Peritoneal equilibration testing: Your questions answered.

The peritoneal equilibration test (PET), first described in 1987, is a semiquantitative assessment of peritoneal transfer characteristics in patients undergoing peritoneal dialysis. It is typically performed as a 4-h exchange using 2.27/2.5% dextrose dialysate with serial measurements of blood and dialysate creatinine, urea, and glucose concentrations. The percentage absorption of glucose and D/P creatinine ratio are used to determine peritoneal solute transfer rates. It is used to both help guide peritoneal dialysis prescriptions and to prognosticate. There are several derivative tests which

Multiple acyl-COA dehydrogenase deficiency in elderly carriers.

Multiple acyl-CoA dehydrogenase deficiency, or glutaric aciduria type II, is an autosomal recessive disorder of fatty acid oxidation due to defects in electron transfer flavoprotein (ETF) encoded by ETFA and ETFB, or in electron transfer flavoprotein dehydrogenase (ETFDH) encoded by the ETFDH gene. The disease may present as a severe neonatal onset form and a mild late-onset form which is heterogeneous for the age at onset and clinical presentation. We describe two patients in their seventies, referred for a nonspecific myopathy, which resulted to manifest carriers of ETFDH gene mutation. Trea

Impact of daylight savings time on spontaneous pregnancy loss in in vitro fertilization patients.

Transition into daylight savings time (DST) has studied negative impacts on health, but little is known regarding impact on fertility. This retrospective cohort study evaluates DST impact on pregnancy and pregnancy loss rates in 1,654 autologous in vitro fertilization cycles (2009 to 2012). Study groups were identified based on the relationship of DST to embryo transfer. Pregnancy rates were similar in Spring and Fall (41.4%, 42.2%). Pregnancy loss rates were also comparable between Spring and Fall (15.5%, 17.1%), but rates of loss were significantly higher in Spring when DST occurred after em

Microvascular free tissue transfer for reconstruction of head and neck cancer defects.

Over the last 10 years, microvascular free tissue transfer techniques have broadened the range of the head and neck surgeon, allowing for successful reconstruction of extensive curative extirpations in one operation with minimal morbidity. Success rates of more than 90% are now being achieved consistently. This article provides an overview of these revolutionary techniques for oral cavity, mandibular, and hypopharyngeal reconstructions. Patients are selected for microvascular free tissue transfer because of the large size of their defect, its location (eg, anterior mandibular loss), or complex

Is there an optimal window of time for transferring single frozen-thawed euploid blastocysts? A cohort study of 1170 embryo transfers.

STUDY QUESTION: Is there an optimal window of time when the transfer of single frozen-thawed euploid blastocysts is associated with a maximal live birth rate (LBR)? SUMMARY ANSWER: Performing a single frozen-thawed euploid blastocyst transfer at 160 ± 4 h post-hCG trigger in modified-natural frozen-thawed embryo transfer (FET) cycles was independently associated with a higher LBR as compared to transfers outside this window; however, in natural FET cycles, LBRs were comparable across a wider range of time intervals. WHAT IS KNOWN ALREADY: There is compelling evidence for maintaining embryo-e

Foreign Body Reaction Following Achilles Tendon Reconstruction With the Ligament Advanced Reconstructive System: Patient Outcomes and Clinical Course.

The Ligament Advanced Reinforcement System (LARS) is a common choice for ligament reconstruction in the lower limb due to its good functional and quality of life (QoL)-related outcomes. It is commonly used for Achilles tendon repair following a rupture. While it facilitates tissue ingrowth and boasts good biocompatibility, we report on multiple cases whereby foreign body reactions have led to the growth of granulomas requiring surgical excision and Flexor Hallucis Longus (FHL) transfer. Following these cases, patients have been shown to have excellent functional and QoL-related outcomes using

T2-blackout effect on DWI as a sign of early bone infarct and sequestration in a patient with sickle cell disease.

