BACKGROUND: Syringomyelia is an uncommon, poorly understood finding in patients with myotonic dystrophy.
METHODS: We describe a patient with myotonic dystrophy and neck pain in whom an extensive neuroradiologic diagnostic work-up was carried out.
RESULTS: Magnetic resonance imaging revealed a large intramedullary cavity extending from the bulbo-medullary junction to the conus medullaris. After intravenous Gadolinium-DTPA administration, an enhanced nodule was seen at T6. Spinal arteriography showed a single hypervascular nodule and slow flow perimedullary draining veins consistent with heman

We treated 12 patients with multilevel stenosis of the cervical canal after spondylosis or ossification of the posterior longitudinal ligament by an expansive open-door laminoplasty, stabilised by using an anchor system. The preoperative sagittal diameter of the canal was 9.8 mm(+/-2.2) which was increased to 16.1 mm(+/-2.9) after surgery. The mean expansion ratio of the canal was 64% (42 to 100). The anchoring systems did not fail during the follow-up period (mean 29.5 months), and the decompression was maintained. The use of anchor systems to stabilise the posterior elements after laminoplas

Since surgeons sometimes encounter difficulty in keeping self- retaining soft tissue retractors in the proper position for anterior cervical spinal surgery, we have developed a new, simple soft tissue retractor system, which is fixed to the side rails of the operating table via retractor stands. All three joints of the retractor can be tightened simultaneously with a single handle. Each of two retractor blades can keep its position independent of the other thereby maintaining a well-exposed operative field for a long period of time. Fine adjustments of the blade position, after fixation of the

Anterior decompression of a cervical ossified posterior longitudinal ligament (OPLL) which involves the dura mater occasionally results in a postoperative cerebrospinal fluid fistula. Bone window computed tomography (CT) was carried out to detect dural defects in 21 patients with cervical OPLL. Twelve of 21 patients had double-layer OPLL, and nine had single-layer OPLL. Dural defects were present in 10 of the 12 patients with double-layer OPLL compared with only one of the nine patients with single-layer OPLL. In addition, greater stenosis of the spinal canal by the OPLL was also associated wi

Cervical spine lesions require early stabilisation. The fibroscope was used in order to avoid irreversible damage to the medulla during intubation. Twenty-five patients aged between 18 and 70 years old were treated. Pre-medication and local anesthesia of the nostril, rhinopharynx and oropharynx was performed. Local anesthesia of the hypopharynx was omitted to avoid the tussigenic reflex which is dangerous in this pathology. The patient was then deeply sedated maintaining respiratory activity. The tracheal tube was then introduced into the nostril and the fibroscope was then positioned using th

The therapeutic goal in treating patients with degenerative neck pain is prompt return to normal activity with the least diagnostic and therapeutic expense. Although many noninvasive treatment modalities exist, most are based on empiricism and tradition and lack scientific validation. With few exceptions, degenerative cervical pain syndromes require an initial period of conservative therapy, since their natural course favors spontaneous resolution. Patients with a clinical history suggestive of either progressive myelopathy, infection, or malignancy warrant more aggressive initial assessment a

This article considers the entities of cervical radiculopathy and myelopathy. Beginning with a brief historical note and neuroanatomy review of the cord and nerve roots, the discussion proceeds with analysis of symptoms and signs of cervical root disease and then compressive cervical myelopathy. A discussion of diagnostic testing and a differential diagnosis follows. A brief review of conservative medical therapy concludes the article.

OBJECTIVE: To present the clinical and neurological features of a patient suffering from cervical spondylotic myelopathy.
CLINICAL FEATURES: Cervical spondylotic myelopathy (CSM) is a condition in which the vascular and neural structures are compressed by bony spurring and soft tissue hypertrophy, causing ischemic damage to the spinal cord. Although cervical spondylotic myelopathy is the most common cord disorder in older adults, the diagnosis is often missed because the initial symptoms are subtle and the condition usually presents with associated conditions such as nerve root involvement.

This article discusses infectious and inflammatory processes of the cervical spine. Major emphasis is placed on infectious discitis/osteomyelitis and epidural abscess, particularly the epidemiologic, bacteriologic, pathophysiologic, and clinical aspects, as well as the major role played by magnetic resonance imaging and other imaging modalities used in the detection and diagnosis of these processes.

PURPOSE: To describe the CT findings in occipital condyle fractures in patients suffering craniocervical trauma.
METHODS: Six occipital condyle fractures in five patients were analyzed. Because of clinical or plain-film findings, the craniocervical junction in each patient was imaged using thin-section, high-resolution CT. Axial data were reformatted in the coronal plane or in both coronal and sagittal planes. Clinical and radiologic findings associated with occipital condyle fractures reported in the English medical literature were correlated with our cases to determine conclusive predictive

OBJECTIVE: This study determined the effect of change in graft height on the forces across a Smith-Robinson graft as well as across the posterior elements of the same motion segment.
STUDY DESIGN: The study utilizes a strain gauge technique for the measurement of facet joint loading and a subminiature load cell for the measurement of graft loads.
SUMMARY OF BACKGROUND DATA: A number of cases of Smith-Robinson procedures have had some form of collapse of the interspace and graft material after surgery. Some patients with collapse of the graft go on to have prolonged sclerotomal-type pain or p

Cervical myelography and survey radiography was performed on 12 light breed wobbling foals and a new radiographic measurement method was applied for more accurate diagnosis of cervical vertebral instability. Ratios of stenosis of the spinal canal on survey radiography and myelography using relative values in an individual foal were defined on radiograms of lateral flexed position of mid-cervical region, and coincidence between the ratios and histopathological lesions in the cervical spinal cord was investigated. Five of 6 foals had ratios of stenosis on myelography more than 40% at the interve

Clinicians now appreciate the full chain of tissue damage of the interconnecting muscles, tendons, ligaments and fascia: this compromised linkage from the skull through the suboccipital musculature to the cervical spine and anterior/posterior cervical muscles, from the check-rein ligaments and muscles extending from the skull and maxilla to the mandible, from the suprahyoid musculature connecting the mandible through to the hyoid bone, from the hyoid through the infrahyoid to the supporting shoulder girdle, all contribute to damaged interconnecting matrices. Consequently, unresolved tissue dam

Two cases of an unusual association-fibrodysplasia ossificans progressiva and synovial chondromatosis-in non-related children are presented. This association does not seem coincidental and raises several questions about the pathogenesis. A genetic hypothesis related to G proteins is proposed. This is supported by the fact that such abnormalities have been demonstrated in pseudohypoparathyroidism and fibrous dysplasia; these diseases can also be associated with fibrodysplasia ossificans progressiva.

Two nonrelated, moderately mentally retarded girls are described with identical clinical signs and symptoms, i.e. brachycephaly, typical facies with thin lips and microstomia, ectomorphic habitus with extreme long, thin fingers and toes and hypoplastic external genitalia. The clinical findings in the present patients allow for the delineation of a new MCA/MR syndrome.

Nine horses with clinical and radiographic findings of cervical vertebral malformation that were necropsied and examined using frozen cervical spinal cord cross sections were reviewed. Only cases with actual distortion of the spinal cord due to compression were selected. The goal of the study was to determine the morphologic features responsible for narrowing of the spinal canal and compression of the spinal cord. In individual cases, bony changes are associated with osteochondrosis and osteomyelitis of the dorsal articular facets and osteosclerosis of the dorsal cervical lamina. Soft tissue p
