Prepuberal stress and obesity: effects on serum corticosterone, prolactin, testosterone and precancerous prostate lesions in adult rats.

BACKGROUND: Gluteal muscle contracture (GMC) may cause abnormal spinal alignment as well as hip and pelvic deformities. The spine-pelvis alignment of GMC patients is unclear. This study aimed to describe the spine-pelvis sagittal alignment in patients with GMC and to explore the impact of GMC on the pathogenesis of low back pain (LBP). METHODS: Radiological analysis was performed in 100 patients with GMC and 100 asymptomatic volunteers who acted as the control group. Sagittal parameters were measured by two independent raters and their averages were presented on lateral radiographs of the who

Operative treatment of malunion of a fracture of the proximal aspect of the humerus.

We retrospectively reviewed the medical records, operative reports, and preoperative and postoperative radiographs of thirty-nine patients who had been managed operatively for malunion of a fracture of the proximal aspect of the humerus. The malunions were categorized according to the presence of osseous abnormalities, including malposition of the greater or lesser tuberosity (type I; twenty-eight patients), incongruity of the articular surface (type II; twenty-six patients), and malalignment of the articular segment (type III; sixteen patients). Soft-tissue abnormalities, such as soft-tissue

A Rare Case of Suspected Malignant Hyperthermia in a Three-Day-Old Neonate: A Case Report.

Malignant hyperthermia (MH) is a rare, life-threatening pharmacogenetic disorder of skeletal muscle calcium regulation, most commonly triggered by volatile anesthetics and depolarizing muscle relaxants. While pediatric patients are at increased risk compared to adults, MH is exceedingly rare in neonates, and its clinical presentation may differ by age group, with hyperthermia and generalized rigidity more prominent in the youngest patients. This report describes a three-day-old female who developed a rapid rise in end-tidal CO2, tachycardia, metabolic acidosis, and hyperthermia intraoperativel

Nonarthroplasty Treatment Options for Management of Primary Elbow Osteoarthritis.

Primary elbow osteoarthritis is a functionally limiting condition, predominantly affecting middle-aged men engaged in repetitive manual labor, and is characterized by pain, stiffness, and mechanical symptoms. Evaluation involves a detailed history focusing on pain, loss of motion, and mechanical symptoms, followed by clinical examination for range of motion deficits, crepitus, and instability. Radiographic assessment, including standard radiographs and, when indicated, computed tomography imaging, is essential for staging disease severity, identifying osteophytes, capsular contracture, and joi

Case Report: Dupuytren's Disease in a 12-Year-Old Child; Unusual Clinical Presentation.

INTRODUCTION: Dupuytren's disease is a progressive fibroproliferative disorder primarily affecting men over 40. Its occurrence in children is extremely rare, with fewer than 25 cases reported since 1955. Due to its rarity, it is often overlooked in pediatric patients presenting with palmar masses. The etiology in children remains unclear, and clinical presentation may differ from the adult form, sometimes mimicking other fibromatoses or even malignancies. CASE REPORT: We report the case of a 12-year-old boy with a painful, progressively enlarging palmar mass. Examination revealed a firm subcu

Fournier's Gangrene and Extensive Abdominal Necrotizing Fasciitis Secondary to Perforated Appendicitis Within an Inguinoscrotal Hernia: A Rare Case Report.

Fournier's gangrene (FG) is a life-threatening necrotizing infection rarely triggered by an intra-abdominal process. We report an unusual case of FG and extensive abdominal necrotizing fasciitis secondary to a perforated appendix within an inguinoscrotal hernia. A 66-year-old male presented with a three-day history of progressive abdominal pain and systemic toxicity. Physical examination revealed abdominal contracture, extensive erythema, subcutaneous emphysema of the lower abdomen, and necrotic patches on the right scrotal skin. A computed tomography (CT) scan confirmed FG secondary to acute

Free functional gracilis muscle opponensplasty for thenar reconstruction: Indications, technique, and long-term outcomes.

Thumb opposition is an essential component of human hand function, and loss of the thenar musculature creates substantial disability of the hand. Additionally, first web space contracture as a result of trauma is a substantial limitation in reconstruction of thumb opposition. Reconstruction of thenar function is most commonly performed via opponensplasty tendon transfers. Although beneficial, tendon transfers require cortical retraining and fall short of native function. These may be the only option in the case of median nerve palsy. However, at times, thenar function is lost owing to thenar m

Intramedullary Compression Screw Fixation of Proximal Phalanx Fractures: An Updated Systematic Review.

Proximal phalangeal fractures occur at a higher frequency than other phalanges, but the optimal management of these injuries remains subject to debate. The use of intramedullary screw (IMS) fixation has gained popularity over the last decade and aims to minimize extensor apparatus violation whilst providing adequate construct stability to allow early motion and rehabilitation. This systematic review aims to provide an updated analysis of patient- and clinician-reported outcome measures following fracture fixation using IMS in the proximal phalanx. A literature search was conducted using PubMed

Subcondylar Fossa Reconstruction in Pediatric Patients Following Malunion of Proximal Phalangeal Neck Fractures: Technique and Case Series.

