Progression of hearing loss in experimental pneumococcal meningitis: correlation with cerebrospinal fluid cytochemistry.

The development of hearing loss and concomitant cerebrospinal fluid (CSF) cytochemical changes in a model of pneumococcal meningitis were examined. Rabbits were injected intracisternally with 10(5) pneumococci. Auditory evoked potentials to clicks and to 10- and 1-kHz tone bursts were recorded hourly; CSF was analyzed every 4 h. Sensorineural hearing loss developed in all animals beginning 12 h after infection and progressed to severe deafness. The onset of hearing loss was preceded by a CSF leukocytosis of > 2000 cells/microL and elevation of CSF protein and lactate concentrations to > or = 1

N24 spinal response to tibial nerve stimulation and magnetic resonance imaging in lesions of the lumbosacral spinal cord.

We studied tibial nerve somatosensory evoked potentials (SEPs) in 22 patients with progressive clinical signs of sensory and motor involvement of the lower limbs. Magnetic resonance imaging (MRI) in 21 patients revealed lesions of the thoracolumbar cord in 18 and of the cauda equina in the remaining three. SEP recordings enabled us to clearly differentiate the response generated by the dorsal horns of the lumbosacral cord from the activity generated in the cauda equina dorsal roots. Neurophysiologic findings strongly correlated with the lesion site demonstrated by the MRI (cauda equina, lumbos

Body volume changes during simulated microgravity: auditory changes, segmental fluid redistribution, and regional hemodynamics.

Space adaptation syndrome (SAS), manifested by cephalad fluid shifts, spacial disorientation, nausea, and vomiting, is of varied expression and uncertain etiology. One theory is that fluid shift to the upper body alters the function of the vestibular apparatus to create an entity similar to Meniere's disease. Since clinical vestibular dysfunction syndromes are mirrored by altered cochlear function, this experiment was undertaken to study the relation between fluid redistribution and the auditory effects of initial antiorthostatic bed rest. Manual and bone audiometry, impedance tympanometry, an

Actions of noradrenaline on myenteric neurons in the guinea pig gastric antrum.

We used intracellular electrophysiological recording to study the actions of noradrenaline on myenteric neurons in the guinea pig gastric antrum. Noradrenaline caused a dose-dependent inhibition of the stimulus-evoked cholinergic fast excitatory postsynaptic potentials (EPSPs). Noradrenaline had no effect on the postsynaptic response to acetylcholine, suggesting a presynaptic site of action. The slow EPSP was also presynaptically inhibited by noradrenaline. In only 5% of the neurons, noradrenaline caused a postsynaptic depolarization, accompanied by increased input resistance and enhanced exci

Effects of nilvadipine on neuronal function in the ischemic cat brain.

The effect of nilvadipine, a dihydropyridine-type calcium entry blocker, on neuronal function during and following ischemia was investigated with a model of focal cerebral ischemia in cats and was compared with that of nicardipine. Drugs were given intravenously 30 minutes before occlusion of the left middle cerebral artery (MCA). Occlusion of the left MCA for 60 minutes was followed by reperfusion for 90 minutes. The amplitude of the somatosensory evoked potentials (SEPs), and the residual relative regional cortical blood flow in the left ectosylvian gyrus and the left posterior sigmoid gyrus

Visual impairment in hysteria.

We have reviewed the charts of 45 neuro-ophthamological patients diagnosed with 79 monocular visual field or visual acuity losses secondary to non-organic etiology. Our aim was to determine the percentage of patients that have improvement in vision. As part of the protocol, all patients had magnetic resonance images, pattern visual evoked potentials, and flash electroretinography in addition to complete neuro-ophthalmological examinations. A single physician performed both the initial and follow-up examinations of all patients. Thirty-three percent of these patients had visual field defects on

"Numb, clumsy hands" and tactile agnosia secondary to high cervical spondylotic myelopathy: a clinical and electrophysiological correlation.

