Ultra-high dose radiation for Liver metastasis using MR-guided TReatment with stereotactic Ablative Single-fraction (ULTRAS): study protocol for a phase III randomized controlled trial.

INTRODUCTION: Liver metastases are common in several malignancies, with only 15-20% of patients eligible for surgical resection. For non-surgical candidates, stereotactic body radiation therapy (SBRT) is an effective alternative with acceptable local control (LC) rates. Higher biologically effective doses (BED10 > 100 Gy10) are associated with improved LC, and single-fraction ultra-high dose SBRT (35-40 Gy) has demonstrated excellent LC with minimal toxicity. MR-guided adaptive SBRT enhances precision and may optimizing single-fraction SBRT for liver metastases. METHODS AND ANALYSIS: ULTRAS i

Immunosenescence in Older Patients with Psoriasis: Mechanistic Insights and Opportunities for Biologic Therapy.

Psoriasis is a chronic systemic inflammatory disease that is increasingly seen in older patients. As the immune system ages, the mechanisms that normally keep immune responses in balance become less precise. The adaptive immune system is reshaped both quantitatively and structurally, whereas innate immunity becomes less well regulated rather than simply less active. Age-related changes in the skin microenvironment may also help sustain local inflammatory signaling. In older patients with psoriasis, these age-related immune changes intersect with the pathways already known to drive disease. Th1

GraESM-FuseDTA: adaptive gated multimodal fusion of graph neural networks and protein language models for robust drug-target affinity prediction.

Accurate drug-target affinity prediction is essential for virtual screening, lead compound prioritization, and drug repositioning, especially under cold-start scenarios involving unseen compounds, unseen targets, or unseen drug-target combinations. However, existing methods still face limitations in transferable molecular representation, contextual protein encoding, statistically reliable performance comparison, and biologically interpretable candidate prioritization. In this study, we propose GraESM-FuseDTA, a graph-enhanced protein language model framework for drug-target affinity prediction

Profound cell wall remodeling in Candida parapsilosis during systemic infection confers simultaneous tolerance to echinocandins and host immunity.

Antifungal tolerance can promote the emergence of resistance yet often incurs fitness costs for fungal pathogens. How tolerant populations compensate for these deficits and how they may be therapeutically targeted remain poorly understood. Here, we investigate four sequential Candida parapsilosis isolates recovered from a patient with persistent candidemia and failure of micafungin therapy. The infection was ultimately cleared with liposomal amphotericin B (LAMB). Whole-genome sequencing (WGS) confirmed clonal relatedness and the absence of known resistance mutations. Later isolates displayed

Role of health digital twins in oncology drug development - a primer.

Health Digital Twins (HDTs) have attracted increasing interest as a potential tool for improving both patient care and clinical research, particularly in oncology where clinical trials remain slow, costly, and operationally complex. This primer introduces the concept of digital twins in a form accessible to professionals involved in oncology clinical research and outlines the main components of health digital twins, describes how these systems are generated, and highlights their potential relevance to oncology trials. HDTs are personalized, dynamically updated, predictive, and decision-informi

Collagen-elastin microstructural network and its mechanical implications in the anterior cruciate ligament: A feasibility study.

BACKGROUND: Understanding the microstructural basis of anterior cruciate ligament (ACL) mechanics is important for advancing reconstruction strategies and rehabilitation. Histological studies indicate that collagen and elastin are distributed in a region-specific manner within the ligament, but such heterogeneity has rarely been incorporated into computational models. Finite element (FE) simulations are widely applied in ligament biomechanics, yet their validation against clinically used tools such as the Lachman test with KT2000 arthrometry remains limited. This study aimed to investigate how

Bioinspired additive manufacturing material optimization for increased stiffness and improved strain sensing in robotic limbs.

