Multidisciplinary approach to lymphedema diagnosis and management.

Lymphedema is a chronic, progressive condition characterized by the accumulation of protein-rich interstitial fluid due to impaired lymphatic transport. It significantly impairs quality of life and presents complex diagnostic and therapeutic challenges. Despite its prevalence, lymphedema remains underdiagnosed and undertreated, in part due to limited provider education and a lack of access to coordinated care. For this reason, a multidisciplinary approach to lymphedema management is crucial. Early diagnosis is critical and requires collaboration across primary care, oncology, vascular medicine

From Weight Gain to Heart Strain: HIV Antiretroviral Adverse Effect Management in Primary Care.

BACKGROUND: More than 40 years after discovering HIV, the prognosis has transformed from an acute, fatal illness, to an average life expectancy approximating that of the general population. Correspondingly, new concerns regarding the management of older adults with HIV and multimorbidity are beginning to emerge. To support the HIV care continuum, people with HIV benefit from having a primary care medical home to address multimorbidity associated with age, genetic disposition, chronic HIV, and/or antiretroviral adverse effects. The interplay between HIV, antiretrovirals, and metabolic condition

Emergency department pain management in special populations.

Pain is a leading cause of emergency department (ED) visits globally, yet certain patient populations experience persistent disparities in their pain management due to physiological complexities, comorbidities, and gaps in evidence-based guidelines. This clinical review focuses on individualized, evidence-based approaches to ED pain management in four vulnerable groups: pregnant and breastfeeding patients, patients with sickle cell disease, geriatric populations, and patients with cancer pain and requiring palliative care. The practical recommendations presented in this review for optimal ED p

Cathelicidin antimicrobial peptides mediate immune protection in marsupial neonates.

Marsupial neonates are born with immature immune systems, making them vulnerable to pathogens. While neonates receive maternal protection, they can also independently combat pathogens, although the mechanisms remain unknown. Using the sugar glider (Petaurus breviceps) as a model, we investigated immunological defense strategies of marsupial neonates. Cathelicidins-a family of antimicrobial peptides expanded in the genomes of marsupials-are highly expressed in developing neutrophils. Sugar glider cathelicidins reside in two genomic clusters, and their coordinated expression is achieved by enhan

Palladium-free Wacker-inspired oxidation: challenges and opportunities in catalysis.

Palladium-catalysed Wacker-type oxidation of olefins to ketones or aldehydes is one of the most prominent homogeneous reactions globally, even performed at a multi-tonne scale. From a fundamental and an applied point of view, it is extremely appealing to replace palladium catalysts by other metal-based catalysts to increase the efficiency, selectivity and sustainability, particularly considering the reactivity of well-defined first-row transition metal complexes as catalysts. In this case, the ligand(s) coordinating to the metal(s) play a major role in controlling selectivity and activity, tha

ATR-hippo drives force signaling to nuclear F-actin and links mechanotransduction to neurological disorders.

The mechanical environment is sensed through cell-matrix contacts with the cytoskeleton, but how signals transit the nuclear envelope to affect cell fate decisions remains unknown. Nuclear actin coordinates chromatin motility during differentiation and genome maintenance, yet it remains unclear how nuclear actin responds to mechanical force. The DNA-damage kinase ataxia telangiectasia and Rad3-related protein (ATR) translocates to the nuclear envelope to protect the nucleus during cell motility or compression. Here, we show that ATR drives nuclear actin assembly via recruitment of Filamin-A to

From Awareness to Understanding: A Knowledge-Focused Framework for Parkinson's Disease Communication.

The 2024 passage of the National Plan to End Parkinson's Act promises federal coordination and resources, creating a pivotal moment to sharpen public communication. Building on this opportunity means addressing unique challenges associated with Parkinson's disease, including the subtle and complex nature of symptoms, a public image dominated by a "tremor-only, older white male" stereotype, and uneven access to specialty care. These factors complicate early detection, stigma reduction, and equitable service use. Drawing on stigma research and communication science, this commentary reviews recen

Genetic Predisposition and Multifocal Cancer: A Complex Case of Lynch Syndrome.

This case report describes the complex presentation and management of a 35-year-old man with synchronous malignancies: A well-differentiated grade 2 neuroendocrine tumor of the pancreas metastasized to the liver and bone, and invasive adenocarcinoma of the sigmoid colon. Strong clinical suspicion for Lynch syndrome arose due to the patient's family history and the loss of MSH1 and PMS2 protein expression in the colon adenocarcinoma. The patient presented with hypoglycemia, abdominal pain, and diarrhea. A multidisciplinary approach confirmed the diagnoses, including imaging studies (computed to

Sphingolipid and methionine metabolism in aging.