Differentiation of bone infarct from osteomyelitis is one of the most challenging issues in the evaluation of acute bone pain in sickle cell patients. The imaging modalities that are currently being used for assessment of bone marrow in this population have several limitations. We present a case of an 18-year-old male with a history of sickle cell disease, who was transferred to our emergency department with progressively severe headache and jaw pain for one-week. Initial evaluation was concerning for osteomyelitis and epidural abscess formation. Due to the lack of response to the current anti

Role of platelet inhibition in microvascular surgery.

Thrombosis remains a significant complication of microvascular free tissue transfer. Recent discoveries in the field of vascular biology have led to a greater understanding of thrombogenesis and the pivotal role that platelets play in the formation of a clot. However, current antithrombotic strategies in the clinical practice of free tissue transfer have not typically focused on platelet inhibition. Decades of cardiovascular clinical trials have delineated the essential role of platelet inhibitor therapy in patients with acute coronary syndromes and those undergoing percutaneous coronary inter

[CKD-MBD (Chronic Kidney Disease-Mineral and Bone Disorder). Gene therapy for secondary hyperparathyroidism].

Secondary hyperparathyroidism (SHPT) is one of the most important complications of chronic kidney disease. Although 1,25-D pulse therapy is effective for SHPT, hyperplastic parathyroid (PT) cells gradually develop resistance to 1,25-D. Vitamin D(3) receptor (VDR) levels are decreased in the PT glands of dialysis patients and animal models of renal failure. To develop a new approach to such rebellious tissue, recent research has shown that the functionality protein can be adjusted with the gene transfer to the parathyroid. The experiment using adenovirus clarifies that the gene transfer of VDR

Management of lower limb fractures in polytrauma patients with delayed referral in a mass disaster. The role of the Ilizarov method in conversion osteosynthesis.

Polytrauma cases in mass disasters present several challenges to the orthopaedic surgeon. Delayed referral, multisystem involvement and the requirement to manage coexisting injuries by interhospital transfer often make infection an inevitable risk. 28 patients with polytrauma were studied after being referred after being recovered from the debris of their homes in the Kashmir earthquake. All patients were referred more than 24h after sustaining their injuries. The lower limb fractures were fixed by external fixators in all these cases before interhospital transfer for the management of their c

A clinical and radiographic approach for establishing proper tibial tubercle transfer when using quad active femoral nerve stimulation.

BACKGROUND: The tibial tubercle-trochlear groove measurement (TT-TG), which measures the lateral offset of the tibial tubercle relative to the trochlear groove of the femur, has been utilized as an intraoperative tool to help establish maximum patellofemoral congruency in patients who suffer from patellar instability. We have previously published our approach of establishing how far to transfer the tibial tubercle using intraoperative femoral nerve stimulation in order to achieve congruency from 0-30° of flexion. The technique and clinical outcomes have previously been published in this journa

International Application of Artificial Intelligence for Lesion Detection on Digital Breast Tomosynthesis: Comparing Western and Eastern Databases.

RATIONALE AND OBJECTIVES: The international application of artificial intelligence (AI) for lesion detection based on digital breast tomosynthesis (DBT) is limited due to disease variations among populations. We hypothesized that lesion detection models trained on either the Western or Eastern DBT dataset would exhibit reduced performance on another dataset. We proposed transfer learning to enhance lesion detection across DBT databases. MATERIALS AND METHODS: The Western database (94 patients) was obtained from the Cancer Imaging Archive Breast Cancer Screening DBT, and the Eastern database (

Pregnancy outcomes in freeze-all versus fresh embryo transfer cycles of women with adenomyosis and endometriosis: a systemic review and meta-analysis.

BACKGROUND: Endometriosis (EMS) and adenomyosis have adverse effects on women's fertility. In vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) are effective treatments for these diseases. Research has shown that different embryo transfer strategies in IVF/ICSI can influence gestational outcomes. This systematic review and meta-analysis aimed to evaluate the impact of freeze-all embryo transfer (FET) versus fresh embryo transfer (ET) strategies in IVF/ICSI cycles for infertile women with EMS and adenomyosis. METHOD: A comprehensive search was conducted across PubMed, EMBAS
