Dissection of the posterior inferior cerebellar artery in a young adult with cerebellar infarct.

The posterior inferior cerebellar artery (PICA) is frequently involved in dissection of the vertebral artery (VA); however, isolated PICA dissection has rarely been reported. A 37-year-old man experienced acute and progressive drowsiness, vertigo, occipital headache, vomiting, and ataxia. There was no precedent trauma or chiropractic manipulation. Neurologically, he had dysmetria of the left extremities. His NIHSS score was 3. Brain magnetic resonance imaging showed an acute cerebellar infarct in the left PICA territory. Magnetic resonance angiography showed a faint signal adjacent to the junc

[Spontaneous dissection of the renal artery and kidney infarction: report of two cases].

We report two previously healthy males aged 33 and 37 years, presenting with severe pain in the right and left part of the abdomen, respectively. An abdominal CT scan showed in both a kidney infarction. An angio-CAT scan showed changes compatible with a fibromuscular dysplasia in the renal arterial wall. An angiography showed an intimal tear or complex dissection flap in both cases. Both had a satisfactory evolution with conservative treatment. The relationship between fibromuscular dysplasia and spontaneous dissection of the renal artery is discussed.

Incidence and Treatment of Arterial Access Dissections Occurring during Cardiac Catheterization.

BACKGROUND: Arterial access dissections may complicate cardiac catheterization and can often be treated percutaneously. The goal of this study was to examine the incidence, consequences, and the treatment of arterial access dissections at a tertiary referral hospital with an active training program. METHODS: Patients experiencing arterial access dissection during coronary angiography or intervention at our institution between October 1, 2004, and January 31, 2007, were identified and their records were retrospectively reviewed. RESULTS: Thirteen of the 3,062 consecutive patients (0.42%) had

A "conservative" method of thoracic wall dissection: a proposal for teaching human anatomy.

The common methods of dissection exposing the thoracic organs include crossing of the wall together with wide resection of its muscular planes. In order to preserve these structures, a little demolishing technique of the thoracic wall is proposed, entering the thoracic cavity without extensive resection of the pectoral muscles. This method is based on the fact that these muscles rise up from the wall, like a bridge connecting the costal plane with the upper limb, and that the pectoralis major shows a segmental constitution. SUPERIOR LIMIT: Resect the sternal manubrium transversely between the

Inguinal node dissection for melanoma in the era of sentinel lymph node biopsy.

BACKGROUND: With the introduction of sentinel lymph node (SLN) biopsy for melanoma, inguinal lymph node dissections (ILND) are more commonly performed for microscopic disease than for clinically palpable disease. We sought to examine the effect this change has on the morbidity of the operation. METHODS: A retrospective review was performed of all patients who underwent an ILND for melanoma between October 1997 and April, 2006. Clinical and pathologic data were collected and correlated by multivariate analysis with the incidence of a major wound complication. RESULTS: We identified 212 patien

Spontaneous dissection of the carotid and vertebral arteries: the 10-year UCSD experience.

The etiology of spontaneous dissection of the carotid and vertebral arteries without antecedent trauma remains unclear. The goal of this 10-year review was to examine factors regarding presentation, diagnosis, treatment, and outcome for all patients at our institution who were diagnosed with spontaneous carotid dissections (SCD) or spontaneous vertebral dissections (SVD) with no prior trauma history. A retrospective chart analysis was performed involving all discharges from UCSD Medical Center from 1995 to 2005. Patients were selected for inclusion based on the diagnosis of carotid or vertebra

A carotid artery dissection presenting with dysphagia due to a dilation of upper oesophagus.

Carotid and vertebral artery dissection is one of the major causes of ischemic stroke in young patients and diagnosing it remains a challenge for the clinician due to its variable clinical presentation. We report the case of a 57-years old man admitted to the emergency department complaining of dysphagia and a hoarse voice. Physical examination revealed a left deviation of his tong and some weakness of the left side of his soft palate. A gastroscopy and a cervical tomodensitometry revealed the presence of an important dilation of his upper oesophagus. A cerebral magnetic resonance was performe

What's new in the biochemical diagnosis of acute aortic dissection: problems and perspectives.

Acute aortic dissection of the thoracic aorta represents a medical emergency that, despite its rare occurrence, is characterized by severe morbidity and mortality. Prompt diagnosis is the key to a desired outcome. According to current clinical data, diagnosis is mostly based on a high index of suspicion and diagnostic imaging and less on clinical manifestations. On the other hand, a biochemical approach to the entity is still falling short in terms of diagnostic practice, although optimistic efforts are being made towards the development of such methods. Lethal vertebral artery dissection in pregnancy: a case report and review of the literature.

Subarachnoid hemorrhage represents a rare event in pregnancy with a high mortality rate. We present the case of a 39-year-old pregnant woman who developed right vertebral artery dissection with subsequent massive subarachnoid hemorrhage with fatal outcome. The macroscopic and microscopic autopsy findings are described. A review of the literature with a discussion of the varied predisposing factors for vertebral artery dissection and subarachnoid hemorrhage and the rarity of these events in pregnancy is provided.

Long-term outcomes after repaired acute type A aortic dissections.

The aim was to evaluate long-term outcomes after surgical treatment of acute type A dissection. Between 1993 and 2004, 199 consecutive patients were operated on for acute type A dissection. All survivors were included in an MRI follow-up program. Follow-up time averaged 4.45 years. Overall survival was 96%, 80% and 65% at 1, 5 and 10 years. The false lumen remained patent in 101 cases (69%). Predictive factors for thrombosis of the false lumen were age >70 years old and valve-sparing aortic root reconstruction. Significant risk factors for patency of the distal false lumen were age <50 years o

Dissection of the V4 segment of the vertebral artery: clinicoradiologic manifestations and endovascular treatment.

Intracranial vertebral artery (VA) dissection has three clinical presentations: ischemia, hemorrhage, and mass effect. Imaging findings of intracranial VA dissections vary according to clinical presentation. Irregular stenosis or occlusion of the VA is the most common finding in patients with posterior fossa infarction, whereas a dissecting aneurysm is the main feature in those with acute subarachnoid hemorrhage. A chronic, giant, dissecting aneurysm can cause mass effect on the brain stem or cranial nerves, as well as distal embolism. Magnetic resonance imaging is useful for detection of intr

Prediction of rebleeding from angiographic features in vertebral artery dissecting aneurysms.

To identify patient characteristics and angiographic features that predict high risk for rebleeding in vertebral artery (VA) dissecting aneurysms. We analyzed 62 patients treated for subarachnoid hemorrhage (SAH) from VA dissecting aneurysms (male: female, 46:16; mean age, 51.7 +/- 8 years). Univariate and multivariate stepwise logistic regression analyses were performed to assess relationships between rebleeding rate and age, gender, history of hypertension, sidedness of the aneurysm, angiographic configuration, and location relative to the origin of the posterior inferior cerebellar artery (

Abdominal aortic dissection with acute mesenteric ischemia in a patient with Marfan syndrome.

Marfan syndrome is an autosomal dominant inherited disorder of connective tissue, with various complications manifested primarily in the cardiovascular system. It potentially leads to aortic dissection and rupture, these being the major causes of death. We report a patient who complained of acute abdominal pain, which presented as acute mesenteric ischemia combined with abdominal aortic dissection. Echocardiography showed enlargement of the aortic root and mitral valve prolapse. Abdominal computed tomography scan revealed acute mesenteric ischemia due to abdominal aortic dissection. Finally, t
