Inferior vena cava filter use at a large community hospital: a retrospective cohort study.

Inferior vena cava (IVC) filters are considered when patients with venous thromboembolism (VTE) develop a contraindication to anticoagulation. Use of IVC filters is increasing, despite associated complications and lack of data on efficacy in reducing VTE-related mortality. We characterized the pattern of IVC filter use at a large community hospital between 2018 and 2022. Specifically, we assessed the indications for IVC filter insertion, filter removal rates, and filter-associated complications. Indications for IVC filters were compared to those outlined by current clinical practice guidelines

Managing COPD exacerbations in primary care.

Chronic obstructive pulmonary disease (COPD) is a common but underdiagnosed lung condition that is frequently managed inappropriately. It impacts poorest communities most, where health inequalities are greatest. New acute symptoms of breathlessness, cough, sputum production and wheeze should prompt clinical suspicion of underlying COPD in someone who is a current or ex-smoker (or has exposure to other risk factors) and be followed by referral for quality-assured spirometry once recovered. Management of COPD exacerbations in primary care includes use of short-acting bronchodilators if mild, and

Glycan cross-feeding supports mutualism between Fusobacterium and the vaginal microbiota.

Women with bacterial vaginosis (BV), an imbalance of the vaginal microbiome, are more likely to be colonized by potential pathogens such as Fusobacterium nucleatum, a bacterium linked with intrauterine infection and preterm birth. However, the conditions and mechanisms supporting pathogen colonization during vaginal dysbiosis remain obscure. We demonstrate that sialidase activity, a diagnostic feature of BV, promoted F. nucleatum foraging and growth on mammalian sialoglycans, a nutrient resource that was otherwise inaccessible because of the lack of endogenous F. nucleatum sialidase. In mice w

3D Synthetic Peptide-based Architectures for the Engineering of the Enteric Nervous System.

Damage of enteric neurons and partial or total loss of selective neuronal populations are reported in intestinal disorders including inflammatory bowel diseases and necrotizing enterocolitis. To develop three-dimensional scaffolds for enteric neurons we propose the decoration of ionic-complementary self-assembling peptide (SAP) hydrogels, namely EAK or EAbuK, with bioactive motives. Our results showed the ability of EAK in supporting neuronal cell attachment and neurite development. Therefore, EAK was covalently conjugated to: RGD, (GRGDSP)4K (fibronectin), FRHRNRKGY (h-vitronectin, named HVP)

Temporal Themes in Periviable Birth: A Qualitative Analysis of Patient Experiences.

Objective Periviable birth accounts for a very small percentage of preterm deliveries but a large proportion of perinatal and neonatal morbidity. Understanding parental experiences during and after periviable deliveries may help healthcare providers determine how to best support women during these medically complex, emotionally charged clinical encounters. Methods This is a qualitative study with a voluntary sample of women who delivered between 22 and 25 weeks gestation at an academic medical center from 2014 to 2016. Women's narratives of each periviable birth experience were transcribed and

Toward the development of a supported employment program for individuals with high-functioning autism in Germany.

Human-human interactions are of central relevance for the success in professional and occupational environments, which also substantially influence quality of life. This is especially true in the case of individuals with high-functioning autism (HFA), who experience deficits in social cognition that often lead to social exclusion and unemployment. Despite good education and high motivation, individuals with HFA do not reach employment rates that are substantially higher than 50 %. This is an alarmingly high rate of unemployment considering that the United Nations have recently emphasized the i

Participant recruitment and retention in a longitudinal study: experience from SARS-CoV-2 cohort in Ethiopia.

BACKGROUND: Longitudinal cohort studies are essential for understanding changes in health over time, yet participant recruitment and retention remain major challenges, especially in low-resource settings. This study describes the strategies, outcomes, and lessons learned from a two-year longitudinal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) cohort study conducted in Ethiopia, focusing on engagement among healthcare workers (HCWs) and community members. METHODS: This prospective observational cohort study was nested within a larger longitudinal SARS-CoV-2 project conducted fr

Parent Perspectives on Optimal Communication About a New Prenatal Diagnosis of Complex Congenital Heart Disease: A Concept Mapping Study.

Prenatal diagnosis of congenital heart disease (CHD) causes parental distress, at least in part due to uncertainties regarding the diagnosis, prognosis, treatment, and family impact. In initial fetal cardiology consultations, over half of dialogue focuses on uncertainties, and clinician communication can influence how parents cope. No prior study has gathered parental perspectives on what would be helpful when communicating about uncertainties in these initial conversations. The objective of this study was to gather parental perspectives on optimal clinician-caregiver communication about uncer

HIV post-exposure prophylaxis in community settings and by lay health workers or through task sharing: a systematic review of effectiveness, case studies, values and preferences, and costs.

INTRODUCTION: Post-exposure prophylaxis (PEP) for HIV prevention has been inadequately promoted, provided and used. Expanded access and task sharing could increase the HIV prevention impact of PEP, but scientific evidence to inform programmatic and policy decisions has not been synthesized. METHODS: To inform World Health Organization guidelines, we conducted a systematic review of studies examining the provision of PEP in community settings, and by trained lay health workers or through task sharing. We searched CINAHL, PsycINFO, PubMed, EMBASE and scientific conferences for studies published

Relationships with caregivers and mental health outcomes among adolescents living with HIV: a prospective cohort study in South Africa.

