Asymptomatic ALCAPA with Preserved Myocardial Function in a 3-Year-Old Child.

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital anomaly. Its clinical course is typically severe in infancy, leading to left ventricular ischemia, cardiogenic shock, and high mortality without surgical intervention. We describe a rare case of a 3-year-old girl diagnosed with ALCAPA, showing extensive right-to-left collaterals, preserved left ventricular function, and minimal myocardial injury.

Percutaneous device closure of hemodynamically significant coronary fistula following endomyocardial biopsy in a pediatric heart transplant patient.

Coronary artery fistula is a rare but well-documented complication of right ventricular endomyocardial biopsy, particularly in the adult population. Typically, these fistulae never reach clinical or hemodynamic significance, but some may cause coronary steal and ventricular dysfunction. We report a case of a significant coronary artery fistula requiring device closure in the cardiac catheterization laboratory with subsequent improvement of clinical symptoms and cardiac function.

Transcatheter management of mid-aortic syndrome secondary to myofibroma presenting in infancy with severe left ventricular dysfunction: a case report.

In this study, we summarise a case of a myofibroma causing mid-aortic syndrome due to obstruction of the distal thoracic and abdominal aorta leading to severe left ventricular dysfunction. The patient was managed with percutaneous intervention via balloon dilation and stent placement. On follow-up, the patient has normalisation of ventricular function, is off anti-hypertensives, and is being monitored for re-stenosis.

Use of extended polytetrafluoroethylene (ePTFE) chordae to re-establish annular-papillary connection after mitral valve excision.

Preserving mitral annular-papillary muscle continuity during mitral valve replacement improves left ventricular function and may help prevent ventricular rupture. In patients with severe rheumatic or myxomatous mitral valve involvement, retaining native chordae may not be possible. We describe a technique to recreate the papillary muscle-mitral annular continuity with ePTFE sutures when excision of a portion or all of the subvalvular apparatus is required.

Ventricular dysfunction consequences of mechanical dyssynchrony in isolated complete right bundle branch block versus left bundle branch block.

BACKGROUND: Complete bundle branch block in individuals without structural heart disease is known as isolated complete bundle branch block. Isolated complete left bundle branch block (CLBBB) is correlated with ventricular dysfunction secondary to dyssynchrony; however, few studies have investigated isolated complete right bundle branch block (CRBBB), which was previously considered benign but was recently found to be associated with adverse cardiovascular outcomes. This study aimed to evaluate cardiac mechanical synchrony, and systolic and diastolic function in patients with isolated CRBBB and

A case of progressive right ventricular failure with ventricular arrhythmia and aortic insufficiency after implantable left ventricular assist device implantation.

UNLABELLED: Right ventricular failure (RVF) is a serious complication after left ventricular assist device (LVAD) implantation. In this report, a case of RVF that developed over two years after LVAD implantation is presented. The patient was a 12-year-old male with dilated phase of hypertrophic cardiomyopathy. He had no risk factors for early or late-onset RVF. However, his right ventricular function worsened after he developed ventricular arrhythmia (VA), and right ventricular dysfunction became exacerbated with an increasing frequency of VAs. He also developed moderate aortic insufficiency (

Importance of venting the left ventricle in aortic valve surgery.

Routine use of left ventricular vent is controversial in patients undergoing open heart surgery. However, surgeons use it during valvular surgery to maintain a dry field to make the operation easier. In addition it helps to prevent left ventricular distention during the critical period of rewarming and reperfusion, if ventricular function does not return immediately following the release of aortic cross clamp. In our country, patients present for valvular surgery at a very late stage and they often have severe left ventricular hypertrophy. This may affect the return of cardiac rhythm after the

End-to-End Resection for Neonatal Aortic Coarctation with Severe Left Ventricular Dysfunction and Large Ventricular Septal Defect: A Case Report in a 23-Day-Old Infant.

Coarctation of the aorta (CoA) may occur alone or in association with other congenital heart defects, including ventricular septal defect (VSD). The coexistence of CoA with a large VSD and decreased left ventricular ejection fraction (LVEF) in neonates is uncommon and presents significant diagnostic and therapeutic challenges. We report the case of a 23-day-old female infant who presented with acute respiratory distress and was found to have CoA, a large membranous VSD, severe pulmonary hypertension, and markedly decreased LVEF. The patient underwent urgent surgical repair consisting of end-to

Right ventricular dysfunction in the critically ill. Echocardiographic evaluation.

