Impact of Chronic Pain on Patients' Quality of Life: A Comparative Mixed-Methods Study.

BACKGROUND: Chronic pain has become a common problem within primary care and can negatively impact patients' lives. OBJECTIVE: To assess and explore the impact of chronic pain on patients' quality of life (QoL) using quantitative and qualitative data, respectively. METHODS: A convergent parallel mixed-methods design was used. Chronic pain patients were recruited from a community-based pain clinic located in the North of England. Quality of life was assessed using Short-Form 36 version 2. Quality of life data were also extracted from the Third Oxford and Lifestyles Survey and Welsh Health Sur

Pathways from physical frailty to activity limitation in older people: Identifying moderators and mediators in the English Longitudinal Study of Ageing.

Physical frailty increases the risk of future activity limitation, which in turn, compromises independent living of older people and limits their healthspan. Thus, we seek to identify moderators and mediators of the effect of physical frailty on activity limitation change in older people, including gender- and age-specific effects. In a longitudinal study using data from waves 2, 4, and 6 of the English Longitudinal Study of Ageing, unique physical frailty factor scores of 4638 respondents aged 65 to 89years are obtained from confirmatory factor analysis of physical frailty, which is specified

Discussing the Effects of Poor Health Literacy on Patients Facing HIV: A Narrative Literature Review.

BACKGROUND: Scholars describe poor health literacy as a "silent epidemic," which is challenging the functioning of healthcare systems all over the world. Health literacy is mainly meant as an individual trait which concerns the ability to obtain, process, and understand basic health information in order to effectively navigate the health system. Low health literate patients perceive poor self-efficacy dealing with their health conditions, are not willing to be involved in the provision of care, show larger risks of hospitalization and mortality, and are not aware about the determinants of well

Racial Differences in Chronic Conditions and Sociodemographic Characteristics Among High-Utilizing Veterans.

PURPOSE: African-Americans are disproportionally represented among high-risk, high-utilizing patients. To inform program development for this vulnerable population, the current study describes racial variation in chronic conditions and sociodemographic characteristics among high-utilizing patients in the Veterans Affairs Healthcare System (VA). METHODS: We identified the 5 % most costly Veterans who used inpatient or outpatient care at the VA during fiscal year 2010 (N = 237,691) based on costs of inpatient and outpatient care, pharmacy services, and VA-sponsored contract care. Patient costs

Photodermatoses: environmentally induced conditions with high psychological impact.

Photodermatoses are a group of skin disorders caused or exacerbated by ultraviolet and/or visible radiation, which collectively affect a high proportion of the population and substantially affect quality of life (QoL). Our objective was to assess the psychological impact of these conditions. Patients with a range of photodermatoses diagnosed at a specialist investigation centre in the UK completed questionnaires evaluating (i) anxiety and (ii) depression, using the Hospital Anxiety and Depression Scale (HADS), (iii) social anxiety, using the Fear of Negative Evaluation measure (FNE), (iv) copi

Brain stimulation therapies for neuropsychiatric disease.

ECT is a well-established, effective, and safe treatment for many neuropsychiatric conditions, especially major depression. However, ECT is also associated with a high relapse rate, notable side-effects, and significant social stigma. Additionally, ECT is ineffective in a sizable minority of patients. Based on this, several other brain stimulation therapies are currently under active investigation. VNS is approved in many countries for the treatment of treatment-resistant epilepsy and TRD. The available data suggest long-term VNS may have clinically significant antidepressant effects, though m

Characterizing major depression phenotypes by presence and type of psychomotor disturbance in adolescents and young adults.

Major depressive disorder (MDD) is phenomenologically heterogeneous, which has prompted investigation of intermediate MDD phenotypes based on specific key symptoms. Presence and type of psychomotor disturbance may be an important psychopathologic feature that differentiates clinically distinct forms of juvenile MDD. This study examined the phenotypic status of three putative MDD phenotypes in a community sample of 941 youths: (1) agitated depression (MDD with psychomotor agitation), (2) retarded depression (MDD with psychomotor retardation), and (3) agitated-retarded depression (MDD with psych

Determinants of allergen concentrations in apartments of asthmatic children living in public housing.

There is growing evidence linking poor housing conditions and respiratory diseases, including asthma. The association between housing conditions and asthma in the inner city has been attributed in part to cockroach and mouse infestation and the resulting allergen exposures. Multiple social and behavioral factors can influence environmental exposures and health conditions, necessitating a thorough examination of such factors. As part of the Healthy Public Housing Initiative, we evaluated the association between physical and household characteristics and pest-related allergen levels in three pub

Depressive symptoms, chronic medical conditions and functional status: a comparison of urban and rural elders in Taiwan.

BACKGROUND AND PURPOSE: The associations of depression, chronic condition and functional disability have not been studied well in Taiwan. The present study hypothesized that strong relations between chronic medical conditions, functional status, and the manifestation of depression would be evident; the interrelationships of the interested measures would differ in urban and rural populations METHODS: The study sample was recruited in a community survey in 2001 that included urban and rural elderly dwelling in southern Taiwan. The Chinese-version Geriatric Depression Scale (GDS) was used for da

Diagnosis of nonpsychotic patients in community clinics.