Four patients presented with a distinctive syndrome of "numb, clumsy hand" and tactile agnosia. Myelography and computed tomographic myelography (CTM) of the cervical spine documented major spondylotic compressive lesions mainly between the C3 and C5 levels. The cortical responses of dermatomal somatosensory evoked potentials (DSEPs) revealed progressively prolonged peak latencies and progressively decreased amplitudes of early components from C6 to C8 dermatomal stimulation. In comparison, the C5 and L2 DSEPs were affected to a lesser extent. This finding suggests that high cervical cord comp

Clinical relevance of phase of steady-state VEPs to P100 latency of transient VEPs.

Pattern visual evoked potentials (VEPs) to transient and steady-state stimulation were recorded in 10 normal subjects at 4 levels of luminance (180, 57, 22 and 11 cd/m2). VEPs were also recorded in 5 patients with optic neuropathy at a fixed luminance (180 cd/m2). The relationship between P100 latency of transient VEPs (T-VEPs) and the phase of steady-state VEPs (S-VEPs) was analyzed. As luminance decreased in normal subjects, P100 latency was prolonged and the phase lag increased. A significant linear relationship between the P100 latency and phase was found. Patients showed both the prolonge

Variable expression of vision in sibs with albinism.

Oculocutaneous albinism is defined by the presence of cutaneous and ocular hypopigmentation, the latter associated with nystagmus, iris transillumination, reduced retinal pigment, foveal hypoplasia, and misrouting of the optic fibers at the chiasm. The visual acuity is variable but almost always reduced. We report on two brothers with oculocutaneous albinism and markedly different visual acuity. One brother has a visual acuity of 20/100, while the second has similar cutaneous pigmentation and visual acuity of 20/20 and had not previously been recognized as having oculocutaneous albinism. Both

The effects of prenatal alcohol on the central nervous system.

Prenatal alcohol exposure has a profound effect on the developing brain. In fetal alcohol syndrome (FAS), mental retardation and microcephaly are commonly observed. A partial syndrome, fetal alcohol effects (FAE) can result in neurobehavioral sequelae, which may present at birth or appear later in development. This chapter discusses the clinical evidence supporting the concept of FAE, the range of cognitive disturbances seen in FAS and FAE children, and studies on long-term outcome. We review studies that suggest that even in the absence of the stigmata of FAS, neonatal signs of central nervou

Albinism in childhood: a flash VEP and ERG study.

Flash visual evoked potentials (F. VEPs) and electroretinograms (ERGs) were recorded in a total of 20 young children with albinism (age range 5 months to 11 years, mean 4 years). All recordings were made without sedation. There were 13 oculocutaneous cases (one with Hermansky-Pudlak syndrome) and seven ocular albinos. Monocular flash stimulation commonly elicited an asymmetrical occipital VEP distribution with a well lateralised component at around 80 ms which was of opposite polarity in a comparison of VEPs from each eye. None of the normally pigmented matched controls or obligate female carr

Comparison of methods to assess neurosensory alterations following orthognathic surgery.

Orthognathic surgery may damage branches of the trigeminal nerve, resulting in postoperative neurosensory disturbances. Alterations may be due to surgical edema, stretching, or direct trauma to the nerve. Lack of a standard and objective method of assessment hinders efforts to study and/or reduce the incidence of neurosensory disturbances. This study compared three methods of assessing neurosensory disturbances in patients who underwent bilateral mandibular ramus sagittal split osteotomies. Forty patients (26 female, 14 male) ranging in age from 23 to 47 years participated in the study. All of

Pelizaeus-Merzbacher disease. Contribution of magnetic resonance imaging to an early diagnosis.

The lesions of the cerebral white matter which characterize Pelizaeus-Merzbacher disease are classically observed at pathological examination. An early diagnosis can now be obtained by magnetic resonance imaging (MRI). In an eutrophic first son born at term, stridor, nystagmus-like eye movements and axial hypotonia were noted immediately after birth and pyramidal signs appeared at 2 months, the only extra-clinical finding at that age being prolonged latencies of evoked visual potentials. An MRI exploration, performed at 3 months, showed that compared with the grey matter the white matter emitt

Management and visual acuity results of monocular congenital cataracts and persistent hyperplastic primary vitreous.