Right ventricular dysfunction is common in critically ill patients, and is associated with increased mortality. Its diagnosis moreover remains challenging. In this review, we aim to outline the potential mechanisms underlying abnormal biomechanics of the right ventricle and the different injury phenotypes. A comprehensive understanding of the pathophysiology and natural history of right ventricular injury can be informative for the intensivist in the diagnosis and management of this condition, and may serve to guide individualized treatment strategies. We describe the main recommended paramete

Irregular Ventricular Tachycardia as a Mechanism of Stabilization of Mechanoelectrical Processes in Canine Heart under Conditions of Antiorthostatic Hypokinesia.

We studied electrophysiological mechanisms of ventricular arrhythmias in dogs (n=7) under conditions of antiorthostatic hypokinesia (head-down tilt 45°). Abnormal transmural heterogeneity of repolarization in the base and apex of the left ventricle and increased dispersion of myocardial repolarization were revealed. By minute 30 of antiorthostatic hypokinesia, an increase in the duration of repolarization was revealed after a period of ventricular arrhythmia in all segments and regions of heart ventricles, which was accompanied by impairment of the pumping function of the heart. Physiologic versus anatomic repair of congenitally corrected transposition of the great arteries.

When biventricular repair of congenitally corrected transposition of the great arteries (ccTGA) is possible, complete anatomic (left ventricle to the aorta, right ventricle to the pulmonary artery, and associated atrial baffle operation) or physiologic (left ventricle to pulmonary artery, right ventricle to aorta) connections are complex and associated with significant long-term problems such as arrhythmias, subaortic obstruction, valvar dysfunction, ventricular-to-pulmonary artery conduit changes, and deteriorating ventricular function. Recent advances have supported physiologic, anatomic, an

[Selective increase of ventricular pacing threshold with complete exit block following DC cardioversion in a patient with a dual chamber pacemaker].

We report the case of a 78-year old patient with a dual chamber pacemaker, who was admitted for cardioversion of atrial tachycardia. Transthoracic DC shock of 160 J was followed by transient loss of ventricular capture with complete exit-block and severe nodal bradycardia. Subsequent analysis of stimulation thresholds revealed a marked rise in the ventricular threshold only, whereas atrial threshold was unchanged. The selective dysfunction of ventricular capture is most likely caused by current-induced tissue damage at the electrode-endomyocardial interface by preferential shunting of high ele

Early Detection of Bi-ventricular and Atrial Mechanical Dysfunction Using Two-Dimensional Speckle Tracking Echocardiography in Patients with Sarcoidosis.

PURPOSE: Sarcoidosis is a systemic inflammatory disease with unknown etiology involving several organs. Myocardial involvement, pericarditis, severe rhythm abnormalities, and heart valve disease due to papillary muscle dysfunction are some of the cardiac manifestations. Conventional echocardiographic methods remain insufficient for the determination of subclinical myocardial dysfunction in patients with sarcoidosis. In our study, we investigated the impact of sarcoidosis on bi-ventricular and atrial functions using two-dimensional (2D) speckle tracking echocardiography (STE). METHODS: Forty p

Improving Repair Durability in Severe Ischemic Mitral Regurgitation: Revisiting Patient Selection and Adjunctive Repair Techniques.

Ischemic mitral regurgitation (IMR) is a complex heterogeneous complication following myocardial infarction, characterized by left ventricular (LV) remodeling and subsequent valvular distortion. The primary mechanisms include papillary muscle displacement, mitral leaflet tethering, and impaired coaptation following annular dilatation. IMR is associated with poor prognosis and an increased incidence of heart failure. We reviewed studies on the surgical management of IMR published over the past 2 decades. While mitral valve repair has been favored for its advantages of low perioperative mortalit

Single-Barrel, Double-Barrel, and Fenestrated Endografts to Facilitate Transcatheter Pulmonary Valve Replacement in Large RVOT.

OBJECTIVES: The aim of this study was to test the hypothesis that narrowing the landing zone using commercially available endografts would enable transcatheter pulmonary valve replacement (TPVR) using commercially available transcatheter heart valves. BACKGROUND: TPVR is challenging in an outsized native or patch-repaired right ventricular outflow tract (RVOT). Downsizing the RVOT for TPVR is currently possible only using investigational devices. In patients ineligible because of excessive RVOT size, TPVR landing zones were created using commercially available endografts. METHODS: Consecutiv

The relationship between right heart and aerobic capacity in large cohort of young elite athletes.

We sought to investigate right heart remodeling and function in elite athlees, as well as the relationship between parameters of right ventricular (RV) and right atrial (RA) remodeling and indices of aerobic capacity. Elite male athletes (n = 352) underwent echocardiographic examination including the evaluation of RV and RA parameters. Maximal cardiopulmonary exercise testing was performed to measure maximal oxygen consumption (VO2max), ventilatory anaerobic threshold (VAT) and heart rate reserve (HRR). The right heart remodeling was different between groups. Soccer players had significantly h
