INTRODUCTION: This paper deals with the dynamic modelling of human walking. The main focus of this research was to optimise the function of the orthosis in patients with neuropathic feet, based on the kinematics data from different categories of neuropathic patients.
METHODS: The patient's body on the sagittal plane was modelled for calculating the torques generated in joints. The kinematics data required for mathematical modelling of the patients were obtained from the films of patients captured by high speed camera, and then the films were analysed through a motion analysis software. BACKGROUND AND OBJECTIVE: The carotid-femoral pulse wave velocity is a well-established index of aortic stiffness, and the brachial-ankle pulse wave velocity (baPWV) has recently been developed as a new method for evaluating arterial stiffness. However, whether the baPWV is also useful in evaluating the risk of stroke is currently controversial. Moreover, it is also unknown which types of stroke and neuroradiological findings are associated with increased arterial stiffness. We investigated the association between the baPWV and risk of stroke using a case-control design.

Charcot osteoarthropathy of the foot is a chronic and progressive disease of bone and joint associated with a risk of amputation. The main problems encountered in this process are osteopenia, fragmentation of the bones of the foot and ankle, joint subluxation or even dislocation, ulceration of the skin and the development of deep sepsis. We report our experience of a series of 20 patients with Charcot osteoarthropathy of the foot and ankle treated with an Ilizarov external fixator. The mean age of the group was 30 years (21 to 50). Diabetes mellitus was the underlying cause in 18 patients. Fiv

AIM: In contrast to the single evaluation methods used in the past, the combination of multiple tests allows one to obtain a global assessment of the ankle joint. The aim of this study was to determine the reliability of the different tests in a functional test battery.
METHODS: Twenty-four male recreational athletes with unilateral functional ankle instability (FAI) were recruited for this study. One component of the test battery included five different functional ability tests. These tests included a single limb hopping course, single-legged and triple-legged hop for distance, and six and c

BACKGROUND: The potential benefit of hyaluronans in alleviating pain associated with osteoarthritis (OA) in joints other than the knee is of increasing interest. This double-blind, randomized, controlled study examined the safety and efficacy of intraarticular sodium hyaluronate (Hyalgan) in the treatment of pain associated with ankle OA.
MATERIALS AND METHODS: Thirty consecutive patients with ankle OA documented by X-ray were randomized to treatment with five weekly injections of either sodium hyaluronate 2 mL (HYL) or phosphate-buffered saline 2 mL (control) in the tibiotalar joint. The pri

Several lines of evidence indicate that very large G-protein-coupled receptor 1 (Vlgr1) makes up the ankle links that connect the stereocilia of hair cells at their base. Here, we show that the transmembrane protein usherin, the putative transmembrane protein vezatin, and the PDZ (postsynaptic density-95/Discs large/zona occludens-1) domain-containing submembrane protein whirlin are colocalized with Vlgr1 at the stereocilia base in developing cochlear hair cells and are absent in Vlgr1-/- mice that lack the ankle links. Direct in vitro interactions between these four proteins further support t

BACKGROUND: Peripheral arterial disease (PAD) is a common disease that is diagnosed with a screening test called the Ankle Brachial Index (ABI). Different methods of ABI have been described in the literature. We wanted to estimate and compare the sensitivity and specificity of an alternative method of calculating the ABI (LAP ABI, low ankle pressure ABI) with the current method (named high ankle pressure (HAP)), using digital subtraction angiography (DSA) as the gold standard.
METHODS: We reviewed the records of all patients who had undergone DSA at a major academic center between August 2003

Patients with diabetes mellitus may develop plantar flexion contractures (equinus) which may increase forefoot pressure during walking. In order to determine the relationship between equinus and forefoot pressure, we measured forefoot pressure during walking in 27 adult diabetics with a mean age of 66.3 years (sd 7.4) and a mean duration of the condition of 13.4 years (sd 12.6) using an Emed mat. Maximum dorsiflexion of the ankle was determined using a custom device which an examiner used to apply a dorsiflexing torque of 10 Nm (sd 1) for five seconds. Simple linear regression showed that the

INTRODUCTION: Cyclooxygenase (COX)-2 selective inhibitors are attractive candidates for treatment of ankle sprain because of their efficacy as anti-inflammatory and analgesic agents and their overall safety, including lack of effect on platelet aggregation. The objective of this study was to assess the efficacy and tolerability of celecoxib compared with diclofenac slow release (SR) in the treatment of acute ankle sprain in an Asian population.
METHODS: In this seven-day, multicentre, double-blind, randomised, parallel-group trial, 370 patients with first- or second-degree ankle sprain occurr

Prophylactic ankle taping has been considered the mainstay of ankle injury prevention and has been used at all levels of competitive football. An alternative to taping is a semirigid ankle orthosis. This study prospectively compared the incidence of ankle sprains in high school football players during a single season, after randomization to either prophylactic bracing or taping of both ankles. Of 83 athletes followed up for an entire season, 6 ankle sprains occurred, 3 in each treatment group; and there was no statistically significant difference in the incidence of ankle sprains between the 2

BACKGROUND: The relationship between pes planus and injuries of the lower extremity is controversial. However, few studies have used standardized means of evaluating and defining pes planus, and none have had a controlled patient population. The objective of this study was to evaluate an ideal population of physically active individuals to establish a potential correlation between pes planus, as defined by a standardized method, and injuries to the lower extremity.
METHODS: A standardized technique for evaluating arch height, based on a midfoot ratio established by Harris mat print calculatio

STUDY DESIGN: Systematic literature review.
OBJECTIVE: To compare the effectiveness of conventional treatment complemented by supervised rehabilitation training (supervised exercises) with conventional treatment alone for the rehabilitation of acute lateral ankle sprains.
BACKGROUND: Conventional treatment is advocated as a preferable treatment strategy. Whether supervised exercises should complement conventional treatment is unclear.
METHODS AND MEASURES: We searched 5 computerized databases from 1966 to March 2004, checked the reference lists of all studies that fulfilled our eligibility

Fourteen patients, at a mean age of 9.1 years (range 4.1-16.6 years), who had spastic diplegic cerebral palsy were evaluated before and after tendo Achilles lengthening (TAL). Follow-up (by gait analysis) after TAL ranged from 8 to 30 months. A Vicon motion analysis system with six CCD cameras and two AMTI force plates provided three-dimensional measurements of joint motion and moments. The TAL procedure resulted in normal passive dorsiflexion of the ankle joint with the knee at 0 degrees of extension and 90 degrees of flexion, reduced plantarflexion during swing phase, and reduced premature p

Inflamed synovia of the ankle joint after 2-4 weeks of adjuvant injection receives dense sensory innervation. To study the role of sensory nerves on the local inflammation, the relative expression of T helper 1 and 2 lymphocyte (Th1 and Th2) markers was investigated on both axotomized adjuvant arthritic (AA) rats, whose sciatic nerves were resected before adjuvant injection, and on sham-operated ones. Immunohistochemical expressions of CXC chemokine receptor 3 (CXCR3) and CC chemokine receptor 4 (CCR4) were examined and compared with those of Th1 cytokine (interferon-gamma, IFN-gamma), Th2 cyt

A case history of a 25-year-old field hockey player, a member of the German National Field Hockey Team, is presented. The patient could not remember any specific ankle injury, but since the World Indoor Championship in February 2003, he experienced significant but diffuse pain around the posterior ankle, especially while loading the forefoot in hockey training and competition. For 2 months, the patient was unable to run. Conservative treatment failed, and surgery was performed. Posterior ankle arthroscopy revealed a frayed posterior intermalleolar ligament and meniscoid-like scar tissue at the
