Ten-year experience with one-step correction of constriction rings by complete circular resection and linear circumferential skin closure.

BACKGROUND: In amniotic band syndrome (ABS) constriction rings affecting the limbs are regularly seen at birth. Circular resection and closure with multiple Z-plasties has been described as the treatment of choice. However, direct circular closure creating linear circumferential scars might replace multiple Z- or W-plasties to reduce scars and improve aesthetic outcome. METHODS: From 2000 to 2009 we treated 160 patients with ABS. In 43 cases release of isolated constriction rings was performed by circular excision and closure. Constriction rings were completely resected and skin closure could

Symmetrical upper limb peromelia and lower limb phocomelia associated with a de novo apparently balanced reciprocal translocation: 46,XX,t(2; 12)(p25.1;q24.1).

We report a female fetus of 20 weeks gestation with severe symmetrical deformity affecting all four limbs. These deformities were unusual in that there was upper limb peromelia and lower limb phocomelia. No additional major malformations were identified on postmortem examination. In particular there was no evidence of splenogonadal fusion or micrognathia and hypoglossia. The limb malformations in this case are associated with a de novo apparently balanced reciprocal translocation 46,XX,t(2;12)(p25.1;q24.1). The cytogenetic features of Roberts-SC phocomelia syndrome were not detected. Unfortuna

Paget-Schroetter syndrome and pulmonary thromboembolism: clinical follow-up over 5 years.

A scaffold constructor lost consciousness at work, leading to emergency admission to our hospital. He had noted gradual worsening of exertional dyspnea over the previous 1 month. Chest radiography showed localized oligemia and enlarged hilar vessels. Pulmonary perfusion scintigraphy confirmed the existence of multiple perfusion defects, so a diagnosis of pulmonary thromboembolism was made. Upper and lower limb venography disclosed interruption of the right subclavian vein (so-called Paget-Schroetter syndrome). However, there was no difference in appearance between the right and left upper extr

[A case of a thoracic extradural arachnoid cyst presenting with slowly progressive muscle weakness in the right upper and lower limbs].

A 49-year-old woman developed slowly progressive muscle weakness of the right upper and lower limbs. Physical examination revealed exaggeration of deep tendon reflexes in bilateral lower extremities and a Th5-Th6 girdle sensation. Weakness in her right upper extremity suggested cervical or intracranial lesion. Neuroradiological studies detected no abnormalities in her cervical cord and cranium. So the symptoms and signs were similar to those of motor neuron disease except for the sensory disturbance. MRI study of thoracic cord demonstrated a thoracic extradural arachnoid cyst. After removal of

Immediate Soft-Tissue Reconstruction in Pediatric Limb-Salvage Extremity Tumor Surgery: Patterns and Outcomes From NSQIP-Pediatric, 2012-2023.

While recent studies have characterized flap-based reconstruction in higher-acuity pediatric extremity tumor cases, the frequency and short-term outcomes of immediate soft-tissue reconstruction across the broader spectrum of pediatric limb-salvage extremity tumor surgery remain poorly defined. The ACS NSQIP-Pediatric database (2012 to 2023) was queried for pediatric upper extremity (UE) and lower extremity (LE) oncologic excisions using comprehensive CPT codes spanning all anatomic levels. We focused exclusively on tumors that required excision and were amenable to limb salvage. Immediate soft

Tone and reflex development before term.

The evolution of tone and reflexes from 25 weeks postmenstrual age (gestational age plus chronologic age) to term in a population of 42 surviving infants is described. The infants were born in 1983 at the Johns Hopkins Hospital, had birth weights less than 1300 g, were examined weekly until neonatal intensive care unit discharge, and did not develop cerebral palsy. Lower-extremity flexor tone was first detectable at 29 weeks post-menstrual age by the popliteal angle and heel to ear maneuvers. Flexor tone, recoil, and hyperreflexia were all noted 2 to 3 weeks earlier in the lower extremities (3

Ipsilateral total shoulder and elbow arthroplasties in patients who have rheumatoid arthritis.

BACKGROUND: The data on seventeen patients with rheumatoid arthritis who had been managed with ipsilateral total shoulder and elbow arthroplasties were analyzed to determine whether the operative technique, the presence of total shoulder and total elbow prostheses in the same upper extremity, or complications of the arthroplasties affected the result in each joint or the overall functional outcome of the upper extremity. METHODS: Seventeen patients with rheumatoid arthritis who were managed with a total of eighteen ipsilateral total shoulder and elbow arthroplasties were evaluated. The most r

Characterizing complex upper limb movements with and without visual feedback in typically developing children.

