Bezoars are masses of indigestible material within the gastrointestinal tract. Trichobezoars, composed of hair, are rare and typically occur in young females with psychiatric conditions. Rapunzel syndrome is a severe variant where the trichobezoar extends into the small intestine. We report a case of a 7-year-old girl presenting with acute abdominal pain, reduced appetite, and a palpable epigastric mass. She had a history of trichophagia and weight loss. Investigations revealed microcytic hypochromic anemia and a large intragastric mass extending into the duodenum, with associated jejunojejuna

BACKGROUND: Congenital insensitivity to pain (CIPA) is an extremely rare syndrome that is caused by a mutation in the NTRK gene. CIPA is characterized by recurrent episodes of unexplained fever, infections, skeletal complications, anhidrosis, mental retardation, and ocular injuries due to the absence of reaction to painful stimuli. Several musculoskeletal complications have been reported and included fractures, avascular necrosis, joint dislocations, soft tissue, and bone infections. The severity of these complications presented complex diagnostic and therapeutic dilemmas. The collaboration of

We present a case of a large bowel obstruction secondary to a large impacted gallstone measuring 5cm in the sigmoid colon. Significant features include a cholecysto-colonic fistula and the presence of a perforation initially missed on imaging. Endoscopic retrieval was attempted, followed by a Hartmann's procedure. This case highlights the significance of operative management in ensuring the safe treatment of colonic gallstone ileus, especially in a rural setting.

BACKGROUND: Degenerative cervical myelopathy (DCM) is the most common cause of adult spinal cord dysfunction. Tourette syndrome (TS) represents a rare cause of DCM that is incompletely characterized in the literature. Patients with TS might develop repetitive involuntary cervical movements that can accelerate spondylotic changes. Therefore, patients with myelopathy secondary to TS or other tic disorders might exhibit DCM symptoms at younger ages. The present study examines the surgical management of DCM secondary to TS in the literature and describes the outcomes of 3 patients who were treated

BACKGROUND: Aggressive vertebral hemangiomas (AVHs) are benign vascular lesions that can exhibit locally invasive behavior. Because of their rarity, standardized treatment guidelines are lacking. We present 2 surgically managed cases, supplemented with intraoperative video, and conduct a thematic literature review evaluating treatment strategies.
OBSERVATIONS: A 66-year-old female presented with low back pain radiating to the groin and gait instability. MRI demonstrated an L1 AVH. She underwent T12-L2 laminectomy, instrumented stabilization, and cement augmentation, followed by radiotherapy.

INTRODUCTION: Hydatid cyst disease, caused by the larval stage of Echinococcus granulosus, is a zoonotic parasitic infection that is endemic in many regions of the world. Musculoskeletal involvement, particularly in the thigh, is rare, accounting for approximately 1-4 % of all hydatid cyst cases. This case series aims to contribute valuable information by presenting three distinct cases of hydatid cyst affecting different compartments of the thigh.
CASES PRESENTATION: We detail the diagnostic process, surgical and medical management strategies, and outcomes observed during the follow-up perio

BACKGROUND: Traumatic injuries to the extrahepatic bile ducts are rare, with an incidence of 0.4%-0.6% in cholecystectomy procedures among adults, particularly following the introduction of laparoscopic cholecystectomy. Among these, the presence of foreign bodies within the biliary tree is exceptionally rare, with obstructive jaundice caused by a bullet lodged in the common hepatic duct being particularly uncommon. This case report aims to share the diagnostic process and the challenges in managing such a rare condition.

The decompression technique can effectively reduce the size of large periapical lesions, minimize tissue damage, and enhance surgical outcomes. This conservative approach allows for better management of extensive lesions, potentially improving patient recovery and decreasing the need for more invasive procedures.

Miller Fisher syndrome (MFS) is a rare variant of Guillain-Barré syndrome (GBS), characterized by the triad of ophthalmoplegia, ataxia, and areflexia, often following a bacterial or viral illness. This case report presents a 58-year-old Hispanic female patient with a preceding streptococcal pharyngitis who developed symptoms of bilateral facial nerve palsy and lower limb ataxia. Cerebrospinal fluid (CSF) analysis revealed albuminocytologic dissociation and, combined with her presentation of ophthalmoplegia, ataxia, and areflexia, supported the diagnosis of MFS. Magnetic resonance imaging (MRI)

First branchial cleft anomalies (FBCAs) are uncommon congenital malformations that develop due to incomplete closure of the first branchial cleft during embryonic development. They represent a small proportion of all branchial cleft anomalies. These lesions, which may present as cysts, sinuses, or fistulas, pose significant diagnostic and therapeutic challenges due to their nonspecific clinical manifestations and complex anatomical relationships, particularly with the facial nerve and parotid gland. This case report describes a 47-year-old male patient presenting with recurrent infections and

