Ventricular Arrhythmia and Life-Threatening Events in Patients With Repaired Tetralogy of Fallot.

Risk stratification for malignant arrhythmias and risk of sudden cardiac death in tetralogy of Fallot (TOF) remains challenging. We aimed to ascertain factors associated with life-threatening arrhythmic events. A multicenter retrospective case-control study including 72 TOF patients with documented cardiac arrest and/or sustained ventricular tachycardia, compared with 216 controls matched for era of surgery. The mean age at event in the cases was 27.3 ± 12.5 years. The majority (57%) presented with sustained ventricular tachycardia. Fatal events occurred in 9. Random forest analysis and a deci

Cardiac Malignant Lymphoma with Diffuse Extension to the Left Ventricle.

Malignant cardiac lymphoma is rare and commonly involves nodules on the right side of the heart. We herein report a case of malignant cardiac lymphoma with diffuse extension into the left ventricle. The patient was a woman in her 60s who complained of dyspnea and malaise. Echocardiography revealed left ventricular hypertrophy (LVH), and magnetic resonance imaging revealed diffuse contrast enhancement on delayed contrast. Cardiac catheterization and a myocardial biopsy suggested heart failure due to cardiac malignant lymphoma, and diastolic dysfunction was mild despite LVH. The patient underwen

Cardiac Mechanoenergetics in Patients with Acute Myocardial Infarction: From Pressure-Volume Loop Diagram Related to Cardiac Oxygen Consumption.

Acute myocardial infarction (AMI) results in significant changes in cardiac structure and functions, leading to left ventricular remodeling and subsequent systolic and diastolic dysfunction. To improve current approaches in diagnoses, treatments, and prevention of cardiovascular diseases, a better understanding of cardiac mechanoenergetics, including systolic performance and energy demand, becomes paramount. In this review, we summarize cardiac mechanics, cardiac energetics, and their relationship in complications related to AMI using 2 important physiologic frameworks, pressure-volume loops a

Transcatheter JenaValve Implantation in a Stentless Prosthesis: A Challenging Case After 4 Previous Aortic Procedures.

A 40-year-old man underwent 4 aortic surgeries because of endocarditis and subsequent prosthesis dehiscence. At the last recurrence he presented with acute severe aortic regurgitation of a Pericarbon Freedom (LivaNova plc, London, UK) stentless bioprosthesis and a morphologically disarranged aortic root. He also presented with left ventricular dysfunction and a very low origin of the left coronary artery. Therefore, a fifth redo aortic valve replacement was considered at high surgical risk. Accordingly, before listing the patient for a heart transplantation, a transcatheter valve-in-valve impl

Effect of Obstructive Sleep Apnea and Positive Airway Pressure Therapy on Cardiac Remodeling as Assessed by Cardiac Biomarker and Magnetic Resonance Imaging in Nonobese and Obese Adults.

It is unknown whether obesity modifies the effect of obstructive sleep apnea (OSA) and positive airway pressure (PAP) therapy on cardiac remodeling and NT-proBNP (N-terminal pro-B-type natriuretic peptide) levels. We compared NT-proBNP and cardiac magnetic resonance imaging in adults without OSA (n=56) and nonobese (n=73; body mass index <30 kg/m2) and obese (n=136; body mass index ≥30 kg/m2) adults with OSA. We also investigated these traits in nonobese (n=45) and obese (n=78) participants with OSA adherent to 4 months of PAP treatment. At baseline, left ventricular mass to end-diastolic volu

A Case of a Small-Breed Dog with Diet-Related Dilated Cardiomyopathy Showing Marked Improvements in Cardiac Morphology and Function after Dietary Modification.

An 11-year-old intact female Papillion weighing 2.1 kg was referred to our institution with the main complaint of shallow, rapid breathing. At the first visit (day 0), although clinical signs improved due to the use of medication from the primary hospital, transthoracic radiography and echocardiography revealed left heart enlargement and left ventricular dysfunction. A clinical diagnosis of dilated cardiomyopathy (DCM) was made and oral administration of pimobendan, temocapril, and taurine was initiated. However, on day 10, the respiratory status worsened and furosemide was prescribed. On day

Torsional motion of the left ventricle does not affect ventricular fluid dynamics of both foetal and adult hearts.

