19q13.32 microdeletion syndrome: three new cases.

A previous report described a unique phenotype associated with an apparently de novo 732 kb 19q13.32 microdeletion, consisting of intellectual disability, facial asymmetry, ptosis, oculomotor abnormalities, orofacial clefts, cardiac defects, scoliosis and chronic constipation. We report three unrelated patients with developmental delay and dysmorphic features, who were all found to have interstitial 19q13.32 microdeletions of varying sizes. Both the previously reported patient and our Patient 1 with a larger, 1.3-Mb deletion have distinctive dysmorphic features and medical problems, allowing u

A voxel-based morphometry (VBM) analysis of regional grey and white matter volume abnormalities within the speech production network of children who stutter.

It is well documented that neuroanatomical differences exist between adults who stutter and their fluently speaking peers. Specifically, adults who stutter have been found to have more grey matter volume (GMV) in speech relevant regions including inferior frontal gyrus, insula and superior temporal gyrus (Beal et al., 2007; Song et al., 2007). Despite stuttering having its onset in childhood only one study has investigated the neuroanatomical differences between children who do and do not stutter. Chang et al. (2008) reported children who stutter had less GMV in the bilateral inferior frontal

Role of magnetic resonance imaging, cerebrospinal fluid, and electroencephalogram in diagnosis of sporadic Creutzfeldt-Jakob disease.

Sporadic Creutzfeldt-Jakob disease (sCJD) is a rapidly progressive dementia (RPD) that can be difficult to identify antemortem, with definitive diagnosis requiring tissue confirmation. We describe the clinical, magnetic resonance imaging (MRI), cerebrospinal fluid (CSF), and electroencephalogram (EEG) measures of a small cohort of 30 patients evaluated for RPD. Clinical and diagnostic measures were cross-sectionally obtained from 17 sCJD patients (15 definite, two probable), 13 non-prion rapidly progressive dementia patients (npRPD), and 18 unimpaired controls. In a subset of patients (nine sC

Left-right differences on timed motor examination in children.

Age-related change in the difference between left- and right-side speed on motor examination may be an important indicator of maturation. Cortical maturation and myelination of the corpus callosum are considered to be related to increased bilateral skill and speed on timed motor tasks. We compared left minus right foot, hand, and finger speed differences using the Revised Physical and Neurological Assessment for Subtle Signs (PANESS; Denckla, 1985); examining 130 typically developing right-handed children (65 boys, 65 girls) ages 7-14. Timed tasks included right and left sets of 20 toe taps, 1

Ipsilateral silent period: a marker of callosal conduction abnormality in early relapsing-remitting multiple sclerosis?

OBJECTIVE: The corpus callosum (CC) is commonly affected in multiple sclerosis (MS). The ipsilateral silent period (iSP) is a putative electrophysiological marker of callosal demyelination. The purpose of this study was to re-assess, under recently established optimised protocol conditions [Jung P., Ziemann U. Differences of the ipsilateral silent period in small hand muscles. Muscle Nerve in press.], its diagnostic sensitivity in MS, about which conflicting results were reported in previous studies. METHODS: ISP measurements (onset, duration, and depth) were obtained in the abductor pollicis

Leukoencephalopathy and raised brain lactate from heroin vapor inhalation ("chasing the dragon").

BACKGROUND: Inhalation of heated heroin vapor ("chasing the dragon"), which is gaining popularity among drug users seeking to avoid the risks of parenteral drug administration, can produce progressive spongiform leukoencephalopathy. METHODS: We studied the clinical phenotype and course, MRI, MRS, and brain pathology in the first American patients described with this syndrome. RESULTS: Two of the three heroin users studied inhaled heroin pyrolysate together daily over the course of 2 weeks. They developed ataxia, dysmetria, and dysarthria. Patient 1 progressed to an akinetic mute state with d

Midline cerebral morphometry distinguishes frontotemporal dementia and Alzheimer's disease.

We investigated and contrasted midline cerebral structures in frontotemporal dementia (FTD) and Alzheimer's disease (AD). FTD and AD may be difficult to distinguish clinically. FTD typically affects frontal and anterior temporal regions, whereas AD tends to involve more posterior temporal and parietal areas. We hypothesized that disease-specific cerebral alterations would be differentially reflected in corresponding regions of the corpus callosum (CC), pericallosal CSF space (PCS), or their ratio (CC:PCS). Regions-of-interest (ROIs) from midsagittal MRIs in 17 AD, 16 FTD, and 12 elderly contro

Decompressive hemicraniectomy for fulminant acute disseminated encephalomyelitis following dengue fever: Case report and literature review.