The subcondylar fossa is the bony depression between the articulating surface of the proximal and middle phalanges of the fingers that allows for full flexion at the proximal interphalangeal joint. A bony block may form in the subcondylar fossa secondary to the malunion of a proximal phalangeal neck fracture. If there is a substantial loss of flexion, surgical correction could be indicated. Corrective osteotomy is technically challenging in the proximal interphalangeal joint area as the phalangeal head is small, creating fixation challenges and carrying the risk of avascular necrosis and joint

SMI-Capsular Fibrosis and Biofilm Dynamics: Molecular Mechanisms, Clinical Implications, and Antimicrobial Approaches.

Silicone mammary implants (SMIs) frequently result in capsular fibrosis, which is marked by the overproduction of fibrous tissue surrounding the implant. This review provides a detailed examination of the molecular and immunological mechanisms driving capsular fibrosis, focusing on the role of foreign body responses (FBRs) and microbial biofilm formation. We investigate how microbial adhesion to implant surfaces and biofilm development contribute to persistent inflammation and fibrotic responses. The review critically evaluates antimicrobial strategies, including preoperative antiseptic protoc

Parietal abdominal pain with lower leg discrepancy: a case report.

BACKGROUND: This report involves the first publication describing a case of parietal abdominal pain due to lower limb length discrepancy. CASE PRESENTATION: A Caucasian male patient in his 50s was referred to our rehabilitation department with chronic abdominal pain that began in childhood. This chronic pain was associated with episodes of acute pain that were partially relieved by grade 3 analgesics. The patient was unable to sit for long periods, had recently lost his job, and was unable to participate in recreational activities with his children. Investigations revealed contracture and hyp

Tailored Physical Therapy in a Case of Tenotomy Post Hip Flexion Deformity With Structural Scoliosis: A Case Report.

Retroperitoneal tumours, mainly classified as malignant and benign, have a rare incidence. It includes major parts of the peritoneum, and surgical excision remains the optimal pathway to remove the tumour. As with any surgery, tumour resection comes with its own complications. These complications would manifest differently depending on patient adaptation or compensation for these drawbacks. Physiotherapy and its positive effects as a need after any surgical procedure become a boon when implemented as and when required. The present study describes the case of a 32-year-old woman who has complai

Reconstruction Using Negative Pressure Wound Therapy with a Cotton Filler for Fixation of Male Genital Skin Grafts in Cases of Fournier's Gangrene.

The reliable engraftment of skin grafts into areas with complex shapes can be challenging. Here, we report a case of successful fixation of a genital skin graft using negative pressure wound therapy (NPWT) with RENASYS® Cotton Filler. A 44-year-old male with no relevant medical history underwent split-thickness skin grafting for a genital skin defect caused by Fournier's gangrene. A 0.4-mm sheet graft was applied for the penile skin defect, while 0.4-mm 1.5 times mesh grafting was applied for the testis and spermatic cord. NPWT with a cotton filler was used for seven days of fixation. No posto

A rare case of aplasia cutis congenita.

In this study, we retrospectively analyzed the clinical data of a newborn with aplasia cutis congenita (ACC) to provide insights for diagnosing and treating the disease. It is believed that ACC with an intact skull and a skin defect diameter of less than 2 cm can be treated conservatively. The main strategies include local disinfection and regular dressing changes to promote epithelial regeneration. The lesion can heal over weeks or months through epithelization adjacent to the defect tissue, resulting in a healed contracture scar with a smooth, hairless surface that can be surgically removed

Reconstruction of Postburn Bone Exposed Defect in the Head and Face Using Two Separate Anterolateral Thigh Chimeric Flap: A Case Report.

Reconstruction of soft-tissue defects in the head and face region requires attention to both functional and aesthetic outcomes. In general, large postburn scars are still a daunting challenge for plastic surgeons. Various types of free flaps were previously performed for head and face reconstruction, including the anterolateral thigh (ALT) flap. However, to fully cover large and complex defects, the skin pedicle needs a sufficient width. Thus, we have combined dual ALT flaps that were harvested from both the lateral thighs. In this article, we described the case of a 49-year-old female who was

A Novel Tilted-Plane Ergometer System for Subject-Specific Rehabilitation.

Immobilization due to various reasons can lead to disuse muscle atrophy. If prolonged, the circumstance is exacerbated and may lead to joint contracture, dysfunction, and long-term sequela. Thus, a balanced exercise regimen is crucial. While able-bodied individuals can perform a variety of exercises, bedridden patients typically resort to exercising primarily with bicycle ergometers. However, since the pedaling trajectory with ergometers is confined to the sagittal plane, muscles responsible for medial-lateral movement and balance are not effectively trained. Furthermore, the direction of join

Skin graft fixation with negative pressure wound therapy with instillation and dwelling (NPWTi-d) for contaminated complex wounds of the extremities.

OBJECTIVE: Negative Pressure Wound Therapy (NPWT) is increasingly being used as a major method of skin graft dressing and fixation. Negative Pressure Wound Therapy with Instillation and Dwelling (NPWTi-d) further enhances wound care over regular NPWT. However, only a few reports have been made on its use for skin graft fixation due to concerns of graft maceration or detachment. We used NPWTi-d to fix skin grafts for 4 cases of severely contaminated complex posttraumatic wounds. METHODS: The age ranged from 37 to 72 years, and included trauma of the lower leg, forearm dog bite and incomplete a