Left ventricular torsion is caused by shortening and relaxation of the helical fibres in the myocardium, and is thought to be an optimal configuration for minimizing myocardial tissue strains. Characteristics of torsional motion has also been proposed to be markers for cardiac dysfunction. However, its effects on fluid and energy dynamics in the left ventricle have not been comprehensively investigated. To investigate this, we performed image-based flow simulations on five healthy adult porcine and two healthy human foetal left ventricles (representing two different length scales) at different

Catheter intervention for congenital heart disease at risk of circulatory failure.

Percutaneous interventions complement other therapeutic modalities in the management of patients with heart failure secondary to congenital heart disease. The systemic left ventricle can fail when submitted to excessive afterload as seen with critical aortic stenosis or coarctation. Balloon dilation of the aortic valve is safe and effective in establishing adequate forward flow and improving myocardial function in neonates. A large left-to-right shunt caused by a ventricular septal defect or a patent ductus arteriosus can lead to left ventricular failure. In selected infants, device closure of

Arrhythmogenic Right Ventricular Cardiomyopathy in an Older Private Pilot.

BACKGROUND: Arrhythmogenic right ventricular cardiomyopathy (ARVC) results in progressive replacement of right ventricular myocardium with fibrofatty tissue. This condition commonly presents with sudden cardiac death, cardiac arrest, or sustained ventricular tachycardia among young adults. Considering the likelihood of sudden incapacitation, a diagnosis of ARVC usually signals the end of a career in aviation. However, the morbidity pattern among those diagnosed later in life may differ as compared to the young.CASE REPORT: A private pilot, in his 60s, applied for renewal of an ICAO Class 2 med

Diastolic heart failure versus diastolic dysfunction: difference in renal function.

BACKGROUND: Despite the common finding of diastolic dysfunction with a preserved ejection fraction on routine echocardiography in elderly patients, it is unknown why some patients with isolated diastolic dysfunction are asymptomatic whereas others develop diastolic heart failure (ie, signs and symptoms of congestive heart failure). HYPOTHESIS: We hypothesized that renal insufficiency is more common in those patients with diastolic heart failure than those with diastolic dysfunction; it is intrinsic renal insufficiency that determines whether diastolic dysfunction becomes symptomatic. Mitochondrial Oxidative Stress and Cardiac Dysfunction in TERT Deficient Progeria Mice.

BACKGROUND: Heart failure (HF) is a major cause of hospitalisation in the elderly. Its incidence increases with aging and the presence of risk factors such as hypertension and diabetes. Aging-related myocardial fibrosis, characterised by alterations in the extracellular matrix and myocardial structure, leads to impairment of cardiac function. However, the specific mechanisms linking mitochondrial oxidative stress to cardiac remodelling in aging remain unclear. OBJECTIVE: To elucidate the mechanisms underlying aging-related ventricular electrical and structural remodelling and to determine the

Influence of right ventricular pressure and volume overload on right and left ventricular diastolic function.

BACKGROUND: Ventricular interdependence may account for altered ventricular mechanics in congenital heart disease. The present study aimed to identify differences in load-dependent right ventricular (RV)-left ventricular (LV) interactions in porcine models of pulmonary stenosis (PS) and pulmonary insufficiency (PI) by invasive admittance-derived hemodynamics in conjunction with noninvasive cardiovascular magnetic resonance (CMR). METHODS: Seventeen pigs were used in the study (7 with PS, 7 with PI, and 3 controls). Progressive PS was created by tightening a Teflon tape around the pulmonary ar

Current indication and practical management of percutaneous left ventricular assist device support therapy in Japan.