OBJECTIVE: Diagnosis-specific, proven efficacious treatments are a major recent advance in psychiatry. Appropriate use of such treatments presupposes patients who meet the diagnostic criteria and clinicians who have accurately diagnosed the target disorder and comorbid conditions. Since little is known about whether these prerequisites are commonly met, the authors conducted a study at two community treatment sites to determine the frequency of various axis I diagnoses and the concordance between the diagnoses recorded in patient charts and those obtained by a structured interview. Given that

Posterior nuchal cystic hygroma.

When cystic hygroma is detected antenatally, the prognosis is usually poor, and intrauterine or neonatal death are common. This seems especially true for Turner syndrome (the largest etiologic group associated with cystic hygroma) and the lethal multiple pterygium syndromes. However, some fetuses with Turner syndrome, and also with certain other diagnoses, had a good outcome, and further study of these conditions is required before more definite prognostic indicators and risk figures will be available. Increasing numbers of syndromes are being reported in association with fetal cystic hygroma,

Developing a program theory for implementing a Danish social prescribing model: an ethnographic study.

INTRODUCTION: Social prescribing (SP) is increasingly promoted internationally as a cross-sector strategy to address complex health and social needs by linking primary care patients with community-based, non-clinical support. Despite increasing policy uptake and expanding evaluations of outcomes, evidence on how SP models are implemented and become embedded in routine practice across diverse welfare and organizational contexts remains limited and fragmented. Existing studies tend to catalogue implementation activities and contextual barriers without sufficiently explaining how implementation s

Dr Google: Which Parents are Performing the Most Online Research Prior to the First Clinic Visit?

BACKGROUND: Parents commonly use the internet to research their child's diagnosis, read provider reviews, and find additional resources like support groups before or after a clinic visit. This study aimed to characterize demographic and diagnosis-specific variables associated with more frequent internet-based research prior to initial outpatient evaluation. METHODS: Parents of children presenting to an outpatient orthopaedic clinic completed a survey reporting their history of internet use in preparation for the initial visit. Data on demographics, diagnosis, and types of websites used were c

Association of Combined Marijuana and Opioid Use with Major Depressive Disorder Among Adults with Chronic Conditions in the United States.

BACKGROUND: Individuals with chronic conditions often use opioids for pain and the broadening legalization of marijuana and its potential analgesic effects may prompt some individuals to use marijuana as an additional option. Both substances have been individually linked to adverse effects on major depressive disorder (MDD), but little is known about their co-use. This study examined the association between co-use and MDD among US adults with chronic conditions. METHODS: This cross-sectional analysis used 2022 National Survey of Drug Use and Health data from 35 585 adults older than 18 years

The relationship between clinical characteristics, sociodemographic factors and sedentary behavior in adults with chronic conditions: a cross-sectional study from the Lolland-Falster Health Study.

BACKGROUND: Sedentary behavior (SB) increases the risk of disease and mortality, even in physically active individuals. Adults living with multimorbidity may be particularly vulnerable. Although factors such as age, body mass index (BMI), and social support have been suggested, evidence is largely based on self-reported activity that tends to underestimate SB; therefore, correlates of objectively measured SB in adults with medical conditions remain insufficiently described. We investigated clinical and sociodemographic correlates of accelerometer-measured SB among adults with medical condition

An occupational health survey on health utility and occupational diseases in Chinese university staff to inform cost-utility analysis.

BACKGROUND: The occupational health of university staff bears great social and economic value for which health utility is an indivisible aspect. Utility is also the primary data for the cost-utility analysis of occupational health programs. Health utility and occupational diseases have not been reported for the university staff in China. In the light of "Healthy China," we conducted this study aiming to (1) estimate the health utility of university staff to inform cost-utility analysis and (2) screen and identify potential occupational diseases for this occupation and examine their impacts on

"Mad, Mentally Ill, Permanently Unstable"? A Culture-Centered Approach to Jinn Possession Stigmas and Management Strategies Among Nigerian Muslims.

This study examines how Nigerian Muslims with jinn possession experiences construct jinn possession stigmas, their socio-relational impacts, and management communication strategies. Interviews with 14 participants revealed that public, self, and enacted stigmas induced by family members and friends were reinforced by religio-cultural beliefs framing jinn possession as contagion, disease, or spiritual cultism. These stigmas led to social isolation, discrimination, emotional abuse, and internalized distress among participants. To navigate the stigmas, participants accepted jinn possession stigma

"They learned fear before letters": Palestinian mothers' narratives of childhood in households affected by addiction.

Parental substance use is widely recognized as a source of household instability with adverse consequences for children, yet limited research has examined how such harm is lived and interpreted within contexts of prolonged structural constraint. This qualitative study explores Palestinian mothers' narratives of childhood in households affected by paternal substance use in the occupied West Bank. Drawing on 30 first-person narratives shared on digital platforms, the analysis examines how fear, violence, emotional neglect, and stigma shape children's everyday lives over time. Using thematic anal

MetALD: Terminology, Implications, and Therapeutic Considerations.

With the introduction of steatotic liver disease (SLD) as an umbrella term and metabolic dysfunction-associated steatotic liver disease (MASLD) as one of its major subcategories, a new diagnostic category was proposed to better reflect the coexistence of metabolic dysfunction and alcohol consumption: MetALD. MetALD identifies individuals with hepatic steatosis and cardiometabolic risk factors whose alcohol intake exceeds MASLD thresholds but remains below the levels typical of alcohol-related liver disease. The prevalence of MetALD in the general population is approximately 4%. Evidence sugges
