Poliomyelitis and its elimination in Cuba: an historical overview.

INTRODUCTION: Polio was first detected in Cuba in the late 19th century among residents of the US community on the Isla de Pinos (Isle of Pines, now Isle of Youth), apparently introduced through migration from the USA. The first outbreak was reported in 1906 on the Isle, with the first epidemic reported in the former province of Las Villas in 1909. The epidemics subsequently intensified, by 1934 becoming periodic every four to five years, and accompanied by high morbidity, mortality and crippling sequelae, primarily among children. OBJECTIVE: To review and analyze the history of polio and its

Short-term hormone therapy suspension and mammography recall: a randomized trial.

BACKGROUND: Without population-based evidence, some clinicians recommend short-term suspension of hormone therapy to improve the performance of mammography. Hormone therapy increases breast density, and abnormal screening mammograms are more common among women with denser breasts and among women using hormone therapy. OBJECTIVE: To test whether 1 to 2 months of hormone therapy suspension before screening mammography decreases additional mammographic imaging (recall) in women age 45 to 80 years. DESIGN: 3-group randomized, controlled trial. SETTING: Integrated health plan in western Washingt

Decision-Making Approaches Used to Limit Potentially Nonbeneficial Life-Prolonging Interventions.

IMPORTANCE: Professional society policy statements recommend that clinicians limit (ie, withhold or withdraw) potentially nonbeneficial life-prolonging interventions by (1) achieving a shared decision with patients or surrogates or (2) initiating an institutional process to address disagreement with patients or surrogates. However, in the context of a health care system with a default tendency toward life prolongation, it is unclear whether clinicians rely entirely on these recommended approaches or resort to alternate approaches. OBJECTIVE: To characterize the range of decision-making approa

Feasibility and safety of early discharge after robot-assisted uro-oncologic surgery using wearable telemonitoring.

Early discharge after robot-assisted uro-oncologic surgery remains limited by concerns regarding patient safety and the timely detection of postoperative complications. Wearable telemonitoring may facilitate earlier discharge while maintaining clinical surveillance during recovery. In this multicenter prospective study, 86 patients undergoing robot-assisted radical prostatectomy, partial nephrectomy, or radical nephrectomy were allocated to a control group (C, n = 43) receiving standard postoperative care or a wearable group (W, n = 43) managed through an early-discharge telemonitoring protoco

Hospital Disaster Preparedness and Response to a Pediatric Mass Casualty Incident in Central India: A Retrospective Quality Improvement Analysis.

Background Mass casualty incidents (MCIs) place extreme demands on health systems, and pediatric victims introduce additional physiological and psychosocial challenges. Although disaster preparedness principles are well described, reports of coordinated pediatric MCI responses from India remain limited. On 26 November 2024, a school bus accident near Nagpur led to the simultaneous arrival of 49 casualties at a tertiary trauma center. This study uses retrospective operational data to evaluate hospital surge capacity, triage, imaging workflow, and clinical outcomes and interprets these findings

The Efficiency and Cost-Effectiveness of 3D-Printed Patient-Specific Guide Plate for Patients Undergoing Open-Wedge High Tibial Osteotomy: A Multicentered Randomized Controlled Trial.

PURPOSE: Open-wedge high tibial osteotomy (OWHTO) is established for young, active patients with medial knee osteoarthritis. Patient-specific 3D-printed guide plates have been introduced to improve surgical precision and efficiency, but evidence of clinical and economic benefit is limited. We aimed to determine whether a 3D-printed patient-specific guide plate improves efficiency, functional outcomes, and cost-effectiveness compared to standard OWHTO. METHODS: In this multicenter randomized trial, patients scheduled for OWHTO were allocated to either conventional planning (control) or surgery

Zika virus and male reproductive health: essential updates for andrologists and fertility specialists.

BACKGROUND: Zika virus (ZIKV), a mosquito-borne flavivirus, remains a concern for reproductive health despite the waning of the 2015-2016 epidemic. Unique among arboviruses, ZIKV can be sexually transmitted, with viral RNA persisting in semen beyond the acute phase, posing potential risks to male fertility and assisted reproduction. This narrative review provides clinicians with a contemporary understanding of ZIKV epidemiology, virology, and its implications for male reproductive care. RESULTS: ZIKV transmission has stabilised into low-level endemicity in Central and South America, with Euro

Towards precision medicine in placenta accreta spectrum: results from a survey-based evaluation of interactive academic international webinars.

BACKGROUND: Placenta accreta spectrum (PAS) has emerged as a pressing global health concern as cesarean delivery rates continue to rise. Once regarded primarily as a placental disorder, PAS is now understood as an abnormal implantation process occurring within uterine scars. Although cesarean hysterectomy remains the predominant management strategy, growing evidence suggests that individualized, uterine-sparing approaches may reduce morbidity and better align with patient preferences. Traditional histopathologic classifications neither correlate with prenatal imaging findings nor reflect surgi

Psychological Factors and Personality Traits Influencing the Experience and Perception of Pain in Women Undergoing Mammography: Protocol for a Cross-Sectional Study.

BACKGROUND: Breast cancer screening through mammography is essential for early detection and improved survival; however, pain and discomfort during the procedure remain major barriers to women's participation. Pain perception during mammography is influenced not only by technical factors but also by psychological variables such as anxiety, depression, pain catastrophizing, and personality traits. Despite growing evidence, findings on the role of psychological factors remain inconsistent, and the contribution of personality traits remains largely unexplored, highlighting the need for further in