Hydatidosis is an infection caused by the parasite Echinococcus granulosus (E. granulosus). The liver and lungs are the most commonly affected organs. However, ovarian hydatid cyst (OHC) is a relatively rare manifestation. Herein, we report a case of primary OHC in a 65-year-old woman. Additionally, upon review of reported OHC cases in Iran, a total of 13 cases have been documented in various studies, with our case being the second reported from Tehran. Most cases originated from different regions of Iran, except for the southwest and southeast, with an average patient age of 38 years. The pre

Myiasis refers to an infestation by maggots or fly larvae. Urogenital myiasis, particularly in the uterine cavity, is extremely rare. Previously reported cases involved uterine prolapses, typically managed with hysterectomy. We describe the successful conservative management of myiasis in an episiotomy wound with extension into the uterine cavity. The case involved a woman in her 30s who gave birth to her third child eight days prior and presented with a painful and infected episiotomy site with "worms" emerging from her vagina. Her poverty had led to her malnourishment and poor hygiene; she w

INTRODUCTION: Scrotal lymphedema is a chronic lymphatic disorder characterised by swelling of the scrotum due to lymphatic obstruction, often caused by filarial infection, malignancy, or inguinal surgery. This condition can lead to fibrosis, functional impairment, and recurrent infections. Treatment mainly involves surgical intervention. The presence of pan-urethral warts, as seen in this rare case, complicates management.
CASE PRESENTATION: A 38-year-old Maasai man presented with chronic, massive penoscrotal lymphedema following childhood bilateral inguinal surgery. He developed a secondary

Squamoid eccrine ductal carcinoma (SEDC) is a rare cutaneous malignancy that demonstrates both squamous and eccrine differentiation. With a limited number of published cases in the literature, SEDC remains a diagnostic challenge. This retrospective chart review reports on 27 cases of SEDC identified at our institution and summarizes their clinical and histopathologic characteristics. Clinically, these tumors demonstrated a predilection for sun-exposed areas, favoring the head and neck (59.2%, n=16) of older individuals (median age 74 years). They presented as a papule, nodule, or plaque (48.1%

An uncommon cause of intestinal obstruction is an abdominal cocoon, also known as sclerosing encapsulating peritonitis (SEP). We present the case of a 24-year-old female peritoneal dialysis patient who presented with a picture of complete intestinal obstruction. After reviewing the patient's medical history and acquiring relevant laboratory and imaging data, the decision was made to proceed with surgery. Intraoperatively, however, she had a picture of sclerosing peritonitis. The decision was to terminate the surgery and to take a conservative approach, including total parenteral nutrition. Her

Oral habits in children are of prime concern to a pediatric dentist as the child is in an active growth phase, and it may alter the development of the orofacial area. One of the major etiological factors in the development of malocclusion and other adverse effects on orofacial structures is oral habits during and after preschool age. The habit of sucking one's lower lip is rarely examined; it appears that practitioners tend to associate it with less adverse clinical consequences. It is important to identify harmful oral habits and eliminate them as early as possible. This case report focuses o

IMPORTANCE: Large language models (LLMs) are revolutionizing medical diagnosis and treatment, offering unprecedented accuracy and ease surpassing conventional search engines. Their integration into medical assistance programs will become pivotal for ophthalmologists as an adjunct for practicing evidence-based medicine. Therefore, the diagnostic and treatment accuracy of LLM-generated responses compared with fellowship-trained ophthalmologists can help assess their accuracy and validate their potential utility in ophthalmic subspecialties.
OBJECTIVE: To compare the diagnostic accuracy and comp

BACKGROUND: The occurrence of ovarian hematoma during controlled ovarian stimulation (COS) is very rare. Until now, there is no such case reported in the literature. In this study, an attempt was made to discuss the possible mechanisms for the development of hematoma in such patients, the clinical presentation, monitoring, and management of these cases.
CASE PRESENTATION: A rare case of periovarian hematoma was reported in a patient with a history of endometriosis undergoing ovarian stimulation for in vitro fertilization. On the seventh day of stimulation, the patient complained of severe pai

A primary epithelioid angiosarcoma of the pericardium is a rare soft tissue malignancy. This report describes a young adult woman who presented with progressive dyspnea and pericardial effusion. She was found to have pericardial mass on imaging along with extensive myocardial infiltration. The biopsy of the mass revealed epithelioid angiosarcoma, which was CD34 and CD31-immuno-positive. Due to unresectable disease, she was given a trial of immunotherapy followed by conventional chemotherapy. She showed partial response on interim assessment, but progressed soon after the completion of six cycl
