A Peircean framework for explainable and human-centred artificial intelligence.

This article develops a philosophical-conceptual framework for rethinking Explainable Artificial Intelligence (XAI) and Human-Centred Artificial Intelligence (HCAI) through the pragmatic semiotics of Charles Sanders Peirce. Rather than treating explainability solely as algorithmic transparency, the article conceptualizes it as a socially situated epistemic process through which AI-generated outputs are interpreted, evaluated, contested, and revised within human practices of inquiry. Drawing on Peirce's concepts of semiosis, abduction, inquiry, fallibilism, pragmaticism, and the community of in

A systematic review of clinical practice guidelines for suicide and self-harm prevention in adults.

Clinical practice guidelines are used to guide clinical decision making but are rarely formally evaluated. As high-quality evidence for suicide prevention is scarce, partly due to limited understanding of underlying causes and low baseline rates, CPGs are important for guiding care in this area. This systematic review focused on clinical practice guidelines for the assessment, management and treatment of adults with self-harm, with or without suicidal intent. We searched 14 medical databases and repositories for guidelines to identify eligible documents published between Jan 1, 2000, and Jan 1

Implications for maintaining vascular access device patency and performance: Application of science to practice.

INTRODUCTION: Vascular access devices are commonly inserted devices that facilitate the administration of fluids and drugs, as well as blood sampling. Despite their common use in clinical settings, these devices are prone to occlusion and failure, requiring replacement and exposing the patient to ongoing discomfort/pain, local vessel inflammation and risk of infection. A range of insertion and maintenance strategies are employed to optimize device performance; however, the evidence base for many of these mechanisms is limited and the mechanisms contributing to the failure of these devices are

CAN gambling with other people's children:

UNLABELLED: In order to give the best care to patients and families, paediatricians need to integrate the highest quality scientific evidence with clinical expertise and the opinions of the family.1Archimedes seeks to assist practising clinicians by providing “evidence‐based” answers to common questions which are not at the forefront of research but are at the core of practice. In doing this, we are adapting a format which has been successfully developed by Kevin Macaway‐Jones and the group at the Emergency Medicine Journal—“BestBets”. UNLABELLED: A word of warning. Local Understandings and First Aid Responses to Burn Injuries: A Phenomenological Study in an Urban Indonesian Community.

BACKGROUND: Burn injuries remain a significant global public health problem, causing substantial morbidity, mortality, and economic burden, particularly in low- and middle-income countries where nearly 90% of cases occur. Despite the importance of timely and appropriate first aid in reducing complications, community responses to burn injuries in many settings continue to rely on traditional or non-evidence-based practices. OBJECTIVE: This qualitative phenomenological study explored community-based knowledge and responses related to the causes and first aid of burn injuries. METHODS: The stud

Scaling up kangaroo mother care in the Philippines using policy, regulatory and systems reform to drive changes in birth practices.

The WHO recommends kangaroo mother care (KMC) for stable preterm and low birthweight babies because it has been demonstrated to reduce mortality by up to half compared with conventional incubator-based care. Uptake of KMC in low/middle-income countries has been limited, despite its suitability for low-resource environments. This paper reviews factors that contributed to the adoption and expansion of KMC in the Philippines. Early introduction began in 1999 but national scale-up was slow until 2014 after which a significant improvement in national adoption was observed. The proportion of target

Exploration of Factors Influencing Participation of Primary Eye Care Clinicians in Low Vision Services.

PURPOSE: Low vision (LV) services are likely to become more in demand as the prevalence of vision impairment increases. Primary eye care clinicians represent a substantially underutilised resource for LV service provision and increasing their participation could considerably improve service capacity and accessibility. This study aimed to gain a comprehensive in-depth theory-based understanding of factors influencing participation of primary eye care clinicians in LV services to inform the evidence base for future behaviour change intervention design. METHODS: Semi-structured one-to-one interv

Food Allergy in India: Delphi Consensus Statement by Indian Academy of Pediatrics (IAP)-Allergy and Applied Immunology Chapter.

Food allergy is an emerging public health concern in India, driven by rapid urbanisation, changing dietary patterns, environmental influences, and increasing recognition of allergic diseases. Despite the growing burden, food allergy care in India is challenged by limited epidemiological data, variable diagnostic practices, and inadequate access to specialised services. These gaps underscore the need for standardised, context-specific guidance tailored to the diverse sociocultural and dietary landscape of the country. This consensus document was developed by a multidisciplinary panel of allergy

Using Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) to improve access to obesity pharmacotherapy: A practical guide for clinicians caring for youth with obesity across United States Medicaid programs: Abbreviated title: Using EPSDT to Improve Access to Obesity Pharmacotherapy.

BACKGROUND: Coverage of obesity pharmacotherapy for youth varies substantially across state Medicaid programs. This variability limits access to evidence-based treatment for pediatric obesity. The Early and Periodic Screening, Diagnostic, and Treatment (EPSDT) benefit is a federally mandated Medicaid provision requiring coverage of medically necessary services for individuals under age 21, even when such services are not included in standard state Medicaid benefits. Despite its broad scope, EPSDT remains underutilized due to limited awareness and operational complexity. METHODS: We developed

Sex steroid priming for growth hormone stimulation testing in children and adolescents with short stature: A systematic review.

OBJECTIVE: Growth hormone stimulation testing (GHST) is used to diagnose growth hormone deficiency (GHD) in children. As sex steroids impact on anterior pituitary function, there is concern around the efficacy of GHST in peripubertal children, where endogenous sex steroid levels are low. Sex steroid priming before GHST is thought to improve test efficacy in these children, however evidence to support its use in clinical practice is limited. In this systematic review, we addressed the following research questions: Does priming increase GH stimulation test efficacy in peripubertal children? Does

Ecologically based management of rodents in the real world: applied to a mixed agroecosystem in Vietnam.

Rodents cause significant damage to lowland irrigated rice crops in the Red River Delta of Vietnam. A four-year study was conducted in 1999-2002 to examine the effectiveness of applying rodent control practices using the principles of ecologically based pest management. Four 100-150 ha study sites adjacent to villages were selected and farmers on two treated sites were asked to follow a set of rodent management practices, while farmers on the untreated sites were asked not to change their typical practices. Farmers on the treated sites were encouraged to use trap-barrier systems (TBS's; 0.065-

Good clinical practice for the use of vasopressor and inotropic drugs in critically ill patients: state-of-the-art and expert consensus.

Vasopressors and inotropic agents are widely used in critical care. However, strong evidence supporting their use in critically ill patients is lacking in many clinical scenarios. Thus, the Italian Society of Anesthesia and Intensive Care (SIAARTI) promoted a project aimed to provide indications for good clinical practice on the use of vasopressors and inotropes, and on the management of critically ill patients with shock. A panel of 16 experts in the field of intensive care medicine and hemodynamics has been established. Systematic review of the available literature was performed based on PIC