Sixty-two patients, 48 with a diagnosis of persistent hyperplastic primary vitreous (PHPV) and 14 with monocular congenital cataract (MCC) are reported. Nineteen patients with a diagnosis of PHPV and all patients with MCC received surgery with attempted visual rehabilitation. Early age of surgery, prompt optical correction with contact lens and aggressive patching therapy are required for successful visual rehabilitation. Glaucoma, retinal or optic nerve pathology, surgery later than three months of age or combinations of these factors were associated with poor prognosis. Isolated microphthalm

Intracranial stimulation of facial nerve in humans with the magnetic coil.

Using ourselves as subjects, maximal compound motor action potentials (CMAPs) were evoked in ipsilateral nasal and orbicularis oculi muscles (onset latency 4.9-5.4 msec) by a magnetic coil (MC) tangentially oriented over parieto-occipital scalp. The facial nerve was also electrically stimulated sequentially at the posterior tragus near the stylomastoid foramen, anterior tragus and 3 cm more distally. Onset latency of the CMAP elicited at posterior tragus ranged from 1.0 to 1.3 msec less than that elicited by the MC over scalp. Because the measured distal facial nerve motor conduction velocity

Intraoperative identification of the central sulcus: a practical method.

A practical method of intraoperative identification of the central sulcus using cortical evoked potentials (EP) was reported. The method herein proposed is unique with stimulation to the trigeminal and tibial nerve in addition to median nerve stimulation, depending on the location of the lesion. Thus, a wide coverage of the rolandic fissure was possible. Eighteen adult patients with peri-rolandic lesions were investigated. Fourteen cases had a malignant tumour but all except 3 showed only a minor neurological deficit preoperatively. In 13 cases, the central sulcus could be identified through t

[Pure spastic hemiplegia of pyramidal origin].

The relationship between the interruption of the human pyramidal tract and its attendant clinical manifestations has been a matter of concern to neurologists and neurosurgeons for over a century. We presently report three cases of unilateral pyramidal tract ischemic lesions within the cerebral hemispheres who presented with a contralateral pure spastic hemiplegia syndrome. In none could we find any disturbance in the somatosensitive evoked potentials of the four limbs. The review of some cases on record since the time of Charcot and Erb has made it clear that the pyramidal syndrome is a valid

[How to examine an non-contractile bladder].

Bladder contractility is dependent on the quality of the detrusor muscle and its innervation, and acontractility can therefore result from muscular or neurologic dysfunction. Etiologic factors involved in this functional disorder include collagen overload of the bladder wall and/or denervation or inhibition phenomena. Cystometric conditions necessary and sufficient for recognition of the disorder are indicated, the stop-test being an indispensable exploration to establish diagnosis. The Lapidès test is reliable when peripheral neurologic lesions are involved but the lesion must be complete; fo

Vasopressin and electrophysiological signs of attention in man.

Seventeen pairs of monozygotic twins, females and males, were tested in a dichotic listening task, containing several types of pips: standard and deviating target pips, which the subject either attended to, or not. Averaged auditory evoked potentials (AEPs) to the pips provided measures of different attentional processes. Furthermore, EEG power spectra, heart rate and blood pressure and behavioral performance were measured. Subjects received treatments (20 I.U. lysine-vasopressin vs. placebo) intranasally 48, 24, and 1 hour prior to the experimental sessions according to a co-twin control desi

Neurophysiological evaluation of newborns with congenital hydrocephalus.

EEGs and visual and auditory evoked potentials (VEP and AEP) were studied in 27 newborns with congenital hydrocephalus. Asynchronous sleep patterns were the main EEG abnormality in the newborn as well as in the older infants and children. Asynchronous tracé alternant patterns, the most common finding, were more frequently encountered in infants with more severe hydrocephalus. Asynchrony of bifrontal humps was another finding observed in a few infants. Intermittent semirhythmic theta or delta waves were noted in some infants. Depression of the EEG background activity was an infrequent finding o
