Female orgasmic disorder.

Female orgasmic disorder (FOD) is defined by the Diagnostic and Statistical Manual of Mental Disorders, 4th Edition, Text Revision, as a persistent or recurrent delay in, or absence of, orgasm following a 'normal' sexual excitement phase. FOD is a common problem affecting sexual function in a substantial proportion of women. Studies suggest that it is prevalent in 11-41% of women worldwide and can have a tremendous impact on the individual's quality of life, relational satisfaction and general well-being. The etiology of FOD tends to be multifactorial as it relates to genetics, medical conditi

The movement of services out of the hospital.

Certificate of Need (CON) laws have been used in the United States since the 1960s to restrict the availability of new and expensive technology in the health system. However, as medical technology is used in non-institutional settings, the value of such a regulatory system is called into question. This article examines changes occurring in the health system in the United States and OECD countries such as the movement of technology out of the hospital, the push by manufacturers of medical equipment to have greater sales and expand their markets, the internecine fights between different specialt

Human immunodeficiency virus transmission and surgeons: update.

Concerns of patients and health care providers regarding disease transmission in the health care setting are not unique to the current human immunodeficiency virus (HIV) epidemic. The data show that the risk to health care workers (HCWs) from patients is far greater than the risk to patients from HCWs. Despite the high frequency of contact with potentially infected fluids, surgeons appear to have a low occupational risk of acquiring HIV infection. Further, the actual risk of surgeon-to-patient transmission appears to be extremely small. Recommendations that HIV-infected surgeons restrict their

Novel use of Lieberman self-retainers for nasal alar retraction.

BACKGROUND: Nasal specula such as Thudichum and Killian have been in use in rhinology for over 100 years. They serve to widen the nasal alar and retract nasal hairs when performing rhinoscopy. METHODS: We describe a novel nasal speculum: the repurposing of the Lieberman eye speculum for use in rhinological procedures. This speculum is adjusted to fit the individual and is self-retaining, achieving equivalent retraction to traditional specula and enabling use of a second hand for intra-nasal surgery. RESULTS: The Lieberman speculum can reach a maximal diameter of 14mm and has successfully bee

Efficacy and Safety Profile of Suvorexant for the Treatment of Insomnia.

Insomnia is becoming a concern in general practice as it affects around 30% of adults. It is a sleep disorder characterized by trouble getting asleep, staying asleep, waking up early, and difficulty falling back to sleep. The most commonly used hypnotics, such as benzodiazepines, Z drugs, etc, are associated with various issues, including psychomotor and cognitive impairment. Dual orexin receptor antagonists are an emerging class of hypnotics used to treat insomnia. Suvorexant is a first-in-class dual orexin receptor antagonist approved by the US FDA in 2014 for the treatment of insomnia. Suvo

[Otorhinolaryngologic diseases during lactation-which medications are compatible with breastfeeding?].

Breastmilk is well established as the best source of nutrition for infants. Any interruption to breastfeeding comes with risks such as mastitis, decreased breastmilk supply, infant bottle refusal, and infant allergic reaction if formula is used. The majority of common medications used in general otolaryngology are safe for breastfeeding patients. Insufficient knowledge can lead to clinicians and patients alike being concerned about continued breastfeeding, resulting in interruption or cessation of breastfeeding. On the other hand, much-needed treatment is often withheld for fear of putting the

Low-dose spinal anesthesia in cesarean section: a narrative review.

Spinal anesthesia is the technique of choice for cesarean delivery, offering rapid onset and reliable anesthesia, but conventional intrathecal doses of hyperbaric bupivacaine frequently cause significant hypotension and delayed recovery. Low-dose spinal anesthesia (LDSA), generally defined as ≤ 8 mg of hyperbaric bupivacaine, often combined with intrathecal opioids, has been investigated as a strategy to mitigate these effects. Current evidence indicates that LDSA can attenuate hemodynamic fluctuations and reduce intraoperative nausea without compromising neonatal outcomes; however, excessivel

[Thoracic electrical impedance tomography: Technical aspects and clinical applications].

Thoracic Electrical Impedance Tomography (EIT) is a functional respiratory imaging technique first developed in the 1970s. It provides real-time functional information at the patient's bedside, without ionizing radiation. Its growing interest in clinical practice in recent years is the result of numerous technological advances, providing the practitioner with a tool for monitoring pulmonary ventilation, assessing the efficacy of therapeutic interventions, detecting complications such as pneumothorax, and obtaining data on pulmonary perfusion. Its use is gradually extending beyond the field of

Epidural Anesthesia in a Patient With Turner Syndrome: A Case Report.

Turner syndrome (TS) is associated with short stature, cardiac abnormalities, and musculoskeletal deformity, which pose unique obstetric anesthesia challenges in the small proportion of TS patients who go through spontaneous or assisted pregnancy. There is very limited data available on the anesthetic management of these patients. This case report presents the management of a 32-year-old gravida 2 para 0 (G2P0010) patient with mosaic TS undergoing labor induction at 37 weeks of gestation. An indwelling epidural catheter placed under ultrasound guidance provided adequate labor analgesia with ab

More Parents With Intellectual Disabilities than we Thought: A Short Report from England.

