Risk factors for intensive care unit admission in hospitalized patients with acute pancreatitis.

BACKGROUND: Acute pancreatitis (AP) is afflicted by substantial morbidity and mortality. Besides local/systemic complications and pancreatic necrosis, patients' comorbidities might be underrated as risk factor for intensive care unit (ICU) admission in AP. METHODS: We screened the German nationwide inpatient statistics identifying patients admitted due to AP (ICD-code K85) 2005-2019. AP hospitalizations were stratified for necessity of ICU admission and risk-factors for ICU admission were investigated. RESULTS: Overall, 797,364 patient-hospitalizations due to AP (median age 56.0 [IQR 44.0-71

Early fluid therapy trajectories in acute pancreatitis: a retrospective cohort study.

BACKGROUND: Individualized management of early fluid therapy in acute pancreatitis (AP) remains challenging. Traditional studies primarily focus on static fluid balance, overlooking the heterogeneity of dynamic trajectories. METHODS: Based on a single-center retrospective cohort, 3,142 AP patients admitted within 72 h of onset were included. Daily fluid therapy-to-body weight ratio (FWR) and CumFWR (trapezoidal AUC) were calculated. Latent Class Growth Modeling (LCGM) identified dynamic FWR trajectories within 72 h. Multivariate Cox regression analyzed the relationship between FWR and in-hosp

A prediction model for 30-day mortality in patients with ARDS admitted to the intensive care unit.

BACKGROUND: The development of acute respiratory distress syndrome (ARDS) is intricate and uncertain. Although intensive care has made substantial progress in the past several decades, the in-hospital mortality rate of patients with ARDS remains as high as 40%, which imposes a large burden on hospitals and intensive care. This study aimed to develop and validate a nomogram to predict the 30-day mortality of patients with ARDS admitted in the intensive care unit (ICU). METHODS: Data of 4920 patients with ARDS were collected from the MIMIC-IV database, as well as data of 248 patients were colle

Use of rescue noninvasive ventilation for post-extubation respiratory failure.

BACKGROUND: Robust evidence supports the use of preemptive non-invasive ventilation (NIV) after extubation in selected high-risk patient cohorts. In contrast, current guidelines discourage the use of NIV as a rescue therapy for respiratory failure that develops later after extubation, based on earlier studies indicating a potential increase in hospital mortality due to delayed reintubation. Nonetheless, NIV continues to be employed in this setting. We conducted a post-hoc analysis of a randomized trial to assess the clinical outcomes of rescue NIV for post-extubation respiratory failure. METH

Triglyceride-glucose index and mortality in congestive heart failure with diabetes: a machine learning predictive model.

BACKGROUND: The triglyceride-glucose (TyG) index serves as a marker for insulin resistance. Research exploring the link between the TyG index and adverse outcomes among patients suffering from congestive heart failure (CHF) and diabetes mellitus (DM) is limited. This investigation endeavors to uncover the connection between the TyG index and mortality risk in subjects suffering from CHF and DM. METHODS: We obtained clinical data for patients with CHF and DM from the MIMIC-IV (3.1) database. The optimal cutoff value for the TyG index was determined using X-tile software, and patients were clas

Predicting mortality and risk factors of sepsis related ARDS using machine learning models.

Sepsis related acute respiratory distress syndrome (ARDS) is a common and serious disease in clinic. Accurate prediction of in-hospital mortality of patients is crucial to optimize treatment and improve prognosis under the new global definition of ARDS. Our study aimed to use machine learning models to develop models that can effectively predict the in-hospital mortality of patients with sepsis related ARDS, calculate the mortality, and to identify related risk factors under the new global definition of ARDS. Based on MIMIC database, our study included 3470 first-time admission records of pati