The development of upper limb movements has been primarily described through reaching movements, which may not have the complex motor planning and execution demands of many daily tasks. In this study, we introduced a complex task in which individuals had to navigate their hand from a start target through two openings in a simple maze to reach an end target. In half the trials, participants received visual feedback of their hand position, and in half of the trials, they did not. A total of 31 participants aged 8-17 yr completed the study. We found that with visual feedback, reaction time, numbe

Efficacy of Brain-Computer Interface Therapy for Upper Limb Rehabilitation in Chronic Stroke: Systematic Review and Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Over 50% of people with chronic stroke experience persistent upper limb dysfunction. Brain-computer interface (BCI) therapy, creating a sensorimotor loop via neural feedback, is a promising alternative; yet, its optimal application remains unclear. OBJECTIVE: This meta-analysis evaluates BCI's efficacy on motor function, tone, and activities of daily living (ADL) in chronic stroke and identifies optimal feedback modalities and intervention parameters. METHODS: We systematically searched Cochrane Library, Embase, PubMed, Scopus, Web of Science, and Wanfang Data from inception to O

Recovery of daily life upper limb use during stroke rehabilitation: neuroanatomical correlates and associated variables.

BACKGROUND: Early after stroke, the upper limb is impaired in ~50% of patients who had a stroke, posing a significant and restrictive challenge to their daily lives. It is unknown how many subacute stroke patients regain good upper limb use in everyday life (ie, performance) during inpatient neurorehabilitation and which clinical and stroke-related neuroanatomical factors are associated with recovery. This study explores these questions using real-world clinical data. METHODS: Analysis of data prospectively collected on a weekly basis in the clinical routine of patients who had a subacute str

Impact of upper-limb aerobic exercise on the prevention of chronic postsurgical pain after total knee arthroplasty: a randomized controlled trial.

OBJECTIVE: To investigate whether adding early upper-limb aerobic exercise to usual postoperative rehabilitation reduces the incidence of chronic postsurgical pain (CPSP) at 6 months after total knee arthroplasty (TKA). RESULTS: Eighty patients undergoing primary unilateral TKA were randomized to the intervention or control groups. Both groups received usual postoperative rehabilitation, and the intervention group additionally performed upper-limb ergometer exercise on postoperative days 2-14. The primary outcome, CPSP at 6 months, occurred at very low rates in both the intervention (2.6%) an

Mapping neural representations of fine and gross upper-limb movements across dorsoventral subthalamic nucleus subregions in Parkinson's disease.

Objective. Deep brain stimulation (DBS) of the subthalamic nucleus (STN) is an established therapy for Parkinson's disease (PD), yet optimizing outcomes remains challenging due to the complexity of STN functional architecture and the subjectivity of intraoperative assessments. Furthermore, the encoding of fine versus gross movements across STN dorsoventral subregions remains poorly understood. This study aimed to characterize region- and movement-specific modulation of STN neuronal activity using synchronized electrophysiology and markerless motion tracking-derived kinematics.Approach. Combined transcranial direct current stimulation and mirror therapy for upper limb rehabilitation in patients with stroke: a systematic review and meta-analysis.

Upper limb impairment is among the common consequences of stroke. Combining transcranial direct current stimulation (tDCS) and mirror therapy (MT) have recently been used to improve neural plasticity and functional recovery. This review aims to evaluate the effects of combined tDCS and MT on upper limb function in patients with stroke. "PubMed, Scopus, Web of Science, CINAHL, PEDro, and EMBASE" were searched from their inception to March 2026. Randomized controlled trials investigating the combination of tDCS and MT in patients with stroke and assessing upper limb function were included. The q

Vagus nerve stimulation to improve post-stroke motor function and activity.

RATIONALE: After a stroke, many people experience persistent upper extremity (UE) motor impairments (that is, deficits in strength, coordination, and muscle tone), which can limit activities such as reaching, grasping, and manipulation. Despite conventional rehabilitation, recovery of UE function often remains limited. Vagus nerve stimulation (VNS), applied invasively or non-invasively (transcutaneously), has been proposed as an adjunct to rehabilitation to enhance neuroplasticity and improve motor outcomes after stroke. While early trials have reported mixed findings, the overall effectivenes

Kinematic and Muscle Activation Differences Between High-Performance and Intermediate Tennis Players During the Forehand Drive.

This study compared the kinematic and neuromuscular characteristics of the tennis forehand drive between high-performance (HP) and intermediate (INT) players. Eighteen right-handed male players (HP: n = 9; INT: n = 9) performed cross-court forehands while three-dimensional motion capture and surface electromyography (EMG) were recorded from the dominant upper limb and trunk. Kinematic and EMG data were time-normalized to the forward swing. One-dimensional statistical parametric mapping two-sample t-tests were used to compare joint angles, angular and linear velocities, and EMG amplitude wavefo

Effects of white cane handle angle and technique on upper limb biomechanics and comfort in visually impaired users.

The white cane is a fundamental mobility aid for individuals with visual impairments. While previous studies have examined cane characteristics, the ergonomic interaction between handle angle and usage technique remains underexplored. This quasi‑experimental study investigated the effects of white cane handle angle and technique on upper-limb posture, discomfort, and usability. Sixteen visually impaired participants completed eight experimental conditions combining four handle angles (0°, 10°, 20°, and 30°) with two cane techniques (two-point touch and constant contact) while walking an eight-