Sphingolipids are essential for cell membrane structure and the regulation of organelle functions. Sphingolipid synthesis requires the coordinated activity of multiple organelles, including the endoplasmic reticulum, Golgi, lysosomes and mitochondria, which are connected via membrane contact sites. Metabolic remodeling of sphingolipid pathways is observed in aging and numerous age-related disorders. However, numerous studies have highlighted the complex and species-specific roles of sphingolipid metabolism in aging. In budding yeast, inhibition of sphingolipid synthesis extends lifespan by a m

Helicopter Air Transport of Newborns With Respiratory Distress: Pitfalls, Challenges, and Solutions for Safe Neonatal Airlifting.

Transporting critically ill neonates by helicopter is complex and requires planning, coordination, and management. Neonates are vulnerable to stress at altitude and during rotary-wing transport. Environmental stressors can compromise clinical stability and hinder care. Successful transport begins with stabilization, particularly in cases of respiratory distress. Specialized neonatal transport systems are essential. They should be equipped with continuous physiological monitoring and mechanical support tools that maintain homeostasis throughout the flight. Aviation dynamics are also a contribut

A Rare Case of Bilateral Mesiodens With Dens Invaginatus Obstructing Permanent Incisor Eruption.

The co-occurrence of mesiodens and dens invaginatus (DI) is very uncommon. Such anomalies may impede the eruption of permanent maxillary central incisors and can appear radiographically similar to normally developing teeth. An 11-year-old girl presented with failure of eruption of the permanent maxillary central incisors. Panoramic radiography and cone-beam computed tomography (CBCT) revealed two palatobuccally positioned supernumerary mesiodens, both of which exhibited DI. Surgical extraction of the mesiodens was performed, and guided bone regeneration (GBR) was carried out to preserve the al

The human social cognitive network contains multiple regions within the amygdala.

Reasoning about someone's thoughts and intentions-i.e., forming a "theory of mind"-is a core aspect of social cognition and relies on association areas of the brain that have expanded disproportionately in the human lineage. We recently showed that these association zones comprise parallel distributed networks that, despite occupying adjacent and interdigitated regions, serve dissociable functions. One network is selectively recruited by social cognitive processes. What circuit properties differentiate these parallel networks? Here, we show that social cognitive association areas are intrinsic

[Diagnosis and Therapeutic Management of Bone Disease in Patients with Chronic Kidney Disease or Kidney Transplant Recipients].

Osteoporosis is a chronic skeletal disease characterized by reduced bone mineral density and deteriorated bone microarchitecture, which increases fracture risk. In patients with chronic kidney disease (CKD), osteoporosis management is complicated by disturbances in mineral and bone metabolism (CKD-MBD) that adversely affect bone health. Diagnosis requires a thorough clinical evaluation, including bone mineral density measurement via DEXA, bone microarchitecture assessment with TBS, and analysis of bone turnover biomarkers. Therapeutic management must be personalized and may include anti-resorp

Development of eating skills in infants and toddlers from a neuropediatric perspective.

Early infant feeding and swallowing are complex motor processes involving numerous muscles in coordination, e.g. the orofacial muscles as well as the muscles of the pharynx, larynx and esophagus. The newborn's reflexive drinking develops into the ability to ingest pureed complementary food as infancy progresses. Finally, in the last part of the first year of life, a differentiated eating, chewing and swallowing process develops allowing the voluntary intake of different foods of the family diet. The dietary schedule for the first year of life, which describes the recommended nutrition of infan

Potentiating Activity of GmhA Inhibitors on Gram-Negative Bacteria.

Inhibition of the biosynthesis of bacterial heptoses opens novel perspectives for antimicrobial therapies. The enzyme GmhA responsible for the first committed biosynthetic step catalyzes the conversion of sedoheptulose 7-phosphate into d-glycero-d-manno-heptose 7-phosphate and harbors a Zn2+ ion in the active site. A series of phosphoryl- and phosphonyl-substituted derivatives featuring a hydroxamate moiety were designed and prepared from suitably protected ribose or hexose derivatives. High-resolution crystal structures of GmhA complexed to two N-formyl hydroxamate inhibitors confirmed the bi

From 0D-complex to 3D-MOF: changing the antimicrobial activity of zinc(II) via reaction with aminocinnamic acids.

Combining zinc nitrate with 3- and/or 4- aminocinnamic acid (3-ACA and 4-ACA, respectively) leads to the formation of the 0D complex [Zn(4-AC)2(H2O)2], the 1D coordination polymer [Zn(3-AC)(4-AC)], and the 2D and 3D MOFs [Zn(3-AC)2]∙2H2O and [Zn(4-AC)2]∙H2O, respectively. These compounds result from the deprotonation of the acid molecules, with the resulting 3- and 4-aminocinnamate anions serving as bidentate terminal or bridging ligands. All solids were fully characterized via single crystal and powder X-ray diffraction and thermal techniques. Given the mild antimicrobial properties of cinnam
