The effectiveness and safety of cricothyroidotomy was reviewed at our institution and in the literature. The literature review yielded nine reports on emergent cricothyroidotomy. Out of 320 patients, there were 308 successful airways and 99 survivors. In 172 cases (eight reports), the patient was not in cardiac arrest, and this group accounted for 84 of 99 survivors. Acute complications were rare and included bleeding and misplacement of the airway. Long-term complications were difficult to evaluate because of the small number of survivors, many of whom were in a permanent vegetative state. At

Silicone oil as endotamponade in cases with attached retina is only indicated in a few very selected cases. These are cases with a high risk of intravitreal hemorrhage and a need for quick visual rehabilitation. We examined 23 patients, who had undergone pars plana vitrectomy and silicone oil filling. All patients were diabetics with proliferative retinopathy and recurrent intravitreal hemorrhage. In 14 cases it was the only eye; 11 eyes previously had a vitrectomy. To investigate the increased tendency of hemorrhage in patients receiving valproate (VPA) therapy, we studied coagulation parameters in 30 randomized children of a group of 83 children receiving antiepileptic drug (AED) therapy. Besides a reduction in fibrinogen concentration and platelet count, we observed a significant decrease in factor VIII-complex. In vitro investigations have demonstrated a high F VIII:Rcof potency and a high F VIII:Rcof/F VIII R:Ag ratio of two heat-treated F VIII concentrates. Management of the perineal wound following rectal excision was assessed in 57 patients at the Toronto General Hospital; 40 had ulcerative colitis, 4 had Crohn's disease, 10 had carcinoma of the rectum, 2 had carcinoma of the anus and 1 had anal incontinence. Ileostomy of the distal end of the bypassed segment of small intestine was done twenty-three months after a 28 to 20 cm (12 to 8 inch) end-to-end jejunoileal bypass for obesity (Scott operation) in a forty-eight year old white female, thus creating a Thiry fistula. The ideal operative timing for laparoscopic cholecystectomy (LC) remains controversial, particularly in emergency patients. This study aimed to evaluate the necessity of operative timing for emergency LC. One hundred ninety-four patients who had undergone operative timings were classified into groups of <72h and >72h from the onset of symptoms to the operation. Primary aortoenteric fistula (AEF) is a rare but fatal cause of upper gastrointestinal bleed. A 27-year-old previously healthy man presented with massive hematemesis, melena, severe anemia and hemodynamic instability. Point-of-care ultrasound and chest radiography suggested thoracic aortic pathology, and CT aortography confirmed a descending thoracic aneurysm with contained rupture and fistulous communication with the esophagus. This case highlights the need to consider rare vascular causes in massive hematemesis, where early imaging can facilitate rapid diagnosis and timely intervention.

Von Willebrand disease (VWD) is the most common inherited bleeding disorder and is divided into three types, namely type 1, type 2 (2A, 2B, 2M, 2N), and type 3. We report a case of a 24-year-old Caucasian woman with a rare variety of type 2M VWD. Her von Willebrand factor versus antigen ratio was 0.44 (normal ratio is greater than 0.7) . She was asymptomatic and hence not treated but followed up regularly. VWD is not life-threatening when treated timely.

OBJECTIVE: This feasibility study was designed to evaluate the therapeutic potential of intraarterial urokinase combined with abciximab, an IV platelet glycoprotein IIb/IIIa receptor antagonist, in peripheral arterial thrombosis.
CONCLUSION: The administration of a highly potent glycoprotein IIb/IIIa inhibitor in conjunction with intraarterial fibrinolysis was safe in 14 patients. One distal embolization and no major bleeding occurred. The primary success rate was 100%. Treatment times ranged from 50 min to 8 hr. We found a reocclusion rate of 7% (1/14) at intermediate follow-up.
