Use of adhesive tissue for repair in experimental vesico-vaginal fistulas in dogs.

Experimental vesico-vaginal fistulas were successfully treated using the transvaginal approach and strengthening the bladder sutures with adhesive tissue. Despite inflammation of the surrounding tissues good results were obtained, without cystotomy, permanent catheterization and antibiotics.

Modified All-Inside Arthroscopic Anterior Tibiofibular Ligament Repair and Reinforcement in Chronic Ankle Instability.

Chronic ankle instability often requires surgical intervention, with arthroscopic anterior talofibular ligament repair being a preferred technique; however, outcomes in cases with poor ligament quality remain challenging. This study introduces a "modified suture augmentation" technique, which uses an individualized portal placement and arthroscopic lasso-loop suturing, followed by a flexible "turn-back" reinforcement of the anterior talofibular ligament using the suture limbs. This all-inside, suture-based augmentation method aims to provide anatomic reconstruction and enhanced stability while

Laser tube ligature release following aqueous shunt implantation in young children.

The implantation of non-valved aqueous shunting devices in children is often complicated by intraocular pressure elevation in the early postoperative period, during which time the tube is closed with a temporary suture ligature to avoid hypotony. Release of a polyglactin suture ligature using conventional laser lysis is not possible in young children in the clinic setting. The authors describe a minimally invasive technique using a portable green diode (532-nm) laser delivered through a standard endoprobe and a Hoskins laser suture lysis lens to disrupt a polyglactin suture ligature following

Use of an inverse dynamics method to compare the three-dimensional motion of the pelvic limb among clinically normal dogs and dogs with cranial cruciate ligament-deficient stifle joints following tibial plateau leveling osteotomy or lateral fabellar-tibial suture stabilization.

OBJECTIVE: To compare the 3-D motion of the pelvic limb among clinically normal dogs and dogs with cranial cruciate ligament (CCL)-deficient stifle joints following tibial plateau leveling osteotomy (TPLO) or lateral fabellar-tibial suture (LFS) stabilization by use of an inverse dynamics method. ANIMALS: 6 clinically normal dogs and 19 dogs with CCL-deficient stifle joints that had undergone TPLO (n = 13) or LFS (6) stabilization at a mean of 4 and 8 years, respectively, prior to evaluation. PROCEDURES: For all dogs, an inverse dynamics method was used to describe the motion of the pelvic l

Cadaveric Comparison of Work of Flexion Following Flexor Tendon Repair: Knotted Versus Knotless Techniques.

BACKGROUND: Few prior studies have compared work of flexion of digital flexor tendons repaired with barbed suture to those performed with conventional knotted techniques. The available evidence is mixed, with one study indicating no difference between the techniques and another showing increased work of flexion with knotless techniques. This study aimed to bolster the current body of literature by comparing work of flexion and strength of digital flexor tendon repairs using knotted and knotless techniques. METHODS: Two knotted and 2 knotless techniques for flexor tendon repair were assessed.

The strength of surgical knots involves a critical interplay between friction and elastoplasticity.

Knots are the weakest link in surgical sutures, serving as mechanical ligatures between filaments. Exceeding their safe operational limits can cause fatal complications. The empirical nature of present guidelines calls for a predictive understanding of the mechanisms underlying knot strength. We identify the primary ingredients dictating the mechanics of surgical sliding knots, highlighting the previously overlooked but critical effect of plasticity and its interplay with friction. The characterization of surgeon-tied knots reveals the relevant ranges of tightness and geometric features. Using

Direct Repair of Flexor Tendons Close to Bony Insertion and Ruptured Collateral Ligaments.

Lacerated flexor tendons close to bony junction are commonly repaired using a pullout suture. However, these injuries very close to the tendon-bone junction can be repaired with robust direct suture repair of the proximal tendon stump with the short residual tendon stump and any local tissues such as periosteum and joint volar plate. Subacute or chronic traumatic rupture at the midpart of the collateral ligaments can also be repaired by "refreshing" the divided ligament ends and repairing the ligament stumps to local tissues with multiple sutures often combined with tightening the elongated jo

Optimizing Fixation for Distal Biceps Tendon Repairs: A Systematic Review and Meta-regression of Cadaveric Biomechanical Testing.

