Retrospective assessment of tailored exercise regimens in the rehabilitation of lower extremity arteriosclerosis obliterans.

Lower extremity arteriosclerosis obliterans (LEAO) is a prevalent peripheral vascular disorder that results in diminished blood circulation and compromised lower-limb function. Conventional rehabilitation typically relies on uniform exercise instructions, which may fail to accommodate the diverse needs of individual patients. This study aimed to assess the therapeutic value of personalized exercise prescriptions (PEPs) in enhancing lower-limb perfusion, walking performance, comfort, pain relief, and self-care ability among patients with LEAO, compared with standard exercise guidance. Evolution and Outcome of a Pediatric Pulmonary Rehabilitation Program in Hong Kong Over the Past Decade.

BACKGROUND: Pulmonary rehabilitation (PR) is a well-established treatment for adults with chronic obstructive respiratory disease and other chronic respiratory diseases. Although the treatment has proved effective for pediatric asthma and cystic fibrosis, its utility for other conditions is in need of study. Since 2015, the Duchess of Kent Children's Hospital (DKCH) in Hong Kong has been running an inpatient PR program for children with compromised lung function resulting from various underlying causes. During the lockdown amid the COVID-19 pandemic, community-based PR was developed to address

Comparative Efficacy and Acceptability of Non-surgical Treatments with or without Exercise for Diastasis Recti Abdominis in Postpartum Women: A Network Meta-Analysis of Randomized Controlled Trials.

BACKGROUND: Diastasis recti abdominis (DRA), commonly occurring in postpartum women, is not only an aesthetic issue but is also highly associated with functional impairments. Various conservative treatment modalities have been employed in clinical practice to alleviate DRA. However, the comparative efficacy of these non-surgical treatments for improving the inter-recti distance (IRD) remains to be determined. AIM: This current network meta-analysis (NMA) aims to compare the efficacy and acceptability of different non-surgical treatments with or without exercise for improving DRA in postpartum

A phase II RCT and economic analysis of three exercise delivery methods in men with prostate cancer on androgen deprivation therapy.

BACKGROUND: Androgen deprivation therapy is commonly used to treat prostate cancer, the most common visceral cancer in men. However, various side effects often worsen physical functioning and reduce well-being among men on this treatment. Based on existing evidence, both resistance and aerobic training provide benefits for this population yet adherence rates are often low. The method of exercise delivery (supervised in-center or home-based) may be important, yet few studies have compared different models. Additionally, long-term exercise adherence is critical to achieve sustained benefits but

Development and Validation of an Artificial Intelligence-Based Motion Analysis System for Upper Extremity Rehabilitation Exercises in Patients with Spinal Cord Injury: A Randomized Controlled Trial.

In this study, we developed an AI-based real-time motion feedback system for patients with spinal cord injury (SCI) during rehabilitation, aiming to enhance their interest and motivation. The effectiveness of the system in improving upper-limb muscle strength during the Thera band exercises was evaluated. The motion analysis program, including exercise repetition counts and calorie consumption, was developed using MediaPipe, focusing on three key motions (chest press, shoulder press, and arm curl) for upper extremity exercises. The participants with SCI were randomly assigned to the experiment

Simulation exercises to strengthen polio outbreak preparedness: experience of the World Health Organization European Region.

BACKGROUND: Poliovirus importations and related outbreaks continue to occur in polio-free countries, including those in the World Health Organization (WHO) European Region. National preparedness plans for responding to poliovirus introduction are insufficient in many countries of the European Region. We describe a series of polio outbreak simulation exercises that were implemented to formally test polio outbreak preparedness plans in the European Region. METHODS: We designed and implemented the exercises, reviewed the results, made recommendations, and assessed the role of outbreak simulation

Feasibility and acceptability of a remotely delivered, home-based "exercise snacking" to improve physical function in community-dwelling older adults: a 28-day pilot study.

BACKGROUND: Age-related declines in muscle strength and physical function increase the risk of frailty, falls, and loss of independence in older adults. Traditional structured exercise programs often have low participation because of time constraints, limited access to facilities, and health-related barriers. "Exercise snacking" defined as brief bouts of resistance exercise accumulated across the day, may offer a more flexible and feasible option. METHODS: In this 28-day pilot randomized controlled trial, 36 community-dwelling older adults aged 65-80 years were randomly assigned to a home-bas

Effectiveness of motor control exercise, aerobic walking, and muscle strengthening programs in improving outcomes in a subgroup of population with chronic low back pain positive for central sensitization: a study protocol for a randomized controlled trial.

BACKGROUND: The role of pain sensitivity in the development and maintenance of chronic pain states, impaired executive functioning, and patient recovery is being investigated. Conditioned pain modulation (CPM) is widely used to measure musculoskeletal pain associated with central sensitization (CS). Despite the recommendations of many reviews and clinical practice guidelines that exercise programs reduce pain and disability, the overall confidence in these results is considered "critically low." The "active ingredient" of exercise programs and the dominant factor influencing CPM remain largely

Gender and racial differences in endothelial oxidative stress and inflammation in patients with symptomatic peripheral artery disease.

BACKGROUND: We compared (1) cellular reactive oxygen species (ROS) production, inflammation, and apoptosis of cultured endothelial cells treated with sera and (2) circulating inflammatory measures, antioxidant capacity, vascular biomarkers, and calf muscle hemoglobin oxygen saturation (StO2) in men and women with peripheral artery disease (PAD). A secondary aim was to compare exercise performance and daily ambulatory activity between men and women. We hypothesized that women would have more impaired endothelial cellular ROS, inflammation, and apoptosis than men as well as worse systemic inflam

Exercise-induced arterial hypoxaemia in healthy human subjects at sea level.