BACKGROUND: Acute disseminated encephalomyelitis (ADEM) is a rare immune-mediated demyelinating disorder of the central nervous system that typically occurs after viral or bacterial infections. Although most patients respond favorably to immunomodulatory therapy, a small subset develops fulminant ADEM characterized by malignant cerebral edema, refractory intracranial hypertension, and a high risk of mortality. Decompressive craniectomy has rarely been reported as a life-saving intervention in such cases. CASE DESCRIPTION: A 25-year-old woman developed fulminant ADEM following dengue fever and

Pediatric Repetitive Mild Traumatic Brain Injury Elicits T Cell-Mediated Neuroinflammation.

Pediatric repetitive mild traumatic brain injury (rmTBI) is a major public health concern with links to chronic cognitive dysfunction. Neuroinflammation represents a significant maladaptive outcome after rmTBI. Persistent innate and adaptive immune cell responses can lead to neurodegeneration and deficits in brain development. Therefore, a deeper understanding of the early dynamics of peripheral immune cell infiltration after pediatric rmTBI is critical for the development of effective treatment. We hypothesize that pediatric rmTBI alters neuroinflammation through T cell infiltration. We used

Getting off-tract: Correlational tractography of white matter integrity and cognitive intraindividual variability in a large sample of community-dwelling older adults.

OBJECTIVE: Cognitive intraindividual variability (IIV), or the dispersion across neuropsychological task performance, is a sensitive marker of neurocognitive aging, but its tract-specific white matter correlates are not well characterized. This study used correlational tractography to identify pathways whose microstructural integrity relates to IIV in older adults. METHOD: Participants were a diverse sample of 510 community-dwelling older adults (Mage = 66.6 years, SDage = 8.4) from the Health and Aging Brain Study-Health Disparities cohort. IIV was computed as the intraindividual standard de

Multiplex gray and white matter networks in autism spectrum disorder: differential topological alterations and transcriptomic associations.

Autism spectrum disorder (ASD) is classically conceptualized as a dysconnectivity syndrome. However, most studies have examined structural and functional connectivity in isolation, leaving the coupling mechanisms between brain structure and function, particularly the contribution of white matter, poorly understood. To systematically characterize connectome pathology in ASD, we analyzed multimodal imaging data from 580 participants (240 with ASD, 340 typical controls) in the Autism Brain Imaging Data Exchange II dataset. We constructed multilayer brain networks integrating gray and white matter

Pbx1 and Pbx3 cooperatively regulate intermediate progenitor genesis and corticogenesis in the mouse neocortex.

Intermediate progenitor cells (IPCs) are key amplifying neuronal precursors that generate the majority of glutamatergic projection neurons during neocortical development. Despite their central role in corticogenesis, the transcriptional mechanisms controlling IPC proliferation and lineage progression remain incompletely defined. Here we combined single-nucleus transcriptomics with bulk RNA sequencing of purified IPCs to identify the TALE homeodomain transcription factors Pbx1 and Pbx3 as prominent regulators in cortical progenitor populations. Single-nucleus RNA-seq of Tbr2+ IPCs reveals broad

Clinical and Molecular Spectrum of Proximal 16p11.2 BP4-5 Copy Number Variations: Insights From a Pediatric Cohort.

Copy number variations (CNVs) at the proximal 16p11.2 BP4-BP5 locus are among the most well-characterized genomic regions associated with neurodevelopmental disorders. We aimed to define the clinical and molecular spectrum of these CNVs in a pediatric cohort. A total of 19 patients with pathogenic or likely pathogenic 16p11.2 BP4-BP5 CNVs were identified among 2200 individuals referred for chromosomal microarray analysis (prevalence: 1/116, 0.86%). Deletions were detected in 16/19 (84.2%) patients and duplications in 3/19 (15.8%). Developmental delay/intellectual disability was observed in 12/

Longitudinal White Matter Changes in Concussed Adolescents with Adverse Childhood Experiences.

Traumatic brain injury (TBI) is a leading cause of death and long-term disability in children, with many experiencing persistent symptoms even after mild TBIs. Exposure to adverse childhood experiences (ACEs) can have physiological effects that may alter how the brain responds to injury, yet the effects of ACEs on white matter injury and recovery processes remain unclear. This study examined whether a history of ACEs is associated with patterns of longitudinal change in white matter microstructure in children with mTBI. Ninety-six concussed adolescents (mean age=14.9 years, range =11.4-17.9, 5

Environmental Exposures and Neuroimaging in Children with Neurodevelopmental Disorders: A Scoping Review.

PURPOSE OF REVIEW: Neurodevelopmental disorders (NDDs) such as autism spectrum disorder (ASD) and attention-deficit/hyperactivity disorder (ADHD) have been linked to environmental exposures, yet the underlying neurobiological mechanisms remain poorly understood. Magnetic resonance imaging (MRI) offers an important in vivo tool for examining how environmental neurotoxicants impact brain development. This scoping review synthesizes current evidence on the associations between air pollution, metals, environmental tobacco smoke, and endocrine-disrupting chemicals (EDCs) assessed from pregnancy thr
