[Recurrent instability and luxation of the shoulder].

Instability and recurrent dislocation of the shoulder result from injuries affecting the capsule, ligaments or bones. The positive diagnosis rests on careful clinical investigation where a well-oriented questioning plays an essential role. Paraclinical examinations, such as radiography, ultrasonography, CT and MRI, provide a very accurate assessment of the state of relevant structures. Surgical treatment consists of a stabilizing operation which may be Bankart operation (i.e. suture of the detached capsule onto the anterior part of the glenoid labrum), or a Latarget operation (i.e. screwing of

Treatment of tibial defect with vascularized osteocutaneous pedicled transfer of fibula.

Free osteocutaneous fibular flap transfer has been used for long bone and soft tissue defects in both the lower and upper extremity. Since July, 1984, two cases of tibial defect, with severe scar formation on the anteromedial aspect, have been treated. An ipsilateral, vascularized, osteocutaneous, pedicled flap of the fibula was used to bridge the bony defect and to resurface the wound, after scar excision and bony debridement. The peroneal vessels were severed, leaving one side intact. The entire fibular flap was shifted anteromedially through a tunnel between the anterior and posterior tibia

Treatment of posttraumatic bone defects of the forearm with vascularized fibular grafts. Follow up after fourteen years.

BACKGROUND: Among several techniques proposed for the reconstruction of posttraumatic bone defects of the forearm, that of free vascularised fibular graft (FVFG) is one of the most widely used. PATIENTS AND METHOD: We study the long-term outcomes of 14 patients who underwent FVFG between 1994 and 2009, with a minimum follow up of 8 years and a maximum of 23 years (mean: 13.9 years). Demographic, clinical and radiological variables were collected retrospectively. The DASH scale was used for clinical assessment. RESULTS: Fourteen patients were operated on by the same surgeon, applying FVFG for

Reduction Malarplasty that Uses Malar Setback Without Resection of Malar Body Strip.

BACKGROUND: Various surgical methods have been developed and used to reduce prominent malar bones. The most common reduction malarplasty methods are resection of the bone strip of the malar bone with L-osteotomy or I-osteotomy, followed by setback and fixation. However, these methods could be associated with complications due to the bone strip resection. The present article introduces an effective and safe method that reduces the zygoma without resection of a malar bone strip. METHODS: Through preauricular and intraoral incisions, we performed the current L-osteotomy without resection of the

Clinical outcome of dental implants placed with high insertion torques (up to 176 Ncm).

BACKGROUND: Primary stability can be improved by using a tapered implant in a slightly underprepared implant site. This may lead to high compression forces and elevated insertion torques. It has been postulated that disturbance of the local microcirculation may occur, leading to necrosis of the osteocytes and bone resorption. PURPOSE: Report on the clinical outcome of 42 implants placed with an insertion torque equal or greater than 70 Ncm and evaluate bone levels around these implants. MATERIALS AND METHODS: This prospective study included 48 patients treated with 66 4.5 mm diameter Tapered

Hydroxyapatite-coated acetabular components. Histological and histomorphometric analysis of six cups retrieved at autopsy between three and seven years after successful implantation.

BACKGROUND: Important questions remain regarding the use of hydroxyapatite-coated acetabular components in total hip arthroplasty. What is the relation of resorption of the hydroxyapatite coating to enduring fixation? Will unresorbed or dislodged hydroxyapatite particles cause adverse tissue reactions? Retrieval studies of clinically well-functioning acetabular components should help to answer these questions. METHODS: We examined six clinically successful hydroxyapatite-coated cementless acetabular components that were retrieved at autopsy between 3.3 and 6.6 years after implantation. All co

Union Outcomes in Medial and Central Column Tarsometatarsal Fusions: Single vs Multiple Joints.

BACKGROUND: Tarsometatarsal (TMT) joint arthrodesis is essential for managing surgical management of severe bunion deformities, complex midfoot trauma, and osteoarthritis. Various fixation methods and bone grafting types have been described, but the impact of the number of joints fused on union rates remains unclear. This study aimed to evaluate whether fusing multiple joints affects arthrodesis rates and to compare union outcomes among different fixation constructs and grafting techniques. METHODS: We conducted a single-center retrospective cohort study of 582 TMT fusions, comparing single-

Biomechanical effects of a novel anatomic fusion cage for anterior cervical corpectomy: a finite element study.

BACKGROUND: Anterior cervical corpectomy and fusion (ACCF) is a crucial surgical procedure for addressing cervical degenerative diseases. However, traditional titanium mesh cages (TTMC) are susceptible to complications, such as cage subsidence and internal fixation failure. In this study, we developed a novel anatomic titanium mesh cage (NATMC) and assessed its biomechanical properties through finite element analysis. METHODS: A conventional three-dimensional finite element model comprising cervical vertebrae C2–C7 was established using computed tomography (CT) data. Subsequently, two surgical

Case Report: Soft-tissue-preserving fixation for displaced segmental maxillary fractures involving partially detached bone segments.