BACKGROUND: Various surgical techniques can be used to repair acute distal biceps tendon (DBT) tears; however, it is unknown which type of repair or implant has the greatest biomechanical strength and presents the lowest risk of type 2 failure. PURPOSE: To identify associations between the type of implant or construct used and the biomechanical performance of DBT repairs in a review of human cadaveric studies. STUDY DESIGN: Systematic review and meta-regression. METHODS: We systematically searched the EMBASE and Medline (PubMed) databases for biomechanical studies that evaluated DBT repair

What Do You Do With The Achilles if You Have No Fancy Toys?

Surgical management of Achilles disorders warrants excision of the degenerated tendon and removal of impinging bone. Resulting defects can be bridged by various methods. Although FHL is the most commonly used tendon for transfer, large defects in cases of chronic Achilles ruptures may be bridged by use of a distant donor tendon. Bony anchorage of a lengthened or transferred tendon into the calcaneus can be done with suture anchors or with interference screws. In developing countries, such implants may not be available or affordable, necessitating the adoption of innovative ways to anchor tendo

Single-Armed Longitudinal Intussusception Vasoepididymostomy in an Ex Vivo Rat Model With Video Demonstration.

Microsurgical vasoepididymostomy (VE) is a technically demanding procedure that benefits from deliberate, reproducible skills training. Practice opportunities may be limited by the cost and availability of double-armed microsutures used in traditional two-needle intussusception techniques. Single-armed longitudinal intussusception vasoepididymostomy (SA-LIVE) reduces reliance on double-armed sutures. Although SA-LIVE has been described in prior publications, still images alone may not fully capture the sequence and microsurgical nuances; a video demonstration can improve clarity for learners.

The pericranium.

The purpose of this chapter is to present how past surgeons have viewed the pericranium and how they have reacted to its appearances. In ancient times, the membrane was considered formed by the dura through the sutures and it retained a relationship with the dura via vessels in the sutures. It was considered advisable to strip it totally from any area to be examined for fissure fractures and also for any area to be trepanned, as pericranial injury was thought to lead to fever and inflammation. In the 18th century, a new idea arose that posttraumatic spontaneous separation of the pericranium fr

Exceptional laceration of flexor digitorum tendons proximal to a severe palmar hand wound: a case report with literature review.

Hand wounds are common, poor functional outcomes are marked because of sequelae inherent to posttraumatic and postoperative complications. Suitable surgery repair in emergency can ensure best results. Classically, tendon's injuries occur near the injured area and their repair depend on traumatized zone, sutures techniques, associated lesions and surgeon's abilities. We report a case of a farmer who has sustained of a severe hand wound due to blades of a combine harvester. Clinical examination showed exceptional laceration of 2(nd) and 3(rd) flexor digitorum tendons from musculo-tendinous junct

[Combined valve surgery in end-stage renal disease patient with porcelain aorta and mitral annulus calcification].

A 66-year-old woman, who had been receiving regular hemodialysis for 11 years, was referred to our hospital because of heart failure due to combined valve disease complicated by porcelain aorta and mitral annulus calcification. We performed ascending aortic replacement under hypothermic arrest, and double valve replacement and tricuspid valve annuloplasty during rewarming. It was noted that the annulus of P3 of mitral valve had been replaced with atherosclerotic plaque containing calcification. We did not perform débridement. We placed non-everted horizontal mattress sutures from the left vent

Current approaches for management of postpenetrating keratoplasty astigmatism.

A successful corneal graft requires both clarity and an acceptable refraction. A clear corneal graft may be an optical failure if high astigmatism limits visual acuity. Intraoperative measures to reduce postkeratoplasty astigmatism include round and central trephination of cornea with an adequate size, appropriate sutures with evenly distributed tension, and perfect graft-host apposition. Suture manipulation has been described for minimising early postoperative astigmatism. If significant astigmatism remains after suture removal, which cannot be corrected by optical means, then further surgica

Pediatric craniofacial surgery for craniosynostosis: Our experience and current concepts: Part -1.

Craniostenosis is a disease characterized by untimely fusion of cranial sutures resulting in a variety of craniofacial deformities and neurological sequelae due to alteration in cranial volume and restriction of brain growth. This involves vault sutures predominantly, but cranial base is not immune. Association with a variety of syndromes makes the management decision complex. These children need careful evaluation by multiple specialists to have strategic treatment options. Parental counseling is an important and integral part of the treatment. Recent advancements in the surgical techniques a
