Comparison of cardiometabolic benefits and tolerability in patients on sustained low doses of semaglutide or tirzepatide.

Semaglutide and tirzepatide are typically evaluated through dose escalation, yet many patients in routine care do not reach a recommended maintenance dose due to tolerability, access, cost, supply, or individualized goals. Here, we used repeated 0.25 mg only semaglutide and 2.5 mg only tirzepatide prescription records as real-world proxies for sustained treatment at the lowest approved dose, a prescribing pattern that may overlap with, but does not establish, intentional microdosing. We analyzed de-identified electronic health records (EHRs) from a federated system of 29 million patients. Amon

Functional Neurological Disorder Management in a Pediatric Hospital: A Retrospective Chart Review.

BACKGROUND: Functional neurological disorder (FND) is a complex, multinetwork brain disorder that is characterized by the presence of a broad range of neurological symptoms in the absence of neurological disease or damage. For pediatric patients requiring an in-patient stay, misdiagnosis, lack of provider consistency and diagnostic education, frequent testing and work-up, and invasive procedures are often reported. This review will characterize the population, identify current state for receiving diagnosis, detect variability in service delivery, and provide information to evaluate use of reso

Artificial intelligence in intimate partner violence risk pathways: a PRISMA-ScR review of femicide prevention and medico-legal accountability.

INTRODUCTION: Artificial intelligence (AI), machine learning, natural language processing and related decision-support methods are increasingly studied in intimate partner violence (IPV), domestic-violence and gender-based violence contexts. The key question is not whether AI can predict femicide as an individual lethal event, but whether AI-related methods may help institutions recognise, document, communicate and act on distributed signs of escalation across clinical, legal, police, social-service and digital settings. METHODS: This PRISMA-ScR scoping review, informed by Joanna Briggs Insti

Sex-specific co-occurrence patterns of Type 2 Diabetes Mellitus and Non-Alcoholic Fatty Liver Disease among patients with colorectal cancer: a retrospective EMR-based series.

BACKGROUND: Colorectal cancer (CRC) is a major global health burden and one of the most prevalent malignancies worldwide. Its association with metabolic comorbidities is receiving increasing attention. Type 2 Diabetes Mellitus (T2DM) and Non-Alcoholic Fatty Liver Disease (NAFLD) are two interrelated metabolic disorders increasingly implicated in CRC pathogenesis, possibly via insulin resistance, chronic inflammation, oxidative stress, and gut-liver axis dysregulation. However, limited evidence exists on their co-occurrence and sex-specific distribution among CRC patients, particularly within r

Prevalence and socio-demographic factors associated with transfusion transmissible infections among voluntary blood donors in Kigali, Rwanda.

BACKGROUND: Transfusion transmissible infections (TTIs) such as Human Immunodeficiency Virus (HIV), hepatitis B virus (HBV), hepatitis C virus (HCV), and syphilis, remain a major public health concern in low-income countries due to inadequate blood donation systems. This highlights the urgent need for continuous surveillance and a deeper understanding of the factors associated with TTIs in Rwanda. METHODS: A retrospective cross-sectional study analyzed voluntary blood donor records from the National Center for Blood Transfusion (NCBT) in Kigali, Rwanda, spanning January 2019 to December 2023.

Custodial and perinatal care patterns of women who received prenatal care while incarcerated in the Arkansas state prison system, 2014-2019.

BACKGROUND: The extraordinary growth in women's incarceration over the past several decades has resulted in calls for expansion of research into their unique needs and experiences, including those related to pregnancy and perinatal care. However, while research into the health outcomes of women who are incarcerated while pregnant has grown, research on women's custodial and perinatal care patterns has remained nearly non-existent. Here, we sought to describe (1) the characteristics of the population of women who came to be incarcerated in a state prison system during pregnancy and (2) the char

Mortality after discharge from a public tertiary cardiovascular referral hospital.

It is critically important for stakeholders with distinct foci of attention on healthcare to understand patient evolution in the presence of an established diagnosis or with a suspected diagnosis of various diseases, specially considering death as an outcome. To study the long-term mortality of patients at a cardiovascular referral hospital. Deterministic binding (selection of pairs of registers from the hospital electronic health records and the mortality records of São Paulo state) from 2002 to 2017 was performed. Studied variables were: age, sex, hospital treatment unit where the first visi

Virtual opioid agonist treatment: Alberta's virtual opioid dependency program and outcomes.

BACKGROUND: Virtually delivered healthcare (telehealth, telemedicine) has the potential to reduce gaps in access to opioid agonist therapy (OAT). Barriers to accessing OAT such as lack of transportation, in-person induction requirements, employment demands and limited childcare options reduce treatment opportunities for clients. A completely virtual model of care has been developed in Alberta, Canada. This paper introduces the unique virtual clinic model and describes outcomes from that model. METHODS: A retrospective chart review was conducted using datasets within existing electronic health

COVID-19 vaccine effectiveness among people living with and without HIV in South Carolina, USA: protocol of a population-based cohort study.

INTRODUCTION: Despite the effectiveness of COVID-19 vaccines in preventing severe COVID-19 outcomes, a small percentage of fully vaccinated persons will develop symptomatic or asymptomatic infections with SARS-CoV-2, which is referred to as 'breakthrough COVID-19'. People living with HIV (PLWH) appear to have an elevated risk of severe COVID-19 outcomes, yet the effectiveness of the COVID-19 vaccine in this population remains unclear due to the limited research efforts in this population in the real world. This study aims to characterise and compare the breakthrough COVID-19 (eg, prevalence an

Natural language processing for prediction of readmission in posterior lumbar fusion patients: which free-text notes have the most utility?

BACKGROUND CONTEXT: The increasing volume of free-text notes available in electronic health records has created an opportunity for natural language processing (NLP) algorithms to mine this unstructured data in order to detect and predict adverse outcomes. Given the volume and diversity of documentation available in spine surgery, it remains unclear which types of documentation offer the greatest value for prediction of adverse outcomes. STUDY DESIGN/SETTING: Retrospective review of medical records at two academic and three community hospitals. PURPOSE: The purpose of this study was to conduc

Can Trans-Syndromal Prototypes ("Types") Improve Depiction of Complex Psychiatric Cases?: An Alternative Way to Consider Concordant Comorbid Psychiatric Disorders and Their Contexts as Coherent Units for Research, Assessment, and Treatment Planning.

BACKGROUND AND AIMS: In many psychiatric settings, patients with complex cases are the rule rather than the exception. These cases are characterized by multidiagnostic conditions, often complicated with distressed social contexts, for which few if any evidence-based practice guidelines exist. The aim of this hypothesis-generating article is to consider whether and how these cases might comprise trans-syndromal prototypes ("types"), coherent units that could serve as the basis for further study, assessment, and treatment planning. METHODS: For context, psychiatric and medical visit diagnoses a

Long-term patient satisfaction and durability of laparoscopic anti-reflux surgery in a large Danish cohort: study protocol for a retrospective cohort study with development of a novel scoring system for patient selection.

INTRODUCTION: Laparoscopic anti-reflux surgery is standard of care in surgical treatment of gastro-oesophageal reflux disease and is not without risks of adverse effects, including disruption of the fundoplication and postfundoplication dysphagia, in some cases leading to reoperation. Non-surgical factors such as pre-existing anxiety or depression influence postoperative satisfaction and symptom relief. Previous studies have focused on a short-term follow-up or only certain aspects of disease, such as reoperation or postoperative quality of life. The aim of this study is to evaluate long-term