BACKGROUND: Displaced segmental maxillary fractures involving partially detached bone segments are difficult to manage because treatment requires both stable fixation and preservation of residual vascularity. Excessive mucoperiosteal elevation may further compromise the blood supply of a displaced segment and increase the risk of bone resorption, sequestration, or necrosis. This report presents two cases of displaced segmental maxillary fractures treated with soft-tissue-preserving fixation strategies. CASE PRESENTATION: Two patients with displaced segmental maxillary fractures were managed u

Clinical Outcomes of Transdiscal Screws for Thoracolumbar Spinal Fractures with Marked Anterior Distraction Gap Accompanied by Diffuse Idiopathic Skeletal Hyperostosis.

Background and Objectives: Diffuse idiopathic skeletal hyperostosis (DISH)-related spinal fractures with marked anterior distraction are highly unstable and pose substantial surgical challenges. The transdiscal screw for diffuse idiopathic skeletal hyperostosis (TSD) technique has been proposed to enhance fixation strength by penetrating adjacent vertebral endplates; however, its clinical utility in large-displacement cases remained unclear. Materials and Methods: In this retrospective study, we reviewed 21 patients with thoracolumbar DISH-related fractures and an anterior fracture gap ≥ 15 mm

Classification and Management Algorithm for Postoperative Wound Complications Following Transforaminal Lumbar Interbody Fusion.

STUDY DESIGN: Retrospective study. PURPOSE: Postoperative wound complications occurring after transforaminal lumbar interbody fusion (TLIF) are unique, as they can involve different tissue zones (subcutaneous, subfascial, osseous, peri-implant, and disc). OVERVIEW OF LITERATURE: Management of postoperative infections occurring after TLIF remains controversial in the context of retention or removal of implants. METHODS: A total of 1,279 consecutive patients (1,520 segments) who underwent TLIF with a minimum follow-up of 1 year were analyzed. Patients with wound complications were classified

Dorsal modified calcaneal plate for extensive midfoot arthrodesis.

BACKGROUND: Extensive midfoot fusions can be challenging because of bone loss, deformity, and soft tissue anatomy. Several options have been advocated, including multiple screw fixation, medial plating, and plantar plating. We report a new technique using a dorsally-placed, modified calcaneal plate for treatment of this difficult clinical problem. METHODS: Patients undergoing extensive (more than four joints) midfoot arthrodeses with a dorsally-placed, modified calcaneal plate between 2000 and 2003 were retrospectively reviewed. Diagnoses included Charcot arthropathy (four), osteoarthritis (t

The relationship between the design, position, and articular wear of acetabular components inserted without cement and the development of pelvic osteolysis.

Between 1983 and 1987, an acetabular component with a unique chamfered-cylinder design was inserted without cement in 134 hips. With use of this design, initial stability is achieved through a cylindrical interference fit with the peripheral rim of the acetabulum, without the need for pegs, spikes, or screws. At an average of sixty-four months (range, forty to ninety-six months) after implantation, follow-up data were available for 113 hips (ninety-three patients). No component had been revised for loosening or was radiographically loose. However, the prevalence of balloon-like osteolysis of t

Percutaneous navigation-assisted versus open posterior fixation for thoracolumbar fractures in ankylosing spinal diseases: a single-institution 15-year cohort analysis.

OBJECTIVE: Ankylosing spinal pathologies including ankylosing spondylitis (AS) and diffuse idiopathic skeletal hyperostosis (DISH) predispose patients to long bone-type unstable fractures after even low-velocity mechanisms of injury. These fractures often require long-segment pedicle screw fixation. Patients with AS and DISH tend to be elderly and have significant comorbidity burdens, emphasizing the importance of minimizing surgical complications. Therefore, the authors investigated outcomes of patients undergoing percutaneous minimally invasive (MIS) pedicle screw fixation versus traditional

Management of Distal Femur Unicondylar Fractures by Calcaneal Buttress Plating: A Prospective Interventional Study.

BACKGROUND: Unicondylar fracture of distal femur accounts for less than 1% of all femoral fractures. As conservative methods have shown substandard results, traditional surgical treatment includes open reduction and internal fixation by Herbert screws, cannulated screws, and conventional lateral locking plates. Though screw fixation has excellent results over long run, there is literature documenting the loosening and loss of articular congruency in elderly patients and in comminuted fracture patterns. Also, the traditional locking plates are precontoured to fit the lateral condyle, whereas th