Robots traversing uneven terrain may require precise calculation of limb kinematics (e.g. foot position from joint angles and segment dimensions) and sensing of force for adaptive walking control. To facilitate these calculations, robots often have very rigid limb segments with a stiff load cell between the limb and end effector (e.g. foot). Insects also must compute kinematics and measure force when walking, but sense forces quite differently, using strain sensors embedded in their exoskeletons distributed across their legs to infer force. Due to this configuration, more rigid leg segments wo

Genome-wide association analysis reveals genetic architecture of fertility traits in Sahiwal heifers and cows.

Fertility traits are pivotal to the productivity and sustainability of dairy production systems; however, their genetic improvement remains constrained due to complex polygenic control and low heritability. Genome-wide association studies (GWAS) offer a powerful approach to identify genomic regions underlying reproductive traits, particularly in tropically adapted indigenous breeds like Sahiwal cattle, which exhibit resilience to environmental stress. This study aimed to elucidate the genetic basis of 8 fertility traits in 303 Sahiwal animals using 50 K SNP genotyping. Animals were genotyped w

Persistent Relief: Protocol for a Feasibility Randomized Trial Comparing Yoga to Pain Education for Patients With Persistent Post-Surgical Pain.

BACKGROUND: Persistent post-surgical pain, a chronic pain syndrome with few effective treatments, has spiked in recent years due to the increase in life expectancy and the number of surgical procedures performed. Studies have shown that non-pharmacological interventions, such as yoga, may help manage chronic pain, reducing pain severity and interference. Despite emerging evidence of the efficacy of yoga in managing chronic pain symptoms, rigorous studies evaluating its use in patients with persistent post-surgical pain have not been conducted. This study describes protocols used in Persistent

Interpretable multimodal PET/CT-EHR fusion via mixture-of-experts for prognostic stratification in mantle cell lymphoma: a multicenter study.

BACKGROUND: Mantle cell lymphoma (MCL) is a rare, biologically heterogeneous B-cell malignancy with highly variable outcomes. Existing prognostic tools are suboptimal. We developed an interpretable deep learning framework integrating baseline [18F]FDG PET/CT and electronic health record (EHR) data for individualized risk stratification. METHODS: In this multicenter study, 187 treatment-naïve MCL patients were analyzed. A mixture-of-experts (MoE) fusion network integrated multimodal representations from PET/CT and EHR data. Expert modules comprising vision encoders, radiomics extractors, and a

Exercise-Induced Shear Stress, Endothelial Glycocalyx Remodeling, and Atherosclerotic Plaque Stability: A Mechanistic Review.

Acute cardiovascular events driven by atherosclerosis primarily originate from thrombosis triggered by vulnerable plaque rupture or endothelial erosion. Endothelial barrier destabilization-characterized by glycocalyx impairment, intercellular junction disassembly, and abnormal cytoskeletal tension-is a core upstream pathological stage that promotes atherogenic lipoprotein leakage, inflammatory cell infiltration, and matrix degradation. Hemodynamics, primarily through wall shear stress (WSS), shape the spatial distribution and plaque phenotypes of atherosclerosis; notably, low or oscillatory sh

A network-centric approach reveals novel pathways impacted by Prader-Willi Syndrome.

Prader-Willi Syndrome (PWS), a rare multi-system disorder characterized by insatiable appetite, growth abnormalities, and cognitive delay, results from genetic defects in a paternally expressed region of chromosome 15, q11.2-q13. This region contains several protein-coding genes and several genes encoding small nucleolar RNA (snoRNAs), including the SNORD116 gene cluster, but their exact role in PWS remains unclear. Since snoRNAs have wide-ranging effects on protein expression and proteins interact in a complex network, the genetic aberrations causing PWS are likely to cause far-reaching indir

AResKGLM: a graph-grounded language-model framework for interpretable multi-hop antimicrobial resistance reasoning.

