right elbow confirmed the diagnosis of right supracondylar fracture. Open reduction and internal fixation by two Kirschner wires was carried out, followed by im

Effective Rehabilitation of a Lisfranc Fracture in a 25-Year-Old Male Patient: A Case Report.

A dislocation or break of the tarsometatarsal joint in the foot is referred to as a Lisfranc fracture, sometimes called a Lisfranc injury. It can be caused by less stressful mechanisms like a twisting fall as well as high-energy events like car crashes or falls from heights. Swelling, bruises, and midfoot pain that gets worse when standing or walking are some of the symptoms. The damage may only affect the ligaments or the foot's bony structures. Nonoperative or surgical treatment may be part of the management, depending on how severe the injury is. In order to realign and stabilize the bones,

Pregnancy-Associated Osteoporosis Presented with Femoral Neck Fracture: A Case Report and Literature Review.

BACKGROUND: Pregnancy-associated osteoporosis (PAO) is a rare condition characterized by reduced bone mineral density during the third trimester or lactation. Multiple risk factors, such as genetic, sedentariness, and 25-hydroxy vitamin D deficiency, are associated with PAO. In the majority of cases, PAO is presented with a compression fracture in vertebras, but in our case, it presented with a fracture of the femoral neck. Case Presentation: A 29-year-old, gravida one woman, developed right hip joint pain during the gestational age 34 weeks who referred to our clinic. Despite conservative man

Novel Presentation of Uncommon Wrist Injury: Simultaneous Lunate and Perilunate Fracture Dislocation (scapho-capitate Syndrome) of Both Wrists.

INTRODUCTION: Perilunate and lunate dislocations are uncommon high energy injuries and have extensive soft tissue, cartilaginous and bony damage. The most common pattern is trans-scaphoid perilunate fracture dislocation which constitutes approximately 50% of these injuries. Unusual injury pattern like Scaphocapitate syndrome can occur with fracture through scaphoid waist and capitate that rotates the proximal capitate 180° so that its proximal articular surface points distally. With this case report, we would like to present a novel presentation of an uncommon wrist injury. CASE REPORT: We pr

Internal fixation of displaced proximal humeral fractures with two one-third tubular plates.

BACKGROUND: Operative treatment of displaced proximal humeral fractures often results in poor functional outcome. We report a technique that provides improved rotational stability of the fracture and thus allows early functional treatment. METHODS: Seventy-one consecutive patients (67 +/- 18 years) with displaced fractures of the proximal humerus (two-part, n = 10; three-part, n = 41; four-part, n = 20) were treated by open reduction and internal fixation using two one-third tubular plates on the anterior and lateral aspects of the proximal humerus. Passive motion was started on the third pos

Operative Management of a Late-Presenting McKee Type IV Capitellum Coronal Shear Fracture via an Anterolateral Approach: A Rare Case Report.

INTRODUCTION: Coronal shear fractures of the distal humerus are rare, comprising <1% of elbow fractures. McKee Type IV fractures involve both the capitellum and lateral trochlea. These injuries are uncommon and may result in stiffness and functional impairment if diagnosis is delayed or treated conservatively. CASE REPORT: A 15-year-old boy presented 6 months after an untreated McKee Type IV coronal shear fracture of the distal humerus with pain, stiffness, and restricted elbow motion. Radiographs and computed tomography demonstrated a malunited shear fragment with a "double-arc" sign. Osteot

Multilevel spinal fractures in ankylosing spondylitis: illustrative case.

BACKGROUND: Ankylosing spondylitis (AS) is a chronic inflammatory disease marked by spinal rigidity, which predisposes patients to unstable fractures even after minor trauma. While isolated spinal fractures in AS are well described, multilevel fractures across the cervical, thoracic, and lumbar spine following a single traumatic event are exceedingly rare and pose substantial diagnostic and surgical challenges. OBSERVATIONS: The authors present the case of a 41-year-old male with advanced AS and a history of alcohol use and diabetes who sustained six unstable spinal fractures spanning the cer

Post-operative peri-prosthetic fracture rates following the use of cemented polished taper-slip stems for primary total hip arthroplasty: a systematic review.

INTRODUCTION: Peri-prosthetic fractures (PPFs) are a serious complication of total hip arthroplasty (THA) associated with significant morbidity, mortality and re-operation similar to the rates seen following femoral neck fractures. When exploring the PPF rates around cemented stems, many studies combine the results of composite beam and taper-slip stems despite their different philosophies. This review looks specifically at PPFs with the use of cemented stems of the taper-slip philosophy to evaluate the demographics, PPF rate and fracture patterns in this patient cohort. Monteggia variant with posterior elbow dislocation and radial shaft fracture: A case report.

INTRODUCTION: The most commonly used Bado classification encompasses the large majority of Monteggia fractures, however, certain patterns cannot be classified with this system. Monteggia injuries and Monteggia-Variants are uncommon but serious injuries that can have dire consequences if not identified and treated appropriately. This paper aims to review relevant literature, discuss current classification systems, and present a case of an atypical fracture pattern. PRESENTATION OF CASE: We present a case of a unique Monteggia variant in an adult that to our knowledge has not been discussed in

Anterior horizontal rafting plate to treat complex osteoporotic tibial plateau fractures: a technical note.

Complex tibial plateau fractures in elderly patients exhibiting severe osteoporosis and articular surface collapse are challenging. Decision-making is difficult when the posterior column is involved. Open reduction and internal fixation of complex tibial plateau fractures in patients with severe osteoporosis are prone to failure. In this paper, we describe a new method for the maintenance of the articular surface of complex tibial plateau fractures in elderly patients. An anterior horizontal rafting plate (3.5-mm-thick reconstruction and locking plate [Zimmer Inc., Warsaw, IN, USA]) is placed

A single centre experience of pre-contoured clavicle plates by an anterior approach.

AIM: In recent years there has been increasing evidence that comminuted and shortened clavicle fractures should be treated surgically. The aim of this study was to assess the outcome of patients who had their clavicle fracture fixed using a pre-contoured, clavicle specific plate from an anterior approach. METHOD: A retrospective analysis was carried out of all patients treated with pre-contoured clavicle plates from an anterior approach between May 2014 and June 2016.Medical records were analysed, and a phone survey was carried out to assess patient function and note any complications. [Fractures of the anterior process of the calcaneus-frequently overlooked injuries following ankle sprains].

Fractures to the anterior process of the calcaneus (PAC) have long been considered rare injuries and have received little attention in clinical research. On the contrary, recent studies have reported a distinct higher incidence, especially following ankle sprains. Decisive reasons are that fractures of the PAC are regularly missed on plain radiographs and that a clinical differentiation from injuries to the lateral ankle ligaments is difficult. With the broad availability of cross-sectional imaging modalities fractures of the PAC are diagnosed more frequently and more reliably. The purpose of
