Disappointment and adherence among parents of newborns allocated to the control group: a qualitative study of a randomized clinical trial.

BACKGROUND: When a child participates in a clinical trial, informed consent has to be given by the parents. Parental motives for participation are complex, but the hope of getting a new and better treatment for the child is important. We wondered how parents react when their child is allocated to the control group of a randomized controlled trial, and how it will affect their future engagement in the trial. METHODS: We included parents of newborns randomized to the control arm in the Danish Calmette study at Rigshospitalet in Copenhagen. The Calmette study is a randomized clinical trial inves

How to Optimize Treatment for Patients With Poor-Risk Testicular Cancer.

Male germ cell tumors, of which testicular is the most common, are rare but generally highly curable malignancies. Cure rates in the metastatic setting are among the highest for any solid tumor, although outcomes in the poor-risk subgroup remain suboptimal. Although most men are cured, risk prognostication tools have identified poor-risk patients with inferior outcomes leading to significant mortality in this predominantly young patient population. To optimize survival in this group, proper therapy selection throughout the treatment course of poor-risk patients is vital to maximize survival op

Comparative evaluation of standardized imaging-guided contact selection for subthalamic deep brain stimulation in Parkinson's disease: study protocol for a randomized double-blind crossover trial.

BACKGROUND: Subthalamic deep brain stimulation (STN-DBS) effectively treats motor symptoms in appropriately selected patients with Parkinson's disease, but individual responses vary. Despite modern directional leads allowing more precise stimulation, optimal contact selection strategies have not yet been standardized. This study compares a standardized imaging-guided contact selection protocol to conventional clinical programming. METHODS: We designed a monocentric, randomized, double-blind crossover trial enrolling 30 people with Parkinson's disease with bilateral directional STN-DBS. Partic

Vertical banded gastroplasty versus standard or distal Roux-en-Y gastric bypass based on specific selection criteria in the morbidly obese: preliminary results.

BACKGROUND: Predicting successful outcomes after bariatric surgical procedures has been difficult, and the establishment of specific selection criteria has been a subject of ongoing research. In an effort to choose the most appropriate surgical procedure for each patient, we have established a specific set of selection criteria for each procedure based on degree of obesity, preoperative dietary habits, eating behavior, and various metabolic features. METHODS: From June 1994 to December 1998, 90 bariatric surgical procedures were performed at the authors' institution by a single surgeon (F.K.)

What is the impact on recruitment of a shortened compared with a standard-length participant information leaflet? PROMETHEUS in IBD-BOOST: study within a trial, a decentralised UK randomised controlled trial.

BACKGROUND: Participant Information Leaflets (PILs) are lengthy and increasingly complex, and could deter research participation. A shortened PIL may be more appealing as it is likely to provide a more a manageable volume of information. Previous research has found that shortened PILs are no less effective for recruitment outcomes, and we deemed it useful to replicate this in an online setting. We also decided to compare retention rates, given the potential for more information to increase participants' motivation. AIM: To evaluate the efficacy of a shortened vs standard-length PIL on trial r

Interdisciplinary Periprocedural Management of Patients Undergoing Transapical TMVI with the Tendyne System: A Narrative Review and Institutional Experience.

OBJECTIVES: Mitral regurgitation (MR) represents the most common valvular heart disease (VHD) in the Western world. While transcatheter mitral valve repair (M-TEER) is the leading interventional treatment for surgically high-risk patients, transcatheter mitral valve implantation (TMVI) is reserved for selected patients with unsuitable anatomy for M-TEER. This review aims to summarize our institutional experience with transapical TMVI using the Tendyne valve (Abbott Vascular, CA, USA), focusing on interdisciplinary preoperative, intraoperative, and postoperative management strategies. METHODS:

The impact of seropositivity on the effectiveness of biologic anti-rheumatic agents: results from a collaboration of 16 registries.

OBJECTIVES: RF and ACPA are used as diagnostic tools and their presence has been associated with clinical response to some biologic DMARDs (bDMARDs) in RA. This study compared the impact of seropositivity on drug discontinuation and effectiveness of bDMARDs in patients with RA, using head-to-head comparisons in a real-world setting. METHODS: We conducted a pooled analysis of 16 observational RA registries. Inclusion criteria were a diagnosis of RA, initiation of treatment with rituximab (RTX), abatacept (ABA), tocilizumab (TCZ) or TNF inhibitors (TNFis) and available information on RF and/or

Sexually transmitted infections (STI) and antenatal care (ANC) clinics in Malawi: effective platforms for improving engagement of men at high HIV risk with voluntary medical male circumcision services.

INTRODUCTION: Voluntary medical male circumcision (VMMC), an effective HIV prevention programme for men, is implemented in East and Southern Africa. Approximately 50% of VMMC clients are aged below 15 years. More targeted interventions to reach older men and others at higher short-term HIV risk are needed. METHODS: We implemented a quality improvement project testing the effectiveness of an active referral-based VMMC recruitment approach, targeting men attending STI clinics and those escorting partners to antenatal care (ANC) clinics, at Bwaila Hospital in Lilongwe, Malawi. We compared the pr