Antimicrobial resistance (AMR) threatens microbiology and microbiome bioinformatics because resistance phenotypes are shaped by interactions among genes, mobile genetic elements, and functional environments across microbial communities. Prioritizing resistance determinants requires models that reason across knowledge graphs (KGs) linking genes, proteins, pathways, drugs, and microbial phenotypes. Existing graph-based methods compress this evidence into scalar scores, whereas large language models can produce explanations not grounded in structured evidence. We developed AResKGLM (Antimicrobial

Childhood dyslexia risk elevated by heavy metal mixtures from e-waste: A machine learning-driven mixture modeling study.

Environmental heavy metal mixtures from informal e-waste recycling are potential neurotoxicants, but their link to developmental dyslexia remains unclear. This study aimed to investigate whether exposure to heavy metal mixtures in an e-waste recycling environment is associated with dyslexia risk, to identify key contributing metals and their interactions, and to explore potential biological mechanisms. This two-stage study in Guiyu, China-a major global e-waste recycling area-involved initial screening of 2520 primary-school children for dyslexia, followed by a case-control analysis including

Functional validation of driver mutation-specific uveal melanoma biomarkers: role of COL9A3 in cancer cell plasticity.

Uveal melanoma (UM) is a deadly ocular malignancy with well-described genetic alterations that predict disease outcome. However, our current understanding of the biological underpinnings of high-risk uveal melanoma progression remains relatively limited. Using RNA expression profiles from 250 patients with UM, we identified 12 novel biomarkers associated with high-risk UM, with protein expression level validation of the top two candidates: the transcriptional regulator RBFOX2 and matrix protein COL9A3. Moreover, we investigated the functional contribution of COL9A3 via its overexpression in EI

A recurrent CCDC82 frameshift variant associated with syndromic neurodevelopmental disorder in a consanguineous Pakistani family.

BACKGROUND: Intellectual disabilities (IDs) are part of neurodevelopmental disorders (NDDs) and are genetically heterogeneous conditions characterized by impairments in cognition, learning, and adaptive functioning. Despite advances in gene discovery, many individuals, particularly those from understudied populations, remain without a molecular diagnosis. Recent reports implicate CCDC82 (HGNC: 26282) as an autosomal recessive ID gene, although the phenotypic spectrum and biological context remain incompletely defined. METHODS: Exome sequencing (ES) was performed in a consanguineous Pakistani

Endometriosis as a chronic endothelial disorder: thromboinflammation, vascular reprogramming, and systemic cardiovascular risk.

Microvascular thromboinflammation is a pervasive yet under-recognized driver of multisystem morbidity that links focal endothelial injury to distal organ dysfunction through interacting cellular and plasmatic networks. Contemporary paradigms remain fragmented: clinicians often treat symptoms or isolated pathways, trials enroll biologically heterogenous populations, and candidate biomarkers lack harmonized standards-together impeding translation from mechanistic insight to durable patient benefit. In this review we synthesize the evidence into a cohesive, systems-level architecture in which end

Progress in reprogramming failed graft-versus-leukemia immunity in acute myeloid leukemia relapse after allogeneic transplantation.

Relapse of acute myeloid leukemia (AML) after allogeneic hematopoietic stem cell transplantation (allo-HSCT) affects 40-50% of recipients and remains the leading cause of posttransplant mortality; median overall survival is approximately 5 months and 1-year survival is below 20%. Conventional salvage-chemotherapy, immunosuppression withdrawal, donor lymphocyte infusion (DLI), hypomethylating agents, targeted agents, and second transplantation-benefits a minority and has improved only incrementally over two decades. Relapse in this setting reflects leukemia that has survived an engrafted alloge

Modelling the impact of larviciding as a supplementary malaria vector control intervention in rural south-eastern Tanzania: A district-level simulation study.

Combining larviciding with insecticide treated nets (ITNs) can reduce malaria transmission. However, most modelling analyses use generalized scenarios rather than incorporating local epidemiological and ecological contexts. In Tanzania and other countries, larviciding is increasingly being prioritized in national strategies, with growing advocacy for its broader implementation, to achieving sustained malaria reduction. District-specific modelling is therefore essential to capture variation in transmission ecology, seasonality, and varying coverage levels, providing evidence that is both rigoro