BACKGROUND: Mental health problems may impact adherence to anti-retroviral treatment, retention in care, and consequently the survival of adolescents living with HIV. The adolescent-caregiver relationship is an important potential source of resilience. However, there is a lack of longitudinal research in sub-Saharan Africa on which aspects of adolescent-caregiver relationships can promote mental health among adolescents living with HIV. We draw on a prospective longitudinal cohort study undertaken in South Africa to address this question. METHODS: The study traced adolescents aged 10-19 initi

Why do community members believe mothers and babies are dying? Behavioral versus situational attribution in rural northern Ghana.

OBJECTIVE: Rates of maternal and neonatal death remain high in the Global South, especially in Sub-Saharan Africa. In addition, indicators vary significantly by geography. This study aimed to understand what communities in northern Ghana with frequent maternal and newborn deaths or near deaths (near-misses) perceive to be the causes. As part of a larger study, four communities in Ghana's Northern Region were identified as areas with high concentrations of deaths and near-misses of mothers and babies. DESIGN: Stakeholders were interviewed using in-depth interviews (IDIs) and focus-group discus

Do elderly clubs in Alexandria meet the needs of their consumers?

The rapid rise in the number of the independent elderly persons in the community has become a world-wide phenomenon. Recently, more attention has been devoted to community based programs for the elderly. One type of these programs offering multiple services is the elderly clubs. These clubs may provide health and physical care beside social interaction and peer group support for the independent elderly in the community. In Alexandria, elderly health and social care provided are not coping with the increasing numbers of the aged population and consequently with their needs and problems. So the

Mental Health Promotion Among Black, Caribbean, and African Immigrants in Canada: Protocol for a Scoping Review.

BACKGROUND: Black, Caribbean, and African immigrant communities in Canada face compounding systemic inequities that undermine mental well-being and limit access to culturally grounded mental health promotion (MHP) strategies. Despite growing national policy attention to these disparities, the landscape of MHP programs and initiatives developed for, with, or by Black, Caribbean, and African populations remains fragmented and poorly synthesized. OBJECTIVE: This scoping review aims to (1) map existing MHP strategies, programs, and activities targeting Black, Caribbean, and African immigrant comm

Exploring the barriers and facilities migrants face in accessing COVID-19 vaccines in Malaysia: A qualitative study.

BACKGROUND: Malaysia provided COVID-19 vaccines for all residents, regardless of citizenship status. However, the extent of vaccine accessibility for migrant populations remains unclear, given the complex healthcare barriers they face. This study explored the barriers migrants faced in accessing COVID-19 vaccines and the measures taken to facilitate their access. METHODS: This qualitative study, conducted between April 2022 and February 2023, involved 32 purposively selected key informants from non-governmental organisations (7), international organisations (2), labour unions (2), healthcare

Addressing key gaps in implementation of mosquito larviciding to accelerate malaria vector control in southern Tanzania: results of a stakeholder engagement process in local district councils.

BACKGROUND: Larval source management was historically one of the most effective malaria control methods but is now widely deprioritized in Africa, where insecticide-treated nets (ITNs) and indoor residual spraying (IRS) are preferred. However, in Tanzania, following initial successes in urban Dar-es-Salaam starting early-2000s, the government now encourages larviciding in both rural and urban councils nationwide to complement other efforts; and a biolarvicide production-plant has been established outside the commercial capital. This study investigated key obstacles and opportunities relevant t

Bacterial Community Dynamics across the Gastrointestinal Tracts of Dairy Calves during Preweaning Development.

Microbial communities play critical roles in the gastrointestinal tracts (GIT) of preruminant calves by influencing performance and health. However, little is known about the establishment of microbial communities in the calf GIT or their dynamics during development. In this study, next-generation sequencing was used to assess changes in the bacterial communities of the rumen, jejunum, cecum, and colon in 26 crossbred calves at four developmental stages (7, 28, 49, and 63 days old). Alpha diversity differed among GIT regions with the lowest diversity and evenness in the jejunum, whereas no cha

Genotype-specific communication profiles in 79,518 individuals with neurodevelopmental disorders.

RATIONALE: Neurodevelopmental disorders (NDDs) are characterised by significant challenges in communication, social interaction, and adaptive function, often impacting quality of life. Previous studies support genetic influences on the communication abilities of individuals with NDD, but were either limited to single genetic conditions or to small cohorts with a limited selection of communication measures. METHODS: We analysed caregiver-reported communication abilities in 79,518 individuals with NDD from the Simons Searchlight and SPARK registries: 4,439 with a CNV-based or monogenic NDD and

Feasibility of Establishing Community Living Labs (CoLLabs) to Improve Access to Prostate Health Resources, Education, Amenities, and Community Health: Protocol for a Pragmatic Clinical Trial.

BACKGROUND: In 2022, nearly 60,000 prostate cancer (CaP) cases were reported among Black men, who face an estimated lifetime risk of approximately 1 in 6, compared with 1 in 8 among White men. The disproportionate burden of CaP in Black men has been attributed to a combination of health-system factors, variations in care processes, and individual- or patient-level determinants. Addressing these multilevel contributors will require innovative strategies to advance prostate health equity. OBJECTIVE: In this paper, we discuss the protocol for assessing the feasibility of establishing a Community

A Pragmatic Trial of a Multilevel Community-Centered Intervention to Reduce Pregnancy Related and Associated Morbidity and Mortality and Disparities: A Study Protocol.

INTRODUCTION: Maternal morbidity and mortality in the US disproportionately affect some women, including the Medicaid-insured, rural, and Black, Hispanic, and Native American populations. The drivers of disparities are complex and there is an urgent need for multilevel interventions focused on pregnancy-related and associated morbidity and mortality (PRAMM). This study will be the first large-scale test of Community Health Worker (CHW) inclusive home visiting and provider/practice/system level improvement strategies that support empowered patients, pro-active providers, and integrates communit
