Surgical treatment of Allman type III acromio-clavicular dislocation. A long-term follow-up study.

The authors report the results of a long-term follow-up study of surgical treatment of Allman type III acromio-clavicular dislocation. Despite numerous publications regarding the validity of closed treatment, the authors believe, in accordance with the anatomo functional and pathological knowledge, that open treatment is justified not only in young people but in older ones too. Among the different techniques, the authors prefer acromio-clavicular fixation with threaded wires because the operation time is short and the functional results are good with the lowest number of complications.

[Pathology of the sesamoid bones of the hand].

Sesamoid bones are osseous parts of certain joints and can either be included in pathological changes of these joints or separately in those of the sesamoidal joint itself. The clinical relevance of such findings is described in the following examples: Rheumatoid variants, psoriasis, different types of hyperostotic forms, chronic sesamoiditis as well as post-trauma. In three out of four cases of fracture, the ulnar sesamoid bone of the metacarpophalangeal joint of the thumb was affected.

Case report 725. Posterior displacement of proximal end of the right radius and capitulum with supracondylar fracture causing a displaced fracture rather than a fracture dislocation.

An 11-month-old boy with a swollen, painful right elbow demonstrated a supracondylar fracture on plain film radiographic studies. Sonography was subsequently performed to exclude dislocation of the elbow and demonstrated the supracondylar fracture as well as a normal relationship of the capitulum and radius, signifying a displaced fracture without associated dislocation. A bone dislocation would generally require dosed reduction alone, whereas distal humeral epiphyseal separation usually requires open reduction or percutaneous (pin) fixation. This case demonstrates the utility of ultrasound in

Scapho-lunate dissociation occurring with scaphoid fracture.

In the most widely accepted model of injuries to the carpus, scaphoid fracture and scapho-lunate dissociation are held to be mutually exclusive. Four cases in which they occurred together are reported, and the likely mechanism proposed.

Temporomandibular joint: surgically created disk displacement causes arthrosis in the rabbit.

To document a causal relationship between temporomandibular joint disk displacement and arthrosis, the disk was surgically displaced in one temporomandibular joint in each of three rabbits. The rabbits were sacrificed after 4 weeks and the mandibular condyles were studied radiographically and histologically. All three joints that underwent disk displacement had radiographic and histologic evidence of arthrosis, which included erosion of the bone, irregularity and fissure formation of the articular soft tissue cover, disruption of the subchondral layer of cartilage cells, and chondrocyte prolif

Current concepts in the management of pediatric hip disease.

A wide variety of disorders affects the pediatric hip. The most common of these are developmental dysplasia of the hip, Legg-Calvé-Perthes disease, and slipped capital femoral epiphysis. Magnetic resonance imaging and ultrasound improve the imaging of these diseases, but the ultimate impact of this technology on treatment is unknown. Complications of treatment for these and other pediatric hip diseases can be devastating; so much recent research concentrates on the etiology, prevention, and effects of these complications. Finally, these diseases occur in skeletally immature individuals, and lo

[Surgical treatment of anterior instability of the shoulder joint].

In the article are presented data on the operative treatment of 52 patients (54 operations) with anterior instability of the arm joints. Patients have been distributed into 4 groups depending upon the character of intra-articular damages, revealed in the process of arthrotomy. Each group of patients was subjected to adequate operative intervention, based upon Bankart's method. The authors proceeded from the principle that the arm joint instability treatment should be aimed at removal of intra-articular damages. There have been restored integrity of the damaged cartilage cushion and the joint c

Recurrent dislocation of the elbow in a child: case report and review of the literature.

Recurrent dislocation of the elbow is extremely rare in children. Surgical correction must be aimed at the specific pathology present in each case. Transfer of the biceps tendon and a central slip of the triceps were successful in an 8-year-old boy who had sustained six dislocations in 2 years.

[The significance of the fissurae petrotympanica, petrosquamosa and tympanosquamosa for disk displacements in the temporomandibular joint].

The correlation between disc displacements in the temporomandibular joint and the petrotympanic, petrosquamous and tympanosquamous fissures was investigated in 38 corpses. Only 12 joints had no ossification of the fissures, while 26 joints were partially ossified. With one exception, none of the joints with non-ossified fissures revealed disc displacement. In contrast, all the joints with ossified fissures revealed disc displacement. Women proved to have more displaced discs than men. On the basis of the findings, a model of the etiology of disc displacement is discussed.

Hereditary onycho-osteodysplasia (nail-patella syndrome). A three-generation familial study.

Hereditary onycho-osteodysplasia or nail-patella syndrome is an uncommon familial musculoskeletal condition that is primarily seen initially in orthopaedic clinics. This three-generation familial review identifies three affected family members, one of whom demonstrated spondylolisthesis, a condition not previously associated with nail-patella syndrome. All affected family members demonstrated the typical radial nail dysplasias, hypoplastic patellae, iliac horns, and posterior dislocation of the radial heads. Overgrowth of the medial femoral condyles caused valgus deformities of the knees that

[Clinical examination of the hips in newborn infants].

The author describes the criteria for screening a hip at risk in the newborn. The following criteria are important: evaluation of abduction, fetal position of the lower limbs and instability. After examination, the hips are classified as normal, irreducible and questionable. A good clinical examination can clarify most of these diagnoses.

Grisel's syndrome. Cervical spine clinical, pathologic, and neurologic manifestations.

Grisel's syndrome involves the subluxation of the atlantoaxial joint from inflammatory ligamentous laxity following an infectious process. Even though it was first described in 1830, it is a rare disease usually affecting children, but infrequent adult cases do occur. Patients generally seek treatment for progressive unrelenting throat and neck pain followed by torticollis and subluxation. Neurologic complications occur in approximately 15% of cases and can range from radiculopathy to myelopathy and even death. Principles of management include bacteriologic cure and correction of bony deformit

Osteoarthrosis as the cause of craniomandibular pain and dysfunction: a unifying concept.

It has been demonstrated that osteoarthrotic changes in the temporomandibular joint (TMJ) and in other synovial joints show a similar course, both clinically and (ultra)microscopically. Initially, cartilage changes and possibly also changes in the synovial membrane set up a vicious cycle of cartilage breakdown accompanied by attempts at repair. When the degradative process exceeds the response of repair, the osteoarthrotic disorder progresses into clinically detectable stages. Frequently, the gliding capacity of the articular disc is also impaired, giving rise to an internal derangement. In th

[The ankle joint fork and tarsus].

Severe soft tissue injury occurring in sprains and dislocations may lead to painful disability after plaster-immobilisation for 6-12 weeks. Therefore, primary operation is indicated for ligamentous repair, osteochondral fracture-fixation and internal K-wire stabilization of dislocations after reduction. After 1 week of immobilisation, the patient should begin active daily muscle exercises; in the 2nd week patients are mobilized with support of a dynamic, partial weight-bearing splint, allowing continued muscle exercises until weight-bearing is possible 6-8 weeks after surgery. In 23 patients u