BACKGROUND: This paper recognises that there has been a long history of research into support for parents with intellectual disabilities in England and a helpful approach to integrating adults with intellectual disabilities in society called 'Valuing People'. This focus has now faded. METHOD: The paper draws together findings from three recent English studies. RESULTS: One third of the cases involving babies in the child protection system involved parents with diagnosed intellectual disability or a borderline or specific learning disability. The other two studies found that general adult soc

A clinical update on chronic rhinosinusitis and nasal polyposis.

Chronic rhinosinusitis (CRS) is a complex heterogeneous disease of the nose and paranasal sinuses that presents different phenotypes and endotypes. CRS is a common health problem associated with significant morbidity, as well as with high health care expenditure. As our knowledge on inflammation, tissue remodeling and pathophysiological mechanisms develops, both diagnosis and therapeutic approaches to CRS improve. This review outlines key drivers in the pathogenesis of CRS with and without nasal polyps, current diagnostic tools clinicians can rely on in clinical practice, and current and futur

Serological Survey of Lymphocytic Choriomeningitis Virus (LCMV) Infection - Gansu Province, China, 2023.

WHAT IS ALREADY KNOWN ABOUT THIS TOPIC?: Lymphocytic choriomeningitis virus (LCMV) is transmitted primarily through rodent excreta, with humans being universally susceptible to infection. Transmission via organ transplantation occurs rarely. The predominantly nonspecific clinical presentation in humans has contributed to LCMV remaining an underrecognized zoonotic pathogen. WHAT IS ADDED BY THIS REPORT?: This study provides the first serological confirmation of past LCMV infection among the general population in Gansu Province, with seroprevalence rates of 18.31% by enzyme-linked immunosorbent

Low-Grade Appendiceal Mucinous Neoplasm Causing Equivocal Appendicitis: A Case Report.

Appendicitis is one of the most common diagnoses that general surgeons encounter in practice. An exceedingly rare cause of this disease is neoplasm. We report the case of a 24-year-old female who presented with non-specific right lower quadrant abdominal pain and equivocal findings of appendicitis and pelvic congestion syndrome on CT imaging. After an extensive work-up, the patient underwent a diagnostic laparoscopy with an appendectomy. The appendix appeared grossly normal; however, on a pathologic review of the specimen, a low-grade appendiceal mucinous neoplasm (LAMN) was found. This case i

Sodium-glucose cotransporter 2 inhibitors for chronic kidney disease: Why, when and when not.

BACKGROUND: Chronic kidney disease (CKD) is a significant healthcare problem. More advanced stages are associated with increased mortality, morbidity and cost. Instigating measures to slow down disease progression at an early stage can save lives, and millions of dollars of taxpayers' money. OBJECTIVE: This article aims to provide evidence-based information to general practitioners, aiding the decision to initiate sodium-glucose cotransporter 2 (SGLT2) inhibitors for CKD patients in their day-to-day practice. DISCUSSION: SGLT2 inhibitors have emerged as a promising and safe addition to the r

A Hidden Threat: Hydatid Cyst Masked by Bacterial Infection.

Hydatid cysts are parasitic infections that primarily affect the liver, but complications like secondary bacterial infections can obscure the clinical presentation. Though infections are common in general practice, hepatic abscesses, particularly those associated with hydatid cysts, are rare and often manifest with non-specific symptoms, leading to delayed diagnosis. Given the risk of serious complications such as rupture and sepsis, maintaining a high index of suspicion is critical, especially in patients with risk factors like a history of travel to endemic regions, liver disease, or excessi

Lethal Gram-negative sepsis in healthy pigs during anaesthesia with contaminated propofol.

Two healthy Landrace pigs anaesthetized with propofol suffered rapid onset of fatal sepsis. Clinical signs included severe arterial hypotension, loss of peripheral oxygenation, low end-tidal CO2, clinical onset of pulmonary oedema and cardiac dysfunction. Gross and histopathological examination revealed loss of vascular integrity with severe lung oedema and congestion, haemorrhages in several organs and fluid leakage into body cavities. Large numbers of Gram-negative bacteria, primarily Klebsiella sp., were present in the anaesthetic infusion containing propofol and were also cultured from int

Pilonidal sinus: an overview of historical and current management modalities.

Pilonidal disease is a common condition that commonly affects the younger adult population and is often seen in both the general practice and the hospital setting. Multiple treatment methods have gained and lost popularity over the last several decades, but more recent intervention principles show promising results. This article details the different methods of managing acute and chronic pilonidal disease ranging from treatments in the primary care setting to those in hospital theatres, with special attention to newer modalities of minimally invasive interventions. As a chronic illness that of