We determined the incidence of exercise-induced arterial hypoxaemia and its determinants in sixteen highly trained, healthy runners who were capable of achieving and sustaining very high metabolic rates (maximal O2 uptake = 72 +/- 2 ml kg-1 min-1 or 4.81 +/- 0.13 l min-1). Arterial blood gases and acid-base status were determined at each load of a progressive short-term exercise test and repeatedly during constant-load treadmill running while breathing air and during inhalation of mildly hypoxic, hyperoxic, and helium-enriched gases. Three types of responses to heavy and maximum exercise were

Concentration-effect relationships with FM 24: a new long acting beta-adrenergic receptor antagonist.

1 FM 24, 1-(2-exo-bicyclo[2,2,1] hept-2-tl phenoxy-3[(1-methyl/ethyl) amino]-2-propanol is a new β-adrenoceptor antagonist. The drug-receptor complex dissociates very slowly in vitro, probably because of the rigidity of the norbornyl ring (Le Fur et al., 1978). 2 Five healthy male volunteers were given single oral doses of placebo, 20, 40, 80, 160 and 320 mg of FM 24. Two hours later a sub-maximal exercise test was carried out with measurement of heart rate and blood pressure and a blood sample was taken for measurement of the plasma concentration of FM 24. Progressive inhibition of exercise t

The use of exercise echocardiography in the evaluation of mitral regurgitation.

Mitral regurgitation (MR) is the second most common valvular disease in western countries after aortic stenosis. Optimal management of patients with MR depends on the etiology of the regurgitation and is based predominantly on left ventricular function and functional status. Recent outcome studies report high risk subsets of asymptomatic patients with MR, and practice guidelines underscore the importance of a well-established estimation of exercise tolerance and recommend exercise testing to objectively assess functional status and hemodynamic factors.

[The assessment of adaptation potential of circulatory system in non-coronarogenic myocardial disease].

Adaptive potential of the circulatory system was estimated in 223 patients with non-coronary myocardial lesions (80 patients had spinal osteochondrosis with cardialgia, 78 patients were obese and 65 patients had neurocirculatory asthenia of the cardial type) in tests with submaximal exercise. 21 healthy subjects served control. It was found that submaximal exercise in rehabilitation does not change the adaptive potential in healthy subjects, lowers this potential in patients with neurocirculatory asthenia and raises in obese patients.

Exercise Echocardiography for Risk Stratification in Unexplained Dyspnea: The Incremental Value of the Mean Pulmonary Artery Pressure/Slope.

BACKGROUND: Patients with unexplained dyspnea and an elevated mean pulmonary artery pressure (mPAP)/cardiac output (CO) slope on invasive hemodynamic assessment during exercise have worse clinical outcomes. The aim of this study was to evaluate the incremental prognostic value of the noninvasive mPAP/CO slope in addition to heart failure with preserved ejection fraction (HFpEF) probability scores and diastolic stress testing in patients with unexplained dyspnea. METHODS: In a multicenter cohort study involving six Belgian dyspnea clinics, patients with unexplained dyspnea underwent exercise e

Multi-Organ Physiologic Deficits During Exercise Identify Clinical and Molecular Predisposition to Heart Failure with Preserved Ejection Fraction.

BACKGROUND: Exercise unmasks limitations in multi-organ system reserve capacity characteristic of heart failure with preserved ejection fraction (HFpEF). However, the metabolic and genetic underpinnings of exercise deficits, and their cumulative contribution to HFpEF severity and prognosis, remain incompletely understood. METHODS: We used invasive cardiopulmonary exercise testing (iCPET), metabolite profiling, and genomics to simultaneously characterize seven exercise physiologic deficits in HFpEF patients: reduced exercise stroke volume and heart rate, steep pulmonary capillary wedge pressur

Effect of Exercise on Acute Senescent Lymphocyte Counts: A Systematic Review and Meta-Analysis.

BACKGROUND: Highly differentiated, senescent lymphocytes are pro-inflammatory and contribute to age-related systemic inflammation, called inflammageing. There are several reports of acute changes in senescent lymphocyte counts post exercise, which potentially have consequences for systemic inflammation. However, there is little consensus since the studies differ with respect to participants, exercise protocols, cellular markers assessed, and the time point of assessment post exercise. OBJECTIVE: We performed a systematic review and meta-analysis to assess the impact of exercise on senescent l

Lifelong Physical Activity Regardless of Dose Is Not Associated With Myocardial Fibrosis.

BACKGROUND: Recent reports have suggested that long-term, intensive physical training may be associated with adverse cardiovascular effects, including the development of myocardial fibrosis. However, the dose-response association of different levels of lifelong physical activity on myocardial fibrosis has not been evaluated. METHODS AND RESULTS: Seniors free of major chronic illnesses were recruited from predefined populations based on the consistent documentation of stable physical activity over >25 years and were classified into 4 groups by the number of sessions/week of aerobic activities

"Chronic Cervical Midline Contusion in Rats Disrupts Aerobic, Muscular, and Cardiovascular Function".

Cardiovascular dysfunction significantly contributes to morbidity and mortality following cervical spinal cord injury (SCI). Unfortunately, only a limited number of preclinical models have been developed for investigating cardiovascular dysfunction following cervical SCI. Furthermore, the broader consequences of cervical SCI on aerobic capacity and muscle endurance during physiological stress testing also remains understudied preclinically. Therefore, in this study we assessed potential deficits across multiple physiological systems in a rat model of cervical SCI using a battery of stress test