Impella (Abiomed, Danvers, MA, USA) is a recently-innovated (commercially available from 2017 in Japan) percutaneous left ventricular assist device which is inserted percutaneously and transfers blood from the left ventricle to the ascending aorta, improving systemic circulation and end-organ dysfunction as well as unloading left ventricle in patients with cardiogenic shock. Impella has not yet shown a significant survival benefit in patients with cardiogenic shock compared to intra-aortic balloon pump in randomized control trials, but gives powerful circulatory support immediately with minima

Influence of pharmacologic intervention and valvular heart disease on left ventricular ejection phase indices: a review.

Echocardiography represents an established noninvasive technique to evaluate left ventricular (LV) performance. The LV ejection phase indices may be divided into three categories: (1) volume-dependent, (2) circumferential, and (3) wall thickening. Since the latter two are not volume dependent, they are clinically more useful in patients with dilated left ventricles. These parameters have limited clinical value in the presence of left-sided volume overload conditions (e g, mitral regurgitation and aortic regurgitation) since significant LV dysfunction may exist in the presence of normal ejectio

Afterload mismatch in aortic and mitral valve disease: implications for surgical therapy.

In the management of patients with valvular heart disease, an understanding of the effects of altered loading conditions on the left ventricle is important in reaching a proper decision concerning the timing of corrective operation. In acquired valvular aortic stenosis, concentric hypertrophy generally maintains left ventricular chamber size and ejection fraction within normal limits, but in late stage disease function can deteriorate as preload reserve is lost and aortic stenosis progresses. In this setting, even when the ejection fraction is markedly reduced (less than 25%), it can improve t

Successful surgical transmitral removal of left ventricular thrombus after acute anterior myocardial infarction without left ventriculotomy.

Left ventricular thrombus (LVT) is known as a life-threatening complication of acute myocardial infarction, in terms of sequential systemic embolization. When an LVT is found to be sufficiently large or mobile, not only anticoagulation therapy but also surgical thrombectomy should be administered immediately to prevent embolic events. Generally, since infarcted myocardium is comparatively fragile, ventriculotomy may result in anastomotic failure or further deterioration of LV function. We report herein a case of transmitral removal of LVT by which we successfully avoided ventriculotomy. A 50-y

Integrated MR imaging approach to valvular heart disease.

With development of cine and velocity encoded magnetic resonance imaging, it is now feasible to detect and quantify aortic and mitral stenosis and regurgitation accurately. In addition, magnetic resonance imaging has the capabilities to assess simultaneously left and right ventricular mass, volumes, and function precisely. The high accuracy and reproducibility of magnetic resonance imaging in quantification of regurgitation and ventricular function has the potential to provide improved monitoring of therapy and optimal timing of surgery in patients with valvular dysfunction. In comparison to e

Preserved and reduced ejection fraction manifest as two unique phenotypes of diastolic dysfunction-a mechanistic study.

AIMS: Diastolic dysfunction is commonplace in patients with preserved (pEF) and reduced (rEF) ejection fraction (EF). Differences in treatment response suggest that these classifications represent two unique phenotypes of diastolic dysfunction. We aimed to evaluate if patients with pEF and rEF would exhibit different mechanics of diastolic dysfunction. Patients with normal EF and normal diastolic function, or grade 2 or 3 diastolic dysfunction with pEF or rEF by echocardiography, were retrospectively identified. METHODS AND RESULTS: Diastolic function was assessed using the parameterized dias

The influence of accessory pathway localization on left ventricular mechanics in adult patients with Wolff-Parkinson-White syndrome: a speckle-tracking echocardiographic investigation.

BACKGROUND: Ventricular pre-excitation can cause left ventricular (LV) mechanical dyssynchrony and dysfunction. However, the specific impact of accessory pathway (AP) location on LV mechanics remains incompletely characterized. This study utilized two-dimensional speckle-tracking echocardiography (2D-STE) to quantify and compare LV deformation and synchrony across these anatomical subgroups. METHODS: We retrospectively analyzed 91 patients with Wolff-Parkinson-White (WPW) syndrome (age 38.2±1.4 years) and 56 matched controls using 2D-STE. Global and regional LV deformation and synchrony were
